RESUMO

OBJECTIVES: To assess the resectability of pancreatic ductal adenocarcinoma (PDAC), the evaluation of tumor vascular contact holds paramount significance. This study aimed to compare the image quality and diagnostic performance of high-resolution (HR) pancreas computed tomography (CT) using an 80 kVp tube voltage and a thin slice (1 mm) for assessing PDAC resectability, in comparison with the standard protocol CT using 120 kVp. METHODS: This research constitutes a secondary analysis originating from a multicenter prospective study. All participants underwent both the standard protocol pancreas CT using 120 kVp with 3 mm slice thickness (ST) and HR-CT utilizing an 80 kVp tube voltage and 1 mm ST. The contrast-to-noise ratio (CNR) between parenchyma and tumor, along with the degree of enhancement of the abdominal aorta and main portal vein (MPV), were measured and subsequently compared. Additionally, the likelihood of margin-negative resection (R0) was evaluated using a five-point scale. The diagnostic performance of both CT protocols in predicting R0 resection was assessed through the area under the receiver operating characteristic curve (AUC). RESULTS: A total of 69 patients (37 males and 32 females; median age, 66.5 years) were included in the study. The median CNR of PDAC was 10.4 in HR-CT, which was significantly higher than the 7.1 in the standard CT (P=0.006). Furthermore, HR-CT demonstrated notably higher median attenuation values for both the abdominal aorta (579.5 HU vs. 327.2 HU; P=0.002) and the MPV (263.0 HU vs. 175.6 HU; P=0.004) in comparison with standard CT. Following surgery, R0 resection was achieved in 51 patients. The pooled AUC for HR-CT in predicting R0 resection was 0.727, slightly exceeding the 0.699 of standard CT, albeit lacking a significant statistical distinction (P=0.128). CONCLUSION: While HR pancreas CT using 80 kVp offered a notably greater degree of contrast enhancement in vessels and a higher CNR for PDAC compared to standard CT, its diagnostic performance in predicting R0 resection remained statistically comparable.

RESUMO

As an alternative to high-cost shoe insole pressure sensors that measure the insole pressure distribution and calculate the center of pressure (CoP), researchers developed a foot sensor with FSR sensors on the bottom of the insole. However, the calculations for the center of pressure and ground reaction force (GRF) were not sufficiently accurate because of the fundamental limitations, fixed coordinates and narrow sensing areas, which cannot cover the whole insole. To address these issues, in this paper, we describe an algorithm of virtual forces and corresponding coordinates with an artificial neural network (ANN) for low-cost flexible insole pressure measurement sensors. The proposed algorithm estimates the magnitude of the GRF and the location of the foot plantar CoP. To compose the algorithm, we divided the insole area into six areas and created six virtual forces and the corresponding coordinates. We used the ANN algorithm with the input of magnitudes of FSR sensors, 1st and 2nd derivatives of them to estimate the virtual forces and coordinates. Eight healthy males were selected for data acquisition. They performed an experiment composed of the following motions: standing with weight shifting, walking with 1 km/h and 2 km/h, squatting and getting up from a sitting position to a standing position. The ANN for estimating virtual forces and corresponding coordinates was fitted according to those data, converted to c script, and downloaded to a microcontroller for validation experiments in real time. The results showed an average RMSE the whole experiment of 31.154 N for GRF estimation and 8.07 mm for CoP calibration. The correlation coefficients of the algorithm were 0.94 for GRF, 0.92 and 0.76 for the X and Y coordinate respectively.

RESUMO

An extensive and year-round survey was conducted to assess metal pollution in vast watershed areas of the Selenga River Basin (2007-2009), which provided baseline heavy metal database for the future management. Sources and environmental hazard and risk indices associated with metal pollution were evidenced across the countries of Mongolia and Russia (Buryatia Republic). In general, the concentrations of heavy metals in river water of Mongolia were greater than those of Russia, expect for the upstream of the Dzhida River in Russia. The spatial distribution generally indicated that metal pollution in the Selenga River was mainly associated with the activities in the Mongolian upstream regions. Similar pollution sources of metals between river water and wastewater associated with surrounding activities were found across the industrial and mining areas. Compositional patterns of metals suggested their sources were independent of each other, with hot spots in certain sites. Our measurements indicated that about 63 % of the locations surveyed (48 of 76) exceeded the critical heavy metal pollution index of 100, identifying possible harmful effects on aquatic ecosystems through metal pollution. Zinc was found to be the chemical of priority concern, as more than half of the locations exceeded the corresponding water quality guideline. Other metals including Mn, Fe, Cr, Cu, and As might be problematic in the Selenga River Basin considering the occurrence and their concentrations. Results of our extensive survey during the period of 3 years indicated that urgent action would be necessary in timely manner to improve water quality and mitigate the impact of heavy metals on aquatic environment of the Selenga River Basin.

RESUMO

BACKGROUND & AIMS: Recently, new methods, including the concept of viable enhancing tumor such as EASL and mRECIST, have been proposed for substitution of the conventional WHO and RECIST criteria in hepatocellular carcinoma (HCC) undergoing transarterial chemoembolization (TACE). Herein, we evaluated the differences of four methods and compared the association of these methods with the prognosis of HCC patients undergoing TACE. METHODS: We retrospectively reviewed 114 consecutive newly diagnosed HCC patients who underwent TACE as initial treatment. We evaluated the intermethod agreement (κ values) between the methods and compared their association with the prognosis of HCC patients. RESULTS: The κ values for EASL vs. WHO, EASL vs. RECIST, mRECIST vs. WHO, and mRECIST vs. RECIST were low, of 0.102, 0.088, 0.112, and 0.122, respectively. However, good correlations were observed for WHO vs. RECIST and EASL vs. mRECIST (κ=0.883, κ=0.759, respectively p<0.001). The median OS was 32.3 months. Hazard ratios (HR) for survival in responders compared with non-responders were 0.21 (95% CI; 0.12-0.37, p<0.001) for EASL and 0.27 (95% CI; 0.15-0.48, p<0.001) for mRECIST. The mean survival of responders was significantly longer than that of non-responders in both EASL (40.8 vs. 16.9 months, p<0.001) and mRECIST (41.1 vs. 20.7 months, p<0.001). In multivariate analysis, EASL response (HR 0.21, 95% CI 0.11-0.40, p<0.001) and mRECIST response (HR; 0.31, 95% CI, 0.17-0.59, p<0.001) were independently associated with survival. CONCLUSIONS: The response assessment by EASL and mRECIST could reliably predict the survival of HCC patients undergoing TACE and could be applicable in practice in preference to the conventional WHO and RECIST criteria.

RESUMO

A study was performed to evaluate the estimated daily intakes (EDI) of benzoates for the average and high (90th percentile) consumers by age and sex categories in Korea. The estimation of daily intakes of benzoates was based on individual dietary intake data from the National Health and Nutrition Survey in 1998 and on the determination of benzoates in eight food categories. The EDI of benzoates for average consumers of different age groups ranged from 0.009 to 0.025 mg kg(-1) bw day(-1). For high consumers, the range of EDI of benzoates was 0.195-1.878 mg kg(-1) bw day(-1). The intakes represented 0.18-0.50% of the acceptable daily intake (ADI) of benzoates for average consumers and 3.9-37.6% of the ADI for high consumers. Foods that contributed most to the daily intakes of benzoates were mixed beverages and soy sauce in Korea.
