Soil environmental safety has received much attention during the past few decades due to its significance in agricultural production and human health. Special attention is required for soil pesticide residues and ecological risks. This study examined 197 soil samples from industrial, residential and agricultural areas for the presence of 12 organophosphorus pesticides (OPPs) and 8 synthetic pyrethroids (SYPs) in the 16 cities in Henan Province, and the center of CPUA, based on the Central Plains Urban Agglomeration (CPUA) concept proposed by China. The total average concentrations of ∑12OPPs in industrial, residential and agricultural soils were 194, 217, 267 ng/g dry weight, and those of ∑8SYPs were 26.8, 35.7, 25.5 ng/g dry weight, respectively. The two pollutants with the greatest concentrations in the soils were malathion and fenpropathrin, respectively, the dominant components of OPPs and SYPs. The soil environmental carrying capacity (SECC) analysis, representing the maximum residual load that can be supported, shows that acephate and cyhalothrin were overloaded, with a predicted period of over 500 years. Among the 16 cities of CPUA, a higher frequency of high ecological risk could be observed only in Shangqiu. The OPPs in children had total non-carcinogenic risk values of more than 1.0. Similarly, the non-carcinogenic risks of SYPs in adults and children in the residential areas were more than 1.0. The study provides knowledge on how to effectively manage soil safety in Henan Province, which is the center of the CPUA, with a large population and grain province to protect ecosystems and reduce the risks of soil pesticide residues in humans.

Background: Geriatric diseases (e.g., chronic diseases and geriatric syndromes) may result in impaired physical performance and a decline in the quality of life. The results of previous studies reported the positive effects of comprehensive community-based rehabilitation (CBR) services on physical and social functioning and psychosocial wellbeing. However, to provide adequate and personalised rehabilitation services, it is essential to understand the needs of the older adults population. There have been no studies on the need for CBR in older adults populations that consider their heterogeneity. Therefore, high-quality studies are required to recognise the heterogeneity and latent classes of CBR needs in older adults population groups. This study aims to identify the heterogeneity of the rehabilitation needs of older adults in the community and explore whether older adults with similar characteristics have similar needs through a cross-sectional survey and latent class analysis (LCA) to provide support for personalised rehabilitation services. Methods: The study is structured into four phases. The first phase will focus on constructing a comprehensive questionnaire to assess rehabilitation needs. In the second phase, a pilot study will be conducted to evaluate the reliability and validity of the completed questionnaire. This step ensures the robustness of the instrument for data collection. The third phase will involve cross-sectional surveys using the finalised questionnaires to collect the necessary data from the targeted population. The fourth phase will focus on conducting LCA to determine the CBR needs of the older adult population. Discussion: The results of this study will provide novel and critical information for a better understanding of the rehabilitation needs, potential categories, and influencing factors of older adults in the community. The study will be conducted in Guizhou Province in western China, where economic and social development is relatively low, and the results will inform and benefit other regions and developing countries facing similar challenges. However, because of the complete social security and rehabilitation service systems in developed areas, our research results may not fully reflect the situation in these areas. The occurrence of microplastics (MPs) has been widely reported in human foodstuffs, and their potential negative effects on human health have been brought into focus. Processed foods are more susceptible to MPs as contamination can be introduced during processing and packaging. However, the risk posed by MPs in processed foods remained unclear. This work aims to critically review the available data for MPs in 11 types of possessed foods and to conduct a preliminary risk assessment of MPs in processed foods. For a comprehensive evaluation, three indicators were selected and determined, namely chemical risk, pollution load, and estimated daily intake (EDI). Our results suggest that nori has the highest chemical risk, followed by canned fish, beverages, table salt, and other food items. In the case of pollution load, nori and milk fall into the risk category of Ⅳ and Ⅲ respectively. Table salts, bottled water, and sugar exhibited lower MPs pollution load (risk category of Ⅱ), whereas the pollution loads of other foods were calculated to be category Ⅰ. Moreover, a correlation between the pollution load of sea salts and MPs pollution level in ambient seawater was found. Regarding EDI of MPs from different processed foods, MPs intakes through bottled water (14.3 ± 3.4 n kg-1 d-1) and milk (6.6 ± 2.4 n kg-1 d-1) are significantly higher than that of the other foods (< 1 n kg-1 d-1). The probabilistic estimation of MPs daily intake indicated that children (19.7 n kg-1 d-1) are at a higher health risk than adults (female: 17.6 n kg-1 d-1, male: 12.6 n kg-1 d-1). Nevertheless, the exposure dose used in toxicological studies was about 10 times higher than the MPs intake via processed foods. Therefore, we argued that MPs in processed foods only carry limited risk. Overall, this study would provide the basis for risk management of MPs in processed food products.

It is widely accepted that elevated protease activity (EPA) in chronic wounds impedes healing. However, little progress has occurred in quantifying the level of protease activity that is detrimental for healing. The aim of this study was to determine the relationship between inflammatory protease activity and wound healing status, and to establish the level of EPA above which human neutrophil-derived elastase (HNE) and matrix metalloproteases (MMP) activities correlate with nonhealing wounds. Chronic wound swab samples (n = 290) were collected from four wound centers across the USA to measure HNE and MMP activity. Healing status was determined according to percentage reduction in wound area over the previous 2-4 weeks; this was available for 211 wounds. Association between protease activity and nonhealing wounds was determined by receiver operating characteristic analysis (ROC), a statistical technique used for visualizing and analyzing the performance of diagnostic tests. ROC analysis showed that area under the curve (AUC) for HNE were 0.69 for all wounds and 0.78 for wounds with the most reliable wound trajectory information, respectively. For MMP, the corresponding AUC values were 0.70 and 0.82. Analysis suggested that chronic wounds having values of HNE >5 and/or MMP ≥13, should be considered wound healing impaired. EPA is indicative of nonhealing wounds. Use of a diagnostic test to detect EPA in clinical practice could enable clinicians to identify wounds that are nonhealing, thus enabling targeted treatment with protease modulating therapies.

Leaf economics spectrum (LES), characterizing covariation among a suite of leaf traits relevant to carbon and nutrient economics, has been examined largely among species but hardly within species. In addition, very little attempt has been made to examine whether the existence of LES depends on spatial scales. To address these questions, we quantified the variation and covariation of four leaf economic traits (specific leaf area, leaf dry matter content, leaf nitrogen and phosphorus contents) in a cosmopolitan wetland species (Phragmites australis) at three spatial (inter-regional, regional, and site) scales across most of the species range in China. The species expressed large intraspecific variation in the leaf economic traits at all of the three spatial scales. It also showed strong covariation among the four leaf economic traits across the species range. The coordination among leaf economic traits resulted in LES at all three scales and the environmental variables determining variation in leaf economic traits were different among the spatial scales. Our results provide novel evidence for within-species LES at multiple spatial scales, indicating that resource trade-off could also constrain intraspecific trait variation mainly driven by climatic and/or edaphic differences.

With volume-of-interest (VOI) cone-beam computed tomography (CBCT) imaging, one set of projection images are acquired with the VOI collimator at a regular or high exposure level and the second set of projection images are acquired without the collimator at a reduced exposure level. The high exposure VOI scan data inside the VOI and the low exposure full-field scan data outside the VOI are then combined together to generate composite projection images for image reconstruction. To investigate and quantify scatter reduction, dose saving and image quality improvement in VOI CBCT imaging, a flat panel detector-based bench-top experimental CBCT system was built to measure the dose, the scatter-to-primary ratio (SPR), the image contrast, noise level, the contrast-to-noise ratio (CNR) and the figure of merit (FOM) in the CBCT reconstructed images for two polycarbonate cylinders simulating the small and the large phantoms. The results showed that, compared to the full field CBCT technique, radiation doses for the VOI CBCT technique were reduced by a factor of 1.20 and 1.36 for the small and the large phantoms at the phantom center, respectively, and from 2.7 to 3.0 on the edge of the phantom, respectively. Inside the VOI, the SPRs were substantially reduced by a factor of 6.6 and 10.3 for the small and the large phantoms, the contrast signals were improved by a factor of 1.35 and 1.8, and the noise levels were increased by a factor of 1.27 and 1.6, respectively. As a result, the CNRs were improved by a factor of 1.06 and 1.13 for the small and the large phantoms and the FOM improved by a factor of 1.4 and 1.7, respectively.

PURPOSE: In this study, the authors investigated the feasibility of a dual resolution volume-of-interest (VOI) cone beam breast CT technique and compared two implementation approaches in terms of dose saving and scatter reduction. METHODS: With this technique, a lead VOI mask with an opening is inserted between the x-ray source and the breast to deliver x-ray exposure to the VOI while blocking x rays outside the VOI. A CCD detector is used to collect the high resolution projection data of the VOI. Low resolution cone beam CT (CBCT) images of the entire breast, acquired with a flat panel (FP) detector, were used to calculate the projection data outside the VOI with the ray-tracing reprojection method. The Feldkamp-Davis-Kress filtered backprojection algorithm was used to reconstruct the dual resolution 3D images. Breast phantoms with 180 microm and smaller microcalcifications (MCs) were imaged with both FP and FP-CCD dual resolution CBCT systems, respectively. Two approaches of implementing the dual resolution technique, breast-centered approach and VOI-centered approach, were investigated and evaluated for dose saving and scatter reduction with Monte Carlo simulation using a GEANT4 package. RESULTS: The results showed that the breast-centered approach saved more breast absorbed dose than did VOI-centered approach with similar scatter reduction. The MCs in fatty breast phantom, which were invisible with FP CBCT scan, became visible with the FP-CCD dual resolution CBCT scan. CONCLUSIONS: These results indicate potential improvement of the image quality inside the VOI with reduced breast dose both inside and outside the VOI.

This work is to demonstrate that high quality cone beam CT images can be generated for a volume of interest (VOI) and to investigate the exposure reduction effect, dose saving, and scatter reduction with the VOI scanning technique. The VOI scanning technique involves inserting a filtering mask between the x-ray source and the breast during image acquisition. The mask has an opening to allow full x-ray exposure to be delivered to a preselected VOI and a lower, filtered exposure to the region outside the VOI. To investigate the effects of increased noise due to reduced exposure outside the VOI on the reconstructed VOI image, we directly extracted the projection data inside the VOI from the full-field projection data and added additional data to the projection outside the VOI to simulate the relative noise increase due to reduced exposure. The nonuniform reference images were simulated in an identical manner to normalize the projection images and measure the x-ray attenuation factor for the object. Regular Feldkamp-Davis-Kress filtered backprojection algorithm was used to reconstruct the 3D images. The noise level inside the VOI was evaluated and compared with that of the full-field higher exposure image. Calcifications phantom and low contrast phantom were imaged. Dose reduction was investigated by estimating the dose distribution in a cylindrical water phantom using Monte Carlo simulation based Geant4 package. Scatter reduction at the detector input was also studied. Our results show that with the exposure level reduced by the VOI mask, the dose levels were significantly reduced both inside and outside the VOI without compromising the accuracy of image reconstruction, allowing for the VOI to be imaged with more clarity and helping to reduce the breast dose. The contrast-to-noise ratio inside the VOI was improved. The VOI images were not adversely affected by noisier projection data outside the VOI. Scatter intensities at the detector input were also shown to decrease significantly both inside and outside the VOI in the projection images, indicating potential improvement of image quality inside the VOI and contribution to dose reduction both inside and outside the VOI.
