RESUMO
Os cuidados paliativos têm uma abordagem que melhora a qualidade de vida dos pacientes e seus familiares, que enfrentam doenças que ameacem a vida. Previne e alivia o sofrimento através da identificação precoce, avaliação correta e tratamento da dor e outros problemas físicos, psíquicos, sócio familiares e espirituais. Diante da importância, é necessário ampliar o conhecimento sobre os cuidados paliativos oferecidos aos pacientes em tratamento oncológico.

Objetivo:

avaliar a qualidade de vida de pacientes com câncer em cuidados paliativos submetidos a tratamentos ambulatorial e hospitalar.

Método:

foi um estudo transversal e observacional, com amostra de conveniência de 101 pacientes, realizado no Hospital de Câncer de Pernambuco entre junho a dezembro de 2017. O HCP é uma instituição filantrópica, sem fins lucrativos, que atende exclusivamente os pacientes com câncer do SUS. Foram avaliados dois grupos de pacientes em tratamento paliativo, um grupo n=42, em regime de internação hospitalar (INT) e o outro n=59, em regime de tratamento ambulatorial (AMB). Foi utilizado o questionário de qualidade de vida WHOQOL-bref com 26 perguntas e 4 domínios específicos.

Resultados:

verificou-se que a maioria dos pacientes do grupo AMB tinha idade maior ou igual a 65 anos, enquanto no grupo INT a maioria tinha idade menor que 65 anos. Houve predomínio do sexo feminino, sendo, 61,01% do grupo AMB, enquanto no grupo INT foram 42,85%. A neoplasia mais frequente foi o câncer de cabeça e pescoço, com 28,81% no grupo AMB e 23,80% no INT. A maior parte dos pacientes do grupo AMB tinha escala de desempenho paliativo PPS de 40; 47,4% e o grupo INT PPS de 30; 42,85%. No escore global dos quatro domínios avaliados físico, psicológico, relações sociais e meio ambiente, a qualidade de vida do grupo AMB obteve escore médio maior (49,8%) que o grupo INT (31,9%) (p =0.0004). A avaliação do escore médio da qualidade de vida QV1 foi de 49,6% e o grupo INT 32,7%. Com relação a satisfação com a saúde QV2, o grupo AMB obteve escore de 42,7%, sendo duas vezes maior que o do grupo INT 20,8%.

Conclusões:

Os resultados deste estudo mostraram que os cuidados paliativos realizados em casa, junto aos seus familiares, proporcionaram uma melhor qualidade de vida para os pacientes com câncer, e por conseguinte, isso pode impactar na redução de custos operacionais dos serviços de saúde do SUS, à medida em que diminui o tempo médio de internação (AU)
ABSTRACT
Palliative care has an approach that improves the quality of life of patients and their families facing life-threatening illnesses. Prevents and alleviates suffering through early identification, correct assessment and treatment of pain and other physical, psychic, family and spiritual problems. Given the importance, it is necessary to increase the knowledge about the palliative care offered to patients on cancer treatment.

Objective:

to evaluate the quality of life of cancer patients in palliative care submitted to outpatient and hospital treatments.

Method:

was a cross-sectional, observational study with the convenience assessment of 101 patients performed at the Hospital de Cancer de Pernambuco between June 2017. HCP is a nonprofit philanthropic institution that exclusively serves patients with SUS cancer. Two groups of patients undergoing palliative treatment were evaluated, one group n = 42, in hospital admission (INT) and the other group n = 59, in outpatient treatment (AMB). The WHOQOL-bref quality of life questionnaire was used with 26 questions and 4 specific domains.

Results:

most of the patients in the AMB group were older than or equal to 65 years of age, whereas in the INT group the majority were younger than 65 years. Females predominated, being 61.01% of the AMB group, while in the INT group they were 42.85%. The most frequent neoplasm was head and neck cancer, with 28.81% in the AMB group and 23.80% in the INT group. Most patients in the AMB group had a PPS palliative performance scale of 40; 47.4% and the INT PPS group of 30; 42.85%. In the overall score of the four domains evaluated physical, psychological, social relations and environment, the quality of life of the AMB group had a higher mean score (49.8%) than the INT group (31.9%) (p = 0.0004). The mean QV1 quality of life score was 49.6% and the INT group was 32.7%. Regarding the QV2 health satisfaction, the AMB group had a score of 42.7%, being twice that of the INT group 20.8%.

Conclusions:

The results of this study showed that palliative care performed at home, with their relatives, provided a better quality of life for cancer patients, and therefore, this may impact on the reduction of the operational costs of SUS health services, as the average length of hospital stay decreases (AU)
