RESUMO

Knowledge, attitudes and practices of 40 randomly selected physicians was assessed in the North Sharqiya region of Oman in 2003. We evaluated non-ophthalmologist physicians' knowledge of techniques of eye examination of diabetic patients, attitudes towards fundus examination and practices of detailed eye examination. Knowledge about different parts of the eye was satisfactory in only 58% of physicians and knowledge about method of fundus examination for diabetic retinopathy was poor in 40%. Attitudes towards eye examination by non-ophthalmologists at primary level were positive. In practice 20 physicians had attempted to use an ophthalmoscope and only 9 could see details of the retina. Our general physicians would need detailed training if they are to be involved in early detection of diabetic retinopathy.

This community-based study was carried out to estimate the prevalence and identify the determinants of glaucoma in the Omani population in 2005-06. Residents of 75 randomly selected houses in 25 clusters participated in the survey. We interviewed and examined 3324 individuals aged > or = 30 years in their homes and in eye clinics to note personal details, glaucoma status and presence of selected risk factors: 180 were diagnosed as having glaucoma. Projected prevalence of glaucoma among the Omani population > or = 30 years was 4.75%. Open angle and angle closure types of glaucoma contributed 40.6% and 37.8% respectively. History of hypertension was negatively, and older age and family history of glaucoma were positively, associated with glaucoma.

RESUMO

We carried out a retrospective analytical study of 3525 bilaterally blind people registered in Oman to the end of 2000. We described the examination method, maintenance of the register and analysis of data. Lists of different categories of blind people were distributed to various organizations for management and rehabilitation purposes. The role of the national register in addressing blinding cataract, corneal blindness, eradication of congenital rubella syndrome and care of blind people in Oman was also examined. The register was useful in identifying candidates for rehabilitation and also for monitoring regional efforts to manage curable blindness. This multiple use of the register could make it an important tool for health programme management in Oman.

Audiometric screening was conducted in Dhofar region to study the magnitude of ear problems and cost-effectiveness of screening first-year preparatory-school children in Oman. None of the 1894 pupils had otitis media with effusion or sensory neuronal hearing loss. Six children (0.32%) had impacted wax, 4 (0.21%) chronic suppurative otitis media and 2 (0.11%) dry perforation of eardrum. In all, 14 children (0.74%) with suspected hearing impairment were referred to a specialist but only 2 attended. Physicians and nurses spent 8-10 minutes for ear examination per child for a yield of less than 1%. The screening expenditure was US$ 5 per pupil. As the prevalence of serious ear conditions was low, we conclude that expanding the audiometric screening of schoolchildren to first-year preparatory pupils is not cost-effective.

Population exposures to toxic trace metals are of great concern due to their nonbiodegradable nature and long biological half-lives for elimination from the body. Response to a toxic metal varies with age group; children are more sensitive and hence more at risk than others. The present study was therefore undertaken on 6- to 10-year-old children residing in various localities of Greater Mumbai and Thane. Blood samples from 566 children residing in 13 locations in Mumbai along with 410 air particulate samples and 64 "duplicate diet" samples were collected for this study. Levels of Pb, Cd, Cu, and Zn in these samples were estimated by differential pulse anodic stripping voltammetric technique. Intake of Pb, Cd, Cu, and Zn for 6- to 10-year-old children through ingestion and inhalation pathways have also been assessed. A correlation coefficient of 0.88 is observed between air lead and blood lead. It is also seen that every microgram increase in the Pb concentration in air (m-3) results in 3.56 microg increase in the blood Pb concentration (dl-1) in children. Similar correlation, however, was not observed in cases of Cd, Cu, and Zn.
