BACKGROUND: Delayed or missed dosages caused by poor medication compliance significantly affected the treatment of diseases in children. AIMS: The present study aimed to investigate the influence of delayed or missed dosages on sirolimus pharmacokinetics (PK) in pediatric tuberous sclerosis complex (TSC) patients and to recommend remedial dosages for nonadherent patients. METHODS: A published sirolimus population PK model in pediatric TSC patients was used to assess the influence of different nonadherence scenarios and recommend optimally remedial dosages based on Monte Carlo simulation. Thirteen nonadherent scenarios were simulated in this study, including delayed 2h, 4 h, 6 h, 8 h, 10 h, 12 h, 14 h, 16 h, 18 h, 20 h, 22 h, 23.5 h, and missed one dosage. Remedial dosing strategies contained 10-200% of scheduled dosages. The optimal remedial dosage was that with the maximum probability of returning the individual therapeutic range. RESULTS: For delayed or missed sirolimus dosages in pediatric TSC patients, when the delayed time was 0-8 h, 8-10 h, 10-18 h, 18-22.7 h, 22.7-24 h, 70%, 60%, 40%, 30%, 20% scheduled dosages were recommended to take immediately. When one dosage was missed, 120% of scheduled dosages were recommended at the next dose. CONCLUSION: It was the first time to recommend remedial dosages for delayed or missed sirolimus therapy caused by poor medication compliance in pediatric TSC patients based on Monte Carlo simulation. Meanwhile, the present study provided a potential solution for delayed or missed dosages in clinical practice.

Reasonable carbon price can effectively promote the low-carbon transformation of economy. The future carbon price has an important guiding significance for enterprises and the country. However, the nonlinear and high noise characteristics inherent in carbon price make them challenging to predict accurately. A hybrid decomposition and integration prediction model is proposed using the Hodrick-Prescott filter, an improved grey model and an extreme learning machine to solve this problem. First, a large number of factors that influence carbon price are collected by meta-analysis. The final input is selected through a two-stage feature selection process. Second, the HP filter is used to decompose the input into long-term trends and short-term fluctuations predicted by the improved GM and ELM, respectively. Finally, the two prediction sequences are compared to obtain the final result. European Union Allowances futures price data are applied for empirical analysis. The results show that the prediction performance of this model is better than the other 10 benchmark models, the T-bill, Stoxx50, S&P clean energy index and Brent oil price in the financial and energy markets are helpful in the carbon price's prediction. T-bill affects carbon price frequently, Stoxx50 has a negative correlation with the carbon price in the influence period. Under normal circumstances, the S&P clean energy index is positively correlated with the carbon price. However, when the economic situation is depressed, resulting in a short-term negative correlation between them. In general, carbon market is significantly affected by cross spill over between different markets. The method not only improves the accuracy of carbon price forecast, but also the application of the improved GM explains the reasons for the change of carbon price, which is helpful to promote the realization of carbon neutralization by market-oriented means.

INTRODUCTION: Sonographic technologies can estimate extravascular lung water (EVLW) in hemodialysis (HD) patients. This study investigated the suitability of a handheld scanner in contrast to a portable scanner for quantifying EVLW in hospitalized patients requiring HD. METHODS: In this prospective study, 54 hospitalized HD patients were enrolled. Bedside lung ultrasound was performed within 30 min before and after dialysis using handheld (phased array transducer, 1.7-3.8 MHz) and portable (curved probe, 5-2 MHz) ultrasound devices. Eight lung zones were scanned for total B-lines number (TBLN). The maximum diameter of inferior vena cava (IVC) was measured. We performed Passing-Bablok regression, Deming regression, Bland-Altman, and logistic regression analysis. RESULTS: The 2 devices did not differ in measuring TBLN and IVC (p > 0.05), showing a high correlation (r = 0.92 and r = 0.51, respectively). Passing-Bablok regression had a slope of 1.11 and an intercept of 0 for TBLN, and the slope of Deming regression was 1.02 within the CI bands of 0.94 and 1.11 in the full cohort. TBLN was logarithmically transformed for Bland-Altman analysis, showing a bias of 0.06 (TBLN = 1.2) between devices. The slope and intercept of the Deming regression in IVC measurements were 0.77 and 0.46, respectively; Bland-Altman plot showed a bias of -0.07. Compared with predialysis, TBLN significantly (p < 0.001) decreased after dialysis, while IVC was unchanged (p = 0.16). Univariate analysis showed that cardiovascular disease (odds ratio [OR] 8.94 [2.13-61.96], p = 0.002), smoking history (OR 5.75 [1.8-20.46], p = 0.003), and right pleural effusion (OR 5.0 [1.2-25.99], p = 0.03) were strong predictors of EVLW indicated by TBLN ≥ 4. CONCLUSION: The lung and IVC findings obtained from handheld and portable ultrasound scanners are comparable and concordant. Cardiovascular disease and smoking history were strong predictors of EVLW. The use of TBLN to assess EVLW in hospitalized HD patients is feasible. Further studies are needed to determine if TBLN can help guide volume removal in HD patients.

The Atmospheric Infrared Ultraspectral Sounder (AIUS), the first high-resolution (0.02 cm−1) solar occultation sounder, aboard GF5, was launched in May 2018 from China. However, relevant studies about vertical profiles of atmospheric constituents based on its operational data were not conducted until half a year later. Due to an urgent need for Hin-orbit tests, the real spectra (called reference spectra hereafter) were substituted with simulated spectra calculated from the reference forward model (RFM) plus different random noises at different altitudes. In the generation process of the reference spectra for N2O, NO2, and HF species, ACE-FTS (Atmospheric Chemistry Experiment⁻Fourier Transform Spectrometer instrument on the SCISAT satellite) level 2 products replace corresponding profiles included in the atmospheric background profiles. The optimal estimation method is employed to extract N2O, NO2, and HF profiles in this study. Comparing the retrieved results with ACE-FTS level 2 products, the relative deviations for these three species are calculated. For N2O, the average relative deviation is less than 6% at altitudes below 25 km, while larger deviations are observed in the range of 25⁻45 km, with the maximum being at ~25%. Additionally, the difference for NO2 is less than 5% in the 20⁻45 km range, with a larger discrepancy found below 20 km and above 45 km; the maximum deviation reaches ±40%. For HF, the relative deviation is less than 6% for all tangent heights, implying satisfactory retrieval. The vertical resolution, averaging kernel, and number of degrees of freedom are used to assess the retrieval algorithm, which indicate that the retrieved information content is much more attributable to the reference spectra contribution than to the a priori profile. Finally, a large number of retrieval tests are performed for N2O, NO2, and HF in selected areas covering the Arctic region, northern middle latitude, tropics, southern middle latitude, and Antarctic region, and reliable results are obtained. Thus, to a great extent, the algorithm adopted in the AIUS system can process retrievals reliably and precisely.
