RESUMO

New CMOS imaging sensor (CIS) techniques in smartphones have helped user-generated content dominate our lives over traditional DSLRs. However, tiny sensor sizes and fixed focal lengths also lead to more grainy details, especially for zoom photos. Moreover, multi-frame stacking and post-sharpening algorithms would produce zigzag textures and over-sharpened appearances, for which traditional image-quality metrics may over-estimate. To solve this problem, a real-world zoom photo database is first constructed in this paper, which includes 900 tele-photos from 20 different mobile sensors and ISPs. Then we propose a novel no-reference zoom quality metric which incorporates the traditional estimation of sharpness and the concept of image naturalness. More specifically, for the measurement of image sharpness, we are the first to combine the total energy of the predicted gradient image with the entropy of the residual term under the framework of free-energy theory. To further compensate for the influence of over-sharpening effect and other artifacts, a set of model parameters of mean subtracted contrast normalized (MSCN) coefficients are utilized as the natural statistics representatives. Finally, these two measures are combined linearly. Experimental results on the zoom photo database demonstrate that our quality metric can achieve SROCC and PLCC over 0.91, while the performance of single sharpness or naturalness index is around 0.85. Moreover, compared with the best tested general-purpose and sharpness models, our zoom metric outperforms them by 0.072 and 0.064 in SROCC, respectively.

PURPOSE: The aims of the study were to identify factors related to (1) adolescents and young adults (AYA) desire to discuss health topics; (2) whether discussions occurred at their last medical visit; and (3) the gap (unmet need) between desire and actual discussion. METHODS: We used data from a nationally representative, cross-sectional online survey of AYA aged 13-26 years (n = 1,509) who had had a visit in the past 2 years. Bivariate analyses examined 11 topics. Multivariable regression identified health care factors and demographic factors related to unmet need across four salient topics (HIV/sexually transmitted infections, alcohol and drug use, tobacco, and contraception). RESULTS: Across 11 topics, unmet need averaged 28% and ranged as high as 60%; unmet need generally increased with AYA age. In multivariable analyses, ever having discussed confidentiality with a health care provider was associated with greater desire to discuss three of four salient topics, increased discussions (four of four topics), and reduced unmet need (two topics). Patient use of a clinical checklist/questionnaire at the last medical visit was associated with an increase in discussions (four topics) and reduced unmet need (four topics). Longer office visits were associated with an increase in discussions (three topics) and reduced unmet need (two topics). Older and minority youth had greater desire for discussions and unmet need. CONCLUSIONS: A considerable gap exists between young people's desire to discuss health topics with their health care providers and actual practice.

Osimertinib is designed to target the secondary resistant EGFR T790M mutant and has shown outstanding clinical efficacy. However, the prognostic prediction of osimertinib patients is a big problem in clinical practice. The resistance mechanism of osimertinib is also not fully understood. NGS and a 1021 gene capture panel were used to analyse the somatic mutation profile of thirty-six lung adenocarcinoma patients' serial ctDNA samples. Progression-free survival of subgroup patients is analysed. Patients harbour TP53 mutations and patients with higher TMB value in pre-treatment samples showed a shorter PFS. Moreover, compared to CT evaluation, ctDNA changes generally correlated with treatment responses in most patients. Novel resistance mechanisms are discovered including EGFR mutations and alternative activation pathway. Our results implied a broad potential of ctDNA as an adjuvant tool in practical clinical management of NSCLC patients. ctDNA could help with clinical practice during osimertinib treatment, regarding monitoring tumour response, detecting development of heterogeneity, identifying potential resistance mechanisms, predicting treatment efficacy and patient outcome.
