RESUMO

The aim of this study was to ascertain farmers' knowledge of the risk of spread of infection from animals to humans, and their transmission prevention practices. This was a survey of farmers who submitted material to Ireland's Regional Veterinary Laboratories in 2015. There was an 84% response rate (1044 farmers). Ninety per cent of farmers were not aware that infection can be acquired from apparently healthy animals. Over half were not aware that disease could be contracted from sick poultry or pets. Conversely, the knowledge of the risk to pregnant women of infection from birthing animals was high (88%). Four-fifths of farmers sourced drinking water from a private well, and of these, 62% tested their water less frequently than once a year. Of dairy farmers, 39% drank unpasteurised milk once a week or more frequently. Veterinarians were the most commonly cited information source for diseases on farms. The survey findings indicate that the level of farmers' knowledge and awareness of the spread of infection from animals to humans is a concern. Further education of the farming community is needed to increase awareness of both the potential biohazards present on farms and the practical measures that can be taken to mitigate the risk of zoonoses.

To investigate the usefulness of culture for the confirmation of brucellosis in cattle, a comparison of culture and serology was undertaken on 248 animals in four dairy herds where the disease was active. Paired supramammary (SM), retropharyngeal (RP), and internal iliac (IL) lymph nodes were cultured, and five serological tests were deployed: the microserum agglutination test (MSAT), complement fixation test (CFT), the indirect (iELISA) and competitive ELISA, and the fluorescence polarisation assay (FPA). Brucella abortus was isolated from 86.8% of animals on combined culture of all three lymph nodes. Individually, the highest isolation rate was from the RP (90.5% of culture positives). Of culture positive animals, 13.7% and 6.2% were positive from the RP and SM alone, respectively. Approximately half of the positive cultures yielded <10 colonies/culture plate. Although 80.9% of animals were positive in at least one serological test, only 45.2% were positive in all five. For culture-positive animals, the MSAT was the most sensitive test (71.8%). Of the culture-negative animals 67.7% were positive in at least one test, while 12.9% were positive in all five. Titres were higher in animals culture-positive from the SM, and there was a direct correlation between higher titres and higher colony counts in SM cultures. Only 8.9% of animals were both culture-negative and seropositive (in at least one test), while 16.5% were culture-positive and seronegative in all five tests. The results highlight and validate the sensitivity of bacteriological culture in confirming a diagnosis of bovine brucellosis. While the MSAT and FPA were the most sensitive serological tests, a significant percentage of infected animals were undetectable using these standard serological assays.

Mortality from asthma in Chicago is among the highest in the nation for 5-34-year-olds. Data for all hospitalizations among Chicago residents less than 35 years of age were examined to define characteristics that may be contributing to morbidity and mortality from asthma. From 1987 through 1989, the average annual age-adjusted hospitalization rate among persons less than 35 years of age in Chicago was 3.57 per 1000 persons. There were significant associations of community hospitalization rate with median income (r = -0.61, p < 0.001) and with proportion of community asthma hospitalizations using Medicaid, Medicare, or self-payment (r = 0.69, p < 0.001). Among 18-34-year-old men, asthma hospitalization rates for Medicaid recipients were 17.4-34.1-fold higher than among men using other forms of insurance. Asthma admissions using Medicaid, Medicare, or self-payment were more likely than those using other forms of insurance to present through the emergency department (79.3% and 66.4%, respectively, p < 0.001) and be discharged against medical advice (1.8% vs. 0.7%, respectively, p < 0.001). These data suggest that differential access to or utilization of health care may be contributing to asthma morbidity in Chicago.

The aim of this study was to report on the average daily energy expenditure by elderly males and females in their normal day-to-day activities. The study was based on an analysis of data collected over six months from daily diary recording of activities. The subjects, 82 males and 93 females, were non-institutionalized volunteers with a mean age of 68 years. A multivariate analysis of variance indicated that there were no differences between males and females in total mean daily energy expended on Sporting activities. Females expended twice as much energy as males on Household Chores (F = 36.3,p < 0.001). Males expended over three times as much energy as females on gardening (F = 56.7, p < 0.001) and over seven times as much on Maintenance (F = 53.6, p < 0.001). There were also differences between males and females on energy expended while Walking (F = 6.1, p < 0.02) and on Miscellaneous activities (F = 6.3, p<0.05). It is concluded that sex stereotypical roles for activities required at younger ages are continued into old age. The implications of these findings for prescribing regular exercise in the elderly are discussed, indicating specific activities that might prove to be beneficial.

To study cross-cultural differences in perceived health problems in the elderly the Nottingham Health Profile (NHP) developed by Hunt et al. was administered to subjects from the People's Republic of China and Australia. The Australian stratum was further categorized according to cardiovascular status. Analyses of covariance (with age as the covariate) on each of the six subscales of the NHP yielded significant differences for 'Energy', 'Pain', 'Emotional reactions', 'Social isolation' and 'Physical mobility'. No differences were found for the 'Sleep' subscale. Through comparisons between the mean scores for the four strata and from normative data it is concluded that it is likely that the NHP is 'culture free' on the dimensions 'Energy', 'Pain', 'Emotional reactions', 'Social isolation' and 'Physical mobility'.
