RESUMO

Reaction-diffusion models have been proposed for decades to capture the growth of gliomas. Nevertheless, these models require an initial condition: the tumor cell density distribution over the whole brain at diagnosis time. Several works have proposed to relate this distribution to abnormalities visible on magnetic resonance imaging (MRI). In this work, we verify these hypotheses by stereotactic histological analysis of a non-operated brain with glioblastoma using a 3D-printed slicer. Cell density maps are computed from histological slides using a deep learning approach. The density maps are then registered to a postmortem MR image and related to an MR-derived geodesic distance map to the tumor core. The relation between the edema outlines visible on T2-FLAIR MRI and the distance to the core is also investigated. Our results suggest that (i) the previously proposed exponential decrease of the tumor cell density with the distance to the core is reasonable but (ii) the edema outlines would not correspond to a cell density iso-contour and (iii) the suggested tumor cell density at these outlines is likely overestimated. These findings highlight the limitations of conventional MRI to derive glioma cell density maps and the need for other initialization methods for reaction-diffusion models to be used in clinical practice.

PURPOSE: To characterize cardiac-driven liver movements using a harmonic phase image representation (HARP) with an optical flow quantification and motion amplification method. The method was applied to define the cardiac trigger delay providing minimal signal losses in liver DWI images. METHODS: The 16-s breath-hold balanced-SSFP time resolved 20 images/s were acquired at 3T in coronal and sagittal orientations. A peripheral pulse unit signal was recorded. Cardiac-triggered DWI images were acquired after different peripheral pulse unit delays. A steerable pyramid decomposition with multiple orientations and spatial frequencies was applied. The liver motion field-map was derived from temporal variations of the HARP representation filtered around the cardiac frequency. Liver displacements were quantified with an optical flow method; moreover the right liver motion was amplified. RESULTS: The largest displacements were observed in the left liver (feet-head:3.70 ± 1.06 mm; anterior-posterior: 2.35 ± 0.51 mm). Displacements were statistically significantly weaker in the middle right liver (0.47 ± 0.11 mm; P = 0.0156). The average error was 0.013 ± 0.022 mm (coronal plane) and 0.021 ± 0.041 mm (sagittal plane). The velocity field demonstrated opposing movements of the right liver extremities during the cardiac cycle. DWI signal loss was minimized in regions and instants of smallest amplitude of both velocity and velocity gradient. CONCLUSION: Cardiac-driven liver movements were quantified with combined cardiac frequency-filtered HARP and optical flow methods. A motion phase opposition between right liver extremities was demonstrated. Displacement amplitude and velocity were larger in the left liver especially along the vertical direction. Motion amplification visually emphasized cardiac-driven right liver displacements. The optimal cardiac timing minimizing signal loss in liver DWI images was derived.

PURPOSE: To prospectively assess chemotherapy-induced changes in pancreatic ductal adenocarcinoma (PDA) with diffusion-weighted (DW)-MR quantitative metrics, including apparent diffusion coefficient (ADC) and histogram-derived parameters, compared with RECIST 1.1. METHODS: 24 patients underwent DW-MR at baseline, week-2 and week-8 after chemotherapy initiation. Tumour diameter was assessed on T2-weighted images. Regions-of-interest (ROI) were drawn on ADC map for ROI-ADC. Volume segmentation (b = 1000 s/mm2 images) provided DW-volume and histogram-derived diffusion parameters (H-ADC, H-D and H-PF). All variables and their relative change were compared to baseline or between responders and non-responders. Discriminant analysis was performed. RESULTS: 15/24 patients were responders. RECIST 1.1 correctly characterized 6/15 responders at week-8. At week-2, in responders DW-volume decreased (P = .002); ROI-ADC mean H-D increased (P = .047; P = .048;). The 25th percentile H-D increased in responders and decreased in non-responders (P = .016; P = .048). At week-8 in responders DW-volume decreased and ROI-ADC mean, 25th, 50th, 75th percentiles of H-ADC and H-D increased (P < .05). No changes were observed in non-responders (P > .05). At week-2, 25th percentile of H-D and H-PF relative change correctly classified 20/24 patients (P = .003); at week-8, DW-volume relative change correctly classified 22/24 patients (P < .0001). CONCLUSIONS: ROI-ADC, DW-volume and histogram-derived diffusion parameters are more accurate to categorize responding and non-responding PDA patients treated with chemotherapy compared with RECIST 1.1.

BACKGROUND: Cerebral dysfunction is a common feature of both chronic alcohol abusers and binge drinkers. Here, we aimed to study whether, at equated behavioral performance levels, binge drinkers exhibited increased neural activity while performing simple cognitive tasks. METHODS: Thirty-two participants (16 binge drinkers and 16 matched controls) were scanned using functional magnetic resonance imaging (fMRI) while performing an n-back working memory task. In the control zero-back (N0) condition, subjects were required to press a button with the right hand when the number "2" was displayed. In the two-back (N2) condition, subjects had to press a button when the displayed number was identical to the number shown two trials before. RESULTS: fMRI analyses revealed higher bilateral activity in the pre-supplementary motor area in binge drinkers than matched controls, even though behavioral performances were similar. Moreover, binge drinkers showed specific positive correlations between the number of alcohol doses consumed per occasion and higher activity in the dorsomedial prefrontal cortex, as well as between the number of drinking occasions per week and higher activity in cerebellum, thalamus and insula while performing the N2 memory task. CONCLUSIONS: Binge alcohol consumption leads to possible compensatory cerebral changes in binge drinkers that facilitate normal behavioral performance. These changes in cerebral responses may be considered as vulnerability factors for developing adult substance use disorders.

Diffusion tensor imaging (DTI) and tractography are noninvasive MRI methods, providing an insight on microscopic structural information of anisotropic tissues in vivo. The success of this technique stems on a watchful choice of imaging parameters and post-acquisition reconstruction. In the present work, we have focused on the problem of residual linear image misalignment in the DTI data and its effects on the parameters of the diffusion tensor and fiber tracking in human brain. We demonstrate substantial sensitivity of the reconstructed diffusion tensor and fiber tractography on increasing amplitude of artificially induced random image misalignment in the DTI. We show that already a submillimeter image misalignment in the DTI is an important source of error, which may potentially mask pathological presentations of the diseases and may partially explain variations in the results obtained from the DTI. Finally, we evaluated four implementations of image registrations and demonstrate their variable performance. This further supports the fact that a robust image registration must be performed to ensure reliable and reproducible diffusion tensor mapping and reconstruction of white matter (WM) fibers.

PURPOSE: To determine the accuracy of 3.0-T pelvic magnetic resonance (MR) imaging in the preoperative assessment of endometriosis and to evaluate colon wall involvement after intrarectal gel administration. MATERIALS AND METHODS: Institutional review board approval for this study was obtained, and each patient gave written informed consent. Forty-one consecutive patients with clinical suspicion of endometriosis underwent pelvic MR imaging at 3.0 T before surgery. Single-shot and high-spatial-resolution axial T2-weighted, sagittal fat-suppressed T2-weighted, and axial fat-suppressed T1-weighted sequences were performed. T2-weighted sequences were repeated after the rectum was filled with ultrasonographic (US) gel. Two blinded readers interpreted images independently. Image quality was scored by using a four-point scale. Detailed mapping of deep endometriosis was performed. Colon wall infiltration was graded (none, serosa, muscularis, submucosa, mucosa). MR imaging results were compared with surgical and pathologic findings. Interobserver agreement was assessed by using kappa statistics. Nonparametric tests were performed to compare colon wall infiltration scores without and those with US gel and between observers. RESULTS: Twenty-seven of 41 patients had deep endometriosis at surgery and histopathologic examination. Sensitivity, specificity, positive and negative predictive values, and accuracy for the diagnosis of deep endometriosis at MR imaging were 96.3% (26 of 27), 100% (14 of 14), 100% (26 of 26), 93.3% (14 of 15), and 97.6% (40 of 41), respectively. kappa Values ranged from 0.65 to 1.0, depending on the location of deep endometriosis. Colon wall infiltration assessment by both readers correlated well with pathologic findings (Spearman coefficient, >0.93), although median wall involvement scores were lower at pathologic examination than for both readers both before (P = .042 and P = .011) and after (P = .079 and P = .011) intrarectal gel filling. CONCLUSION: MR imaging of the pelvis at 3.0 T is accurate in the diagnosis and staging of deep endometriosis for the preoperative assessment of patients clinically suspected of having endometriosis.

PURPOSE: To prospectively quantify pancreatic regional perfusion with dynamic contrast material-enhanced magnetic resonance (MR) imaging by using a one-compartment model and to assess perfusion changes during secretin stimulation in healthy volunteers. MATERIALS AND METHODS: The study had institutional review board approval, and written informed consent was obtained. Ten healthy volunteers (five men, five women; mean age, 24.7 years +/- 1.9 [standard deviation]; range, 22-29 years) underwent MR imaging pancreatic perfusion studies performed twice without secretin and twice during secretin stimulation. Dynamic contrast-enhanced MR imaging consisted of saturation-recovery T1-weighted turbo-field-echo imaging with peripheral pulse triggering and respiratory tracking. A dose of 0.05 mmol gadodiamide per kilogram of body weight was injected at a rate of 3.5 mL/sec. Regional perfusion parameters were fitted with a one-compartment model. The analysis of variance test for repeated measurements was used to assess differences in pancreatic perfusion without and that with secretin administration. RESULTS: Significant differences in perfusion parameters between the three pancreatic regions were observed (P < .05). During secretin stimulation, a significant difference was observed only between the body and the tail of the pancreas (P = .02). A significant increase (P = .003) in pancreatic perfusion was observed after secretin administration. Mean pancreatic perfusion was 184 mL/min/100 g of tissue +/- 71, 207 mL/min/100 g +/- 77, and 230 mL/min/100 g +/- 87 without secretin and 342 mL/min/100 g +/- 154, 338 mL/min/100 g +/- 156, and 373 mL/min/100 g +/- 176 after secretin stimulation in the head, body, and tail of the pancreas, respectively. Intraindividual variability was 21% without secretin stimulation and 46% with secretin stimulation. CONCLUSION: Dynamic contrast-enhanced MR imaging enables noninvasive quantification of regional pancreatic perfusion in resting conditions and demonstrates the increase in pancreatic perfusion during secretin stimulation in healthy subjects.

OBJECTIVE: The purpose of this study was to evaluate the usefulness of MR imaging in the diagnosis and assessment of fetal sacrococcygeal teratoma and to determine if MR imaging provides information not seen on obstetric sonography. SUBJECTS AND METHODS: Twelve pregnant women were referred for fetal MR imaging of sacrococcygeal teratoma seen at obstetric sonography. The presence, size, content extension, and compressive effects of each mass were determined and correlated with the sonographic findings and with postnatal studies, including surgery and pathology. The extent of each sacrococcygeal teratoma was classified according to the American Association of the Pediatrics Surgery Section (types I-IV). RESULTS: There is a complete agreement of sonographic and MR imaging measurements. The sacrococcygeal teratomas appeared cystic with few septa in three fetuses, markedly septated or even microcystic in eight, and completely solid in one. The sonographic description of the content corresponded well to MR imaging findings in 10 of 12 fetuses. An agreement on the extent of each mass was observed in nine patients, whereas there is a disagreement in three, including in one fetus with an extension of the tumor within the spinal canal recognized only at MR imaging. The MR imaging findings were confirmed by postnatal studies in 10 patients. CONCLUSION: Sacrococcygeal teratomas had characteristic MR imaging appearances that allowed a complete assessment in most fetuses. Because of MR imaging, the prenatal evaluation was changed in some patients and affected counseling of the parents and treatment. MR imaging is a valuable adjunct to obstetric sonography for the prenatal evaluation of sacrococcygeal teratoma.
