BACKGROUND: Vitamin B12 (B12) has a fundamental role in both central and peripheral nervous system function at all ages. Neurologic manifestations may be the earliest and often the only manifestation of B12 deficiency. Mostly because of the poor sensitivity of methods of determination for B12 levels, peripheral neuropathy remains a classical but underdiagnosed complication of B12 deficiency. So the clinical and electrophysiological characteristics of B12-responsive neuropathy are not well known. METHODS: A retrospective study of patients with B12-responsive neuropathy was conducted at our hospital on a 3-year period. The criteria for inclusion were: (a) neuropathy confirmed by the electrophysiological study (nerve conduction study); and (b) improvement of at least 1 point of the total Overall Neuropathy Limitations Scale score after vitamin B12 treatment. RESULTS: Nine patients were identified. Serum B12 level was low in only four. Four patients had sensorimotor (predominantly sensory) axonal polyneuropathy while five had only sensory neuronopathy. Six improved in less than 1 month after B12 supplementation. CONCLUSION: B12-responsive neuropathy is a more heterogeneous group of neuropathy than previously described. B12 deficiency is a cause of peripheral neuropathy and should systematically be ruled out in the clinical setting of idiopathic neuropathy or sensory neuronopathy because of potential reversibility. The goal of this study was to assess a new pencil-beam densitometer, the Stratos (Diagnostic Medical Systems, Pérols, France). Evaluation of the dosimetry and precision were done together with an in vivo cross-calibration study performed with the fan beam densitometer Discovery A (Hologic, Bedford, MA). The results indicated that the Stratos performed bone mineral density (BMD) measurements with a good precision, low radiation dose, and good agreement with the Discovery A. The air dose, measured by an ionization chamber, was 40 µGy. The effective dose was assessed using an anthropomorphic phantom and thermoluminescent detectors resulting in 1.96 and 0.31 µSv for a lumbar spine and proximal femur scan, respectively. The average scattered dose rate at a distance of 1m from the device was 1.06 and 1.21 µSv.h(-1) in the lumbar spine and left proximal femur scan mode, respectively. For the precision evaluation, 30 patients underwent 2 lumbar spine and 2 proximal femur scans with repositioning after each scan. The percentage root-mean-square coefficient of variation was 1.22%, 1.38%, 2.11%, and 0.86% for the lumbar spine (L1-L4), lumbar spine (L2-L4), femoral neck, and total hip, respectively. The cross-calibration studies were done on 57 patients (60 ± 9 yr). Lumbar spine, left neck, and left total hip mean BMD were 3.10% lower and 11.94% and 8.83% higher, respectively, with the Stratos compared with the Discovery A. Cross-calibration equations were calculated with a correlation coefficient of 98% (p<0.01) for the lumbar spine (L2-L4), 94% (p<0.01) for the left neck, and 92% (p<0.01) for the left total hip. After standardizing the Stratos measures using the cross-calibration equations, LIN's concordance correlation coefficient was 0.98, 0.93, and 0.92 for the lumbar spine (L2-L4), left neck, and total hip, respectively.

Oxidation reactions that involve several oxygen and nitrogen reactive species together with nucleobase radical cations give rise among various classes of lesions to modified bases. About 70 of oxidized nucleosides that include diastereomeric forms have been characterized in mechanistic studies involving isolated DNA and related model compounds. However, only eight modified bases have been accurately measured within cellular DNA upon exposure to either gamma or UVA radiations. Emphasis is placed in this survey on recent developments of HPLC associated with tandem mass spectrometry (MS/MS) operating in the mild electrospray ionization mode. Interestingly, the HPLC-MS/MS assay in the multiple reaction monitoring mode appears to be the more sensitive and accurate method currently available for singling out several oxidized nucleosides including 8-oxo-7,8-dihydro-2'-deoxyguanosine, 8-oxo-7,8-dihydro-2'-deoxyadenosine, 5-formyl-2'-deoxyuridine, 5-(hydroxymethyl-2'-deoxyuridine, 5-hydroxy-2'-deoxyuridine, and the four diastereomers of 5,6-dihydroxy-5,6-dihydrothymidine within isolated and cellular DNA. However, one limitation of the assay that also applied to all chromatographic methods is the slight side-oxidation of normal bases during DNA extraction and subsequent work-up. This explains why the combined use of DNA repair glycosylases with either the comet assay or the alkaline elution technique is a better alternative to monitor the formation of low levels of oxidized bases within cellular DNA.
