RESUMO

BACKGROUND: Healthy sleep is crucial for the physical and mental wellbeing of adolescents. However, many adolescents suffer from poor sleep health. Little is known about how to effectively improve adolescent sleep health as it is shaped by a complex adaptive system of many interacting factors. This study aims to provide insights into the system dynamics underlying adolescent sleep health and to identify impactful leverage points for sleep health promotion interventions. METHODS: Three rounds of single-actor workshops, applying Group Model Building techniques, were held with adolescents (n = 23, 12-15 years), parents (n = 14) and relevant professionals (n = 26). The workshops resulted in a multi-actor Causal Loop Diagram (CLD) visualizing the system dynamics underlying adolescent sleep health. This CLD was supplemented with evidence from the literature. Subsystems, feedback loops and underlying causal mechanisms were identified to understand overarching system dynamics. Potential leverage points for action were identified applying the Action Scales Model (ASM). RESULTS: The resulting CLD comprised six subsystems around the following themes: (1) School environment; (2) Mental wellbeing; (3) Digital environment; (4) Family & Home environment; (5) Health behaviors & Leisure activities; (6) Personal system. Within and between these subsystems, 16 reinforcing and 7 balancing feedback loops were identified. Approximately 60 potential leverage points on different levels of the system were identified as well. CONCLUSIONS: The multi-actor CLD and identified system dynamics illustrate the complexity of adolescent sleep health and supports the need for developing a coherent package of activities targeting different leverage points at all system levels to induce system change.

Introduction. Friendly Health Services are spaces of comprehensive and differential care for adolescents and young people, which start from their biological, social and emotional. Objective. Evaluate the perceptions of youth about the quality of care they received and their experience in the Community Family Health Units. Methodology. A mixed study was carried out that collected the perceptions of the participants in two successive stages. First with a questionnaire that evaluated the quality of care and experience of users, then through group and individual interviews that estimated the level of satisfaction. Results. Youth consult health establishments, although 68.2 % are unaware of the exclusive programs for them (Friendly Health Services). The most used service was general medicine (76.6 %). Regarding the care received, the respect, trust and privacy provided by health professionals was rated as excellent or very good (76 %). 39.7 % reported that the waiting time was 30 to 60 minutes, 17.6 % reported waiting more than two hours. Conclusion. Currently, there is a need to improve access to services for adolescents and young people by socializing the offer to encourage their use, increase attention in preventive areas and implement their evaluation with methods different from current ones

RESUMO

OBJECTIVES: Adolescents make up an estimated 22 % of the 11.8 million population of Papua New Guinea (PNG), yet, as in many low and middle income countries, there are limited specific services for adolescents, who in traditional health service design fall between paediatric and adult patients. With more survivors of chronic illnesses, including tuberculosis and HIV, congenital and acquired heart disease, epilepsy and cerebral palsy, diabetes, cancer and other chronic conditions, there are gaps in the care of such young people, and in their eventual transition to adult services. Moreover, traditional health service design rarely addresses many adolescent health concerns - mental health, self-esteem, substance use, and adolescent-friendly sexual and reproductive health. This study, amongst a cohort cared for in a provincial hospital in Milne Bay Province, Papua New Guinea, aimed to explore the perceptions of adolescents on the health care they receive, and their views on their health priorities. METHODS: A qualitative study was carried out from April to August 2022, amongst adolescents (persons aged 10-19 years) attending or admitted to Alotau Provincial Hospital. Data were collected via semi-structured wide-ranging interviews, and contextual data obtained from their medical charts. Thematic data analysis was done. RESULTS: Fifty-four adolescents were interviewed. Adolescents were generally content with the care they received, although many raised concerns regarding feeling out of place in a ward with older adults or infants, and staff attitudes towards them including not directly communicating. Forty four adolescents preferred an adolescent-friendly setting, as opposed to the adult or children's wards, stating that it would allow for positive peer-interaction, improve their experience in the hospital, and improve the quality of health care received. Many adolescents with chronic illnesses had concerns regarding loneliness and isolation, and uncertainty of their future. Pregnant adolescents highlighted the lack of education on sexual and reproductive health, and experiences of gender-based violence. CONCLUSIONS: This study shows that adolescents can perceive the need for adolescent-friendly health services and advocate for improvement in the quality of health care they receive, and the breadth of adolescent health care concerns.

RESUMO

Social media influencers (SMIs) have become an important source of health information for adolescents. However, the lack of expertise and commercial interests of SMIs pose challenges for adolescent health. To gain a better understanding of these challenges, this scoping review aimed to synthesize existing research on the role of SMIs in adolescent health. Using the PRISMA-ScR approach, we conducted a comprehensive search of eight databases and screened articles based on predefined criteria. The final sample included 51 articles published between 2012 and 2022. Results revealed inconsistent definitions and classifications of SMIs. The most common topics included appearance, nutrition, and substance use, while mental and sexual health topics were underrepresented. The ability of SMIs to establish trustworthy relationships with their followers was highlighted in the reviewed studies, shedding light on potential benefits and challenges for adolescent health. However, most studies focused on the negative roles of SMIs, such as the promotion of unrealistic body images, unhealthy diets, substance use, and inaccurate diagnosis and treatment advice. In addition, inappropriate advertising of unhealthy products (e.g., junk food, tobacco products and alcohol) was identified as a major challenge. The reviewed studies identified stricter regulation and improved social media and health literacy as important avenues for policy action. Less attention was given to the potential positive impact of SMIs and how to effectively include them in health promotion campaigns. In addition, the majority of studies were conducted in the Global North and relied on quantitative approaches, resulting in a lack of representation of minority populations and male adolescents.

RESUMO

American Indian (AI) adolescents who practice healthy behaviors of sleep, nutrition, physical activity, and limited screen time can lower their lifetime risk of diet-sensitive disease. Little is known about how AI parenting practices influence the health behaviors of youth. The objective of this qualitative study was to explore how a group of AI parents of youths at risk of disease influenced their youth's health behaviors after a family intervention. A secondary objective was to understand the role of AI parents in supporting and sustaining health behavior change in their youths following the intervention. Semi-structured in-depth interviews were conducted with AI parents (n = 11) and their young adolescents, 10-15 years old (n = 6). Parents reported facilitators to how they enacted healthy lifestyle behaviors, including family togetherness, routines, youth inclusion in cooking, and motivation due to a health condition in the family. Barriers to enacting healthy behaviors included a lack of time, a lack of access to health resources, negative role modeling, and the pervasiveness of screen media. Three major themes about the role of AI parenting emerged inductively from the interview data: "Parenting in nontraditional families", "Living in the American grab-and-go culture", and "Being there and teaching responsibility". The importance of culture in raising youths was emphasized. These findings inform strategies to promote long-term adherence to behavior changes within the intervention. This study contributes to public health conversations regarding approaches for AI youths and families, who are not well represented in previous health behavior research.

RESUMO

Bullying at school affects the health and development of children and adolescents. This study aimed to describe evidence about the characteristics of boys and girls identified as bullies in bullying situations. This is a review with publications from 2015 to 2020, operationalized in SciELO, Scopus, Web of Science, and PsycINFO. Data were independently extracted by two reviewers and submitted to methodological quality assessment. The corpus was composed of 27 primary studies. It was found that boys do more bullying and are more engaged in physical aggression. Girls engage more in verbal or psychological type violence. Social/cultural issues are factors consistently associated with such differences. Regarding the consequences of the aggression perpetrated, bullies, regardless of gender, may also have mental health problems. This study gathers quantitative evidence on characteristics of boys and girls who engage in bullying at school. Differences/similarities should be considered in anti-bullying intervention programs.

BACKGROUND: To improve health, intersectoral cooperation is often advocated. However, only few studies have reported health effects of this approach. Sweden has adopted a national public health policy (NPHP), which focuses on intersectoral primary prevention of disorders and injuries. AIM: To investigate the effects of the NPHP, on child and adolescent health in Sweden during the period 2000-19. METHODS: In the first step, the most important improvements in disorders and injuries, assessed as DALYs and incidences, were identified using the GBD Compare database. In the second step, primary prevention methods for these disorders and injuries were identified. In the third step, the relative importance of various government agents for these preventive measures was assessed using Google searches. RESULTS: Out of 24 groups of causes of disease or injury, only two groups demonstrated an incidence decrease: neoplasms and transport injuries. Leukaemia neoplasms might be prevented by reducing parental smoking, reducing outdoor air pollution and having the mother take folate supplements before getting pregnant. Transport injuries might be prevented by speed restrictions, and physically separating pedestrians from vehicle transport. Most of the primary prevention work was done by government agencies, like the Swedish Transport Agency, which worked independently of the National Institute of Public Health. CONCLUSION: Governmental agencies outside the health carried out most of the effective primary preventive efforts, almost independently of the NPHP.

RESUMO

Policy Points Social indicators of young peoples' conditions and circumstances, such as high school graduation, food insecurity, and smoking, are improving even as subjective indicators of mental health and well-being have been worsening. This divergence suggests policies targeting the social indicators may not have improved overall mental health and well-being. There are several plausible reasons for this seeming contradiction. Available data suggest the culpability of one or several common exposures poorly captured by existing social indicators. Resolving this disconnect requires significant investments in population-level data systems to support a more holistic, child-centric, and up-to-date understanding of young people's lives.

RESUMO

PURPOSE OF REVIEW: This review defines intersectionality, discusses recent studies that use an intersectional framework in adolescent health research, and outlines ways where clinicians can use intersectionality to address health disparities in youth of color through clinical practice, research, and advocacy. RECENT FINDINGS: Research using an intersectional framework can identify populations at risk for certain disorders or behaviors. Recent studies in adolescent health research using an intersectional lens identified lesbian girls of color as an at-risk population for e-cigarette use, demonstrated lower skin color satisfaction among Black girls of all ages predicted greater binge-eating disorder symptoms, and showed that two-thirds of Latine (gender-neutral term that refers to people with Latin American roots) youth who recently immigrated to the United States experienced at least one traumatic event during their migration journey, putting them at risk for PTSD and other mental health disorders. SUMMARY: Intersectionality refers to how multiple social identities intersect to produce a specific experience that reflects overlapping systems of oppression. Diverse youth contain multiple identities that intersect to produce unique experiences and health inequities. Using an intersectional framework acknowledges that youth of color are not homogenous. RESUMO

INTRODUCTION: Over the past 50 years, there has been a decline in average sleep duration and quality, with adverse consequences for overall health. Growing evidence from countries around the world shows the negative impact of using internet access technologies on sleep due to the short wavelength enriched light emitted by these electronic devices. OBJECTIVE: To analyze the exposure to modern technologies/internet and its relationship with the state of daytime sleepiness in teenage high school students. METHODS: a survey was conducted in six public schools located in the northeast of Brazil. The sample consisted of 1,130 students (mean age 16.6 years ± 1.1). The data collection instrument used was an objective criterion. The characteristic analytical study and quantitative interpretation. Quantitative variables were expressed as mean and standard deviation. For association analyses, the chi-square test or Student's t-test was used. It was considered significant p<0.05. RESULTS: We found that 7% and 31.6% of the participants reported a state of sleepiness during and after classes, respectively. 59.3% report using the internet excessively, 39.9% have a feeling of dependence, 32.6% report sleep deprivation due to excessive internet use, and 26.7% are unsuccessful in trying to reduce the use of cell phones. CONCLUSION: The relationships between the variables in this study will allow us, the younger students use internet access equipment, the more they are exposed to the risk of sleep impairment.

RESUMO

INTRODUCTION: Children with COVID-19 may be asymptomatic or present a heterogeneous clinical presentation. The present case series aimed to report clinical manifestations of COVID-19 in children and adolescents admitted to pediatric intensive care units (PICU) in the city of São Carlos, Brazil, during 2020 and 2021. REPORTS: The subjects were six children aged 3 months to 13 years, with COVID-19. The data were collected from electronic charts. All cases were domestic contact with a COVID-19 case. Two cases had multisystem inflammatory syndrome (MIS-C) and one had jaundice and ascites. One case had a seizure. One case required invasive ventilation and two cases presented gastrointestinal symptoms. There were no deaths in the cases. The length of PICU stays varied from one to 16 days. CONCLUSION: In the six cases reported, COVID-19 clinical manifestations in children and adolescents who required intensive care in São Carlos revealed a heterogeneous presentation and no lethality. It is worth emphasizing that a history of contact with a symptomatic respiratory person should guide the suspicion of COVID-19 in children and indicate a proper follow-up, as COVID-19 may be severe in this population.

Purpose: This study employed an intersectional framework to examine impact of inequalities related to sexual minority (SM) and ethnic minority (EM) identities in risk for health, well-being, and health-related behaviors in a nationally representative sample. Methods: Participants included 9789 (51% female) adolescents aged 17 years from the U.K.-wide Millennium Cohort Study, with data on self-identified sexual and ethnic identities. Adolescents were grouped into White heterosexual, White-SM, EM-heterosexual, and EM-SM categories. Questionnaires assessed mental health (e.g., self-reported psychological distress, doctor-diagnosed depression, attempted suicide), general health (self-rated health, chronic illness, body mass index), and health-related behaviors (e.g., smoking, substance use). Associations were analyzed using multivariable logistic regression. Results: SM individuals (White: 18% and EM: 3%) had increased odds for mental health difficulties and attempted suicide, with higher odds for White-SM individuals than for EM-SM individuals. Compared with White heterosexual individuals, White-SM and EM-SM individuals had higher risk for psychological distress (adjusted odds ratios [OR] 3.47/2.24 for White-SM/EM-SM, respectively) and emotional symptoms (OR 3.17/1.65). They had higher odds for attempted suicide (OR 2.78/2.02), self-harm (OR 3.06/1.52), and poor sleep quality (OR 1.88/1.67). In contrast, the White heterosexual and White-SM groups had similarly high proportions reporting risky behaviors except for drug use (OR 1.45) and risky sex (OR 1.40), which were more common in White-SM individuals. EM-heterosexual and EM-SM individuals had decreased odds for health-related behaviors. Conclusion: SM (White and EM) individuals had substantially worse mental health compared with heterosexual peers. Adverse health-related behaviors were more common in White-SM individuals. Investigation into the mechanisms leading to these differences is needed.
