OBJECTIVE: To compare dexmedetomidine and fentanyl constant rate infusions in anesthetic protocols for septic dogs with pyometra, using microcirculatory, hemodynamic and metabolic variables. STUDY DESIGN: Randomized clinical study. ANIMALS: A total of 33 dogs with pyometra with two or more systemic inflammatory response syndrome variables undergoing ovariohysterectomy. METHODS: Dogs were randomized into two groups: group DG, dexmedetomidine (3 µg kg-1 hour-1; 17 dogs) and group FG, fentanyl (5 µg kg-1 hour-1; 16 dogs) infused during isoflurane anesthesia and mechanical ventilation. Microcirculation flow index (MFI), total vessel density and De Backer score were assessed using orthogonal polarization spectral imaging at the sublingual site. Heart rate, invasive blood pressure, temperature, arterial blood gas analysis and lactate concentration were obtained at various time points. Variables were recorded at baseline (BL), immediately before (T0), 30 (T30) and 60 (T60) minutes after infusion, and 60 minutes after surgery. Data were analyzed using the Shapiro-Wilk test. To compare variables between groups, the unpaired Student t test was used. Comparison between evaluation time points was performed with two-way anova for repeated measures. Where statistical significance was detected, the Bonferroni post hoc test was used. RESULTS: MFI was significantly higher in group FG at T30. Mean arterial pressure at T30 was higher in group DG (89 ± 15 mmHg) than in group FG (72 ± 13 mmHg). Lactate concentrations were not significantly different between groups at each time point. Both groups had similar clinical outcomes (mortality, extubation time and occurrence of hypotension and bradyarrhythmias). CONCLUSIONS AND CLINICAL RELEVANCE: Dexmedetomidine (3 µg kg-1 hour-1) without a loading dose can be included in the maintenance of anesthesia in dogs with pyometra and sepsis without compromising microcirculation and hemodynamic values when compared with fentanyl (5 µg kg-1 hour-1).

OBJECTIVES: Web-based interventions for the promotion of maternal mental health could represent a cost-effective strategy to reduce the burden associated with perinatal mental illness. This study aimed to evaluate the cost-utility of Be a Mom, a self-guided web-based cognitive behavioral therapy intervention, compared with a waiting-list control. METHODS: The economic evaluation alongside a randomized controlled trial was conducted from a societal perspective over a 14-month time frame. Postpartum women presenting low risk for postpartum depression were randomized to the intervention (n = 191) or control (n = 176) group and assessed at baseline, postintervention and 4 and 12 months after postintervention. Data regarding healthcare use, productive losses and quality-adjusted life years (QALYs) were collected and used to calculate incremental cost-effectiveness ratios (ICERs). Uncertainty was accounted for with nonparametric bootstrapping and sensitivity analyses. RESULTS: At 14 months, and after accounting for a 3.5 percent discount rate, the intervention resulted in a yearly cost-saving of EUR 165.47 (-361.77, 28.51) and a QALY gain of 0.0064 (-0.0116, 0.0244). Bootstrapping results revealed a dominant ICER for the intervention group. Although results were statistically nonsignificant, cost-effectiveness acceptability curves showed that at a EUR 0 willingness to pay threshold, there is a 96 percent probability that the intervention is cost-effective when compared with the control group. The sensitivity analyses generally supported the acceptable likelihood of the intervention being more cost-effective than the control group. CONCLUSIONS: From a societal perspective, the implementation of Be a Mom among low-risk postpartum women could be a cost-effective way to improve perinatal mental health.

BACKGROUND: During protective mechanical ventilation, electrical impedance tomography (EIT) is used to monitor alveolar recruitment maneuvers as well as the distribution of regional ventilation. This technique can infer atelectasis and lung overdistention during mechanical ventilation in anesthetized patients or in the ICU. Changes in lung tissue stretching are evaluated by monitoring the electrical impedance of lung tissue with each respiratory cycle. OBJECTIVE: This study aimed to evaluate the distribution of regional ventilation during recruitment maneuvers based on the variables obtained in pulmonary electrical impedance tomography during protective mechanical ventilation, focusing on better lung recruitment associated with less or no overdistention. METHODS: Prospective clinical study using seven adult client-owned healthy dogs, weighing 25 ± 6 kg, undergoing elective ovariohysterectomy or orchiectomy. The animals were anesthetized and ventilated in volume-controlled mode (7 ml.kg-1) with stepwise PEEP increases from 0 to 20 cmH2O in steps of 5 cmH2O every 5 min and then a stepwise decrease. EIT, respiratory mechanics, oxygenation, and hemodynamic variables were recorded for each PEEP step. RESULTS: The results show that the regional compliance of the dependent lung significantly increased in the PEEP 10 cmH2O decrease step when compared with baseline (p < 0.027), and for the nondependent lung, there was a decrease in compliance at PEEP 20 cmH2O (p = 0.039) compared with baseline. A higher level of PEEP was associated with a significant increase in silent space of the nondependent regions from the PEEP 10 cmH2O increase step (p = 0.048) until the PEEP 15 cmH2O (0.019) decrease step with the highest values at PEEP 20 cmH20 (p = 0.016), returning to baseline values thereafter. Silent space of the dependent regions did not show any significant changes. Drive pressure decreased significantly in the PEEP 10 and 5 cmH2O decrease steps (p = 0.032) accompanied by increased respiratory static compliance in the same PEEP step (p = 0.035 and 0.018, respectively). CONCLUSIONS: The regional ventilation distribution assessed by EIT showed that the best PEEP value for recruitment maintenance, capable of decreasing areas of pulmonary atelectasis in dependent regions promoting less overinflation in nondependent areas, was from 10 to 5 cmH2O decreased steps.

OBJECTIVE: To evaluate the accuracy of steady-state free precession (SSFP) unenhanced magnetic resonance angiography (MRA) at 1.5 T for the identification of multiple renal arteries, using computed tomography angiography (CTA) as the reference standard. MATERIALS AND METHODS: This was a prospective study involving 39 patients (26 males; mean age, 62.6 years) who underwent CTA and unenhanced MRA to evaluate the proximal and middle segments of the renal arteries. The analysis was performed in two phases: the quality of unenhanced MRA images was classified as diagnostic or nondiagnostic for the presence of multiple renal arteries by two independent readers; two other independent readers then evaluated the images previously classified as being of diagnostic quality. The sensitivity, specificity, and overall accuracy of unenhanced MRA were calculated, CTA being used as the reference standard. The kappa statistic was used in order to calculate interobserver agreement. RESULTS: The image quality of unenhanced MRA was considered diagnostic in 70-90% of the extrarenal arterial segments. The CTA examination revealed 19 multiple renal arteries (8 on the right and 11 on the left). The accuracy of unenhanced MRA for the identification of multiple renal arteries was greater than 90%, with a sensitivity of 72.7-100% and a specificity of 96.3-100%. CONCLUSION: Unenhanced MRA provides high quality imaging of the extrarenal segments of renal arteries. This method may be used as an alternative for the evaluation of the renal arteries, given that it has an accuracy comparable to that of CTA.

MATERIAIS E MÉTODOS: Trinta e nove pacientes (26 do sexo masculino; idade média de 62,6 anos) foram submetidos a angio-TC e angio-RM para avaliação dos segmentos proximais e médios das artérias renais. A análise dividiu-se em duas partes: inicialmente, a qualidade das imagens de angio-RM foi classificada como diagnóstica ou não diagnóstica por dois avaliadores independentes; posteriormente, outros dois avaliadores analisaram as imagens consideradas diagnósticas quanto à presença de múltiplas artérias renais. Sensibilidade, especificidade e acurácia da angio-RM foram calculadas utilizando-se a angio-TC como referência. Estatística kappa foi utilizada para cálculo da concordância interobservador. RESULTADOS: A qualidade das imagens de angio-RM foi considerada diagnóstica em 70-90% dos segmentos arteriais extrarrenais. A angio-TC revelou 19 artérias renais múltiplas (8 direitas e 11 esquerdas). A acurácia da angio-RM para identificação de artérias renais múltiplas foi maior que 90%, com sensibilidade variando de 72,7% a 100% e especificidade variando de 96,3% a 100%. CONCLUSÃO: A angio-RM sem contraste proporciona imagens de alta qualidade dos segmentos extrarrenais das artérias renais. Este método pode ser alternativamente utilizado para avaliação renal, com acurácia comparável à da angio-TC.

Adolescent pregnancy remains a public health concern in both developed and developing countries. Portugal and Brazil represent some of the best examples of this phenomenon. The present study aimed to identify sociodemographic, sexual, and reproductive health-related variables associated with adolescent pregnancy among students from low socioeconomic backgrounds in both countries. The sample included 984 female adolescents, among whom 215 became pregnant. Living with a partner and lack of information about sex and contraception from the family were the best explicative factors for pregnancy occurrence in both countries. Country-specific variables were also identified. Our results may contribute to developing global preventive interventions, addressing the school as an ideal setting for primary intervention and considering culture-specific characteristics of high-risk populations.

INTRODUCTION: Better knowledge about fertility desires/intentions among HIV-serodiscordant partners who face unique challenges when considering childbearing may be helpful in the development of targeted reproductive interventions. The aim of this systematic review was to synthesize the published literature regarding the prevalence of fertility desires/intentions and its associated factors among individuals in HIV-serodiscordant relationships while distinguishing low- and middle-income countries (LMIC) from high-income countries (HIC). METHODS: A systematic search of all papers published prior to February 2017 was conducted in four electronic databases (PubMed/MEDLINE, PsycINFO, Web of Science and Cochrane Library). Empirical studies published in peer-reviewed journals with individuals in HIV-serodiscordant relationships assessing the prevalence of fertility desires/intentions and/or the associated factors were included in this systematic review. This review adhered to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. RESULTS AND DISCUSSION: After screening 1852 references, 29 studies were included, of which 21 were conducted in LMIC and eight in HIC. A great variability in the prevalence of fertility desires/intentions was observed in LMIC (8% to 84% (one member of the dyad included)). In HIC, the results showed a smaller discrepancy between in the prevalence (32% to 58% (one member of the dyad included)); the prevalence was higher when the couple was the unit of analysis (64% to 73%), which may be related to the fact that all these studies were conducted in the context of assisted reproduction. Few studies examined the factors associated with fertility desires/intentions, and all except one were conducted in LMIC. Individuals (e.g. number of children), couple-level (e.g. belief that the partner wanted children) and structural factors (e.g. discussions with health workers) were found to be associated. CONCLUSIONS: The results of this systematic review suggest that many individuals in HIV-serodiscordant relationships have fertility desires/intentions, although the prevalence is particularly heterogeneous in LMIC in comparison to HIC. Well-known factors such as younger age and a fewer number of living children were consistently associated with increased fertility desires/intentions. Different couple-level factors emerged, reflecting the importance of considering both the individual and the couple. However, further studies that specifically focus on the dyad as the unit of analysis are warranted.

Granites are widely used in construction and they may be potential sources of ionizing radiation, due to the presence of radionuclides such as 40Kand decay products from 238Useries and 232Thseries. These radionuclides occur in the minerals constituting the rocks. To evaluate the doses in humans exposed to 40K, and decay products from 238Useries and 232Thseries γ radiation, a room with dimensions of 4.0 × 5.0 × 2.8 m3, with uniformly distributed radiation source on the floor of granitic rocks, was computationally modeled. Adult individuals were represented in the virtual scenario by two virtual anthropomorphic phantoms FASH3 and MASH3, incorporated simultaneously in the software MCNPX 2.7.0. The mean energy deposited on each organ and tissue of FASH3 and MASH3 phantoms was determined using the MCNPX F6 tally (MeV/g/particle), while the photon flux within the room was calculated with the MCNPX F4 tally (MeV/cm2/particle). The organs that obtained the highest conversion coefficients CC[HT](Sv/Gy) were the red bone marrow (0.94), skin (0.90), breast (0.81) and bladder (0.73) for the FASH3; skin (0.89), gonads (0.88), breast (0.79) and bladder (0.70) for the MASH3. The simulated air absorbed dose rates varied between 23.4 (11%) and 25.8 (12%) nGy/h, and the annual dose rates were 0.10 (6%) and 0.11 (6%) mSv/year. These results presented acceptable statistical uncertainties and they are in agreement with the literature. Fluency of photons pointed to the central region of the room floor as the place of greatest exposure. The results showed that the organs closer to the radiation source had the highest deposited energy values. Based on the annual effective dose data obtained, it was possible to note that the values are within the literature. We believe that the methodology used will allow the investigation of any ornamental material that emits natural radiation.

The Plenary of the National Health Council (CNS) approved the Resolution 568/2017, convening the 16th National Health Conference to be held in 2019 and decided to promote thematic activities in eight areas, including Pharmaceutical Policy and Science and Technology. CNS partnership with FIOCRUZ and the National School of Pharmacists proposed the 8th National Symposium on Science and Technology and Pharmaceutical Policy, preceded by ten regional preparatory meetings for the symposium throughout the country. The purpose of this article is to present and analyze the results of the first stage of meetings. A participatory methodology was developed for the meetings that included the presentation of problem situations reported in the form of "cases" built by real narratives, fictitious or adapted from reality. Debates in groups and proposals construction, compilation and weighting, with general discussion completed the meetings. The set of 150 proposals from the 5 meetings was read individually by each of the members of the analysis team and pre-categorized. The 5 meetings had a total of 238 participants. Four categories were defined: Health as a right; Consolidation of SUS principles; Adequate and sufficient financing for SUS; Participatory democracy.


O Pleno do Conselho Nacional de Saúde (CNS) aprovou a Resolução nº 568/2017 que convoca a 16ª Conferência Nacional de Saúde, a ser realizada no ano de 2019 e deliberou pela realização de atividades temáticas, em oito áreas, incluindo Assistência Farmacêutica e Ciência e Tecnologia. Uma parceria do CNS com a Fiocruz e a Escola Nacional dos Farmacêuticos propôs a realização do 8º Simpósio Nacional de Ciência e Tecnologia e Assistência Farmacêutica, antecedido por dez encontros regionais preparatórios para o simpósio em todo o país. O objetivo deste artigo é apresentar e analisar os resultados dessa primeira etapa de encontros. Foi desenvolvida uma metodologia participativa para os encontros que inclui a apresentação de situações-problema em forma de "casos" com narrativas reais, fictícias ou adaptadas da realidade. Debates em grupos e construção de propostas, compilação e ponderação, com discussão geral completaram os encontros. As 150 propostas oriundas de cinco encontros foram lidas por cada membro da equipe de análise e pré-categorizado. Esses encontros reuniram 238 participantes. Quatro categorias foram definidas: Saúde como direito; Consolidação dos princípios do SUS; Financiamento adequado e suficiente para o SUS; Democracia participativa.

Although pain is considered the 4th vital sign and one of the most frequently observed clinical signs in domestic animals' clinical practice, its treatment is still inadequate despite significant improvement in the last few years. Acute post-operative pain has aroused great interest due to its potential risk of developing into chronic pain, and if not treated properly, it might worsen the recovery and the patient's quality of life. Cats are one of the least studied species of domestic animals regarding pain recognition and control. Some of the difficulties lie in pain assessment and perception. The consensus published in February 2016 about behavioral signs of pain in cats considered some signs to be reliable and sensitive for the assessment of pain in this species in many different clinical conditions, however it still states that more studies will be necessary in order to evaluate its clinical validity and applicability, especially considering the various pain intensities. As an attempt to quantify pain intensity in cats, several types of traditional subjective scales and others that facilitate pain assessment by combining the observation of spontaneous behavioral signals of pain and qualitative response to palpation of surgical wound are used as tools. It is necessary to use specific scales for each type of pain and for each specific animal species so to minimize the subjectivity and the partiality of the observers, reducing bias and improving efficacy, thus leading to a better patient care.(AU)


Embora a dor seja considerada o quarto sinal vital e uma das manifestações mais comumente encontradas na prática médica veterinária dos animais domésticos, seu tratamento ainda é inadequado. A dor aguda pós-operatória tem suscitado grande interesse por seu potencial risco de cronificação caso não adequadamente tratada, podendo piorar a recuperação e a qualidade de vida do paciente. O gato é uma das espécies domésticas menos estudadas no que diz respeito ao reconhecimento e controle da dor, e algumas das dificuldades residem na avaliação e na percepção da dor. O consenso sobre os sinais comportamentais da dor nesta espécie publicado em fevereiro de 2016 considerou alguns sinais como confiáveis e sensíveis para a avaliação da dor em gatos, em toda uma gama de diferentes condições clínicas, porém afirma a necessidade da realização de estudos que analisem a sua validade e aplicabilidade clínica, especialmente em relação a diferentes intensidades de dor. Na tentativa de se quantificar a dor são utilizados vários tipos de escalas subjetivas tradicionais e outras que facilitam a avaliação da efetividade da analgesia, a partir da observação de sinais comportamentais espontâneos indicativos de dor, combinada a uma resposta qualitativa à palpação da ferida cirúrgica. Faz-se necessária a utilização de escalas específicas para o tipo de dor (aguda ou crônica) e para a espécie, de modo a minimizar a subjetividade e a parcialidade dos observadores e possibilitando uma melhor assistência ao paciente.(AU)

This study compared the quality of life (QoL) of HIV-infected patients with and without hepatitis C and examined the sociodemographic, HIV-related and psychological symptoms associated with the QoL domains in patients with HIV/HCV co-infection. The sample consisted of 248 HIV/HCV co-infected patients (18-74 years, 81.5 % male) and 482 patients only with HIV (24-78 years, 62.7 % male). Participants completed the WHOQOL-HIV-Bref questionnaire and the Brief Symptom Inventory. The HIV/HCV co-infected patients reported significantly lower QoL in all domains, as well as significantly lower scores in 10 of the 17 specific facets. Overall, among the co-infected patients, male gender, employment, combination antiretroviral therapy use and fewer depressive and anxiety symptoms were significantly associated with higher QoL. Symptoms of psychological distress accounted for significant variability in the QoL scores of co-infected patients. These data reinforce the need for tailored interventions to improve the overall well-being of HIV/HCV co-infected patients.

No Brasil, o direito de acesso à saúde é um preceito constitucional garantido enquanto dever do Estado, com base em princípios que preveem que este seja universal, integral, igualitário e com controle social. Desta forma, a saúde deve ser compreendida enquanto política pública que articule ações que compreendam os diferentes níveis de complexidade de atenção e a intersetorialidade inerente a este setor.A assistência farmacêutica, prevista enquanto campo de atuação do SUS, está organizada com base em uma Política Nacional aprovada no âmbito do Conselho Nacional de Saúde. As diversas ações que garantam o acesso aos medicamentos essenciais, de forma qualificada e racional, devem ser vistas enquanto parte integrante da Política Nacional de Saúde. Nesta lógica, em 2004, foi criado o Programa Farmácia Popular do Brasil - PFPB. Em caráter complementar às demais ações executadas no campo da assistência farmacêutica, o Programa foi concebido enquanto uma alternativa de acesso aos medicamentos, buscando atender àqueles que pudessem despender de um pequeno valor pelo seu medicamento, permitindo desta forma que os usuários de medicamentos não tivessem interrompido seu tratamento em virtude de dificuldades de acesso. Em 2006, além das parcerias com Estados e Municípios, o Programa foi ampliado através de parcerias com farmácias e drogarias, chamado de “Aqui Tem Farmácia Popular”. Considerando a dimensão do Programa, este trabalho objetivou analisar o Programa Farmácia Popular do Brasil enquanto alternativa de acesso aos medicamentos e sua articulação com as demais ações estratégicas do MS, com base na Política Nacional de Medicamentos e na Política Nacional de Assistência Farmacêutica. Além disso, analisar a relação entre Programa Farmácia Popular do Brasil e o Complexo Econômico- Industrial da Saúde - CEIS, com base na dimensão da dispensação e investimentos feitos pelo Ministério da Saúde, em especial no fortalecimento da oferta de medicamentos genéricos. Ao considerarmos os resultados de capilaridade do Programa, ampliação do número de pessoas atendidas, os valores gastos pelo MS e estímulo aos medicamentos genéricos, é possível afirmar que estes apontam que o PFPB articula-se com o CEIS, na sua dimensão social e econômica, e com as demais ações estratégicas do Governo, fortalecendo o acesso os medicamentos.

The assessment of quality of life (QOL) in HIV infection has emerged as being vital to research and clinical practice. This assessment is also a challenge due to the specific characteristics of the infection, the increased availability of therapeutics, as well as the epidemiological variability inherent to HIV infection. The purpose of this study was to investigate the psychometric properties of the European Portuguese version of the World Health Organization's QOL Instrument in HIV Infection (WHOQOL-HIV) and to test its performance in a sample of HIV-infected patients. The European Portuguese version of WHOQOL-HIV was administered in a sample of 200 HIV-positive patients. The patients also completed the Portuguese versions of Beck Depression Inventory (BDI) and Brief Symptom Inventory (BSI). The WHOQOL-HIV showed quite an acceptable internal consistency (Cronbach's α ranged from 0.86 to 0.95 across domains). Convergent validity with BDI and BSI was satisfactory for all domains (all r>0.50; p<0.001). Moreover, correlations between domains and between domains and overall QOL were all statistically significant (p<0.001). The reliability and validity studies of the European Portuguese version of the WHOQOL-HIV revealed good psychometric characteristics, which allows for the use of this version of WHOQOL in our country, and cross-cultural comparability.

Abortamento é a expulsão ou extração de um produto da concepção sem sinais de vida com menos de 500 gramas ou 22 semanas de gestação. O aborto infectado ou séptico decorre da eliminação incompleta do ovo, do embrião ou da placenta, que mantém aberto o canal cervical, favorecendo a ascensão de bactérias da microbiota vaginal e intestinal à cavidade uterina. A real magnitude do abortamento é desconhecida devido à sua ilegalidade. No Brasil, há subnotificação de casos e complicações, pois nem todas as mulheres demandam assistência pós-abortamento. Ainda assim, os dados oficiais justificam a adoção de medidas preventivas e de promoção da saúde reprodutiva. O diagnóstico de aborto séptico deve ser considerado quando uma mulher na menacme apresentar atraso menstrual, sangramento vaginal, febre, dor hipogástrica e à mobilização do colo uterino. Exige tratamento imediato. Prefere-se a associação gentamicina/clindamicina.


Abortion is the expulsion or extraction of a product of conception, with no signs of life, with less than 500 grams or 22 weeks of gestation. Infected or septic abortion stems from partial removal of the egg, embryo or placenta, which keeps open the cervical channel, encouraging the rise of intestinal and vaginal bacteria to the uterine cavity. The real magnitude of abortion is unknown due to its illegality. In Brazil, there is underreporting of cases and complications because not all women require assistance in post-abortion care services. Still, official data justify the adoption of preventive measures and of reproductive healthÆs promotion. The diagnosis of septic abortion should be considered when a woman of childbearing age has missed period, vaginal bleeding, fever, hypogastric pain and pain by the cervix mobilization. It requires immediate treatment. The association of gentamicin/clindamycin is chosen.

BACKGROUND: At the beginning of the 1990s, the World Health Organization (WHO) developed a project in order to create a cross-cultural instrument of quality of life assessment: the World Health Organization Quality of Life (WHOQOL). PURPOSE: This paper describes the development of the European Portuguese version of the WHOQOL-100, according to the methodology recommended by the WHO. METHOD: Special attention is given to the qualitative pilot study, which led to the development of the Portuguese Facet [Political P], and to the empirical pilot study and the psychometric studies, based on the application of the Portuguese version of the instrument to a sample of 315 subjects from the general population and 289 patients. The assessment protocol also included the Beck Depression Inventory and the Brief Symptom Inventory. RESULTS: The Portuguese version of WHOQOL-100 showed acceptable internal consistency (alpha range 0.84-0.94) and test-retest reliability in all domains (r range 0.67-0.86). Discriminant validity was significant for all domains, except in Spirituality. Convergent validity with the Beck Depression Inventory and the Brief Symptom Inventory was satisfactory for most domains. CONCLUSION: The WHOQOL showed good psychometric characteristics, suggesting that the Portuguese version of WHOQOL is valid and reliable in the assessment of quality of life in Portugal.
