RESUMO

PURPOSE: To characterize the non-laminar flow dynamics and resultant decreased wall shear stress (WSS) and high oscillatory shear index (OSI) of the infrarenal abdominal aortic dilatation, cardiac phase-resolved 3D phase-contrast MRI (4D-flow MRI) was performed. METHODS: The prospective single-arm study was approved by the Institutional Review Board and included 18 subjects (median 67.5 years) with the dilated infrarenal aorta (median diameter 35 mm). 4D-flow MRI was conducted on a 1.5T MRI system. On 3D streamline images, laminar and non-laminar (i.e., vortex or helical) flow patterns were visually assessed both for the dilated aorta and for the undilated upstream aorta. Cardiac phase-resolved flow velocities, WSS and OSI, were also measured for the dilated aorta and the upstream undilated aorta. RESULTS: Non-laminar flow represented by vortex or helical flow was more frequent and overt in the dilated aorta than in the undilated upstream aorta (P < 0.0156) with a very good interobserver agreement (weighted kappa: 0.82-1.0). The WSS was lower, and the OSI was higher on the dilated aortic wall compared with the proximal undilated segments. In mid-systole, mean spatially-averaged WSS was 0.20 ± 0.016 Pa for the dilated aorta vs. 0.68 ± 0.071 Pa for undilated upstream aorta (P < 0.0001), and OSI on the dilated aortic wall was 0.093 ± 0.010 vs. 0.041 ± 0.0089 (P = 0.013). The maximum values and the amplitudes of the WSS at the dilated aorta were inversely proportional to the ratio of dilated/undilated aortic diameter (r = -0.694, P = 0.0014). CONCLUSION: 4D-flow can characterize abnormal non-laminar flow dynamics within the dilated aorta in vivo. The wall of the infrarenal aortic dilatation is continuously and increasingly affected by atherogenic stimuli due to the flow disturbances represented by vortex or helical flow, which is reflected by lower WSS and higher OSI.

PURPOSE: To evaluate the utility of ring-type dedicated breast positron emission tomography (dbPET) for the detection of the residual tumor after neoadjuvant chemotherapy (NAC). MATERIALS AND METHODS: This prospective study included 27 women with histologically proven breast cancer over a 37-month period. All patients underwent ring-type dbPET followed by whole-body PET-CT (WBPET) for preoperative tumor evaluation and re-staging after NAC. The maximum standardized uptake value (SUVmax) of the tumor lesion and the degree of confidence for the presence of the residual tumor were compared between pathological complete response (pCR) and non-pCR tumors. The sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) for the detection of a non-pCR tumor were compared between dbPET and WBPET. RESULTS: On dbPET, SUVmax was significantly higher in non-pCR than in pCR tumors (P = 0.030). The sensitivity for the detection of a non-pCR tumor was significantly higher with dbPET than with WBPET (84.2% vs 26.3%, P = 0.001). In the qualitative analysis, the sensitivity for the detection of a non-pCR tumor was also significantly higher with dbPET than with WBPET (57.9% vs 21.1%, P = 0.016). CONCLUSION: The dbPET can provide more sensitive detection of residual tumor after NAC than can WBPET.

PURPOSE: The objective of the study was to evaluate the cost-effectiveness of gadoxetic acid-enhanced magnetic resonance imaging (EOB-MRI) in the diagnosis and treatment of hepatocellular carcinoma (HCC) in Japan compared with extracellular contrast media-enhanced MRI (ECCM-MRI) and contrast media-enhanced computed tomography (CE-CT) scanning. METHODS: A 6-stage Markov model was developed to estimate lifetime direct costs and clinical outcomes associated with EOB-MRI. Diagnostic sensitivity and specificity, along with clinical data on HCC survival, recurrence, treatment patterns, costs, and health state utility values, were derived from predominantly Japanese publications. Parameters unavailable from publications were estimated in a Delphi panel of Japanese clinical experts who also confirmed the structure and overall approach of the model. Sensitivity analyses, including one-way, probabilistic, and scenario analyses, were conducted to account for uncertainty in the results. FINDINGS: Over a lifetime horizon, EOB-MRI was associated with lower direct costs (¥2,174,869) and generated a greater number of quality-adjusted life years (QALYs) (9.502) than either ECCM-MRI (¥2,365,421, 9.303 QALYs) or CE-CT (¥2,482,608, 9.215 QALYs). EOB-MRI was superior to the other diagnostic strategies considered, and this finding was robust over sensitivity and scenario analyses. A majority of the direct costs associated with HCC in Japan were found to be costs of treatment. The model results revealed the superior cost-effectiveness of the EOB-MRI diagnostic strategy compared with ECCM-MRI and CE-CT. IMPLICATIONS: EOB-MRI could be the first-choice imaging modality for medical care of HCC among patients with hepatitis or liver cirrhosis in Japan. Widespread implementation of EOB-MRI could reduce health care expenditures, particularly downstream treatment costs, associated with HCC.

PURPOSE: To evaluate the feasibility of diffusion kurtosis (DK) imaging of the pancreas for the assessment of hemoglobin (Hb) A1c values. MATERIALS AND METHODS: Our Institutional Review Board approved this prospective study and written informed consent was obtained. In all, 102 consecutive patients with suspected pancreatic disease underwent magnetic resonance imaging (MRI), including DK imaging. Patients were classified into three groups according to American Diabetes Association criteria: HbA1c < 5.7% (group 1), 5.7% ≤ HbA1c < 6.5% (group 2), and HbA1c ≥ 6.5% (group 3). Mean kurtosis (MK) and apparent diffusion coefficient (ADC) of pancreatic parenchyma were computed. MRI measurements and HbA1c values were then compared. RESULTS: HbA1c values positively correlated with MK (r = 0.66, P < 0.0001). Group 3 was significantly (P < 0.05) higher (P < 0.05) in MK than groups 1 and 2. The sensitivity, specificity, and area under the ROC curve of the MK for the detection of group 3 were 90%, 88%, and 0.92, respectively. CONCLUSION: The MK measurement on DK imaging of the pancreas could be a potential biomarker for assessing HbA1c level.

PURPOSE: To evaluate whether a hepatic fibrosis index (HFI), quantified on the basis of hepatic contour abnormality, is a risk factor for the development of hepatocellular carcinoma (HCC) in patients with chronic hepatitis C. MATERIALS AND METHODS: Our institutional review board approved this retrospective study and written informed consent was waved. During a 14-month period, consecutive 98 patients with chronic hepatitis C who had no medical history of HCC treatment (56 men and 42 women; mean age, 70.7 years; range, 48-91 years) were included in this study. Gadoxetic acid-enhanced hepatocyte specific phase was used to detect and analyze hepatic contour abnormality. Hepatic contour abnormality was quantified and converted to HFI using in-house proto-type software. We compared HFI between patients with (n=54) and without HCC (n=44). Serum levels of albumin, total bilirubin, aspartate transferase, alanine transferase, percent prothrombin time, platelet count, alpha-fetoprotein, protein induced by vitamin K absence-II, and HFI were tested as possible risk factors for the development of HCC by determining the odds ratio with logistic regression analysis. RESULTS: HFIs were significantly higher in patients with HCC (0.58±0.86) than those without (0.36±0.11) (P<0.001). Logistic analysis revealed that only HFI was a significant risk factor for HCC development with an odds ratio (95% confidence interval) of 26.4 (9.0-77.8) using a cutoff value of 0.395. CONCLUSION: The hepatic fibrosis index, generated using a computer-aided assessment of hepatic contour abnormality, may be a useful imaging biomarker for the prediction of HCC development in patients with chronic hepatitis C.
