In the present study, 31 samples (12 fecal, 9 nasal and 10 rectal swabs) from 28/92 (30.43%, 10 captive and 18 free-roaming African green monkeys (AGMs, Chlorocebus sabaeus)) apparently healthy AGMs in the Caribbean Island of St. Kitts tested positive for adenoviruses (AdVs) by DNA-dependent DNA polymerase (pol)-, or hexon-based screening PCR assays. Based on analysis of partial deduced amino acid sequences of Pol- and hexon- of nine AGM AdVs, at least two AdV genetic variants (group-I: seven AdVs with a Simian mastadenovirus-F (SAdV-F)/SAdV-18-like Pol and hexon, and group-II: two AdVs with a SAdV-F/SAdV-18-like Pol and a Human mastadenovirus-F (HAdV-F)/HAdV-40-like hexon) were identified, which was corroborated by analysis of the nearly complete putative Pol, complete hexon, and partial penton base sequences of a representative group-I (strain KNA-08975), and -II (KNA-S6) AdV. SAdV-F-like AdVs were reported for the first time in free-roaming non-human primates (NHPs) and after ~six decades from captive NHPs. Molecular characterization of KNA-S6 (and the other group-II AdV) indicated possible recombination and cross-species transmission events involving SAdV-F-like and HAdV-F-like viruses, corroborating the hypothesis that the evolutionary pathways of HAdVs and SAdVs are intermingled, complicated by recombination and inter-species transmission events, especially between related AdV species, such as HAdV-F and SAdV-F. To our knowledge, this is the first report on detection and molecular characterization of AdVs in AGMs.

BACKGROUND: High levels of public awareness regarding the hazards of asbestos, rights to health, and benefits of an asbestos-free country can increase advocacy and political commitment to a total ban on asbestos. We aimed to investigate asbestos awareness and associated sociodemographic characteristics among the adult population of St. Kitts and Nevis. METHODS: In this cross-sectional study, 1009 participants completed an online questionnaire with questions about sociodemographic data and asbestos awareness. We applied multiple regression models to estimate associations between sociodemographic factors, levels of asbestos knowledge, and attitudes toward asbestos management. RESULTS: We found that 70% of residents of St. Kitts and Nevis considered asbestos exposure to be a general public concern and believed the government should prevent it. Of all participants, 54% were in favor of completely banning the use and importation of all asbestos products and materials; those with higher levels of asbestos knowledge were more likely to favor a total ban. Higher proportions and odds of favoring a total asbestos ban were also observed in participants aged ≥ 30 years, women, those with higher education, and those living in St. Kitts (vs. Nevis). CONCLUSIONS: These findings support implementing policies to regulate and outright ban the use of asbestos products and materials in St. Kitts and Nevis. This data can be used to develop tailored campaigns to improve asbestos knowledge among sociodemographic groups with lower asbestos awareness, such as in the wider Caribbean and other under-resourced countries.

The Caribbean spiny lobster, Panulirus argus (Latreille, 1804) is a highly commercial species and comprises the largest spiny lobster fishery in the world. Although populations have declined throughout its range, there is little known regarding its diseases and pathogens. The objectives of this study were to provide illustrated and standardized methods for postmortem examination, and to describe baseline gross and microscopic pathology for P. argus. From July 2017-March 2019, a postmortem examination including comprehensive histological assessment was performed on 313 fishery-caught lobsters. Epibionts and lesions observed include branchial cirriped infestation (69%), branchial encysted nemertean worm larvae (23%), tail fan necrosis (11%), skeletal muscle necrosis (7%), antennal gland calculi (6%), branchial infarction (2%), and microsporidiosis (0.6%). This report confirms the rare prevalence of microsporidiosis in P. argus and describes nemertean worm larvae in the gill. This study also reports a condition resembling excretory calcinosis in spiny lobster. The methods and data produced by this study facilitate disease diagnosis and sustainable stock management of P. argus.

A 4-month-old intact male domestic shorthair kitten living in St Kitts, West Indies presented with respiratory distress, cachexia, and mucopurulent nasal discharge. Thoracic radiographs revealed a diaphragmatic hernia. The diaphragmatic hernia as well as subpleural pulmonary nodules suspicious for verminous pneumonia were identified during a postmortem examination. Histology showed multifocal to coalescing pyogranulomatous and eosinophilic pneumonia centered on larvae and morulated eggs. The lesion and nematode morphology were consistent with Aelurostrongylus abstrusus. Although Aelurostrongylus abstrusus has been reported worldwide, this is the first report of a metastrongyloid lungworm in cats in St. Kitts and for the West Indies. This case report should increase the awareness of A. abstrusus pneumonia in cats from St. Kitts and other locations in the eastern Caribbean.

We identified multiple extraintestinal cystacanths during routine postmortem examination of 3 small Indian mongooses and 2 African green monkeys from the Caribbean island of St. Kitts. In mongooses, cystacanths were encysted or free in the subcutaneous tissue, skeletal muscle, or peritoneal or pericardial cavities, whereas in the monkeys, they were in the cavity and parietal layer of the, tunica vaginalis, skeletal muscle, and peritoneal cavity. Morphological, histological, and molecular characterization identified these cystacanths as Oncicola venezuelensis (Acanthocephala: Oligacanthorhynchidae). There was minimal to mild lymphoplasmacytic inflammation associated with the parasite in the mongooses and moderate inflammation, mineralization, hemorrhage, and fibrosis in the connective tissue between the testis and epididymis in 1 monkey. We identified a mature male O. venezuelensis attached in the aboral jejunum of a feral cat, confirming it as the definitive host. Termites serve as intermediate hosts and lizards as paratenic hosts. This report emphasizes the role of the small Indian mongoose and African green monkey as paratenic hosts for O. venezuelensis.

Free-roaming chickens on Caribbean islands are important sentinels for local avian diseases and those introduced by birds migrating through the Americas. We studied 81 apparently healthy unvaccinated free-roaming chickens from 9 parishes on St. Kitts, an eastern Caribbean island. Using commercial ELISAs, no chickens had antibodies against avian influenza virus, West Nile virus, or Salmonella Enteritidis, although seropositivity was high to infectious bursal disease virus (86%), infectious bronchitis virus (84%), Mycoplasma (37%), and avian avulavirus 1 (Newcastle disease virus, 31%). Examination of small and large intestinal contents revealed cestodes in 79% and nematodes in 75% of the chickens. Although ectoparasites and endoparasites were common (74% and 79%, respectively), only a few chickens had lesions at postmortem examination, mainly intestinal serosal nodules (12%) and feather loss (6%). Histologic examination of 18 organs from each bird revealed lesions in high percentages of organs, mainly the liver (86%), lung (75%), spleen (60%), small intestine (56%), skin (42%), and kidney (40%). Lesions included degenerative, reactive, inflammatory, and neoplastic, and were not correlated with the serologic status of the chickens except in one case of infectious bursal disease. Microscopically, Paratanaisia bragai was seen in the kidneys of 3 chickens and intestinal coccidiasis in 1 chicken. Pulmonary silicate aggregates were common, were present in intestinal serosal nodules, and were suggestive of environmental exposure.

OBJECTIVE: To characterize high-risk human papillomavirus (HPV) infections in a sample of women in two small English-speaking Caribbean countries: Saint Kitts and Nevis and Saint Vincent and the Grenadines. METHODS: Sexually active women ≥ 30 years old attending primary care health facilities participated in the study. Each participant had a gynecological examination, and two cervical specimens were collected: (1) a specimen for a Papanicolaou (Pap) test and (2) a sample of exfoliated cervical cells for HPV DNA testing, using the HPV High Risk Screen Real-TM (Sacace). High-risk HPV genotypes were assessed in 404 women in Saint Kitts and Nevis and 368 women in Saint Vincent and the Grenadines. RESULTS: High-risk HPV was detected in 102 of 404 (25.2%) in Saint Kitts and Nevis and in 109 of 368 (29.6%) in Saint Vincent and the Grenadines. High-risk HPV genotypes 52, 35, 51, 45, and 31 were the most common high-risk types in Saint Kitts and Nevis. In Saint Vincent and the Grenadines, the most common high-risk HPV genotypes were 45, 35, 31, 18, and 51. Current age was found to be significantly associated with high-risk HPV infection in both countries. In addition, in Saint Vincent and the Grenadines, high parity (> 3 pregnancies) and having had an abnormal Pap smear were found to be independent risk factors for high-risk HPV. CONCLUSIONS: These results contribute to the evidence on HPV prevalence for small island states of the Caribbean and support the accelerated introduction of the 9-valent HPV vaccine in the two countries and elsewhere in the English-speaking Caribbean. Use of the study's results to guide the development of policy regarding implementation of HPV testing as the primary screening modality for older women is recommended.

Objectives Platynosomum species are cat-specific parasitic trematodes that parasitize the biliary ducts and gall bladder. Due to the common connection to the major duodenal papilla of the pancreas and common bile ducts in addition to the periductal proximity of the pancreas, it is possible that platynosomosis could cause pancreatitis. The objective of this study was to determine whether platynosomosis, a commonly diagnosed parasitic disease in cats on St Kitts, has any association with pancreatic disease. Methods To investigate this possibility, the pancreas of free-roaming cats with naturally acquired platynosomosis were evaluated via ultrasound, serum concentrations of feline pancreatic lipase (fPL), cobalamin, folate and feline trypsin-like immunoreactivity (fTLI) and histopathology. Twenty free-roaming, young adult, feral cats, positive for feline immunodeficiency virus, and diagnosed with Platynosomum species infection via fecal analysis were recruited. The liver, biliary system and pancreas were evaluated via ultrasonography during a short duration anesthesia. Serum concentrations of fPL, fTLI, folate and cobalamin were measured. Sections of the right limb, left limb and body of the pancreas were evaluated histopathologically using hematoxylin and eosin (H&E) stain. Results None of the cats had sufficient criteria to fulfill the ultrasonographic diagnosis of pancreatitis. One cat had an elevated fPL concentration in the range consistent with pancreatitis. Four cats had cobalamin deficiencies and 11 had abnormal folate concentration. The fTLI concentration was equivocal for the diagnosis of exocrine pancreatic insufficiency in one cat. With a single exception, histopathology changes, when present (n = 12), were mild, non-specific and predominantly characterized by lymphocytic infiltrates and fibrosis. The exception was a cat that presented a chronic interstitial and eosinophilic pancreatitis of slightly increased severity, likely the result of platynosomosis. Conclusions and relevance The results of this study suggest that platynosomosis rarely induces pancreatic damage in cats. With only one exception, chronic pancreatitis diagnosed in cats with fluke-induced cholangitis and cholangiohepatitis was subtle and interpreted as an incidental background lesion unrelated to platynosomosis.

BACKGROUND: The prevalence of chronic kidney disease (CKD) in St. Kitts and Nevis, islands of the West Indies, is unknown. We sought to determine estimates of CKD and its risk factors (e.g. diabetes, hypertension and obesity) in St. Kitts and Nevis. METHODS: This was a chronic disease screening program. Three community-based locations in St. Kitts and Nevis were included in the program. Participants were adult community residents aged ≥18 years. The main outcome measures were estimated CKD prevalence (by serum creatinine-based estimated glomerular filtration rate (eGFR) and dipstick urine albumin); and estimated prevalence of CKD risk factors (diabetes, hypertension and obesity). Logistic regression was used to determine independent predictors of CKD. RESULTS: One thousand nine hundred seventy eight persons, from Nevis (n = 950) and St. Kitts (n = 1028) were screened by the Caribbean Health and Education Foundation. Participants' mean age was 49 ± 15 years, 65% were female, and 99% were black. Fully, 21.5% had diabetes and 53.1% had hypertension; and 40.3% were obese. Mean estimated eGFR was 98 ml/min/1.73 m2 (standard deviation = 30) and 4.7% had an eGFR <60 ml/min/1.73 m2, indicating CKD. Age [Odds Ratio (OR) = 1.08, 95% Confidence Interval (CI) 1.05-1.11], hypertension (OR = 2.89, 95% CI 1.18-7.07) and diabetes (OR = 3.12, 95% CI 1.80-5.43) were independent predictors of reduced eGFR in models adjusted for age, gender and obesity status. Of those with urine testing in Nevis (n = 929), 13.5% had urine albumin ≥30 mg/dL, and diabetes was an independent predictor of this finding (OR = 2.43, 95% CI 1.53-3.87). CONCLUSIONS: CKD and its risk factors were prevalent among adults in St. Kitts and Nevis. Public policy strategies for prevention and treatment of these conditions may be needed to reduce their associated morbidity, mortality and costs.

RESUMEN Objetivo Caracterizar la infección por el virus del papiloma humano (VPH) de alto riesgo en una muestra de mujeres de dos países pequeños del Caribe de habla inglesa: Saint Kitts y Nevis, y San Vicente y las Granadinas. Métodos Participaron en el estudio mujeres sexualmente activas ≥ 30 años que acudieron a centros de atención primaria. Se realizó un examen ginecológico a todas las participantes y se le tomaron dos muestras cervicouterinas: 1) una muestra para efectuar la prueba de Papanicolaou y 2) células cervicouterinas exfoliadas para efectuar la prueba de ADN del VPH, mediante la prueba de detección del VPH de alto riesgo en tiempo real (HPV High Risk Screen Real-TM, Sacace). Se evaluaron los genotipos de alto riesgo del VPH en 404 mujeres en Saint Kitts y Nevis, y en 368 mujeres en San Vicente y las Granadinas. Resultados Se detectó VPH de alto riesgo en 102 de 404 mujeres (25,2%) de Saint Kitts y Nevis, y en 109 de 368 (29,6%) de San Vicente y las Granadinas. Los genotipos 52, 35, 51, 45 y 31 de VPH de alto riesgo fueron los detectados con más frecuencia en Saint Kitts y Nevis. En San Vicente y las Granadinas, los más frecuentes fueron los genotipos 45, 35, 31, 18 y 51. Se observó una relación significativa entre la edad actual y la infección por el VPH de alto riesgo en los dos países. Además, en San Vicente y las Granadinas, se halló que la multiparidad (> 3 embarazos) y los resultados anómalos de la prueba de Papanicolaou son factores de riesgo independientes de la infección por el VPH de alto riesgo. Conclusiones Estos resultados aportan datos acerca de la prevalencia del VPH en los estados insulares pequeños del Caribe y respaldan la introducción acelerada de la vacuna nonavalente contra el VPH en los dos países y en los demás países del Caribe de habla inglesa. Se recomienda aplicar los resultados de este estudio para guiar la formulación de políticas con respecto a la incorporación de las pruebas de detección del VPH como método primario de tamizaje en las mujeres mayores.

Cada participante foi submetida a exame ginecológico e coleta de duas amostras do colo do útero: (1) uma amostra para exame de Papanicolau e (2) uma amostra de citologia esfoliativa para teste de DNA do HPV, utilizando o kit HPV High Risk Screen Real-TM (Sacace). A presença de genótipos de HPV de alto risco foi avaliada em 404 mulheres em São Cristóvão e Nevis e 368 mulheres em São Vicente e Granadinas. Resultados HPV de alto risco foi detectado em 102/404 mulheres (25,2%) em São Cristóvão e Nevis e em 109/368 (29,6%) em São Vicente e Granadinas. Os genótipos 52, 35, 51, 45 e 31 do HPV foram os tipos de alto risco mais comuns em São Cristóvão e Nevis. Em São Vicente e Granadinas, os genótipos do HPV de alto risco mais comuns foram 45, 35, 31, 18 e 51. Foi constatada associação significante entre idade atual e infecção por HPV de alto risco em ambos os países. BACKGROUND: Toxoplasma gondii is a ubiquitous protozoan parasite capable of infecting all warm-blooded animals including livestock. In these animals, the parasite forms cysts in the tissues which may pose a risk to public health if infected meat is consumed undercooked or raw. The aim of this study was to determine the exposure of livestock to T. gondii in St. Kitts and Nevis. METHODS: Sera and/or heart tissue and meat juice were collected from pigs (n = 124), sheep (n = 116) and goats (n = 66) at the St. Kitts Abattoir. Sera and meat juice were screened for reactive antibodies to T. gondii using an in-house ELISA. Heart tissue was screened for T. gondii DNA using quantitative PCR and positive samples were genotyped using RFLP. RESULTS: Antibodies to T. gondii were detected in sera from 48% of pigs, 26% of sheep and 34% of goats tested. Antibodies were also detected in the meat juice from 55% of pig hearts, 22% of sheep hearts and 31% of goat hearts tested. There was a significant positive correlation between serology and meat juice results. T. gondii DNA was detected in heart tissue of 21% of pigs, 16% of sheep and 23% of goats tested. Preliminary PCR-RFLP analysis identified a predominance of the Type III genotype of T. gondii. CONCLUSIONS: These results suggest widespread environmental contamination with T. gondii oocysts and that livestock could be a potentially important source of T. gondii infection if their infected meat is consumed (or handled) undercooked.

BACKGROUND: Toxoplasma gondii is a protozoan parasite capable of infecting all warm-blooded animals. Humans can become infected by ingesting infective oocysts from the environment or contaminated food or water, or by ingesting tissue cysts in undercooked infected meat or by handling infected meat. Caribbean African green monkeys (Chlorocebus sabaeus) are present in large numbers on the island of St. Kitts in the Caribbean, and it is not uncommon for these animals to be trapped and eaten by islanders. The aim of this study was to determine T. gondii infection in Caribbean African green monkeys. FINDINGS: Sera collected from 79 wild-caught Caribbean African green monkeys were examined for T. gondii antibodies by ELISA. Antibodies were detected in 38 out of 79 (48.1%) monkeys. Significantly more females were infected than males but there was no significant effect of age or location on antibody status. CONCLUSIONS: Results indicate that Caribbean African green monkeys can be infected with T. gondii and that there is widespread environmental contamination of St. Kitts with oocysts. These monkeys could present a potential source of T. gondii infection if their meat is consumed undercooked. This is the first report of T. gondii antibodies in this species.

An outbreak of Streptococcus iniae occurred in the early months of 2008 among wild reef fish in the waters of the Federation of St Kitts and Nevis, lasting almost 2 months. Moribund and dead fish were collected for gross, histological, bacteriological, and molecular analysis. Necropsy findings included diffuse fibrinous pericarditis, pale friable livers, and serosal petechiation. Cytological and histological analysis revealed granulocytic and granulomatous inflammation with abundant coccoid bacterial organisms forming long chains. Necrosis, inflammation, and vasculitis were most severe in the pericardium, meninges, liver, kidneys, and gills. Bacterial isolates revealed ß-hemolytic, Gram-positive coccoid bacteria identified as S. iniae by amplification and 16S ribosomal RNA gene sequencing. Results from biochemical and antimicrobial susceptibility analysis, together with repetitive element palindromic polymerase chain reaction fingerprinting, suggest that a single strain was responsible for the outbreak. The inciting cause for this S. iniae-associated cluster of mortalities is unknown.

Numerous myxozoan cysts (∼ 1 mm) were found in the musculature of blackfin tuna (Thunnus atlanticus) harvested off the Caribbean island of St. Kitts. Myxospores were consistent with quadrate members of the Kudoidae, measuring 8.8 (8.2-9.4) µm wide, 7.3 (6.6-8.3) µm thick, and 6.2 (5.8-6.9) µm long with 4 uniform drop-like polar capsules measuring 2.7 (2.2-3.2) µm long and 2.0 (1.7-2.2) µm wide. The 18S small-subunit (SSU) and 28S large-subunit (LSU) ribosomal DNA sequences did not result in direct matches to any published sequences. However, the SSU sequences (1,786 base pairs [bp]) obtained from 6 individual cysts were identical and demonstrated high homology to Kudoa thunni (99.0%) from albacore (Thunnus alalunga). Alternatively, 33 unique sequences were obtained for the LSU (∼ 800 bp), demonstrating 0.1 to 5.0% variability between them, although a majority of these sequences (60%) demonstrated high homology (>99%) to K. thunni. Morphologically, the case isolate was smaller than published descriptions of K. thunni; however, rDNA sequence homology, and phylogenetic placement based on concatenated SSU and LSU rDNA sequences suggests this case isolate and K. thunni are conspecific. To our knowledge this is the first report of K. thunni infection in blackfin tuna from the Caribbean.

OBJECTIVE: The aim of the present study was to investigate the occurrence of keratinophilic fungi including dermatophytes on feathers of domestic and wild birds in the islands of St Kitts and Nevis. METHOD: During 2010-2011, samples of feathers from ninety-four birds were examined by hair-baiting technique in Petri-dishes containing sterilized soil. Fungal growths appearing on the feathers and the hair-baits were microscopically examined and the cultures obtained were identified on the basis of their microscopic and colonial morphology. RESULTS: Chrysosporium constituted the majority (86.9%) of the 72 isolates of keratinophilic fungi, represented by mainly C tropicum and C indicum. Sepedonium spp isolates were recovered from nine of the feather samples; two of these were identified as Sepedonium chrysospermum, and the other two as S ampullosporum. CONCLUSION: Recovery of four isolates of the dermatophyte, Microsporum gypseum complex (two each of M gyspeum and M fulvum) from feathers of birds is a finding of public health significance.


OBJETIVO: El objetivo del presente estudio fue investigar la presencia de hongos queratinofílicos, incluyendo dermatofitos, en las plumas de aves domésticas y silvestres en las islas de St Kitts y Nieves. MÉTODOS: Durante 2010-2011, se examinaron muestras de plumas de noventa y cuatro aves, utilizando la técnica de anzuelo queratínico (técnica de Vanbreuseghem) en placas de Petri con tierra esterilizada. Los crecimientos fúngicos que aparecieron sobre las plumas y los anzuelos de queratina de pelos (hair baits) fueron examinados bajo el microscopio, y los cultivos obtenidos fueron identificados sobre la base de su morfología microscópica y colonial. RESULTADOS: Chrysosporium constituyó la mayor parte (86.9%) de los 72 aislados de hongos queratinofílicos, representados principalmente por el C tropicum y el C indicum. Aislados de Sepedonium spp fueron obtenidos de nueve muestras de plumas. Dos de ellos fueron identificados como Sepedonium chrysospermum y los otros dos como S ampullosporum. CONCLUSIÓN: La recuperación de cuatro aislados del complejo M gypseum dermatofito (formado por dos M gyspeum y dos M fulvum respectivamente) de las plumas de aves, es un hallazgo de importancia para la salud pública.

ABSTRACT OBJECTIVE: Previous mosquito surveys performed in the Federation of St Kitts and Nevis identified Anopheles albimanus in Nevis but there is no recorded occurrence of this mosquito in St Kitts. To determine the presence of this and other species in St Kitts and Nevis, a mosquito survey was conducted. METHODS: Surveys were performed in two phases - the dry season (March 16-23, 2010, in St Kitts), and the rainy season (October 18-25, 2010) in St Kitts and Nevis. BG SentinelTM mosquito traps baited with BG Lure® and CO2 were set in a variety of habitats (urban, rural, semi-urban, dry forest and mangrove). Identification was performed using morphological keys. RESULTS: The most abundant species during both phases were Culex quinquefasciatus, Aedes taeniorhynchus and Aedes aegypti. A new record for St Kitts was Anopheles albimanus which was trapped during the rainy season near a mangrove site. CONCLUSION: This is the first time a potential malaria vector has been identified in St Kitts.


OBJETIVO: Las encuestas previas sobre mosquitos realizadas en la Federación de Saint Kitts y Nevis identificaron al Anopheles albimanus en Nevis, pero no hay registro de este mosquito en Saint Kitts. El objetivo de esta investigación fue determinar la presencia de esta u otra especie de mosquito en Saint Kitts. MÉTODOS: Se realizaron encuestas en dos fases - la estación seca (16-23 de marzo de 2010, en Saint Kitts), y la estación lluviosa (18-25 de octubre de 2010) en Saint Kitts y Nevis. Trampas BG SentinelTM para mosquitos preparadas con BG Lure® y CO2 se colocaron en diversos habitats (urbano, rural, semi-urbano, bosque seco y manglar). La identificación fue realizada usando claves morfológicas. RESULTADOS: Las especies más abundantes durante ambas fases fueron Culex quinquefasciatus, Aedes taeniorhynchus y Aedes aegypti. Un nuevo registro para Saint Kitts fue Anopheles albimanus, atrapado durante la estación de lluvias cerca de un manglar. CONCLUSIÓN: Esta es la primera vez que se identifica un vector potencial de la malaria en Saint Kitts.

Tumor necrosis factor (TNF) promoter single-nucleotide polymorphisms (SNPs) have been extensively characterized in humans, with numerous reports of associations with obesity-related phenotypes as well an array of infectious, immune-mediated, and inflammatory disease phenotypes. Controlling for the multitude of environmental risk factors in human studies has been a major confounder of efforts to elucidate the role and relative contribution of TNF promoter SNPs. As part of an ongoing initiative to further genetically and phenotypically characterize the St Kitts-origin vervet monkey (Chlorocebus aethiops ssp.) as an animal model of human obesity, we have conducted association analyses between TNF SNPs and previously defined obesity-related phenotypes in 265 pedigreed vervets. We report eight SNPs (-809G, -756A, -352C, -322A, +1285T, +2133T, +2362A, +2405), all contained within the same haplotype block and comprising a single haplotype, to be significantly associated with BMI, waist circumference, total plasma cholesterol (P < 0.05), and high-density lipoprotein-cholesterol (HDL-C) (P < 0.01). This study provides additional validation of the St Kitts-origin vervet model of obesity by demonstrating genetic associations analogous to that shown in humans.

Stray cats trapped in various areas of Basseterre, the capital of St Kitts in the West Indies, were tested for infection with feline immunodeficiency virus (FIV) and feline leukemia virus (FeLV) using commercial kits. Of 99 (51 male and 48 female) cats trapped in 2006/7, 15% (12 males and three females) were positive for FIV while none were positive for FeLV. Of 72 (41 males and 31 females) cats trapped in 2009, 14% (nine males and one female) were positive for FIV while none were positive for FeLV. Polymerase chain reaction analysis revealed DNA of Bartonella species in whole blood collected from 60/95 (63%) cats trapped in 2006/7. Sequencing of the 16S-23S rRNA gene intergenic transcribed spacer (ITS) region of a convenience sample of nine amplicons and the 11 isolates made from 43 blood samples which were cultured using Bartonella alpha Proteobacteria (BAPGM) enrichment medium revealed B henselae (14) and B clarridgeiae (six).

OBJECTIVE: The purpose of this study is to review bacterial isolates from cases of urinary tract infection (UTI) and their antimicrobial susceptibility pattern for the years 2005-2007 in St Kitts. It is hoped that the study will be of use in the treatment of cases of UTI in St Kitts. METHODS: The laboratory records at St Francis Hospital, Basseterre, St Kitts, for bacterial isolates from cases of urinary tract infection and their susceptibility profiles for three years, 2005-2007, were retrospectively reviewed and compared. RESULTS: A total of 595 isolates of 13 species of pathogenic bacteria were recovered from cases of UTI. Escherichia coli was the predominant species recovered each year. Among the other species frequently recovered were Citrobacter spp, Enterobacter spp, Klebsiella pneumoniae and Pseudomonas aeruginosa. CONCLUSION: This study, the first of its kind from St Kitts serves to emphasize that treatment of UTI should be instituted generally on the basis of antimicrobial susceptibility tests.


OBJETIVO: El propósito de este estudio es examinar los aislados bacterianos de casos de infección del tracto urinario (ITU) y su modelo de susceptibilidad antimicrobiana durante los años 2005-2007 en Saint Kitts. Se espera que el estudio sea de utilidad en el tratamiento de casos de ITU en Saint Kitts. MÉTODOS: Los archivos de laboratorio del Hospital Saint Francis, Basseterre, Saint Kitts, sobre los aislados bacterianos de los casos de infección del tracto urinario y sus perfiles de susceptibilidad durante tres años, 2005-2007, fueron examinados y comparados retrospectivamente. RESULTADOS: Un total de 595 aislados de 13 especies de bacterias patógenas fueron recuperadas de casos de ITU. Escherichia coli fue la especie predominante recuperada cada año. Entre las otras especies frecuentemente recuperadas se hallan: Citrobacter spp, Enterobacter spp, Klebsiella pneumoniae y Pseudomonas aeruginosa. CONCLUSIÓN: Este estudio - el primero de su tipo en Saint Kitts - sirve para enfatizar que el tratamiento de la ITU generalmente debe establecerse sobre la base de pruebas de susceptibilidad antimicrobiana.

OBJECTIVE: Cancer screening is one approach that can reduce morbidity and mortality through early detection of precancers; however anxiety, fear, and lack of information are important barriers to universal cancer screening in the Caribbean. Nevis is a small island located in the Eastern Caribbean. We report available data on screening prevalence and cancer mortality for four common cancers: cervical, breast, colon and prostate. METHODS: Demographic information, screening utilization (when available) and cancer mortality statistics were obtained from the Charlestown Health Clinic and the annual reports from the Ministry of Health. Moreover, physicians and key stakeholders were interviewed to assess current guidelines for cancer screening as well as to indicate areas of need. RESULTS: Cervical cancer screening is underutilized in Nevis. Between 2001 and 2007, the overall prevalence of Pap testing fluctuated minutely (mean: 6.87%, range: 6.06 - 7.41%). Systematic screening for breast, colon, and prostate cancer is not routinely performed, thus utilization rates are not available. Cancer mortality varied slightly between 2002 and 2006; prostate cancer had the highest overall crude mortality rate (30.6 deaths/100 000 persons). Physician interviews revealed that adherence to US and European cancer screening guidelines are inappropriate for their population of patients. Breast and prostate cancers are frequently diagnosed in these geographic areas before the age when cancer screening is currently recommended. CONCLUSION: Cancer is perceived as an important health problem by physicians, key stakeholders and citizens, however cancer screening is underutilized in Nevis. Future research should focus on generating screening guidelines that are relevant for this population, as well as methods to promote screening.


OBJETIVO: El pesquisaje de cáncer es un método que puede reducir la morbilidad y la mortalidad mediante la detección precoz de precánceres. Sin embargo, la ansiedad, el miedo, y la falta de información son barreras importantes que se erigen frente al pesquisaje universal del cáncer en el Caribe. Nevis es una pequeña isla situada en el Caribe Oriental. Reportamos datos disponibles sobre la prevalencia del pesquisaje y la mortalidad por cáncer en relación con cuatro tipos de cánceres comunes: el cervical, y el de mamas, colon y próstata. MÉTODOS: La información demográfica, la utilización de pesquisaje (de hallarse disponible) y las estadísticas sobre la mortalidad por cáncer, se obtuvieron de la Clínica de la Salud de Charlestown, y los reportes anuales del Ministerio de Salud. Además, los médicos y las partes interesadas claves, fueron entrevistados a fin de evaluar las guías actuales para el pesquisaje del cáncer así como para indicar las áreas de necesidad. RESULTADOS: El pesquisaje del cáncer cervical es subutilizado en Nevis. Entre 2001 y 2007, la prevalencia general de la prueba citológica tuvo una fluctuación mínima (promedio: 6.87%, rango: 6.06 - 7.41%). El pesquisaje sistemático del cáncer de mamas, colon, y próstata no se realiza como rutina, y por consiguiente no existen tasas de utilización disponibles. La mortalidad por cáncer varió ligeramente entre 2002 y 2006. El cáncer de próstata tuvo la tasa de mortalidad cruda general más alta (30.6 muertes/100 000 personas). Las entrevistas médicas revelaron que las guías de pesquisaje en USA y Europa son inapropiadas para la población de pacientes. Los cánceres de mamas y próstata son diagnosticados con frecuencia en estas áreas geográficas antes de la edad en que se recomienda el pesquisaje actualmente. No obstante, el pesquisaje del cáncer es subutilizado en Nevis. Las investigaciones futuras deben centrarse en producir guías de pesquisaje que sean pertinentes para esta población, así como en métodos de promover el pesquisaje.
