Background: Non-communicable diseases (NCDs) cause long-term impacts on health and can substantially affect people's ability to work. Little is known about how such impacts vary by gender, particularly in low- and middle-income countries (LMICs), where productivity losses may affect economic development. This study assessed the long-term productivity loss caused by major NCDs among adult women and men (20-76 years) in Mexico because of premature death and hospitalisations, between 2005 and 2021. Methods: We conducted an economic valuation based on the Human Capital Approach. We obtained population-based data from the National Employment Survey from 2005 to 2021 to estimate the expected productivity according to age and gender using a two-part model. We utilised expected productivity based on wage rates to calculate the productivity loss, employing Mexican official mortality registries and hospital discharge microdata for the same period. To assess the variability in our estimations, we performed sensitivity analyses under two different scenarios. Results: Premature mortality by cancers, diabetes, chronic cardiovascular diseases (CVD), chronic respiratory diseases (CRD) and chronic kidney disease (CKD) caused a productivity loss of 102.6 billion international US dollars (Intl. USD) from 2.8 million premature deaths. Seventy-three percent of this productivity loss was observed among men. Cancers caused 38.3% of the productivity loss (mainly among women), diabetes 38.1, CVD 15.1, CRD 3.2, and CKD 5.3%. Regarding hospitalisations, the estimated productivity loss was 729.7 million Intl. USD from 54.2 million days of hospitalisation. Men faced 65.4 and women 34.6% of these costs. Cancers caused 41.3% of the productivity loss mainly by women, followed by diabetes (22.1%), CKD (20.4%), CVD (13.6%) and CRD (2.6%). Conclusions: Major NCDs impose substantial costs from lost productivity in Mexico and these tend to be higher amongst men, while for some diseases the economic burden is higher for women. This should be considered to inform policymakers to design effective gender-sensitive health and social protection interventions to tackle the burden of NCDs.

BACKGROUND: Mexico and other low- and middle-income countries (LMICs) present a growing burden of non-communicable diseases (NCDs), with gender-differentiated risk factors and access to prevention, diagnosis and care. However, the political agenda in LMICs as it relates to health and gender is primarily focused on sexual and reproductive health rights and preventing violence against women. This research article analyses public policies related to gender and NCDs, identifying political challenges in the current response to women's health needs, and opportunities to promote interventions that recognize the role of gender in NCDs and NCD care in Mexico. METHODS: We carried out a political mapping and stakeholder analysis during July-October of 2022, based on structured desk research and interviews with eighteen key stakeholders related to healthcare, gender and NCDs in Mexico. We used the PolicyMaker V5 software to identify obstacles and opportunities to promote interventions that recognize the role of gender in NCDs and NCD care, from the perspective of the political stakeholders interviewed. RESULTS: We found as a political obstacle that policies and stakeholders addressing NCDs do not take a gender perspective, while policies and stakeholders addressing gender equality do not adequately consider NCDs. The gendered social and economic aspects of the NCD burden are not widely understood, and the multi-sectoral approach needed to address these aspects is lacking. Economic obstacles show that budget cuts exacerbated by the pandemic are a significant obstacle to social protection mechanisms to support those caring for people living with NCDs. CONCLUSIONS: Moving towards an effective, equity-promoting health and social protection system requires the government to adopt an intersectoral, gender-based approach to the prevention and control of NCDs and the burden of NCD care. Despite significant resource constraints, policy innovation may be possible given the willingness among some stakeholders to collaborate, particularly in the labour and legal sectors. However, care will be needed to ensure the implementation of new policies has a positive impact on both gender equity and health outcomes. Research on successful approaches in other contexts can help to identify relevant learnings for Mexico.

Frequent consumption of sugar sweetened beverages (SSB) is related to the risks of developing overweight, obesity, cavities, diabetes and other diseases. Policies to significantly increase taxes on SSB have proven to be effective in reducing their consumption. The political debate on implementing these taxes in Colombia shows a series of barriers to placing this policy on the political agenda, and therefore, to its approval. This work analyses the political process involved in the struggle for the approval of an SBB tax in Colombia, as well as barriers and opportunities to putting it on the political agenda. This is done through a policy analysis with three research methods: a documentary analysis, political mapping of actors and semistructured interviews with key actors. Among the main findings, we have that actors who are in favour of the SSB tax stated that it is needed due to the health problems caused by SSB consumption, while those who opposed it argue that Colombia regulations are sufficient and already inform and educate consumers on excessive sugar consumption and its health implications. The Colombian political context is a barrier to SSB taxation, as the government favours and has a close connection with the food and SSB industry. In short, the policy issue has been reaching the agenda intermittently throughout the years. Nevertheless, new opportunities are arising after the COVID-19 pandemic and the 2022 administrative changes and further efforts from policy entrepreneurs are required to make this initiative progress in the political agenda.

Latin America has experienced a rise in noncommunicable diseases (NCDs) which is having repercussions on the structuring of healthcare delivery and social protection for vulnerable populations. We examined catastrophic (CHE) and excessive (EHE, impoverishing and/or catastrophic) health care expenditures in Mexican households with and without elderly members (≥65 years), by gender of head of the households, during 2000-2020. We analyzed pooled cross-sectional data for 380,509 households from eleven rounds of the National Household Income and Expenditure Survey. Male- and female-headed households (MHHs and FHHs) were matched using propensity scores to control for gender bias in systematic differences regarding care-seeking (demand for healthcare) preferences. Adjusted probabilities of positive health expenditures, CHE and EHE were estimated using probit and two-stage probit models, respectively. Quintiles of EHE by state among FHHs with elderly members were also mapped. CHE and EHE were greater among FHHs than among MHHs (4.7% vs 3.9% and 5.5% vs 4.6%), and greater in FHHs with elderly members (5.8% vs 4.9% and 6.9% vs 5.8%). EHE in FHHs with elderly members varied geographically from 3.9% to 9.1%, being greater in less developed eastern, north-central and southeastern states. Compared with MHHs, FHHs face greater risks of CHE and EHE. This vulnerability is exacerbated in FHHs with elderly members, because of gender intersectional vulnerability. The present context, marked by a growing burden of NCDs and inequities amplified by COVID-19, makes key interlinkages across multiple Sustainable Development Goals (SDGs) apparent, and calls for urgent measures that strengthen social protection in health.

INTRODUCTION: Efforts to identify the predictors of maternal knowledge on Early Child Development (ECD) have proven inconclusive thus far, particularly with respect to socially deprived contexts in Low- and Middle-Income Countries (LMICs). We quantified the extent of ECD knowledge among mothers who were the primary caregivers of 0-38-month-old infants in marginalized communities in Mexico. We also explored the characteristics of the children, both individually and with regard to their households, given the influence of these factors on childhood development. METHODS: We analyzed primary data obtained through a questionnaire administered to mothers who were the primary caregivers of 1045 girls and boys 0-38 months of age. The instrument was specifically designed for our study in order to explore the knowledge of participants about physical, neurological and psycho-affective development during childhood. We performed fractional regression analysis to assess the predictors of ECD knowledge. RESULTS: The mean score of maternal ECD knowledge increased with their age and schooling as well as with their levels of cognitive ability and self-esteem. Irrespective of age at first birth, mean knowledge was relatively high for women with high school education and low for women with elementary or no formal education, a gradient with respect to age at fist birth was more marked among women with middle school education. ECD knowledge scores increased among mothers from households enjoying higher socioeconomic levels and from households with health insurance. Scores were lower for indigenous households regardless of their participation in social programs. CONCLUSION: Public policies on ECD should promote programs that are not only adapted to specific contexts, but also designed to improve shared child-rearing, early childhood care and as well as psycho-emotional education skills as a pathway to healthier ECD. The participation of families and communities in sensitive childhood care should form part of multisectoral programs involving education, health and wellbeing.

Based on a behavioral economics (BE) approach, we analyzed the decision to participate in an early childhood development (ECD) program implemented in Mexico by a non-governmental organization. We conducted a literature review and a qualitative study of four localities participating in the ECD program. Situated in the state of Oaxaca, these communities are characterized by high and very high levels of social marginalization. From May 20 to 30, 2019, we collected primary data through semi-structured interviews (n = 30) and focus groups (n = 7) with a total of 61 informants (51 women and 10 men). We then performed an inductive systematic analysis of the data to identify documented cognitive bias associated with the decisions of individuals to participate and remain in or abandon social programs. The interviewees were living in conditions of poverty, facing difficulties in meeting even their most basic needs including food. Program participants attached far greater weight to incentives such as the basic food basket than to the other benefits offered by the program. The four localities visited maintained traditional views of domestic roles and practices, particularly regarding child-rearing, where women were in charge of childcare, home care and food preparation. Problems linked to child malnutrition were a decisive factor in the decision of residents to participate and remain in the program. Testimonials gathered during the study demonstrated that the longer the mothers remained in the program, the more they understood and adopted the concepts promoted by the interventions. In contexts marked by economic vulnerability, it is essential that ECD programs create the necessary conditions for maximizing the benefits they offer. Our analysis suggests that cognitive load and present bias were the biases that most severely affected the decision-making capacity of beneficiaries. Therefore, considering loss aversion and improving the management of incentives can help policymakers design actions that "nudge" people into making the kinds of decisions that contribute to their well-being.

This article examines the coverage in the continuum of antenatal-postnatal care for vulnerable women in Mexico according to indigenous status and assesses the influence of public health insurance strategies on the evolution of coverage over the last 25 years. We studied a total of 19 613 567 Mexican women, aged 12-54 years at last birth, based on a pooled cross-sectional analysis of data from the 1997, 2009, 2014 and 2018 waves of the National Survey of Demographic Dynamics. After describing sociodemographic characteristics and maternal-health coverage by indigenous status, we constructed a pooled fixed-effects and interaction multivariable regression model to assess the influence of the Seguro Popular programme on continuum of care. We estimated adjusted continuum of care coverage between 1994 and 2018 according to Seguro Popular affiliation and indigenous status. Prior to the Seguro Popular programme, crude coverage in the continuum of care for non-indigenous women stood at 14.5% [95% confidence interval (CI): 13.2-15.8%] or 11 percentage points higher than for indigenous women. During the last period of the programme, it rose to 46.5% [95% CI: 45.6-47.5%] and 34.1% [95% CI: 30.7-37.4%], respectively. Our regression analysis corroborated findings that, on average, indigenous women faced lower odds of benefiting from continuum of care [adjusted odds ratio (aOR) = 0.48, 95% CI: 0.40-0.57] than did their non-indigenous counterparts. It also revealed that coverage for indigenous women without Seguro Popular affiliation was 26.7% [95% CI: 23.3-30.1%] or 12 percentage points lower than for those with Seguro Popular affiliation (38.6%, 95% CI: 35.7-41.4%). Our regression results confirmed that the latter benefited from higher odds of continuum of care (aOR = 1.67, 95% CI: 1.36-2.26). Gaps between those of indigenous and non-indigenous status have persisted, but the Seguro Popular clearly contributed to reducing the coverage gaps between these two groups of women. Strategies yielding better outcomes are required to improve the structural conditions of indigenous populations.

OBJECTIVE: To present the results of a stakeholder analysis used to construct a map of the actors involved in the delib-eration of a proposal to increase the tax on sugar-sweetened beverages (SSB) in Mexico from 10 to 20 percent per liter. MATERIALS AND METHODS: A literature review and in-terviews to key actors were implemented. The analysis of the actors' power and position was made using Policymaker. RESULTS: There was concern for the obesity epidemic among all stakeholders, but little consensus on the way to solve it. Researchers and non-governmental organizations (NGO) support an increase in the tax on SSB, while government officials and industry representatives oppose this measure. CONCLUSION: Supporters of an increase to the tax on SSB need to build a coalition in order to force government officials to support this policy and successfully confront the soda industry, which has a solid opposing strategy and enormous financial resources to influence public opinion and congressmen.

The aim of this study was to estimate the role of victimization by violence among Mexican adolescents that have considered or attempted migrating to the United States, including mental health variables (emotional self-esteem, self-esteem in school, depression, suicidal ideation, and attempted suicide) as mediators of the effects. The study used a cross-sectional design with a stratified cluster sample of 13,198 adolescents from the 2nd Mexican National Survey on Exclusion, Intolerance, and Violence in public schools in 2009. The analysis used the regression models proposed by Baron & Kenny. Prevalence of having considered or attempted cross-border migration was 23.1%. Mean age was 16.36 years. Female adolescents constituted 54.9% of the sample, and 56% were lower-income. Mental health variables that acted as partial mediators were suicidal ideation (35.9%), depression (19.2%), attempted suicide (17.7%), emotional self-esteem (6.2%), and self-esteem in school (3.4%) for moderate family violence, and emotional self-esteem (17.5%) for social rejection in school and suicidal ideation (8.1%) for physical harm in school. Female adolescents showed greater impact from mediators than men in considering or having attempted cross-border migration. The study discusses the importance of incorporating the prevention of violence in the social contexts studied here and incorporating mental health in dealing with violence in adolescents and in public health programs in transit areas for illegal migrants.

OBJECTIVE:: To show that the administrative regime of specialized hospitals has some influence on the administrative processes to operate the Mexican Fund for Catastrophic Expenditures in Health (FPGC, in Spanish), for providing health care to breast cancer, cervical cancer and child leukemia. MATERIALS AND METHODS:: The variable for estimating administrative efficiency was the time estimated from case notification to reimbursement. For its estimation, semistructured interviews were applied to key actors involved in management of cancer care financed by FPGC. Additionally, a group of experts was organized to make recommendations for improving processes. RESULTS:: Specialized hospitals with a decentralized scheme showed less time to solve the administrative process in comparison with the model on the hospitals dependent on State Health Services, where timing and intermediation levels were higher. CONCLUSIONS:: Decentralized hospitals administrative scheme for specialized care is more efficient, because they tend to be more autonomous.

Resumen: Objetivo: Mostrar que el régimen administrativo de hospitales de especialidad influye en la eficiencia de los procesos administrativos para operar el Fondo de Protección contra Gastos Catastróficos (FPGC), para la atención de cáncer de mama, cáncer cérvicouterino y leucemia linfoblástica aguda. Material y métodos: La variable para estimar la eficiencia del proceso administrativo fue el tiempo entre la notificación del caso y el reembolso. Para su estimación se realizaron entrevistas semiestructuradas con actores clave involucrados en la gestión de casos financiados por el FPGC. Se organizó también un grupo de expertos para emitir recomendaciones de mejora. Resultados: Los hospitales de especialidad con un esquema descentralizado mostraron menor tiempo en el proceso administrativo en contraste con el modelo administrado por los Servicios Estatales de Salud, donde los tiempos fueron mayores y donde hubo mayores niveles de intermediación. Conclusiones: El modelo de hospitales especializados con un esquema descentralizado es más eficiente debido a que tiene mayor autonomía.

Introducción: La actividad física (AF) es un factor de protección ante múltiples enfermedades. Las recomendaciones mundiales para la AF están bien establecidas y son difundidas en todos los escenarios posibles, pero el ambiente construido podría presentar múltiples barreras para seguir dichas recomendaciones. Objetivo: Analizar algunas barreras del ambiente construido para la actividad física por niveles de marginación en la ciudad de Cuernavaca, México. Materiales y métodos: Empleando el programa Atlas ti 5.0, se categorizaron los datos obtenidos de 170 guías de observación. Se realizó el análisis de un acervo fotográfico de 341 imágenes de 20 AGEBS de la ciudad de Cuernavaca, utilizando un análisis de imágenes fijas connotativo, denotativo y por 3 categorías (Calzadas, tránsito y espacios deportivos). Se utilizó el programa Stata 12.0 para obtener frecuencias y porcentajes. Resultados: Se identificaron deficiencias en el diseño y uso de espacios como las banquetas, falta de señalización e inseguridad de las vías públicas. Se encontró carencia de espacios para la práctica de la actividad física. Solo hay una presencia del 10% de espacios deportivos en los 20 AGEBS estudiados. Se encontró que el 82.86% de las calles está en deficiente estado. La deficiente iluminación vial, carencia de ciclovías y grafitis con porcentajes importantes también se identificaron como barreras para la AF. Discusión: En Cuernavaca las posibilidades de acceso a espacios seguros, presentan múltiples barreras y por ende esta tendencia afecta de forma negativa la salud de los habitantes de la ciudad, además que no es congruente con las recomendaciones mundiales para la actividad física. Conclusiones: La identificación y abolición de las barreras deben estar dentro las prioridades de las políticas de salud públicas y las intervenciones deben procurar dicha transformación, de esta forma se superará la tendencia de solamente informar sobre los beneficios de la actividad física que predominan en la gran mayoría de las recomendaciones e intervenciones.


Introduction: Physical activity (PA) is a factor of protection against multiple diseases. Global recommendations for PA are well established and are spread in all possible scenarios, but the built environment could present multiple barriers to follow these recommendations. Objective: To Analyze some built environment barriers for physical activity by levels of marginalization in the city of Cuernavaca, Mexico. Material and methods: Using the software Atlas ti 5.0, data from 170 observation guides and were categorized. The analysis of a photographic collection of 341 images of 20 AGEBS of Cuernavaca was performed using an analysis connotative and denotative by 3 categories (roadways, traffic and sports areas). Stata 12.0 software was used to obtain frequencies and percentages. Results: Deficiencies in the design and use of spaces such as sidewalks, lack of signage and insecurity of public roads were identified. Lack of spaces for the practice of physical activity was found. There is only one presence of 10% of sports facilities in the 20 AGEBS studied. It was found that 82.86% of the streets are in poor condition. Poor street lighting, lack of bicycle paths and graffiti with significant percentages were also identified as barriers to PA. Discussion: In Cuernavaca the possibilities of access to safe spaces for FA have multiple barriers, this negatively affects the health of the habitants of the city, and that is not consistent with global recommendations for physical activity. Conclusions: Identifying and abolishing barriers must be within the priorities of public health policies and interventions should seek this transformation, to improve the tendency to only report on benefits of physical activity prevailing in the vast majority of programs.

OBJECTIVE: To describe a political mapping on discrimination and homophobia associated to human immunodeficiency virus (HIV) in the context of public institutions in Mexico. MATERIALS AND METHODS: The political mapping was conducted in six Mexican states. Stakeholders who were involved in HIV actions from public and private sectors were included. Semistructured interviews were applied to explore homophobia and discrimination associated with HIV. Information was systematized using the Policy Maker software, which is a good support for analyzing health policies. RESULTS: Discriminatory and homophobic practices in the public domain occurred, damaging people´s integrity via insults, derision and hate crimes. Most stakeholders expressed a supportive position to prevent discrimination and homophobia and some of them had great influence on policy-making decisions. It was found that state policy frameworks are less specific in addressing these issues. CONCLUSIONS: Homophobia and discrimination associated to HIV are still considered problematic in Mexico. Homophobia is a very sensitive issue that requires further attention. Also, an actual execution of governmental authority requires greater enforcement of laws against discrimination and homophobia.

RESUMO

Objetivo. Presentar un mapeo político sobre discriminación y homofobia asociadas con la epidemia del virus de la inmunodeficiencia humana (VIH) en entornos institucionales públicos. Material y métodos. Se realizó un mapeo político con actores clave en seis estados de México, a quienes se les aplicó una entrevista semiestructurada para explorar la discriminación y homofobia asociadas con el VIH. La información se sistematizó mediante matrices de contenido por categorías y el software PolicyMaker. Resultados. Se documentaron prácticas discriminatorias y homofóbicas lesivas a la integridad como la denostación, la burla y los crímenes de odio. La mayoría de actores se manifestaron en contra de la discriminación y la homofobia; algunos de ellos influyeron de manera importante en la formulación de políticas para prevenir la discriminación y la homofobia. Sin embargo, los marcos normativos estatales fueron poco específicos. Conclusiones. Son persistentes las prácticas discriminatorias y la homofobia asociadas con el VIH, por lo cual se requiere mayor atención en los estados y es necesario impulsar políticas para prevenirlas, así como la observancia del cumplimiento de las leyes estatales.

OBJECTIVE: Evaluative research projects for identifying good practice have been postponed regarding health system reform. This study was thus aimed at identifying health governance and social protection indicators. METHODS: This study involved evaluative research regarding the health system for the uninsured part of the population in six Mexican states. The primary data was obtained from in-depth interviews with key players from the participating states; official statistics and the results of a macro-project concerned with Mexican health and governance reform and policy was used for secondary. Atlas Ti and Policy Maker software were used for processing and analysing the data. RESULTS: A list of strengths and weaknesses was presented as evidence of health system governance. Accountability at federal level (even though not lacking) was of a prescriptive nature and a system of accountability and transparency regarding the assignment of resources and strategies for the democratisation of health in the states and municipalities was still lacking. CONCLUSIONS: All six states had low levels of governance and experienced difficulty in conducting effective reform programmes and strategies involving a lack of precision regarding the rules and roles adopted by different health system actors.

RESUMO

Este artículo presenta resultados de un estudio cuyo principal objetivo fue identificar los actores clave, sus roles, coaliciones y los espacios de interacción para el desarrollo de protección social en la salud de los migrantes. Se identificaron 10 categorías de actores clave para estrategias de salud para emigrantes, así como metas de política y estrategias binacionales de salud con diferentes niveles de factibilidad y efectividad. Un seguro público binacional de salud para emigrantes indocumentados es factible pero requiere una gran inversión y movilización de recursos públicos. Para ello se deberán desarrollar nuevas iniciativas dirigidas a resolver los problemas de equidad y acceso en la salud de los migrantes.

OBJETIVO: Analisar medidas municipais implementadas para o controle da epidemia da dengue, sobretudo as de coordenação setorial, governança e participação de grupos sociais. MÉTODOS: Estudo observacional, realizado em Morelos, México, 2007. Os dados coletados em entrevistas e observações diretas foram submetidos a análise de conteúdo e mapeamento político. O software Policy Maker foi utilizado para avaliar os pesos atribuídos aos dados do desempenho (i.e., critérios alto, médio e baixo) e papel de atores (ações realizadas, sejam elas de vigilância, controle, sejam administrativas). Foi realizada análise estratégica das oportunidades e desafios no cumprimento das políticas públicas e controle da dengue. RESULTADOS: As bases jurídicas indicam que a resposta à epidemia é uma tarefa multissetorial. Porém, a resposta está centrada em atividades dos serviços da saúde, que estão forçados a dar o maior apoio financeiro e a derivar os recursos humanos necessários, em contraste com a contribuição de outros setores (e.g., água e saneamento básico), que desconhecem as suas responsabilidades. O setor da saúde apresenta alto nível de factibilidade para a vinculação intra-institucional, em termos de otimização de recursos e cumprimento de objetivos, particularmente entre autoridades de saúde nos níveis estatal, jurisdicional, municipal e local. CONCLUSÕES: A abordagem multidisciplinar e o fortalecimento das responsabilidades políticas permitirão a resposta eficaz perante a epidemia da dengue, sustentada na coordenação setorial e envolvimento ativo da população afetada.

RESULTADOS: Las bases jurídicas indican que la respuesta a la epidemia es una tarea multisectorial. Sin embargo, la respuesta está centrada en actividades de los servicios de la salud, que están forzados a dar mayor apoyo financiero y derivar los recursos humanos necesarios, en contraste con la contribución de otros sectores (e.g. agua y saneamiento básico), que desconocen sus responsabilidades. El sector de la salud presenta alto nivel de factibilidad para la vinculación intra?institucional, en términos de optimización de recursos y cumplimiento de objetivos, particularmente entre autoridades de salud en los niveles estatal, jurisdiccional, municipal y local. CONCLUSIONES: El abordaje multidisciplinario y el fortalecimiento de las responsabilidades políticas permitirán la respuesta eficaz ante la epidemia del dengue, sustentada en la coordinación sectorial e involucramiento activo de la población afectada.

OBJECTIVE: To analyze municipal measures implemented to control the dengue epidemic, giving priority to the functions of intersectoral coordination, policy strengthening and community participation. METHODS: The observational study was performed in Morelos, Mexico in 2007. Data collected in interviews and direct observations were submitted to contents analysis and policy mapping. Policy Maker software was used to evaluate the values assigned to the stakeholders' performance (i.e., high, medium and low criteria) and to their role (actions undertaken for surveillance, control or management). A strategic analysis of opportunities and challenges regarding public policies and dengue control was conducted. RESULTS: The legal framework indicates that the approach to the dengue epidemic should be an intersectoral response. However, the burden of activities in terms of financial and human resources tends to fall on local health services, which contrasts with the contribution of other sectors (e.g., water and sanitation) that do not recognize their responsibilities. A high degree of operational feasibility links, in terms of optimization of resources and objectives fulfillment was detected, particularly among health authorities at state, jurisdictional and municipal levels. CONCLUSIONS: A multidisciplinary approach and strengthening of policy stewardship may allow a more efficient response to dengue outbreaks, sustained by intersectoral coordination and the active participation of the affected population.

Objetivo: Identificar los efectos de las reformas en servicios de salud sobre indicadores de financiamiento y gobernanza en sistemas de salud. Material y métodos: Se diseño una investigación evaluativa basada en el método “rapid assesment”. El universo de estudio fue el sistema de salud para no asegurados en México y la población de estudio se delimito a seis estados seleccionados bajo diferentes criterios de factibilidad técnica, política y financiera. Para los datos primarios se realizaron entrevistas a profundidad con informantes clave de los estados participantes; para los datos secundarios se utilizaron estadísticas oficiales y los resultados del proyecto macro sobre reformas, políticas de salud, equidad, financiamiento y gobernanza en México, 1999-2006. El procesamiento y análisis de datos se realizó con 2 paquetes de software: ATLAS Ti y POLICY MAKER. Resultados/Conclusiones: A partir de las reformas se han registrado importantes incrementos en los montos del financiamiento en salud, estos cambios también han coincidido con ganancia en los índices de corresponsabilidad financiera; Existe una nueva fórmula de asignación de recursos, donde el nivel estatal ha jugado un rol protagónico en su revisión, ajustes y aplicación para las adecuaciones sobre los recursos asignados y sobre el modelo de atención a la salud que cada estado adopta en función de sus necesidades; La estrategia de protección financiera en salud en algunos estados ha disminuido gastos catastróficos en salud y por consecuente el mayor empobrecimiento de las familias más vulnerables; La rendición de cuentas en el nivel federal, aunque no está ausente, se mantiene con un carácter prescriptivo, mientras que en los estados y municipios aún está ausente un sistema de rendición de cuentas y de transparencia en la asignación de recursos.


Objective: To identify effects of health system reform on heath financing and health governance. Methods and data sources: A design of evaluative research based on rapid assessment was developed. The research object it was the health system for uninsured population in Mexico, focusing on six States selected considering technical, political and financial feasibility. For primary data gathering structured interviews with key informants of selected States were done; for secondary data, official statistical data and research results of the macro project on heath reform, health policy, equity, financing and health governance, 1999-2006. Information processing and analysis were done by using Atlas-Ti and Policy Maker software. Results / Conclusions: Health system reforms have showed strong influence in financing amounts and important gains in financial co-responsibility. An actualized formula for resources allocation was identified; this formula has involved States and local levels on its revision, adjustments and appliances on allocated resources and health services provision model adopted for the States in relation with their health needs. Financial protection strategy has reported protection versus catastrophic health expenditure and vulnerable families’ impoverishment in some States. Accountability in federal government it is not absent, but it functions as a prescriptive policy, while transparency and accountability mechanisms at the States and municipalities are absent.

OBJECTIVE: To evaluate the implementation of its participative strategies and the creation of support networks for poor pregnant women. MATERIAL AND METHODS: A qualitative and comparative evaluation was carried on in four states. RESULTS: Coordination and community participation were relevant in relation with major resources allocation and availability, particularly housing and transportation. Governmental actors involvement and leadership favoured linking and coordination. Pregnant women used to valuate as the major support source the one provided by their kinship networks. CONCLUSIONS: To strengthen and to stimulate participative strategies is fundamental in zones with high maternal mortality rates. The wide appreciation of kinship networks, midwives and voluntaries' support to pregnant women in housing and transportation, suggests that these actors are a functional component of the support network; it is insufficient focusing the support network on health services and municipal authorities.
