RESUMO

The aims of this study were to evaluate the impact of the COVID-19 pandemic on emergency and elective hand surgery in four Italian regions that had either a high (Lombardy and Piemonte) or a low (Sicilia and Puglia) COVID-19 case load to discuss problems and to elaborate strategies to improve treatment pathways. A panel of hand surgeons from these different regions compared and discussed data from the centers they work in. The COVID-19 pandemic had an enormous impact on both elective and emergency surgery in Italy, not only in highly affected regions but also - and paradoxically even at a higher extent - in regions with a low COVID-19 case load. A durable and flexible redesign of hand surgery activities should be promoted, while changing and hopefully increasing human resources and enhancing administrative support. Telematics must also be implemented, especially for delivering rehabilitation therapy.

The present study describes the preparation of 50.3 nm superparamagnetic nanosorbents with high surface area for the adsorptive removal of lead and methyl orange from water. This material is based on the surface modification of iron oxide superparamagnetic nanoparticles with a double-shell coating of mesoporous silica whose porosity was increased up to 570 m2/g by the addition of a porogenic material and its calcination. The adsorptive performance of the nanosorbent was evaluated as a function of several parameters (e.g. solution pH, pollutant initial concentration, and contact time), concluding that pHs around 5 are needed to avoid precipitation of Pb2+ as Pb(OH)2 and the equilibrium adsorption capacity is reached after 2 h in all cases. The experimental data on the adsorption capacity of lead and methyl orange onto the nanosorbent were fit to a pseudo-second order kinetic model and Langmuir isotherm model. The maximum adsorption capacity value increases from 35 up to 50 mg/gNS for lead removal with increasing nanosorbent surface area. Contrary, for methyl orange the maximum adsorption goes up to 240 mg/gNS, indicating a larger nanosorbent surface affinity for the organic matter that is able to diffuse through the silica pores as probed by the intraparticle diffusion model. In addition, we found an good reusability (100% recovering after 4 sorption/desorption cycles for methyl orange removal), which makes of this magnetic nanosorbent suitable for remediation technologies.

In this study, a global metabolite profile using Raman spectroscopy analysis was obtained in order to predict, by an in silico prediction of activity spectra for substance approach, the bioactivities of the intracellular content (IC) and cell wall (CW) fractions obtained from Lactobacillus casei CRL 431 and Bacillus coagulans GBI-30 strains. Additionally, multifunctional in vitro bioactivity of IC and CW fractions was also assessed. The metabolite profile revealed a variety of compounds (fatty acids, amino acids, coenzyme, protein, amino sugars), with significant probable activities (Pa > 0.7) as immune-stimulant, anti-inflammatory, neuroprotective, antiproliferative, immunomodulator, and antineoplastic, among others. Moreover, in vitro assays exhibited that both IC and CW fractions presented angiotensin-converting enzyme-inhibitory (> 90%), chelating (> 79%), and antioxidant (ca. 22-57 cellular antioxidant activity units) activities. Our findings based on in silico and in vitro analyses suggest that L. casei CRL 431 and B. coagulans GBI-30 strains appear to be promising sources of postbiotics and may impart health benefits by their multifunctional properties.

Milk-derived bioactive peptides with a single activity (e.g., antioxidant, immunomodulatory, or antimicrobial) have been previously well documented; however, few studies describe multifunctional bioactive peptides, which may be preferred over single-activity peptides, as they can simultaneously trigger, modulate, or inhibit multiple physiological pathways. Hence, the aim of this study was to assess the anti-inflammatory, antihemolytic, antioxidant, antimutagenic, and antimicrobial activities of crude extracts (CE) and peptide fractions (<3 and 3-10 kDa) obtained from fermented milks with specific Lactobacillus plantarum strains. Overall, CE showed higher activity than both peptide fractions (<3 and 3-10 kDa) in most of the activities assessed. Furthermore, activity of <3 kDa was generally higher, or at least equal, to the 3 to 10 kDa peptide fractions. In particular, L. plantarum 55 crude extract or their fractions showed the higher anti-inflammatory (723.68-1,759.43µg/mL of diclofenac sodium equivalents), antihemolytic (36.65-74.45% of inhibition), and antioxidant activity [282.8-362.3µmol of Trolox (Sigma-Aldrich, St. Louis, MO) equivalents]. These results provide valuable evidence of multifunctional role of peptides derived of fermented milk by the action of specific L. plantarum strains. Thus, they may be considered for the development of biotechnological products to be used to reduce the risk of disease or to enhance a certain physiological function.

RESUMO

En estudio doble ciego se valoro el efecto analgesico de la combinacion fija mesilato de dihidroergotamina 1 mg, paracetamol 450 mg y cafeina 40 mg, en comparacion con placebo. Se incluyeron en el estudio 50 pacientes ambulatorios de ambos sexos con cefalea de origen tensional, traumatico o vascular con componente psicogeno o migranoso de localizacion e intensidad variables. La intensidad del dolor descendio en forma significativa (p < 0.001) con el preparado en estudio, lo que no sucedio con el placebo. La duracion del efecto terapeutico fue en aumento, alcanzando al final del estudio 6 horas de duracion: el porcentaje de recurrencia del dolor fue menor a los 5 dias de tratamiento que el que se registro al principio de este. Finalmente, la tolerancia fue excelente, hubo 2 casos de mareo y otros tantos de somnolencia, que por su caracter leve, no obligaron a discontinuar el tratamiento.No se observo efecto de rebote o farmacodependencia
