RESUMO

 The current study aimed to examine the outcomes of pharmacists' involvement with elderly people in special nursing homes. We analyzed 58 cases involving regular visits by community pharmacists to 41 residents. The residents' mean age was 87.8±6.9 years, and 68.3% were prescribed 6 or more types of medication. Antipsychotic and insomnia medication was taken by 24.4% and 31.8% of residents, respectively. Pharmaceutical consultation following medication use accounted for 60.3% of pharmacists' involvement with residents. The outcomes of these consultations included improvements in prescription content; the identification and prevention of adverse drug events; improvement in activities of daily living; and improvement in test results, sleep, and urination/bowel control. The results also suggested that pharmacists' intervention reduced drug costs. Information that facilitated involvement was most frequently acquired via conversations (67.2%) and conferences (24.1%) in the facilities. The most common information sources were care workers (72.4%), followed by nurses (37.9%), physicians (6.9%), and functional training instructors (6.9%). Information was also acquired from patients (3.4%) and their family members (5.2%). The findings indicated that regular visits by pharmacists to facilities for elderly people and conversations between residents, their family members, and physicians, nurses and various other professionals improved various pharmacotherapy outcomes.

In Japan the prevalence of dementia has increased considerably, and pharmacists are involved in addressing these patients' medication-related problems. Here, we determined whether pharmacists' comprehensive assessment of medication profiles could reduce the burden of dementia patients' medication-related problems. In this historical cohort study 120 community pharmacies were randomly selected, and participating pharmacists completed questionnaires concerning comprehensive assessment of patient medication profiles, using a "start" questionnaire for patients prescribed medication prior to or during the study period and a "follow-up" questionnaire for patients who subsequently visited pharmacies for prescriptions. Numbers and details of problems and solutions implemented by pharmacists and identified in the start and follow-up questionnaires were compared. Changes in start and follow-up scores were also compared between patients whose problems were identified by pharmacists (identified group) and those whose problems were not (non-identified group). Data were collected for 349 patients issued medication by 60 pharmacies. The most common medication-related problems identified in the start survey were key person's understanding of donepezil (60 cases) and other dementia treatments (60 cases), and adherence to treatment (53 cases). Solutions implemented by pharmacists included gathering information regarding drug administration and dementia awareness from the key person and providing pharmaceutical counseling and instruction. Subsequently, problems related to understanding of dementia treatment, understanding donepezil, and adherence were resolved by 70.0%, 65.0%, and 58.5%, respectively. Pharmacists' comprehensive assessment of medication profiles could effectively solve dementia patients' medication-related problems.

The contents of pharmacist interventions, which were carried out by the ward pharmacists in their routine pharmacy service activities, were sorted and analyzed to evaluate the contributions of pharmacists. In the ward where pharmacists were stationed, there were a total of 196 cases of pharmacist intervention. The prescription was changed in 170 cases, giving a rate of prescription change of 86.7%. The breakdown of the pharmacist intervention was as follows: "efficacy/safety", 106 cases, followed by "dosage regimen" (48 cases) and "compliance" (10 cases). Cost savings achieved during the investigation period were calculated to be 440,639 yen, and cost avoidance was valued at 1,941,847-3,883,695 yen using the Diagnosis Procedure Combination (DPC). The results of the present investigation showed that pharmacists contribute to through not only their pharmacy services, but also through the promotion of proper drug use and risk management, thereby contributing to hospital management through cost savings and avoidance.

In order to examine the structural features of community pharmacy function and to clarify areas meriting priority in efforts to expand pharmacy functions in the home care field, a questionnaire survey was conducted. Using the distribution of responses regarding the actual conditions entailed in the 16 categories of pharmacy services, factor analysis was carried out. Structural equation modeling was performed and the fitness of the path model constructed to study the interrelatedness of the individual factors extracted by factor analysis was considered. Four factors comprised pharmacy services "dispensing", "supply of goods", "offering home care", and "cooperation in community health and medical programs". The interrelatedness of four pharmacy functions by the result of structural equation modeling indicated the following insights. 1) By strengthening "cooperation in community health and medical programs", it is possible to directly improve the functions of "dispensing", and "offering home care". An improvement in "collecting and utilizing patient information", "providing appropriate instruction", "providing introductions to medical institutions and physicians", and "supplying over-the-counter drugs" can be expected as a result of direct benefits accrued by "cooperation in community health and medical programs". 2) By strengthening "offering home care", it is possible to directly improve "supply of goods". An improvement in "stocking long-term care goods and sanitary goods" can be expected as a result of direct benefits accrued by "offering home care". In conclusion, this study indicated that the areas meriting priority in the expansion and upgrading of pharmacy functions is stronger "cooperation in community health and medical programs".

The present study investigated the clinical effects and therapeutic cost of cefazolin (CEZ) and ampicillin/sulbactam (SBT/ABPC) compared to analyze cost-effectiveness for surgical prophylaxis in gastric cancer patients. 157 inpatients who underwent surgery for gastric cancer were investigated. There was no difference between the two groups with regard to sex, age, incidence of complication, stage of cancer, operative time and blood loss, length of hospitalization, the appearance of systematic inflammatory responses syndrome and the prophylactic effect of infection. Meanwhile, decision analysis indicated that the anticipated therapeutic cost per patient in CEZ group was less than that of SBT/ABPC group (USD 142.72 and USD 187.17, respectively). In this case, CEZ use was more cost-effective, insofar as only drug cost was considered.

In order to investigate the implementation about pharmacy services focusing on home care and to identify differences in functional characteristics in accordance with pharmacy location, questionnaire surveys were conducted on all 75 pharmacies belonging to T-City Pharmaceutical Association in a suburban area and on all 161 belonging to K-Ward Pharmaceutical Association in a central downtown district. Using data, a principal component analysis was performed. The following services were investigated: medication guidance for patients and their families; provision of drug information to patients; management of patients' drug history; dispensing; advice to physicians; inquiries; management of prescriptions for home infusion services; sales of items such as over-the-counter [OTC] drugs [commercially available drugs that do not require a physician's prescription]; display and catalogue sales of nursing items; consultation on home care; holding events such as health education programs; guidance for drug management by home visits; and information exchange with other medical professionals and social workers. The results of these surveys were as follows. (1) The following services were regularly performed in both urban and suburban areas: medication guidance for patients and their families; provision drug information to patients; management of patients' drug history; dispensing; advice to physicians; inquiries; and sales of OTC drugs. (2) The following services were not difficiently regularly performed in both areas: display and catalogue sales of nursing items; consultation on home care; holding events such as health education programs; guidance for drug management by home visits; and information exchange. (3) As to principal component analysis, pharmacy functions were divided into 3 components and the dispensing service strongly influenced the functional characteristics in T-City. Whereas, in K-Ward, pharmacy functions were divided into 4 components and consultation and information exchange were strong influences. (4) Home care-relate pharmacy services were closely associated with information exchange on both areas. This implies that enforcing information exchange among pharmacists and other health practitioners and social welfare practitioners is an essential factor to expand implementation of home-care related pharmacy services.

To analyze the reason why home care service which community pharmacists provide has not been effectively utilized, a questionnaire on home care services was submitted to 472 home care service users, 215 general practitioners, 10 visiting nurses and 153 home-helpers, and 75 community pharmacists. We were interested in whether or not gaps existed between users' needs, practitioners' demands, and pharmacists' awareness of professional responsibility to pharmacy services. Also, we were interested in whether or not gaps existed in awareness of the importance of cooperation with community pharmacists and influencing factors between practitioners and home-helpers and visiting nurses. The main results were as follows: users and practitioners considered that counseling about home care and welfare services by community pharmacists was important and home visiting was not so important. However, pharmacists considered that home visiting was important and counseling about home care and welfare services was not so important. Therefore, gaps existed between users' needs, practitioners' demands, and pharmacists' awareness of professional responsibility to pharmacy services. Practitioners' awareness of the importance of cooperation with community pharmacists in comparison to that of other professionals was low as well as that of home-helpers, and visiting nurses. However, over 70% of them recognized the necessity of home visiting services by pharmacists. As far as factors influencing the awareness of the importance of cooperation with community pharmacists, practitioners' recognition to home visiting by pharmacists was higher than that of home-helpers and visiting nurses. However, expectations regarding community pharmacists' participation in home care services was higher than that of practitioners.
