RESUMO

Tunnel excavation induces the stress redistribution of the surrounding rock. Structural cracks may develop in the secondary lining due to this stress redistribution and bias pressure, consequently affecting the overall construction safety of the tunnel. This paper aims to achieve real-time monitoring of the excavation stability of the lining structure by integrating two monitoring technologies: structural deformation monitoring and fiber grating strain monitoring. Additionally, it proposes a method to simultaneously measure the thermal strain and applied stress-strain of the structure. By analyzing the displacement and deformation of the lining structure, its stability can be preliminarily evaluated in the short term. To achieve long-term real-time monitoring and a more accurate assessment of the tunnel structure's stability, the paper introduces fiber Bragg grating (FBG) strain sensor monitoring technology. First, based on the geological stratigraphy information obtained from the exploration, a simulation model of the tunnel under different section bias angles is established. The displacement and stress concentration areas of the lining structure are then analyzed to optimize the sensor deployment array and provide a theoretical basis for the sensor arrangement. FBG strain sensors are installed on the surface of the structure to measure thermal strain and loading stress-strain, whereas FBG temperature sensors measure local temperature. The findings indicate that following tunnel excavation, the maximum daily strain differences at K107+043 and K107+240 were 126.87 µÎµ and 209.38 µÎµ, respectively. After a period of rock disturbance, the average daily strain differences due to applied stress-strain were 16.8 µÎµ and 12.65 µÎµ, respectively. The thermal strain was close to the daily strain difference. Therefore, after the rock disturbance subsided, the strain fluctuations in the lining structure were mainly caused by local temperature changes, and the surrounding rock tended to stabilize. This offers a viable method for evaluating structural stability post-tunnel excavation.

ETHNOPHARMACOLOGICAL RELEVANCE: Psoraleae Fructus (PF), the dried fruit of Psoralea corylifolia L., is a commonly used traditional medicine that has contributed to the treatment of orthopedic diseases for thousands of years in China. However, recent PF-related liver injury reports have drawn widespread attention regarding its potential hepatotoxicity risks. AIM OF THE STUDY: This study was aimed to evaluate the long-term efficacy and chronic toxicity of PF using a 26-week administration experiment on rats in order to simulate the clinical usage situation. MATERIALS AND METHODS: The PF aqueous extract was consecutively administrated to rats daily at dosages of 0.7, 2.0, and 5.6 g/kg (equivalent to 1-8 times the clinical doses for humans) for as long as 26 weeks. Samples were collected after 13, 26, and 32 weeks (withdrawal for 6 weeks) since the first administration. The chronic toxicity of PF was evaluated by conventional toxicological methods, and the efficacy of PF was evaluated by osteogenic effects in the natural growth process. RESULTS: In our experiments, only the H group (5.6 g/kg) for 26-week PF treatment demonstrated liver or kidney injury, which the injuries were reversible after 6 weeks of withdrawal. Notably, the PF treatment beyond 13 weeks showed significant benefits for bone growth and development in rats, with a higher benefit-risk ratio in female rats. CONCLUSIONS: PF displayed a promising benefit-risk ratio in the treatment and prevention of osteoporosis, a disease that lacks effective medicine so far. This is the first study to elucidate the benefit-risk balance associated with clinical dosage and long-term use of PF, thereby providing valuable insights for rational clinical use and risk control of PF.

Accurate quantification on the quantum yields (φ) of both the prompt fluorescence (PF) and the delayed fluorescence (DF) species is quite essential for the clarification of molecular design rationales for thermally activated delayed fluorescence (TADF) luminogens. Currently, most φPF and φDF data of TADF fluorophores were acquired through time-correlated single-photon counting (TCSPC) lifetime measurement systems. However, because of their equal-time-channel working manner, so far all the commercially available TCSPC systems cannot render accurate measurement on φPF of TADF materials due to the lack of enough valid data points in the faster decay region of the corresponding photoluminescence (PL) decay curves. Although an intensified charge coupled device (ICCD) system equipped with a streak camera or an optical parametric oscillation laser has been proven to be a powerful tool for accurate determination of φPF and φDF of TADF fluorophores, the ultrahigh cost of these ICCD systems makes them inaccessible to most users. Herein, by replacing the timing module of a commercial TCSPC system with a low-cost and versatile time-to-digital converter (TDC) module, we developed a modified TCSPC system that can work in an unequal-time-channel manner. The resultant TDC-TCSPC system can not only concurrently determine the accurate lifetime of PF and DF species whose lifetime span even exceeds 5 orders of magnitude in just one time window but also render accurate measurements on φPF and φDF of TADF fluorophores. The reliability of the TDC-TCSPC method was verified through TCSPC- and ICCD-based comparative experiments on ACMPS, a known TADF fluorophore. Our results not only can provide a low-cost and convenient test method for accurate determination of key experimental data of TADF materials but also will facilitate deeper understanding of the molecular design principles for high-performance TADF materials.

The research results are inconsistent that assessing whether the increased obliquity of the distal articular surface of the medial cuneiform leads to an increase in hallux valgus angle. Thus, this study investigated the relationship between distal medial cuneiform obliquity and hallux valgus by measuring various angles in weightbearing anteroposterior radiographs of the foot. In total, 679 feet of 538 patients with the radiographs were included in the study. We measured radiographic parameters including hallux valgus angle, first to second intermetatarsal angle, metatarsus adductus angle, first metatarsus cuneiform angle, distal medial cuneiform angle, and first proximal metatarsal articular angle. The surface morphology (flat or curved) of the first tarsometatarsal joint was also recorded. Our results analysis revealed a weak negative correlation between distal medial cuneiform angle and both hallux valgus angle and first to second intermetatarsal angle, contrary to our assumption. So we believe that distal medial cuneiform angle was relatively constant and it cannot be used as a characteristic angle for quantifying hallux valgus. First metatarsus cuneiform angle was a characteristic indicator of hallux valgus and was positively correlated with its severity (p < .000), indicating that it can be used to measure the size of hallux valgus. It can also be used as a reference factor for the first metatarsal osteotomy in clinical bunion orthopedics. First tarsometatarsal joint morphology was unrelated to hallux valgus, whereas metatarsus adductus angle, and first proximal metatarsal articular angle should be considered in hallux valgus.

BACKGROUND: Panonychus citri is a major citrus pest worldwide. The short life cycle and high reproductive potential of P. citri, combined with heavy acaricide use, have led to high levels of resistance to acaricides, posing a threat to global resistance management programs. Here, resistance monitoring was established to determine the pyridaben resistance status of ten P. citri populations in China from 2014 to 2021 using a leaf-dipping assay. Four characterized strains-the susceptible strain (Lab_S), the resistant strain (Pyr_R), as well as the segregated resistant strain (Pyr_Rs) and the segregated susceptible strain (Pyr_Control) derived from the crossing of the Lab_S and Pyr_R strains, were used to evaluate the life-history characteristics using age-stage, two-sex life tables. RESULTS: Most P. citri populations developed high resistance to pyridaben. Resistance levels exceeded 1000-fold in Yuxi, Anyue, Nanning, and Ganzhou populations compared with the Lab_S strain. Compared with Pyr_Control, two key fitness cost criteria, developmental period and fecundity, showed significant differences in Pyr_Rs under consistent conditions. The intrinsic rate of increase, net reproductive rate and gross reproductive rate were lower in the resistant strain compared with the Pyr_Control strain. The Pyr_Rs strain had a lower relative fitness of 0.934 compared with the Pyr_Control. Moreover, the life-history traits and population parameters of the Pyr_R strain also showed significant differences compared with the Lab_S strain. CONCLUSION: The resistance levels to pyridaben varied greatly among the different P. citri populations and showed regional differences. Substantial fitness costs are associated with pyridaben resistance. This study provides potential implications for developing strategies for resistance management in P. citri. © 2022 Society of Chemical Industry.

This study used panel data for 2166 county-level units in China between 2000 and 2017 to characterize carbon productivity's temporal and spatial evolution in Chinese counties. The "catch-up effect" of carbon offsets between counties was examined, and this was used to measure the carbon offset potential and analyze its spatial differences. The results indicated that: (1) China's carbon productivity increased from 2000 to 2017; carbon productivity in eastern China was the highest, and carbon productivity in northeast China was the lowest. A polarization to unipolarization evolutionary trend was evident. (2) Considering the spatial differences in carbon productivity, the overall difference in carbon productivity in China from 2000 to 2017 first decreased and then increased; the contribution rate of differences between cities and counties to the overall difference was relatively stable, at around 39.88 %. The contribution rate of the differences between zones to the overall difference was the lowest, remaining stable at around 2.49 %. (3) In terms of the spatial distribution of the carbon offset potential, the eastern region exhibited the highest average carbon offset potential. The carbon offset potential of China exhibited a "high in the north and low in the south; low in the west and high in the east" spatial pattern.

China has the highest carbon dioxide emissions worldwide. Exploring the mechanism of fiscal decentralisation on agricultural carbon intensity can help China reach its carbon peak and become carbon neutral. This study used panel data for 30 provinces in Mainland China from 2000 to 2019 and constructed a spatial Durbin model to investigate the spatial-temporal patterns and internal relationships among fiscal decentralisation, environmental regulation, and agricultural carbon intensity. The results indicated that (1) from 2000 to 2019, the agricultural carbon intensity showed a downward trend and showed a spatial pattern of 'high in the north and low in the south'. The degree of fiscal decentralisation has gradually increased, and the spatial pattern of 'high in the north and low in the south' has also emerged. The intensity of environmental regulation continues to increase, and the intensity of environmental regulation in inland areas is higher than that in coastal areas. (2) From 2000 to 2019, the global Moran index of agricultural carbon intensity showed a development trend of first rising and then falling, and the spatial correlation changed from strong to weak. Agricultural carbon intensity tends to develop from polarisation to balanced development. (3) Both fiscal decentralisation and environmental regulation can reduce agricultural carbon intensity, and environmental regulation has a negative spatial spillover effect. (4) Under the influence of fiscal decentralisation, environmental regulation is not conducive to reducing agricultural carbon intensity due to the characteristics of 'race to the bottom', causing the 'green paradox' effect. (5) Environmental regulation and fiscal decentralisation in main grain producing areas have less impact on agricultural carbon intensity than in non-main grain producing areas. Therefore, the central government should focus on optimising the fiscal decentralisation system, formulating a differentiated agricultural carbon emission control system, regulating competition among local governments, and optimising a political performance evaluation system.

OBJECTIVE: To determine the penetration of heated tobacco products (HTPs) into the youth market in Taiwan, with a particular focus on the correlation between IQOS use and the usage of other tobacco products. METHODS: Data from the 2018 Global Youth Tobacco Survey were used to assess previous experience with and current use (within 30 days prior to survey completion) of IQOS products by Taiwanese students aged 12-18 years. Independent variables included the usage patterns of conventional cigarettes and e-cigarettes. The control variables included background information (gender, grade, monthly income/allowance, household educational level, smoking status at home and among close friends), access to free cigarettes, as well as exposure to cigarette advertisements and anti-tobacco courses. Logistic regression was used to identify tobacco usage patterns correlated with IQOS use. RESULTS: In 2018, 2.33% of Taiwan's adolescents were currently using IQOS and 4.17% had tried IQOS. The use of conventional cigarettes and e-cigarettes (individually and together) were associated with an elevated risk of the ever use and current use of IQOS. CONCLUSION: Despite the fact that HTP products are not sold legally in Taiwan, the use of IQOS products by young people is far from negligible. We recommend amending the "Tobacco Hazards Prevention Act" to include regulations pertaining to the sale and marketing of HTPs.

Trichomonas vaginalis (T. vaginalis) detection remains an unsolved problem in using of automated instruments for urinalysis. The study proposes a machine learning (ML)-based strategy to increase the detection rate of T. vaginalis in urine. On the basis of urinalysis data from a teaching hospital during 2009-2013, individuals underwent at least one urinalysis test were included. Logistic regression, support vector machine, and random forest, were used to select specimens with a high risk of T. vaginalis infection for confirmation through microscopic examinations. A total of 410,952 and 428,203 specimens from men and women were tested, of which 91 (0.02%) and 517 (0.12%) T. vaginalis-positive specimens were reported, respectively. The prediction models of T. vaginalis infection attained an area under the receiver operating characteristic curve of more than 0.87 for women and 0.83 for men. The Lift values of the top 5% risky specimens were above eight. While the most risky vigintile was picked out by the models and confirmed by microscopic examination, the incremental cost-effectiveness ratios for T. vaginalis detection in men and women were USD$170.1 and USD$29.7, respectively. On the basis of urinalysis, the proposed strategy can significantly increase the detection rate of T. vaginalis in a cost-effective manner.

The study aimed to characterize the molecular epidemiology, phylogenetic relationship, and population dynamics of the G protein gene in clinical respiratory syncytial virus (RSV) strains isolated from northern Taiwan. We analyzed a total of 160 and 116 G protein gene sequences of RSV-A and RSV-B representative strains, respectively, from 804 clinical viral stocks collected between July 2000 and June 2016. Population dynamic patterns of the RSV G protein gene were analyzed using Bayesian inference through the Markov chain Monte Carlo framework. A phylogenetic analysis revealed that RSV-A from Taiwan could be categorized into GA2, GA5, and GA7 lineages. GA2 of RSV-A could be further divided into NA1, NA2, NA4, and ON1 clades. These RSV-A lineages has been replaced over time, whereas RSV-B strains from Taiwan continually evolved from a single lineage with significant time-dependent waves. Four putative positive selection sites were observed in both RSV-A and RSV-B. The Bayesian skyline plot revealed that the local population dynamics of RSV were associated with lineage displacement events. Both circulating subtypes and population dynamics represented a unique local pattern. Our results affirm the necessity of continuing molecular surveillance of RSV to attain a more comprehensive understanding of epidemics.

Large numbers of DNA sequences deposited in the International Nucleotide Sequence Databases (INSD) are erroneously annotated. The erroneous information may lead to misleading conclusions or cause great economic losses to farmers. Lentinus edodes (= Lentinula edodes (Berk.) Pegler) is one of the most important and popular culinary-medicinal mushrooms with a high nutritional value. In this study, experimental and in silico methods were used to correct the sequences annotated as L. edodes in the INSD. A total of 3,426 nucleotide entries were retrieved from public databases, including 140 different types of genetic sequences. Excluding 1,893 genome sequences, the most abundant signatures represented by ITS (258) and IGS1 (259) sequences accounted for 33.23% of the total entries. A total of 3,058 sequences were annotated correctly, 350 were indeterminate, and 18 were annotated erroneously based on the two methods. Correction of sequences will be beneficial for species identification and annotation. Phylogenic analysis based on ITS sequences suggested that L. edodes segregate in four clades in the tree based on ITS sequences. The isolates from China were distributed into two clades. In L. edodes, the intraspecific variation of the ITS2 sequences was much higher than that of the ITS1 sequences. In addition, the genetic diversity of the L. edodes sequences from China was much higher than that of any other regions included in this study. The northwest and southwest regions of China were L. edodes diversity centers.

The World Health Organization has promoted age-friendly city (AFC) projects in response to the aging population. Taiwan has also promoted AFC policies. This study was conducted in Taitung County, where 15.37% of the population is older adults in Taiwan. The aim was to understand the perceptions of older adults and service providers with regard to the current status of AFC policies to influence future policies. The participants of this study were older adults and service providers in various regions of Taitung. Quantitative questionnaires were completed by older adults and qualitative interviews were held with focus groups. The older adults were the most satisfied with the AFC domains of "respect and social inclusion" and "community and health services", and the least satisfied with "transportation" and "civic participation and employment". Homogeneity existed between the older adults' satisfaction levels in different regions and the service providers' opinions; however, there were notable differences between them. Both economic development and the ethnicity of groups in different regions are influential factors that determine the success of government policies. In promoting AFC policies, local governments should consider their applicability based on local conditions and resources to meet the needs of the aging population in rural areas.

A novel ERP approach was proposed to index variations in mental workload, particularly in attentional reserve, which is complementary to EEG spectral content thought to reflect mental effort. To our knowledge, no study has assessed mental effort and attentional reserve simultaneously in EEG gel-based and, importantly, dry systems, which are particularly well suited for real-world settings. Therefore, by systematically considering ERP, EEG spectral, and importantly the combination of both, this study examined if a small set of dry EEG electrodes could detect changes in both spectral and ERP metrics to assess the mental workload under various challenges with a similar fidelity to their gel-based counterparts in a laboratory setting. By employing both EEG gel-based and dry systems, the ERP and spectral markers were computed while participants executed a visuomotor task under three levels of challenge. For both EEG systems, more challenging levels of difficulty were associated with concomitant changes in ERP amplitude, and spectral power reflected a reduction of the attentional reserve and an increase in cognitive-motor effort, respectively. Those variations in attentional reserve and cognitive-motor effort collectively indexed mental workload with nearly identical fidelity for both gel-based and dry EEG systems. These findings promise to assess the mental workload in situations where the use of dry EEG systems could be advantageously employed to examine human cognitive-motor performance.

We discuss the use of inequalities of the Leggett-Garg type (LGtI) to witness quantum coherence and present the first experimental violation of this type of inequalities using a light-matter interfaced system. By separately benchmarking the Markovian character of the evolution and the translational invariance of the conditional probabilities, the observed violation of a LGtI is attributed to the quantum coherent character of the process. These results provide a general method to benchmark "quantumness" when the absence of memory effects can be independently certified and confirm the persistence of quantum coherent features within systems of increasing complexity.

CYP2C9, one of the most important drug-metabolizing enzymes, is responsible for metabolizing approximately 15% of clinically important drugs, including warfarin, diclofenac, and losartan. Similar to other CYP members, human CYP2C9 exhibits marked genetic polymorphisms among individuals of different ethnicities. In this study, a novel missense mutation (1300A>T) was identified in a warfarin-sensitive patient after a genetic screen of three candidate genes related to high variability in response to warfarin doses. This base transversion leads to an Ile-to-Phe amino acid substitution at codon 434 within the CYP2C9 protein, and this new variant has been named a novel allele, CYP2C9*59, by the Human CYP Allele Nomenclature Committee (http://www.cypalleles.ki.se/cyp2c9.htm). The exogenous expression of CYP2C9.59 in insect cell microsomes revealed that, despite a similar protein expression level as wild-type CYP2C9, variant CYP2C9.59 exhibited significantly reduced maximal velocity, Vmax, and/or increased Michaelis constant, Km, values toward three CYP2C9-specific substrates. Our data suggest that the 1300A>T mutation can greatly decrease the enzymatic activity of the CYP2C9 protein both in vitro and in vivo.

We investigated the efficacy and cost effectiveness of the KineSpring System in the Spanish healthcare system, as compared to other standard treatments methods. Cost-utility ratios were calculated using derived cost data and we calculated quality-adjusted life years (QALYs) gained for each method of treatment. Cost-utility ratios were calculated assuming lifetime and 10-year durability. Assuming lifetime durability, cost-utility ratios of total knee arthroplasty (TKA), unicompartmental knee arthroplasty (UKA), high tibial osteotomy (HTO), KineSpring System, and conservative treatments, compared to no treatment, are €2348 ± 70/QALYs, €2040 ± 61/QALYs, €2281 ± 68/ QALY, €1669 ± 268/QALYs, and €11,688 ± 2185/QALYs, respectively. Assuming a treatment durability of 10 years, the cost-utility ratio of TKA, UKA, HTO, KineSpring System, and conservative treatments, compared to no treatment, are €4884 ± 323/QALYs, €4243 ± 280/QALYs, €4744 ± 313/QALYs, €3757 ± 1353/QALYs, and €10,575 ± 4414/QALYs, respectively. In comparison to current standard-of-care treatments, the KineSpring System has a favorable cost-utility ratio, making it an effective treatment option and a suitable cost-saving alternative. The KineSpring System is associated with lower cost and increased QALYs.

Osteoarthritis (OA) is a chronic, degenerative disease that is highly prevalent in the population, yet the factors that affect purchasing decisions related to this condition are poorly understood. A questionnaire was developed and administered to hospital executives across North America to determine the factors that affect purchasing decisions related to OA. Thirty-four individuals participated in the survey. Clinical evidence and cost effectiveness were deemed to be the most important factors in the process of making purchasing decisions. The most important considerations for adopting new technology were whether there was sufficient evidence in the literature, followed by thoughts of key opinion leaders, and cost of intervention/device. Ongoing research is still needed, but the current study allowed us to identify some trends in the data, providing new insight on how hospital purchasing decisions are made, which could have an immediate impact on those currently involved with making these decisions.

The number of outpatient orthopedic surgeries performed within North America continues to increase. The impact of this change in services on patient outcomes is largely unknown. The objective of this review is to compare patient outcomes and associated costs for outpatient orthopedic surgeries traditionally performed in hospital to inpatient surgeries, as well as to summarize the eligibility and preoperative education requirements for outpatient orthopedic surgery in North America. We performed a systematic review of Medline, Pubmed and Embase databases for articles comparing the clinical and economic impact of outpatient orthopedic surgical procedures versus inpatient procedures in North America. We reported on requirements for inpatient versus outpatient care, preoperative education requirements, complications and patient outcomes, patient satisfaction, and when available total mean costs. Nine studies met the inclusion criteria for this review. Eligibility requirements for outpatient orthopedic surgery within the included studies varied, but generally included: patient consent, a caregiver at home following surgery, close proximity to an outpatient center, and no history of serious medical problems. Preoperative education programs were not always compulsory and practices varied between outpatient centers. All of the reviewed studies reported that outpatient surgeries had similar or improved level of pain and rates of nausea. Outpatients reported increased satisfaction with the care they received. As expected, outpatient procedures were less expensive than inpatient procedures. This review found that outpatient procedures in North America appear to be less expensive and safe alternatives to inpatient care for patients who are at lower risk for complications and procedures that do not necessarily require close hospital level care monitoring following same day surgery.
