RESUMO

The role and place of health care facilities of the highly specialized (tertiary) level in radiation emergencies in the conditions of martial law are considered taking into account the experience of overcoming of the Chornobyl catas-trophe consequences. The classification of emergencies and the main scenarios of radiation emergencies (nuclear terrorism, radiation accidents and incidents, the use of nuclear weapons) are presented. The principles of medical preparedness and providing the highly specialized medical care in a radiological emergency are presented on the base of the Chornobyl catastrophe experience. It is determined that the tertiary health care facilities in a radiological emergency should provide as follow: development and implementation of plans and protocols of highly specialized medical care; organization, direct provision, coordination and quality control of the highly specialized medical care; aggregation and coordination of efforts of all three levels of health care facilities; integration into the radiation emergency management system, and analysis and generalization of the experience gained.

RESUMO

In this review, we summarize information about the current trends in hematopoietic stem cells transplantation (HSCT) in Europe and world. HSCT has represented one of the most innovative and highly expensive method of treatment for a set of malignant and non-malignant disorders. Differences in the number of HSCT and transplant rates (number of HSCT per 10 million inhabitants), indications and types of transplantations between countries have been reported. They were attributed mainly to differences in the economic situation of the countries and differences in prevalence of certain types of diseases. The gross national income per capita, health care expenditures per capita, number of transplant teams per 1 million inhabitants and team experience are the main impact factors influencing on transplant activity. These data provide a basis for health care planning, preparation of normative acts and future research in Ukraine for rapid expansion HSCT

RESUMO

The article considers the possibility of the use of current infrastructure of specialized population registers of Ukraine to study leukemia and other systemic blood diseases revealed in Chernobyl accident liquidators. Advantage and limitation of such registers in the use are discussed in the article. Ukrainian state register of people who suffered from Chernobyl accident and Ukrainian national cancer registers are the largest population registers in the country, which cover all the territory of Ukraine and contain information on each individual and may serve as source base for epidemiological studies. To solve issues on leukemia and other oncological diseases is recommended to use in complex data of specialized registers of Ukraine. It should be also taken into account necessity of using late registered cases and verifying registered diagnoses.

RESUMO

The qualitative and quantitative parameters of hemopoiesis and immunocompetent cells were studied in 365 children in the early "iodine" and long-term periods after the accident. In accordance with primary response, a group consisting of 28 subjects with so-called "radiation injury" was distinguished. The early period was marked by unusual qualitative and quantitative hematological changes which, however, did not lead to the development of the hematological syndrome characteristic of acute radiation injury. Certain deviations that corresponded to somatic pathology were detectable at the late periods after the accident.
