STATEMENT OF PROBLEM: The esthetic assessment of smile lines by laypersons is a subject of ongoing debate. However, smile lines often appear with different types of upper lip curvature, which further complicates the esthetic assessment process, and studies on this combination are lacking. PURPOSE: The purpose of this clinical study was to investigate a layperson's esthetic perception of smile lines and upper lip combined images. MATERIAL AND METHODS: Twenty-six smile images resulting from combinations of 3 upper lip types, 4 anterior smile line types, and 3 posterior smile line types were generated by an image editing software program. Eighty-three laypersons (39 men and 44 women; 18 to 35 years of age) completed rating images using a visual analog scale. Unattractive smiles were designated to be those with scores <50 and attractive ones with scores ≥50. Data were analyzed using 1-way analysis of variance and Bonferroni post hoc tests (α=.05). RESULTS: High anterior smile line with gingival display >4 mm obtained significantly lower scores of <50 when combined with all upper lip curvatures (upward: 28.29 ±22.79, straight: 38.74 ±23.00, downward: 30.67 ±22.25, P<.01). High anterior smile lines with gingival display ≤4 mm combined with upward and straight upper lip curvature images obtained significantly higher scores, and all were ≥50 (upward: 63.24 ±22.22, straight: 61.40 ±21.58, P<.01). CONCLUSIONS: From a layperson's perspective, high anterior smile lines with gingival display >4 mm combined with any lip type were determined to be unattractive. If gingival display was ≤4 mm combined with both upward and straight lip types, the smile was assessed as attractive.

BACKGROUND: Changes in alveolar bone dimension after tooth extraction may affect placement of the subsequent implant, resulting in ridge deficiency that can adversely impact long-term implant stability or aesthetics. Alveolar ridge preservation (ARP) was effective in reducing the amount of ridge resorption following tooth extraction. There is sparse evidence regarding the benefit of ARP at periodontally compromised molar extraction sockets. This study will be a randomized trial to assess the soft tissue contour, radiographical, and histological changes of ARP at molar extraction sites in order to compare severe periodontitis cases with natural healing results and determine the most beneficial and least traumatic clinical treatment for such patients. METHODS: This research is designed as a two-group parallel randomized controlled trial. The total number of tooth extraction sites will be 70 after calculation with power analysis. Teeth will be randomly assigned to two groups with the test group conducting ridge preservation and the control group healing naturally. Periodontal examination, cone beam-computed tomography (CBCT) data, and stereolithographic (STL) files obtained by intraoral scanning will be collected through the follow-up period, and bone biopsy samples would be obtained during implant surgery. The primary outcomes are the vertical and horizontal change of alveolar ridge measured on CBCT images, soft tissue contour changes evaluated by superimposing the digital impressions, alterations of mucosa thickness (as measured by superimposing the CBCT data and STL files), histological features of implant sites and periodontal parameter changes. The secondary outcomes are patient-reported post-operative reaction and conditions of simultaneous bone graft or sinus lifting procedures during implantation. DISCUSSION: This study will provide information about hard and soft tissue dimension changes and histomorphology evaluation following ARP and natural healing in periodontally compromised molar sites, which may contribute to complement the missing information of ARP at periodontally compromised molar extraction sockets. There remains a clinical need for an accurate and non-invasive imaging tool for intraoral evaluation of dental conditions. Optical coherence tomography (OCT) is a potential candidate to meet this need, but the design of current OCT systems limits their utility in the intraoral examinations. The inclusion of light-induced autofluorescence (LIAF) can expedite the image collection process and provides a large field of view for viewing the condition of oral tissues. This study describes a novel LIAF-OCT system equipped with a handheld probe designed for intraoral examination of microstructural (via OCT) and microvascular information (via OCT angiography, OCTA). The handheld probe is optimized for use in clinical studies, maintaining the ability to detect and image changes in the condition of oral tissue (e.g., hard tissue damage, presence of dental restorations, plaque, and tooth stains). The real-time LIAF provides guidance for OCT imaging to achieve a field of view of approximately 6.9 mm × 7.8 mm, and a penetration depth of 1.5 mm to 3 mm depending on the scattering property of the target oral tissue. We demonstrate that the proposed system is successful in capturing reliable depth-resolved images from occlusal and palatal surfaces and offers added design features that can enhance its usability in clinical settings.

Background: Information regarding using a pig cadaver model for teaching purposes in dentistry is limited, especially for periodontal surgery procedures. The aim of this study was to assess the feasibility and efficacy of teaching crown lengthening surgical procedures using a prepared pig cadaver model. Methods: Mandibles of slaughtered pigs with subgingival crown fracture defects on two premolars and two molars on each side were prepared as periodontal surgery teaching cases. A resident group (n = 20) and an instructor group (n = 18) participated in assessing the efficacy of the model by completing questionnaires before and after training sessions. Data was either assessed descriptively or analyzed statistically with Wilcoxon signed-rank test with the significance level at α = 0.05. Results: Results revealed that all the knowledge points showed statistically significant improvements (p < 0.05) except for the procedure to determine the quantity of bone removal during osteotomy procedures. Most residents rated the efficacy of the model obtained with 9.0 out of 10 scale. The data of effectiveness of the pig cadaver model from the instructor group ranged from 7.4 ± 1.4 to 9.0 ± 1.0. Conclusion: Results of this study support feasibility in using prepared pig cadaver models to teach crown lengthening surgical procedures to postgraduates.

STATEMENT OF PROBLEM: The adaptation of interim crowns made by subtractive and additive manufacturing has not been well investigated. PURPOSE: The purpose of this in vitro study was to evaluate the internal fit and marginal discrepancy of interim crowns made by different manufacturing methods. MATERIAL AND METHODS: A dentoform mandibular left first molar was prepared for a ceramic crown and scanned for the fabrication of 48 stereolithical resin dies and interim crowns. Group CAM included 16 ZCAD interim crowns made by computer-aided design and computer-aided manufacturing (CAD-CAM) technology; group 3DP, 16 NextDent MFH interim crowns made by digital light processing technology; and group APP, 16 Jet interim crowns manually made by using autopolymerizing acrylic resin and used as controls. The silicone replica technique was used to determine the internal discrepancy volume before definitive cementation. All crowns were cemented with Temp-Bond NE under a 50-N load and bench set for 10 minutes before microcomputed tomographic (µCT) scan assessment. The volume of cement space was measured by using the µCT scan 3-dimensional (3D) images, and gap distance at assigned locations was recorded by using the µCT scan 2-dimensional (2D) images. The marginal discrepancy was measured by the polyvinyl siloxane (PVS) impression technique and using a stereomicroscope. Data were analyzed by ANOVA and the Tukey honestly significant difference tests (α=.05). The association between different measuring techniques was analyzed by the Pearson correlation test. RESULTS: The gap distance between interim crowns and dies from all 3 groups measured by using the µCT scan 2D images ranged from 0.13 mm to 0.55 mm, with the highest value found at the central occlusal location in group APP. The total average gap distance values recorded for group APP were significantly higher than those for group CAM and group 3DP (P<.05). No significant differences were found in internal discrepancy and cement space volume between group CAM and group 3DP (P>.05). The Pearson correlation test showed a moderate correlation (r=0.69) between the silicone replica technique and the µCT scan technique in determining cement space volume. When the PVS impression technique was used to measure the marginal discrepancy, the mean values obtained from group APP were statistically significantly higher than those from group CAM and group 3DP (P<.05). No statistically significant difference in marginal discrepancy was found between group CAM and group 3DP (P=.70). CONCLUSIONS: Digitally fabricated interim crowns had better internal fit and smaller marginal discrepancy than manually constructed interim crowns. The silicone replica technique and µCT scan technique measurements had a moderate correlation in assessing the adaptation of cemented interim crowns.

PURPOSE: To compare the internal fit and marginal discrepancy of acrylic resin interim crowns fabricated by different manufacturing methods, and to test the consistency of measuring marginal discrepancy and internal fit between different measuring techniques. MATERIALS AND METHODS: A dentoform mandibular left first molar was prepared for an all-ceramic crown. Thirty-six interim crowns were fabricated and divided into three groups (n = 12): group BAC (Bis-acrylic composite, fabricated manually), group CAM (CAD/CAM polymethylmethacrylate resin, milled), and group 3DP (3D printed methacrylic oligomers, printed). The internal fit of the interim crowns was evaluated by the silicone replica technique and by X-ray microcomputed tomography (µCT) technique. The marginal discrepancy of the interim crowns was evaluated by the vinyl polysiloxane (VPS) (Aquasil Ultra XLV) impression technique and by optical coherence tomography (OCT) technique. Data were statistically analyzed using ANOVA and Turkey tests at α = 0.05. Pearson correlation test was used to evaluate the correlation between the different measurement techniques and marginal discrepancy/internal fit. RESULTS: The manually fabricated interim crowns (group BAC) had significantly greater discrepancy of internal fit than did the digitally fabricated crowns (group CAM and group 3DP) measured by both silicone replica technique and µCT 2-dimensional (2D) image measurement. There were no statistically significant differences in the cement space volume values obtained by the µCT image technique between group BAC and group 3DP (p = 0.285). The coefficient of determination between the two volumetric measurement techniques was low (R2 = 0.30). For marginal discrepancy, the manually fabricated interim crowns had a wider absolute marginal discrepancy than both digitally fabricated groups (p < 0.05). In both the VPS impression and OCT assessment, there was no statistically significant difference between group CAM and group 3DP (p = 0.798 and 0.994, respectively). The coefficient of determination between the VPS impression and OCT techniques for marginal discrepancy measurement was low (R2 = 0.23). CONCLUSIONS: Digitally fabricated interim crowns (group CAM and group 3DP) had better internal fit and smaller marginal discrepancy than manually fabricated interim crowns (group BAC). For comparison of the different evaluation techniques, the silicone replica technique and µCT measurements had low correlation for internal fit assessment, as did the PVS impression and OCT techniques for marginal discrepancy test.

Gingiva is the soft tissue that surrounds and protects the teeth. Healthy gingiva provides an effective barrier to periodontal insults to deeper tissue, thus is an important indicator to a patient's periodontal health. Current methods in assessing gingival tissue health, including visual observation and physical examination with probing on the gingiva, are qualitative and subjective. They may become cumbersome when more complex cases are involved, such as variations in gingival biotypes where feature and thickness of the gingiva are considered. A noninvasive imaging technique providing depth-resolved structural and vascular information is necessary for an improved assessment of gingival tissue and more accurate diagnosis of periodontal status. We propose a three-dimensional (3D) imaging technique, optical coherence tomography (OCT), to perform in situ imaging on human gingiva. Ten volunteers (five male, five female, age 25-35) were recruited; and the labial gingival tissues of upper incisors were scanned using the combined use of state-of-the-art swept-source OCT and OCT angiography (OCTA). Information was collected describing the 3D tissue microstructure and capillary vasculature of the gingiva within a penetration depth of up to 2 mm. Results indicate significant structural and vascular differences between the two extreme gingival biotypes (ie, thick and thin gingiva), and demonstrate special features of vascular arrangement and characteristics in gingival inflammation. Within the limit of this study, the OCT/OCTA technique is feasible in quantifying different attributes of gingival biotypes and the severity of gingival inflammation.

STATEMENT OF PROBLEM: Information is lacking about the fatigue resistance of computer-aided design and computer-aided manufacturing (CAD-CAM) tooth-colored implant custom abutment materials. PURPOSE: The purpose of this in vitro study was to investigate the reliability of different types of CAD-CAM tooth-colored implant custom abutments. MATERIAL AND METHODS: Zirconia (Lava Plus), lithium disilicate (IPS e.max CAD), and resin-based composite (Lava Ultimate) abutments were fabricated using CAD-CAM technology and bonded to machined titanium-6 aluminum-4 vanadium (Ti-6Al-4V) alloy inserts for conical connection implants (NobelReplace Conical Connection RP 4.3×10 mm; Nobel Biocare). Three groups (n=19) were assessed: group ZR, CAD-CAM zirconia/Ti-6Al-4V bonded abutments; group RC, CAD-CAM resin-based composite/Ti-6Al-4V bonded abutments; and group LD, CAD-CAM lithium disilicate/Ti-6Al-4V bonded abutments. Fifty-seven implant abutments were secured to implants and embedded in autopolymerizing acrylic resin according to ISO standard 14801. Static failure load (n=5) and fatigue failure load (n=14) were tested. Weibull cumulative damage analysis was used to calculate step-stress reliability at 150-N and 200-N loads with 2-sided 90% confidence limits. Representative fractured specimens were examined using stereomicroscopy and scanning electron microscopy to observe fracture patterns. RESULTS: Weibull plots revealed ß values of 2.59 for group ZR, 0.30 for group RC, and 0.58 for group LD, indicating a wear-out or cumulative fatigue pattern for group ZR and load as the failure accelerating factor for groups RC and LD. Fractographic observation disclosed that failures initiated in the interproximal area where the lingual tensile stresses meet the compressive facial stresses for the early failure specimens. Plastic deformation of titanium inserts with fracture was observed for zirconia abutments in fatigue resistance testing. CONCLUSIONS: Significantly higher reliability was found in group ZR, and no significant differences in reliability were determined between groups RC and LD. Differences were found in the failure characteristics of group ZR between static and fatigue loading.

OBJECTIVES: This study aimed to classify the dynamic smile and to quantify the gingival line (GL), as well as apico-coronal displacement of the gingival zenith (GZ), in the maxillary anterior dentition in a young Chinese population. METHODS: Two-hundred young Chinese subjects (100 men and 100 women; 20-35 years of age) with healthy dentogingival tissue were recruited. The dynamic smile process was captured using a digital camera. The smile type, GL type, the vertical distance of the GZ between the canine and the central incisor on the same side and the GZ of the lateral incisor-GL relationship were measured using a self-developed smile-analysis method. The kappa statistics was used to examine the reliability of the data recorded by the rater. The Pearson chi-square test was used to analyse the differences between subjects regarding the frequencies of smile type and GL type at α = 0.05. RESULTS: Data revealed that 45.5% of subjects had a high smile and 45.5% had an average smile; 58.2% of the subjects presented an upwards GL. The GZ of canine teeth was 0.33 mm apical to the corresponding central incisor and no significant difference between both sides of the GZ was observed. The GZ of the lateral incisor was located coronal to the GL in 87.9% of samples. The vertical distance between the GZ of the lateral incisor and the GL was 0.59 mm and no statistically significant difference was detected. CONCLUSIONS: The most common findings were a high or average smile type, combined with an upward GL. In the majority of subjects, the GZ of the lateral incisor is coronal to the GL. The apico-coronal displacement of the GZ showed bilateral symmetry.

STATEMENT OF PROBLEM: Although various zirconia abutments have been introduced, insufficient data exist regarding the maximum load capacity of internal tri-channel connection zirconia implant abutments with various implant-abutment interfaces. PURPOSE: The purpose of this in vitro study was to compare the maximum load capacity of 3 different types of internal tri-channel connection zirconia abutments and to assess their mode of failure. MATERIAL AND METHODS: The study investigated 3 groups (n=20) of zirconia implant abutments with different implant-abutment interfaces. Group AllZr consisted entirely of zirconia (Aadva CAD/CAM Zirconia Abutment), group FrZr of a titanium insert friction-fitted to the zirconia abutment component (NobelProcera Abutment Zirconia), and group BondZr of a titanium insert bonded to the zirconia abutment component (Lava Zirconia abutment). All the abutments were thermal cycled for 20 000 cycles between 5°C and 55°C. Sixty test implants made of titanium (Dummy NobelReplace) were embedded in autopolymerizing acrylic resin, and 60 zirconia copings (Lava Zirconia) with a uniform thickness of 2.0 mm were fabricated and bonded to the abutments. A universal testing machine was used to statically load all the specimens at a crosshead speed of 1 mm/min. The maximum load was recorded and used as the failure load. The fractured specimens were collected and representative specimens were studied with a stereomicroscope and scanning electron microscope (SEM). One-way ANOVA and post hoc comparisons with the Tukey HSD tests were used for statistical analysis (α=.05). RESULTS: The mean (SD) maximum load capacity was 484.6 (56.6) N for NobelProcera, 503.9 (46.3) N for Aadva, and 729.2 (35.9) N for Lava abutments. The maximum load capacity of Lava abutments was significantly higher than that of Aadva or NobelProcera (P< 05). No significant difference between Aadva and NobelProcera abutments was noted. The mode of failure among the Aadva, NobelProcera, and Lava abutments was different. CONCLUSIONS: With standard diameter internal tri-channel connection implants, the maximum load capacity of the Lava abutment was significantly higher than that of the Aadva or NobelProcera abutment. No significant difference in maximum load capacity was noted between Aadva and NobelProcera abutments. However, the fracture behavior of all 3 abutments was different.

OBJECTIVES: The aim of this clinical study was to determine the effectiveness of progressive loading procedures on preserving crestal bone height and improving peri-implant bone density around maxillary implants restored with single premolar crowns by an accurate longitudinal radiographic assessment technique. MATERIALS AND METHODS: Twenty-three HA-coated, endosseous dental implants were placed in 20 subjects and permitted to heal for 5 months before surgical uncovering. The implants were randomly assigned to either an experimental or control group. Following a conventional healing period, the control group implants were restored with a metal ceramic crown and the experimental group implants underwent a progressive loading protocol. The experimental group was progressively loaded by increasing the height of the occlusal table in increments from a state of infraocclusion to full occlusion by adding acrylic resin to a heat-processed acrylic crown. The progressively loaded crowns were placed in infraocclusion for the first 2 months, light occlusion for the second 2 months, and full occlusion for the third 2 months. At this point, a metal ceramic crown replaced the acrylic crown. Standardized radiographs of each implant were made at the time of restoration, then after 2, 4, 6, 9, and 12 months of function. Digital image analysis and digital subtraction radiography were used to measure changes in crestal bone height and peri-implant bone density. RESULTS: The mean values of crestal bone height loss at 12 months were 0.2+/-0.27 mm for the progressively loaded implants and 0.59+/-0.27 for the conventionally loaded implants, and when tested with repeated-measure ANOVA across the time periods, the differences were statistically significant (P< or =0.05). The progressively loaded group showed a trend for higher bone density gain in the crestal area than the conventionally loaded group, but the conventionally loaded group showed a trend for higher bone density gain at the apex of the implants. CONCLUSION: The peri-implant bone around progressively loaded implants demonstrates less crestal bone loss than the bone around implants placed conventionally into full function. The peri-implant density measurements of the progressively loaded implants show continuous increase in peri-implant bone density by time.
