The objective of the present study was to evaluate the association between social position and anthropometric status in women and men Brazilian adult. This was a cross-sectional study that used baseline data collected from 2008 to 2010 for the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil, in Portuguese), in the six major Brazilian state capital cities. A total of 15,105 active and retired civil servants aged from 35 to 74 years. Two latent variables were defined by latent class analysis, social position and anthropometric status. Both constructs and the analyses were separately evaluated by sex. Associations were assessed using multivariate logistic regression analysis with adjustment for age, self-reported skin color/race, and marital status. Around 44% of the women and 26% of the men were classified as overweight or obese. Social position tended to be lower in women (43.2%) and higher among men (40.4%). Heavier women were more likely to be black and brown-skinned, whereas slimmer women were more likely to be white. After adjustment, women's weight increased as social position decreased (OR = 1.52; 95%CI: 1.36-1.70), whereas in men weight decreased as social position decreased (OR = 0.87; 95%CI: 0.76-0.99). Social position affected the anthropometric status of women and men differently, with body patterns also being affected by ethnicity/skin color, showing the potentiality of taking the intersectional perspective when investigating the possible social determinants of the phenomenon.

Resumo: O objetivo do estudo foi avaliar a associação entre posição social e o estado antropométrico em brasileiros adultos de ambos os sexos. O estudo transversal usou dados coletados entre 2008 e 2010 pelo Estudo Longitudinal de Saúde do Adulto (ELSA-Brasil), nas seis maiores capitais brasileiras. Um total de 15.105 funcionários públicos, ativos e aposentados, de ambos os sexos, entre 35 e 74 anos de idade. Duas variáveis latentes foram definidas pela análise de classes latentes: posição social e estado antropométrico. Os construtos e análises foram avaliados separadamente por sexo. As associações foram avaliadas com o uso de análise de regressão logística multivariada, ajustada para idade, cor/raça e estado civil. Em torno de 44% das mulheres e 26% dos homens foram classificados com sobrepeso ou obesidade. A posição social tendia a ser mais baixa nas mulheres (43,2%) e mais alta nos homens (40,4%). Houve uma proporção maior de mulheres com sobrepeso ou obesidade entre as pretas e pardas, e proporção maior de mulheres magras entre as brancas. Nas mulheres, após ajustes, o peso aumentava na medida em que a posição social diminuía (OR = 1,52; IC95%: 1,36-1,70), enquanto nos homens o peso diminuía junto com a diminuição da posição social (OR = 0,87; IC95%: 0,76-0,99). A posição social afetou de maneira diferente o estado antropométrico de mulheres e homens, com perfis corporais afetados também pela raça/cor da pele, indicando o potencial de levar em conta a perspectiva interseccional ao investigar os possíveis determinantes sociais do fenômeno.


Resumen: El objetivo de este estudio fue evaluar la asociación entre posición social y estatus antropométrico de adultos brasileños de ambos sexos. Fue un estudio transversal, realizado usando datos de referencia recogidos entre 2008 y 2010, del Estudio Longitudinal Brasileño de Salud en Adultos (ELSA-Brasil), llevado a cabo en seis de las mayores capitales de estado brasileñas. Un total de 15.105 activos y jubilados, mujeres y hombres funcionarios públicos de 35 a 74 años de edad. Se definieron dos variables latentes mediante análisis de clases latentes: posición social y estatus antropométrico. Ambos constructos y análisis fueron evaluados separadamente por sexo. Las asociaciones fueron evaluadas usando una regresión logística multivariada con ajuste por edad, color de piel/raza autoinformado y estatus marital. Alrededor de un 44% de las mujeres y un 26% de los hombres fueron clasificados como con sobrepeso u obesos. La posición social tendió a ser más baja en mujeres (43,2%) y más alta entre hombres (40,4%). Las mujeres con más peso tenían más probabilidad de ser negras y mulatas/mestizas y las mujeres más delgadas tenían más probabilidad de ser blancas. En mujeres, tras el ajuste, se incrementó más el peso cuanto mayor decrecía la posición social (OR = 1,52; IC95%: 1,36-1,70), mientras en hombres el peso decrecía al igual que la posición social (OR = 0,87; IC95%: 0,76-0,99). La posición social afectó diferentemente al estatus antropométrico de mujeres y hombres, con los patrones corporales también estando afectados por etnicidad/color de piel, mostrando su potencialidad tomando en consideración la perspectiva transversal, cuando se está investigando los posibles determinantes sociales del fenómeno.

PURPOSE: To analyze the factors associated with commuting by bicycling and walking in adult participants from ELSA-Brasil (Longitudinal Study of Adult Health). DESIGN: Cross-sectional. SETTING: Six teaching/research institutions throughout Brazil. PARTICIPANTS: A total of 15 105 civil servants. MEASURES: Commuting by bicycling and walking was analyzed using the long-form International Physical Activity Questionnaire. ANALYSIS: A hierarchical model containing possible factors associated with commuting by bicycling and walking was constructed. Crude and adjusted odds ratios were calculated using multinomial logistic regression. RESULTS: Considering the 2 forms of commuting, 66% of the participants were being considered inactive or insufficiently active. In women, being "heavier," feeling unsafe practicing physical activity, and being a former smoker were factors negatively associated with commuting by bicycling and walking. In men, active commuting was less common among those who were overweight or had abdominal obesity, those with a negative perception of safety, and those reporting that there was nowhere suitable in the neighborhood to practice physical activity. CONCLUSION: Obesity and negative perceptions in the neighborhood are associated with inactive or insufficiently active commuting. The relevance of this finding for public health is reinforce developing policies aimed at promoting health in Brazil and in other countries with similar characteristics.

CONTEXT AND OBJECTIVE:: The Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) was conducted among civil servants at six higher education institutions located in six Brazilian state capitals. The objective of this review was to identify the publications produced within the scope of ELSA-Brasil that analyzed the participants' physical activity. DESIGN AND SETTING:: Review study using baseline data from ELSA-Brasil. METHODS:: Narrative review of Brazilian studies on physical activity produced using data from ELSA-Brasil participants. RESULTS:: The prevalence of leisure-time physical activity (LTPA) among ELSA-Brasil participants was low (44.1% among men and 33.8% among women). The main factors associated were social (higher schooling and family income), environmental (living in places with conditions and opportunities for physical activity) and individual (not being obese, being retired, not smoking and positive perception of body image). The perception of facilities for walking in the neighborhood was positively associated with both LTPA and commuting-related physical activity. An active lifestyle was a protective factor against several cardiometa-bolic variables (hypertension, diabetes, lipid abnormalities and cardiovascular risk over the next 10 years). Comparison between LTPA and commuting-related physical activity showed that only LTPA had a protective effect against arterial hypertension. CONCLUSIONS:: The prevalence of physical activity among ELSA-Brasil participants was low. The main determinants were social, environmental and personal. LTPA had a greater protective efect on cardio-metabolic outcomes than did commuting-related physical activity.

RESUMO CONTEXTO E OBJETIVO: O Estudo Longitudinal de Saúde do Adulto (ELSA-Brasil) é um estudo longitudinal com servidores públicos de seis instituições de nível superior localizadas em seis capitais brasileiras. O objetivo desta revisão foi identifcar as publicações realizadas no âmbito do ELSA-Brasil que tenham analisado a atividade física dos participantes. TIPO DE ESTUDO E LOCAL: Estudo de revisão com dados da linha de base do ELSA-Brasil. MÉTODOS: Revisão narrativa dos estudos sobre atividade física no Brasil produzidos com dados de participantes do ELSA-Brasil. RESULTADOS: A prevalência da atividade física no tempo livre (AFTL) em participantes do ELSA-Brasil foi baixa, (44,1% em homens e 33,8% em mulheres). Os principais fatores associados foram de ordem social (maior escolaridade e renda familiar), ambiental (viver em locais com condições e oportunidades para prática de atividade física) e individual (não ser obeso, ser aposentado/a, não ser tabagista, e ter percepção positiva da imagem corporal). A percepção de facilidades para caminhar na vizinhança foi positivamente associada tanto a AFTL quanto a atividade física no deslocamento (AFD). O estilo de vida ativo fsicamente foi fator de proteção para diversas variáveis cardiometabólicas (hipertensão arterial, diabetes, alterações lipídicas e risco cardiovascular nos próximos 10 anos). Após comparação entre AFTL e AFD, observou-se que apenas a AFTL apresenta efeito protetor para hipertensão arterial. CONCLUSÕES: A prevalência da atividade física em participantes do ELSA-Brasil foi baixa, os principais determinantes foram de ordem social, ambiental e pessoal. A AFTL apresentou maior efeito de proteção para desfechos cardiometabólicos do que a AFD.

OBJECTIVE: To analyze the prevalence and sociodemographic factors associated with different domains of physical activity among adults of black ethnicity. METHODS: Cross-sectional study with a sample of 2,305 black individuals from 20-96 years of age, 902 (39.1%) of which men living in the city of Salvador, Bahia, Brazil. Sociodemographic variables analyzed were: gender, age, schooling, socioeconomic status (SES), marital status, racial discrimination at work/school (RDWE), in public (RDPUP) and private (RDPRP) places and perception of police (PPN) /violence in the neighborhood (PVN), as well as total physical activity (TPA) in its different domains: leisure time (LTPA), work (WPA), commuting (CPA) and household activity (HPA). Associations were analyzed using chi-square tests, chi-square trend and prevalence ratio (PR). We also used logistic regression analysis to estimate the odds ratio (OR) with a 95%confidence interval. RESULTS: The proportions of active individuals were 39.1% for TPA, 11.2% for LTPA, 9.6% for WPA, 23.7% for CPA and 33.7% for HPA. TPA was positively associated with higher schooling and inversely associated with male gender, age over 60 years and with no PPN. LTPA was positively associated with male gender, higher schooling and higher SES. WPA was inversely associated with age over 60 years and positively associated with male gender, higher schooling and no PPN. CPA was inversely associated with age over 60 years and positively associated with male gender. HPA was inversely associated with male gender, age over 60 years and no PPN and positively associated with higher schooling and SES. CONCLUSION: Socio-demographic factors, particularly gender, age and schooling were associated with different domains of physical activity among adults of black ethnicity.

OBJETIVO: Analisar a prevalência e os fatores sociodemográficos associados aos diferentes domínios da atividade física em adultos de etnia negra. MÉTODOS: Estudo transversal com amostra de 2.305 adultos negros de 20 a 96 anos de idade, sendo 902 (39,1%) homens, residentes na cidade de Salvador, BA, Brasil. Foram analisadas as variáveis sociodemográficas sexo, idade, escolaridade, nível socioeconômico (NSE), estado civil, discriminação racial no trabalho/escola (DRTE), em ambientes públicos (DRAPU) e privados (DRAPR) e percepção de policiamento (PPB)/violência no bairro (PVB), além da atividade física total (AFT) e em diferentes domínios: tempo livre (AFTL), trabalho (AFTR), deslocamento (AFDL) e doméstico (AFDM). As associações foram analisadas por meio dos testes qui-quadrado, qui-quadrado de tendência e razão de prevalência (RP). Utilizou-se também regressão logística para estimar a razão de chances (RC) com intervalo de confiança de 95%. RESULTADOS: As proporções de indivíduos ativos foram 39,1% para AFT, 11,2% para AFTL, 9,6% para AFTR, 23,7% para AFDL e 33,7% para AFDM. A AFT associou-se positivamente com escolaridade e inversamente com sexo masculino, idade maior que 60 anos e com não PPB. A AFTL associou-se positivamente com sexo masculino, maior escolaridade e maior NSE. A AFTR associou-se inversamente à idade maior que 60 anos e positivamente com o sexo masculino, maior escolaridade e não PPB. A AFDL associou-se inversamente à idade maior que 60 anos e positivamente ao sexo masculino. A AFDM associou-se inversamente com o sexo masculino, idade maior que 60 anos e não PPB; e positivamente com maior escolaridade e NSE. CONCLUSÕES: Os fatores sociodemográficos, principalmente sexo, idade e escolaridade se mostraram associados aos diferentes domínios da atividade física em adultos de etnia negra.

INTRODUÇÃO: A atividade física apresenta efeito protetor contra doenças crônicas e fatores de risco cardiovascular; no entanto, o gasto calórico necessário para promover a prevenção do diabetes permanece especulativo. OBJETIVO: Analisar o gasto calórico dos diferentes domínios da atividade física (trabalho, deslocamento, atividade doméstica, tempo livre e atividade física total) como preditores da ausência de diabetes em adultos de ambos os sexos. MÉTODOS: Estudo transversal realizado na cidade de Lauro de Freitas, Bahia, Brasil (2007-2008) com amostra constituída por 522 indivíduos maiores de 18 anos, sendo 302 do sexo feminino e 220 do sexo masculino. Foram construídas curvas Receiver Operating Characteristic (ROC) e comparadas as áreas sob as mesmas, além de verificar-se a sensibilidade e especificidade para identificar os melhores pontos de corte entre os diferentes domínios da atividade física e a ausência de diabetes. Foi utilizado o intervalo de confiança a 95%. RESULTADOS: Entre os diferentes domínios de atividade física analisados encontrou-se significância estatística nas áreas sob a curva ROC para o tempo livre, deslocamento e para atividade física total. Observou-se também que o gasto calórico na atividade física total de 830kcal/semana quando analisados apenas os homens, e 1,774kcal/semana quando analisados homens e mulheres conjuntamente, foram os melhores pontos de corte para predizer a ausência de diabetes. CONCLUSÃO: A prática da atividade física deve ser sugerida em níveis adequados para indivíduos de ambos os sexos visando contribuir para a prevenção do diabetes.


BACKGROUND: Physical activity had a protective effect against chronic diseases and cardiovascular risk factors; however, the caloric expenditure necessary to promote diabetes prevention remains speculative. OBJECTIVE: To analyze the caloric expenditure of different domains of physical activity (work, commuting, household, leisure time and total physical activity) as predictors of the absence of diabetes in adults of both sexes. METHODS: This was a cross-sectional study in the town of Lauro de Freitas, Bahia, Brazil (2007 - 2008) with a sample of 522 individuals over 18 years of age; 302 female and 220 male. Receiver Operating Characteristic Curves (ROC) were constructed and the areas below them were compared. Additionally, the sensitivity and specificity to identify the best cutoff points among the different domains of physical activity and the absence of diabetes were verified. Confidence interval at 95% was used. RESULTS: Among the different domains of physical activity analysed, statistical significance was only found in the areas under the ROC curve for leisure time, commuting and total physical activity. Additionally, it was observed that the caloric expenditure in total physical activity ranging from 830 kcal/week and 1.774 kcal/week were the best cutoff points for predicting the absence of diabetes. CONCLUSION: Physical activity should be suggested at appropriate levels for individuals of both sexes to contribute to diabetes prevention.

A incidência de diabetes, aterosclerose e morte cardíaca súbita é bastante frequente em pessoas obesas, porém quando a obesidade está centralizada na região abdominal as repercussões negativas, tanto de ordem metabólica quanto cardiovascular são mais significativas.O objetivo do estudo foi analisar e comparar entre os indicadores antropométricos de obesidade abdominal mais utilizados, quais deles apresentam maior poder preditivo para discriminar risco coronariano elevado (RCE) e propor pontos de corte para sua utilizaçãona prática clínica e populacional em adultos brasileiros. Foram analisados os manuscritos publicados pelo grupo de pesquisa em doenças crônicas não transmissíveis do Instituto de Saúde Coletiva (ISC) da Universidade Federal da Bahia (UFBA) que compararam diversos indicadores antropométricos como preditores de RCE. As evidências com base nos estudos analisados permitem sugerir para avaliação da obesidade abdominal: na prática clínica, utilizar o índice de conicidade (Índice C) com os pontos de corte de 1,25 para homens, 1,18 e 1,22 para mulheres até 49 anos e a partir de 50 anos de idade, respectivamente; em estudos populacionais, utilizar a razão cintura-estatura (RCest) com a mensagem de que a cintura não deve ser maior que a metade da estatura de determinada pessoa.


The incidence of diabetes, atherosclerosis and sudden cardiac death is high among obese individuals, with significant metabolic and cardiovascular adverse effects being observed when obesity is centered in the abdominal region. The objective of this study was to determinewhich of the anthropometric indicators of abdominal obesity commonly used show the highest predictive power to discriminate a high coronary risk (HCR) and to propose cut-off values for their use in clinical practice and in population studies on Brazilian adults. The studies publishedby the research group on non-transmissible chronic diseases of the Public Health Institute (PHI), Federal University of Bahia (UFBA), that compare different anthropometric indicators as predictors of HCR were analyzed. The evidence provided by the studies analyzed suggests the use of the conicity index for the evaluation of abdominal obesity in clinical practice, with cut-off values of 1.25 for men and of 1.18 and 1.22 for women ? 49 years and > 50 years, respectively. The waist-height ratio should be used in population studies, with the recommendation that waist should not exceed half the height of a particular subject.
