RESUMO

Continuous-time Markov chains are frequently used as stochastic models for chemical reaction networks, especially in the growing field of systems biology. A fundamental problem for these Stochastic Chemical Reaction Networks (SCRNs) is to understand the dependence of the stochastic behavior of these systems on the chemical reaction rate parameters. Towards solving this problem, in this paper we develop theoretical tools called comparison theorems that provide stochastic ordering results for SCRNs. These theorems give sufficient conditions for monotonic dependence on parameters in these network models, which allow us to obtain, under suitable conditions, information about transient and steady-state behavior. These theorems exploit structural properties of SCRNs, beyond those of general continuous-time Markov chains. Furthermore, we derive two theorems to compare stationary distributions and mean first passage times for SCRNs with different parameter values, or with the same parameters and different initial conditions. These tools are developed for SCRNs taking values in a generic (finite or countably infinite) state space and can also be applied for non-mass-action kinetics models. When propensity functions are bounded, our method of proof gives an explicit method for coupling two comparable SCRNs, which can be used to simultaneously simulate their sample paths in a comparable manner. We illustrate our results with applications to models of enzymatic kinetics and epigenetic regulation by chromatin modifications.

RESUMO

The problem of fraud in China's health insurance has existed for a long time and is becoming more serious, which needs to be solved urgently. This article constructs a tripartite evolutionary game model to study the formation mechanism of the Health Insurance fraud, game participants including medical administrative organization (MAO, a government department responsible for health insurance supervision), medical institutions (MI, such as hospitals), and insured individuals (II, who participating in medical and healthcare insurance). By analyzing the equilibrium of the tripartite evolutionary game, this paper makes an in-depth study on the formation and resolution of health insurance fraud. The results show that: (1) How to prevent the fraud behavior of the medical institutions is the difficulty and core of the problem. It is necessary to achieve effective supervision of the MAO, improve the internal management of the MI and give play to the supervisory role of the II. (2) The regulatory behavior of the MAO needs to focus on protecting the interests of the II, not only to encourage them to actively play the role of supervision and reporting but also to prevent their collusion with MI. (3) On the one hand, the MAO needs to strengthen supervision and increase the punishment for fraud. On the other hand, they also need to take incentive measures to guide all subjects to form a sound internal management mechanism.

RESUMO

In order to understand the levels of indoor air pollution and health risks in public places, our research group conducted air quality monitoring and human health risk assessments for five types of public places (offices, classrooms, laboratories, banks, and hospitals) in Xi'an City from December 2017 to July 2020. The test items included formaldehyde, benzene, toluene, xylene, n-butyl acetate, ethylbenzene, styrene, n-undecane, and total volatile organic compounds (TVOC). The results showed that formaldehyde had the highest exceedance rate (59.4%), followed by toluene, TVOC, benzene, and xylene. Among the five types of public places, hospitals had the highest rate of pollutants exceeding the standard (46.7%), and the main pollutants exceeding the standard were formaldehyde, benzene, and toluene. The results showed that the concentrations of formaldehyde and TVOC were positively correlated with temperature and humidity. The health risk assessment results showed that there were carcinogenic risks of formaldehyde and benzene in different places; people working in banks had a higher risk of formaldehyde carcinogenesis, and those working in hospitals had a higher risk of benzene carcinogenesis. This study provides a reference for the level of indoor air pollution in public places in Xi'an City, and is of great significance to the health risk research of related populations.

RESUMO

BACKGROUND: Ideal cardiovascular health (CVH) is associated with a lower risk of cardiovascular disease and freedom from coronary artery calcium (CAC). Prospective data on the association between maintenance of optimal CVH and the progression of subclinical coronary atherosclerosis are limited. We assessed the influence of unfavorable versus favorable CVH on the incidence of CAC progression. METHODS AND RESULTS: The study population consisted of 1119 FHS (Framingham Heart Study) participants who attended the serial FHS MDCT I and MDCT II study (Multi-Detector Computed Tomography) and had a zero Agatston CAC score at baseline. CVH status was defined using 6 CVH metrics from the American Heart Association definition. CAC progression was defined by an increase in Agatston CAC score to ≥3.4. Generalized estimating equations were applied to identify significant associations of CAC progression with both the baseline measurement of CVH and the longitudinal maintenance of CVH. After follow-up (mean, 6.1 years), we observed CAC progression in 191 participants (17.1%). Participants with unfavorable CVH at baseline had a greater risk of CAC progression (odds ratio, 2.43; 95% confidence interval, 1.40-4.23; P=0.0017). In addition, each unit decrease in ideal CVH metric was associated with an increase in CAC progression (odds ratio, 1.15; 95% confidence interval, 0.99-1.34; P=0.067), after adjustment for baseline ideal CVH metrics. CONCLUSIONS: Significant associations between an unfavorable CVH profile and CAC progression support public health measures that seek to prevent cardiovascular disease by promoting favorable CVH profiles in persons free of clinical and subclinical cardiovascular disease.

RESUMO

PURPOSE: To explore the value of in-line phase-contrast imaging with computed tomography (ILPCI-CT) by synchrotron radiation (SR) for liver fibrosis. MATERIALS AND METHODS: Liver fibrosis models were set up in 13 BALB/c mice by peritoneal injections of thioacetamide and evaluated by ILPCI-CT. Histological staging was used to categorize liver fibrosis into normal, mild fibrosis and advanced fibrosis groups. Microvessel density (MVD), the ratio of total vessel length to volume (L/V), the ratio of total number of branching points to liver volume (P/V) and the distribution of vessel diameter were assessed. RESULTS: The CT images showed slightly high-density shadows around the portal tracts in the fibrosis group. Three-dimensional reconstruction can detect vascular and nodular changes on the surface of fibrotic livers. The MVDs between the three groups were significantly different (P = 0.024). L/V was significantly different between the three groups (P = 0.014). There was a positive correlation between MVD and P/V. CONCLUSION: Fibrous material can be detected by ILPCI-CT even in the early stage of fibrosis. MVD, L/V, P/V and the distribution of vessel diameter were consistent with fibrosis-related angiogenesis progress. Three-dimensional reconstruction is a promising method to visualize morphological changes of the fibrotic liver. KEY POINTS: • ILPCI-CT can detect fibrous material even in the early stage of liver fibrosis. • MVD, L/V, P/V, and the distribution of vascular diameter reflect pathological angiogenesis. • 3D reconstruction could be a promising approach for detecting liver fibrosis.

RESUMO

The concentrations of toxic metals As, Co, Cr, Cu, Mn, Ni, Pb, V and Zn in street dust of Tongchuan, China were determined by wavelength dispersive X-ray fluorescence spectrometry. The risk of the analyzed metals to urban ecosystem and human health were evaluated by potential ecological risk index and human exposure model, respectively. The results show that, in comparison with Shaanxi soil, dust samples have elevated metal concentration as a whole expect for As, Mn, V and Ni. The assessment results of ecological risk indicate that the ecological risks of As, Cr, Mn, Ni, Cu, V and Zn in the dust were in the low level, while Pb and Co presented low to moderate level. Health risk assessment shows that ingestion was the main exposure route of all analyzed toxic metals in street dust to children and adults. The non-cancer risks of the studied metals to children and adults were within the safe range, and the cancer risks of As, Co, Cr and Ni were also within the currently acceptable range.

RESUMO

OBJECTIVE: To report in African Americans with type 1 diabetes the association of single-nucleotide polymorphisms in 193 candidate genes with diabetic retinopathy (DR) and/or its progression. METHODS: A custom panel of 1536 single-nucleotide polymorphisms located on 193 candidate genes for DR was genotyped in 437 African Americans with type 1 diabetes who participated in the New Jersey 725 study. Clinical evaluations at baseline and follow-up examinations included structured clinical interview, ocular examination, 7-field stereoscopic fundus photographs, and blood pressure measurements. Severity of DR was determined via masked grading of fundus photographs. Biological evaluations included blood and urine assays. RESULTS: Single-nucleotide polymorphisms in 13 candidate genes for DR involved in glucose metabolism, angiogenesis, inflammation, neurotransmission, hypertension, and retinal development were significantly associated with the prevalence of severe DR. Three of these genes were also significantly associated with progression of DR. Adjusting for sex, duration of diabetes, glycosylated hemoglobin, systemic hypertension, and total cholesterol did not alter the results. CONCLUSIONS: Our data support the role of genetic factors to account for severity and/or progression of DR in African Americans with type 1 diabetes and to identify several prime genes that likely contribute to the risk of DR.

RESUMO

Fly ash was investigated as a raw material for the preparation of a composite coagulant with sulfuric acid. Types of acid solution, H2SO4 concentration, ratio of H2SO4 to fly ash and stirring time were respectively examined as factors that influenced the efficiency of converting the iron and aluminum components into a composite coagulant and coagulation performance on domestic wastewater. The coagulant was attained at the condition of H2SO4-fly ash ratio of 5 mL/g, H2SO4 of 2 mol/L, stirring time of 4h and stabling time of 30 min, and contained Fe3+ of 0.010 8 mol/L with conversion efficiency of 11.4% and Al3+ of 0.035 4 mol/L with conversion efficiency of 4.3%. Removal efficiencies of COD and SS by this type of coagulant reached 70.4% and 91.9% respectively when treating domestic wastewater. This study provides a promising means to utilize fly ash for coagulation, which possibly makes wastewater treatment more economical and more sustainable.

RESUMO

Along with the aging of population, dementia became a social problem disturbing our life. Western researches focused on caregivers found that, the more severe the dementiais, the more burdens on caregivers. Furthermore, considening the different relationship to the patient, different culture background and different types of dementia, there are different problems in care. Based on comprehensive assessments on the demented and the caregiver, specific health education and other assistant services may improve both the mental and physical health of the caregivers, and reduce the disease burden due to dementia.
