BACKGROUND: Soil salinity is one of the major menaces to food security, particularly in dealing with the food demand of the ever-increasing global population. Production of cereal crops such as wheat is severely affected by soil salinity and improper fertilization. The present study aimed to examine the effect of selected microbes and poultry manure (PM) on seedling emergence, physiology, nutrient uptake, and growth of wheat in saline soil. A pot experiment was carried out in research area of Institute of Soil and Environmental Sciences, University of Agriculture, Faisalabad, Pakistan. Saline soil (12 dS m- 1 w/w) was developed by spiking using sodium chloride, and used in experiment along with two microbial strains (i.e., Alcaligenes faecalis MH-2 and Achromobacter denitrificans MH-6) and PM. Finally, wheat seeds (variety Akbar-2019) were sown in amended and unamended soil, and pots were placed following a completely randomized design. The wheat crop was harvested after 140 days of sowing. RESULTS: The results showed a 10-39% increase (compared to non-saline control) in agronomic, physiological, and nutritive attributes of wheat plants when augmented with PM and microbes. Microbes together with PM significantly enhanced seedling emergence (up to 38%), agronomic (up to 36%), and physiological (up to 33%) in saline soil as compared to their respective unamended control. Moreover, the co-use of microbes and PM also improved soil's physicochemical attributes and enhanced N (i.e., 21.7%-17.1%), P (i.e., 24.1-29.3%), and K (i.e., 28.7%-25.3%) availability to the plant (roots and shoots, respectively). Similarly, the co-use of amendments also lowered the Na+ contents in soil (i.e., up to 62%) as compared to unamended saline control. This is the first study reporting the effects of the co-addition of newly identified salt-tolerant bacterial strains and PM on seedling emergence, physiology, nutrient uptake, and growth of wheat in highly saline soil. CONCLUSION: Our findings suggest that co-using a multi-trait bacterial culture and PM could be an appropriate option for sustainable crop production in salt-affected soil.

Road crashes are a major public safety concern in Pakistan. Prior studies in Pakistan investigated the impact of different factors on road crashes but did not consider the temporal stability of crash data. This means that the recommendations based on these studies are not fully effective, as the impact of certain factors may change over time. To address this gap in the literature, this study aims to identify the factors contributing to crash severity in road crashes and examine how their impact varies over time. In this comprehensive study, we utilized Generalised Linear Model (GLM) on the crash data between the years 2013 to 2017, encompassing a total sample of 802 road crashes occurred on the N-5 road section in Pakistan, a 429-kilometer stretch connecting two big cities of Pakistan, i.e., Peshawar and Lahore. The purpose of the GLM was to quantify the temporal stability of the factors contributing crash severity in each year from 2013 to 2017. Within this dataset, 60% (n = 471) were fatal crashes, while the remaining 40% (n = 321) were non-fatal. The results revealed that the factors including the day of the week, the location of the crashes, weather conditions, causes of the crashes, and the types of vehicles involved, exhibited the temporal instability over time. In summary, our study provides in-depth insights aimed at reducing crash severity and potentially aiding in the development of effective crash mitigation policies in Pakistan and other nations having similar road safety problems. This research holds great promise in exploring the dynamic safety implications of emerging transportation technologies, particularly in the context of the widespread adoption of connected and autonomous vehicles.

This research has been designed to analyze the risk factors of major eye diseases and the genetic alterations contributing to the manifestation of such disease. For this purpose, data was collected from 256 patients diagnosed by an ophthalmologist by using a specialized questionnaire. Blood samples were collected from 100 patients to perform a genetic investigation of cataracts. Whole genomic DNA was extracted from blood samples via the phenol-chloroform method. The purified DNA was used as the template for the amplification of about 400 bp fragments amplifying exons 1 and 2 of the CRYAA gene. The statistical analysis showed that 68% of individuals were blind due to cataracts. During molecular analysis, nucleotide sequences obtained have resulted in one silent mutation that occured at 20 positions in exon 2. It was replacing A>G which in turn substitutes the Lysine at position 70 for Arginine. It was interpreted by statistical analysis that this mutation did not result in a significant change in the CRYAA gene. In addition, protein analysis showed no significant changes in the structure of normal and mutated genes. At last, it is concluded that environmental risk factors play a major role in the studied diseases as compared to genetic factors. It is recommended to extend the study to a larger population to study all exons of the CRYAA gene as well as develop better estimates of the magnitude of the problems of visual loss and eye diseases in the Pakistani population.

The vitality contribution is a vital cause for defensible monetary improvement and collective success by eradicating poverty. Adopting the solar home lighting system (SHLS) is advantageous not only in social lifestyles but also improves the health of family members and increases home-based small businesses activities due to the inexpensive and continuous supply of energy. The main aims of the study are to scrutinize the most substantial barriers to adopting SHLS in Pakistan. A comprehensive, structured questionnaire appraisal was conducted for sample size with the help of non-probability sampling (purposive sampling), and primary data was collected. The designated hypotheses were evaluated using partial least square structural equation modeling (PLS-SEM). In the present study, we validate the model using a sample of 271 adopters of SHLS contributed as respondents. The results disclose that entire autonomous variables expressively and positively correlated with adopting SHLS dipping energy disasters and improving home-based small business activities. Correspondingly, social media-based awareness of SHLS significantly moderates and positively affects the selected factors in this study. Empirical results indicate that prudently eradicating maintenance barriers with experienced professionals, subsidy in prices from the government, quality base satisfaction of owners, and social media-based awareness are the primary tools to adopt SHLS. Additionally, the outcomes offer valuable suggestions to the competent authorities that introduce encouragement and maintenance policy for adopting SHLS.

This study endeavors to investigate the environmental cost of FDI inflows in the Sub-Saharan African (SSA) region over the period of 2006 to 2020. There are two opposing theories about how FDI impacts the environment, namely, the pollution halo hypothesis (PHH) and the pollution haven hypothesis (PH). Given the SSA region's poor environmental performance and potential spatial spillover effects on neighboring nations, the study underlines the necessity to look into the pollution hypotheses in the region. The examination is carried out through non-spatial and spatial panel data econometric approaches. The empirical findings provide evidence that an increase in FDI inflow by 1% in SSA is positively associated with increasing levels of CO2 emissions by 0.03% on average, thus validating the notion of a pollution haven in the region. Furthermore, the study reveals that the environmental spillovers of CO2 emissions are not confined to the host country alone, but also extend to neighboring nations. Other key determinants of CO2 emissions, including GDP, population, and urbanization, were also found to be positively linked to CO2 emissions, whereas the use of renewable energy resources was found to have a mitigating effect. The empirical findings offer valuable insights for policymakers and stakeholders in the SSA region. These insights highlight the importance of the adoption of renewable energy sources and enacting regulatory measures to monitor the environmental cost of FDI, with the aim of mitigating the deleterious consequences of CO2 emissions, not only in the host nation but also in the neighboring nations.

BACKGROUND: Existing data and clinical trials could not determine whether faster intravenous thrombolytic therapy (IVT) translates into better long-term functional outcomes after acute ischemic stroke among those treated with endovascular thrombectomy (EVT). Patient-level national data can provide the required large population to study the associations between earlier IVT, versus later, with longitudinal functional outcomes and mortality in patients receiving IVT+EVT combined treatment. METHODS: This cohort study included older US patients (age ≥65 years) who received IVT within 4.5 hours or EVT within 7 hours after acute ischemic stroke using the linked 2015 to 2018 Get With The Guidelines-Stroke and Medicare database (38 913 treated with IVT only and 3946 with IVT+EVT). Primary outcome was home time, a patient-prioritized functional outcome. Secondary outcomes included all-cause mortality in 1 year. Multivariate logistic regression and Cox proportional hazards models were used to evaluate the associations between door-to-needle (DTN) times and outcomes. RESULTS: Among patients treated with IVT+EVT, after adjusting for patient and hospital factors, including onset-to-EVT times, each 15-minute increase in DTN times for IVT was associated with significantly higher odds of zero home time in a year (never discharged to home) (adjusted odds ratio, 1.12 [95% CI, 1.06-1.19]), less home time among those discharged to home (adjusted odds ratio, 0.93 per 1% of 365 days [95% CI, 0.89-0.98]), and higher all-cause mortality (adjusted hazard ratio, 1.07 [95% CI, 1.02-1.11]). These associations were also statistically significant among patients treated with IVT but at a modest degree (adjusted odds ratio, 1.04 for zero home time, 0.96 per 1% home time for those discharged to home, and adjusted hazard ratio 1.03 for mortality). In the secondary analysis where the IVT+EVT group was compared with 3704 patients treated with EVT only, shorter DTN times (≤60, 45, and 30 minutes) achieved incrementally more home time in a year, and more modified Rankin Scale 0 to 2 at discharge (22.3%, 23.4%, and 25.0%, respectively) versus EVT only (16.4%, P<0.001 for each). The benefit dissipated with DTN>60 minutes. CONCLUSIONS: Among older patients with stroke treated with either IVT only or IVT+EVT, shorter DTN times are associated with better long-term functional outcomes and lower mortality. These findings support further efforts to accelerate thrombolytic administration in all eligible patients, including EVT candidates.

OBJECTIVES: To evaluate nationwide pediatric critical care facilities and resources in Pakistan. DESIGN: Cross-sectional observational study. SETTING: Accredited pediatric training facilities in Pakistan. PATIENTS: None. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: A survey was conducted using the Partners in Health 4S (space, staff, stuff, systems) framework, via email or telephone correspondence. We used a scoring system in which each item in our checklist was given a score of 1, if available. Total scores were added up for each component. Additionally, we stratified and analyzed the data between the public and private healthcare sectors. Out of 114 hospitals (accredited for pediatric training), 76 (67%) responded. Fifty-three (70%) of these hospitals had a PICU, with a total of 667 specialized beds and 217 mechanical ventilators. There were 38 (72%) public hospitals and 15 (28%) private hospitals. There were 20 trained intensivists in 16 of 53 PICUs (30%), while 25 of 53 PICUs (47%) had a nurse-patient ratio less than 1:3. Overall, private hospitals were better resourced in many domains of our four Partners in Health framework. The Stuff component scored more than the other three components using analysis of variance testing ( p = 0.003). On cluster analysis, private hospitals ranked higher in Space and Stuff, along with the overall scoring. CONCLUSIONS: There is a general lack of resources, seen disproportionately in the public sector. The scarcity of qualified intensivists and nursing staff poses a challenge to Pakistan's PICU infrastructure.

BACKGROUND: Modern medical education has flourished with an emergent interest in a variety of assessment approaches that entails direct observation of performance and provision of feedback. Workplace-based assessment (WPBA) has been embraced into specialty training in the United Kingdom (UK) within National Health Service (NHS). In Pakistan, this educational framework is incorporated to gauge the clinical proficiency of trainees at the postgraduate and undergraduate levels. The present study was done to identify how WPBA is perceived by dental field postgraduate trainees in Pakistan. OBJECTIVE: The study explored and ascertained trainees' perceptions, attitudes, and preferences considering their experiences with the systematic organization and execution of WPBA in dental educational settings. METHODS: This cross-sectional study design involved the clinical trainees of various cadres in a tertiary care hospital in Multan. A structured and validated questionnaire previously used for another analysis was administered among 90 trainees at the institute. The results were analyzed and tabulated using SPSS-21. Influential statistics (Pearson's Chi-Square Test) was performed and the confidence interval was set at 95% (P≤0.05). RESULTS: The response rate was 88%. Opinions were more positive compared to the former surveys. The majority of the respondents (65.8%) have shown a positive attitude towards WPBA and preferred it to be a valuable assessment system as it fosters reflective practices in educational settings. In addition, it was shown that WPBA has the potential to improve clinical training (72.2%) and aid in the effective implementation of clinical practice (68.4%). As the system of digital education has been introduced ever since the pandemic, the highest percentage of participants (56%) preferred a combination of on-paper and online assessment systems. CONCLUSION: The study reported that WPBA proposes the opportunity to associate teaching, learning, and assessment. The faculty training program is a significant input to upholding the quality of WPBA. Empirical research on WPBA is essential to be carried out to overcome the inadequacies thus endorsing its application universally.

The current study analyzed whether the enhancement in managerial ability accelerates the environmental, social, and economic sustainability practices or not. Using panel data methodology on Chinese listed firms data from 2010 to 2019, we report that CEOs' managerial ability impacts the overall (environmental, social, and economic) sustainability practices of the firms positively. Moreover, we find that social sustainability and economic sustainability also increase with the increase of the CEO's managerial ability in the firm. The results remain robust after several alternative empirical tests. The findings justify the relationship between management skills and sustainability and demonstrate how each one of the sustainability pillars is affected individually. The support for sustainability practices that can be achieved through the communication of management skills is an essential conclusion for practitioners. Findings establish the link between CEO's managerial ability and environmental, social, and economic sustainability performance by taking insights from upper echelon theory.

Current energy systems face multiple problems related to inflation in energy prices, reduction of fossil fuels, and greenhouse gas emissions which are disturbing the comfort zone of energy consumers and the affordability of power for large commercial customers. These kinds of problems can be alleviated with the help of optimal planning of demand response policies and with distributed generators in the distribution system. The objective of this article is to give a strategic proposition of an energy management system for a campus microgrid (µG) to minimize the operating costs and to increase the self-consuming energy of the green distributed generators (DGs). To this end, a real-time based campus is considered that currently takes provision of its loads from the utility grid only. According to the proposed given scenario, it will contain solar panels and a wind turbine as non-dispatchable DGs while a diesel generator is considered as a dispatchable DG. It also incorporates an energy storage system with optimal sizing of BESS to tackle the multiple disturbances that arise from solar radiation. The resultant problem of linear mathematics was simulated and plotted in MATLAB with mixed-integer linear programming. Simulation results show that the proposed given model of energy management (EMS) minimizes the grid electricity costs by 668.8 CC/day ($) which is 36.6% of savings for the campus microgrid. The economic prognosis for the campus to give an optimum result for the UET Taxila, Campus was also analyzed. The general effect of a medium-sized solar PV installation on carbon emissions and energy consumption costs was also determined. The substantial environmental and economic benefits compared to the present situation have prompted the campus owners to invest in the DGs and to install large-scale energy storage.

The occurrence and persistence of pharmaceuticals in the food chain, particularly edible crops, can adversely affect human and environmental health. In this study, the impacts of the absorption, translocation, accumulation, and degradation of paracetamol in different organs of the leafy vegetable crop spinach (Spinacia oleracea) were assessed under controlled laboratory conditions. Spinach plants were exposed to 50 mg/L, 100 mg/L, and 200 mg/L paracetamol in 20% Hoagland solution at the vegetative phase in a hydroponic system. Exposed plants exhibited pronounced phytotoxic effects during the eight days trial period, with highly significant reductions seen in the plants' morphological parameters. The increasing paracetamol stress levels adversely affected the plants' photosynthetic machinery, altering the chlorophyll fluorescence parameters (Fv/Fm and PSII), photosynthetic pigments (Chl a, Chl b and carotenoid contents), and composition of essential nutrients and elements. The LC-MS results indicated that the spinach organs receiving various paracetamol levels on day four exhibited significant uptake and translocation of the drug from roots to aerial parts, while degradation of the drug was observed after eight days. The VITEK® 2 system identified several bacterial strains (e.g., members of Burkhulderia, Sphingomonas, Pseudomonas, Staphylococcus, Stenotrophomonas and Kocuria) isolated from spinach shoots and roots. These microbes have the potential to biodegrade paracetamol and other organic micro-pollutants. Our findings provide novel insights to mitigate the risks associated with pharmaceutical pollution in the environment and explore the bioremediation potential of edible crops and their associated microbial consortium to remove these pollutants effectively.

In South Asia, the high costs of synthetic fertilizers have imposed research on alternative nutrient inputs. We aimed to identify potential trace elements (PTE) present in some organic manure that might be a source of environmental pollutions and risk to public health following consumption. The study aims to evaluate how different organic waste (poultry waste, PW; press mud, PM; and farmyard manure, FYM) influences the heavy metal migration in soil, the accumulation in garlic, and their potential health risks. Organic waste caused a higher accumulation of certain metals (Zn, Cu, Fe, and Co), whereas Mn, Cd, Cr, and Pb were in lower concentrations in soil. Amendments of soil with PM revealed a higher accumulation of Cd, Cr, Fe, and Pb, whereas PW resulted in Cu and Zn accumulation in garlic. Treatment of soil with FYM exhibited higher metal concentration of Co and Mn. An environmental hazard indices study revealed that pollution load index (PLI) was highest for Cu following treatment with PM. The health risk index (HRI) was greater for Cd following amendment with PM. Maximum daily intake of metals (DIM) was observed for Zn after treatment with PW. The Pb exhibited maximum bioconcentration factor (BCF) in PM-amended plants. Based on these findings, we concluded that garlic grown on contaminated soil with organic waste may pose serious health hazards following consumption.

Trace elements (TEs) contamination of agricultural soils requires suitable criteria for regulating their toxicity limits in soil and food crops, which depends on their potential ecological risk spanning regional to global scales. However, no comprehensive study is available that links TE concentrations in paddy soil with ecological and human health risks in less developed regions like Pakistan. Here we evaluated the data set to establish standard guidelines for defining the hazard levels of various potentially toxic TEs (such as As, Cd, Co, Cu, Cr, Fe, Mn, Ni, Pb, Se, Zn) in agricultural paddy soils of Punjab, Pakistan. In total, 100 topsoils (at 0-15 cm depth) and 204 rice plant (shoot and grain) samples were collected from five ecological zones of Punjab (Gujranwala, Hafizabad, Vehari, Mailsi, and Burewala), representing the major rice growing regions in Pakistan. The degree of contamination (Cd) and potential ecological risk index (PERI) established from ecological risk models were substantially higher in 100% and 97% of samples, respectively. The positive matrix factorization (PMF) model revealed that the elevated TEs concentration, notably Cd, As, Cr, Ni, and Pb, in the agricultural paddy soil was attributed to the anthropogenic activities and groundwater irrigation. Moreover, the concentration of these TEs in rice grains was higher than the FAO/WHO's safe limits. This study provided a baseline, albeit critical knowledge, on the impact of TE-allied ecological and human health risks in the paddy soil-rice system in Pakistan; and it opens new avenues for setting TEs guidelines in agro-ecological zones globally, especially in underdeveloped regions.

Present study was conducted to check the heavy metal content in wheat treated with municipal solid waste, Although municipal solid waste was enriched with organic nutrient, micronutrients, and macro-nutrients, considerable amount of cobalt was also witnessed in municipal solid waste that s why pot experiment was executed. The concentration of cobalt in different parts of wheat (root, shoot, and grain) was analyzed by atomic absorption spectrophotometer (AA-6300 Shimadzu Japan). Highest concentration of cobalt in root, shoot, and grain of wheat was observed in the range of 0.91-1.02 mg/kg, 0.92-1.04 mg/kg, and 0.93-1.00 mg/kg, respectively, under the influence of different fertilizer's used, while in field experiment, level of Co was greater in roots followed by shoots and grain. The metal concentration in wheat grown in field was ranged from 0.67 to 0.72 mg/kg for roots, 0.64 to 0.71 mg/kg for shoots, and 0.66 to 1.71 mg/kg for grains. Concentrations of cobalt were found below the permissible limits suggested by FAO/WHO. Various indices (PLI, BCF, DIM, HRI) were calculated and results showed that PLI was above 1 indicating that metal was causing pollution in treatments while value of BCF, DIM, and HRI was within the permissible range. Higher Co content in wheat may cause damage to the pancreatic cells of animals, cause respiratory problems, and effects their kidney, liver, and lungs, if exposure is for long period through feed.

Diabetes mellitus is a multifaceted metabolic disorder, which often required frequent blood glucose monitoring, poly-pharmacy and timely adjustments for its management. The present study focuses to check the effectiveness of empagliflozin add-on therapy in diabetic patients already taking metformin and glimepiride. This was observational, comparative and follow-up cohort study, conducted in a tertiary care hospital of Pakistan. Ninety subjects were enrolled and evenly distributed in Group A (patients on oral therapy of Metformin & Glimepiride) and Group B (patients on oral therapy of metformin, glimepiride and empagliflozin) randomly. The results showed that the addition of empagliflozin to metformin and glimepiride standard therapy provided better control over blood sugar with a significant decrease in HbA1c (16.1% decrease in HbA1c for Group B patients against 8.2% in Group A patients), FBS (23.8% decrease as compared to 14.6% decrease) and BMI (1.5% decrease in Group B patients against 0.06% increase in Group A). The addition of empagliflozin did not exacerbate the toxicity of the existing regimen and is safe to be included in multiple drug regimens. Empagliflozin addition to standard antidiabetic therapy might possess beneficial impacts in managing poorly controlled Type-2 Diabetes Mellitus in the Pakistani population.

This study investigates the impact of urbanization and nonrenewable energy consumption on carbon emissions. The context of the analysis is 54 African Union countries from 1996 to 2019. For estimation, we use panel quantile regression (PQR) and fully modified ordinary least squares (FMOLS). Our regression results demonstrate that there is a positive correlation between urbanization and CO2 emission. Further, our empirical results confirmed that nonrenewable energy consumption increases environmental pollution in African Union countries. The outcomes demonstrate the EKC hypothesis because at the initial stage of development, when economic growth increases, environmental pollution increases; after a threshold point, environmental pollution decreases as economic growth increases. It can find an inverted U-shaped relationship between economic growth and CO2 emission. The findings also show that urbanization should be planned; otherwise, it can lead to environmental degradation in the long run. Africa continent takes strict action and builds a blueprint for efficient and effective energy production and consumption. The only solution to achieve green growth in Africa is to shift from fossil fuel energy supply to renewable energy supply.

Feeding 9 billion by 2050 is one of major challenges for researchers. Use of diversified crops, nonconventional water resources and rehabilitation of marginal lands are alternate options to produce more food to face climate change projections. Adaptation to climate change through climate smart agriculture practices, agroecology activities, and crop-based management packages can help transform the marginal lands from environmental burdens into productive and economic blocks. This review discusses the recent advancements on specialty group of alternate crops (oil seeds, legumes, cereals, medicinal, lignocellulose, and fruit crops) which can adapt in the marginal environments. Availability of alternate water resources (saline water, treated wastewater) for irrigation cannot be omitted. Crop diversification systems involving drought and salt-tolerant crops are likely to be the key to future agricultural and economic growth in the regions where salt-affected soils exist and/or saline aquifers are pumped for irrigation. These systems may tackle three main tasks: sustainable management of land resources and enhancement of per unit productivity; intensification of agroecological practices to increase soil fertility; and improving productivity of marginal lands for diversified climate smart crops. This review explores various aspects of marginal lands and selection of tolerant crop genotypes, crop diversification, and agroecological practices to maximize benefits.
