RESUMO

O presente trabalho objetivou comparar o efeito do flunixin meglumine, cetoprofeno e meloxicam no tratamento da dor pós-operatória de ovinos submetidos à implantação de cânula ruminal e orquiectomia. Foram utilizados 32 ovinos, machos, pesando em média 35,5±3,5kg, distribuídos em três grupos: GFlu (flunixin meglumine 1,1mg/kg i.v.), GCet (cetoprofeno 3,0mg/kg i.v.) e GMel (meloxicam 0,5mg/kg i.v.). Exame clínico e coletas de sangue foram realizados no M0 (pré-avaliação), M1 (10 minutos após a pré-avaliação), M2 (início da sutura para fixação da cânula ruminal), M3 (logo após o término da cirurgia) e em duas, 12, 23, 25, 48 e 72 horas após a cirurgia (M2h, M12h, M23h, M25h, M48h e M72h), quando foram avaliados cortisol, glicose, proteína total, albumina, γ-glutamiltransferase (GGT), aspartato aminotransferase (AST), creatina quinase (CK), ureia, creatinina e hemograma. Nos M2h, M12h, M23h, M25h e M48h, foi realizada avaliação comportamental. O GFlu apresentou maior concentração de cortisol no M12h e no M48h e maior escore de dor na fístula e no testículo no M12h, quando comparado ao GMel. Os animais do GCet apresentaram menor interação com outros membros da baia no M23h. A ação analgésica do meloxicam foi maior em animais submetidos à implantação de cânula ruminal e orquiectomia, quando comparado ao flunixin meglumine e ao cetoprofeno.(AU)


This study aimed to compare the effect of flunixin meglumine, ketoprofen, and meloxicam in the treatment of postoperative pain in sheep submitted to ruminal cannulation and orchiectomy. 32 sheep were submitted to implantation of rumen cannula and orchiectomy, divided into three groups: GFlu (Flunixin meglumine 1,1mg/kg i.v.); GCet (Ketoprofen 3,0mg/kg i.v.); GMel (Meloxicam 0,5mg/kg i.v.). Clinical examination and blood samples were performed at M0 (pre-evaluation), M1 (10 minutes after pre-evaluation), M2 (beginning ruminal cannula), M3 (immediately post-surgery), and M2h, M12h, M23h, M25h, M48h and M72h (2h, 12h, 23h, 25h, 48h and 72 hours post-surgery) with the evaluation of cortisol, glucose, total protein, albumin, γ-glutamyl transferase (GGT), aspartate aminotransferase (AST), creatine kinase (CK), urea, creatinine and blood count. At M2h, M12h, M23h, M25h and M48h a behavioral evaluation was performed. The GFlu showed higher concentration of cortisol at M12h and M48h and greater pain score related with fistula and testis procedures at M12h when compared to GMel. Animals in the GCet group presented lower interaction with other animals in the same M23h paddock. The analgesia provided by Meloxicam was higher than flunixin meglumine and ketoprofen in animals submitted to placement of ruminal cannula and orchiectomy.(AU)

Focal cryoablation (FC), brachytherapy (B) and active surveillance (AS) were offered to patients diagnosed with very low-risk prostate cancer (VLRPC) in an equal access protocol. Comprehensive validated self-report questionnaires accessed patients' erectile (IIEF-5) and voiding (IPSS) functions, Beck scales measured anxiety (BAI), hopelessness (BHS) and depression (BDI), SF-36 reflected patients' quality of life added to the emotional thermometers including five visual analogue scales (distress, anxiety, depression, anger and need for help). Kruskal-Wallis or ANOVA tests and Spearman's correlations were obtained among groups and studied variables. Thirty patients were included, median follow-up 18 months (15-21). Those on AS (n = 11) were older, presented higher hopelessness (BHS) and lower general health perceptions (SF-36) scores than patients opting for FC (n = 10) and B (n = 9), P = 0.0014, P = 0.0268 and P = 0.0168 respectively. Patients on B had higher IPSS scores compared to those under FC and AC, P = 0.0223. For all 30 included patients, Spearman's correlation (rs ) was very strong between BHS and general health perceptions (rs  = -0.800, P < 0.0001), and weak/moderate between age and BHS (rs  = 0.405, P = 0.026) and age and general health perceptions (rs  = -0.564, P = 0.001). The sample power was >60%. To be considered in patients' counselling and care, current study supports the hypothesis that even VLRPC when untreated undermines psychosocial domains.

OBJECTIVE: To evaluate the satisfaction level of multiple sclerosis (MS) patients treated with interferon beta-1b (IFNbeta-1b, Betaferon*) using a newly developed application system compared to their currently used application system. METHODS: A survey was conducted in Portugal in patients treated with IFNbeta-1b for relapsing-remitting MS with the Betaject or Betaject Lite autoinjector. Nurses demonstrated the new application system and supervised the first injection. Patients rated their overall satisfaction retrospectively with their current application system and prospectively after the first, the seventh and the 15th injection with the newly developed application system. Additionally, the ease of use was evaluated for both application systems using a questionnaire consisting of 13 questions. Responses were compiled and descriptive analyses performed. RESULTS: A total of 249 patients evaluated the current and the new system after the first, 235 after the seventh and 174 after the 15th injection. The satisfaction level was high with the current system (70.3%, 'satisfied' or 'very satisfied'). However, compared with the current system, more patients were either 'satisfied' or 'very satisfied' (98%) with the new system after first injection. Only a minority of patients rated 'somewhat satisfied': 2.0% after the first, 8.6% after the seventh, and 4.4% after the 15th injection. Increased overall satisfaction level ('satisfied' or 'very satisfied') with the newly developed system was maintained over time (98% - first, 90.5% - seventh, 93.8% - 15th injection). The thinner, pre-attached 30-gauge needle and the visual signalling of injection completion were among the changes considered as strong improvements to the new system by up to 80.3% of patients. LIMITATION: Retrospective analysis of current system. CONCLUSIONS: This survey documented patient satisfaction with different application systems of IFNbeta-1b. The increased satisfaction with the new application system indicates an improvement to the currently used injection system, which may contribute to further advancement in adherence and consequently higher clinical efficacy of treatment.

RESUMO

This paper considers the implementation of a current control method for switched reluctance motors (SRMs) and presents a novel approach to the accurate on-line modeling of an SRM drive. A simple autotuning technique for the SRM drives using a PWM controller is considered. Furthermore, conventional PI control and Internal Model Control (IMC) are considered to validate this method and present corresponding robust control analysis for the process. The control structures are comparatively analyzed using standard robustness measures for stability and performance. The proposed PWM controller is simulated and a hardware prototype is then implemented using digital signal processor control to evaluate the method using a 12/8, three-phase SRM. The experimental results of the SRM drive model validates the performance of the current loop.

RESUMO

The management of 50 AIDS patients by traditional hospital admission (25 cases) and outpatient clinics (25 cases) was studied between August and November 1995 in the Department of Infectious and Parasitic Diseases of the Federal University Hospital. The most costly items of expenditure were hospital services and consumable materials. Comparison of costs was complicated by differences in clinical status of the patients in the two groups. The choice of treatment was much more dependent on clinical status than on sociodemographic factors. Traditional hospital admission tended to be associated with the poorest patients. The rationalization of care based on cost-benefit analysis requires much future work.

RESUMO

The therapeutic progress achieved in the second half of this century was made possible only due to the interest taken by the pharmaceutical industry in the research field. It can be considered as the primum movens of the therapeutic revolution which has strongly benefited the humanity in these last decades. The clinical trials of new drugs constitute an integral part of the pharmaceutical and medical research. However its complexity and the costs involved make its execution impossible without the help or sponsorship of the pharmaceutical firms. Even in the more advanced countries it would be unthinkable that clinical trials could be carried out without the assistance of the pharmaceutical industry. The sponsorship by the pharmaceutical firms turns the relationship doctor-firm into a symbiotic association as both the parties reap benefits thereof: the physician because he is able to satisfy his inquisitiveness and his interest for the clinical research and the pharmaceutical firm because through these trials it will collect the information it requires. Besides having to bear the expenses inherent to the research, the sponsoring firms equally contribute for other pursuits usually of academical character and of interest to the investigator. Therefore it is not rare that these firms assume the costs of presence of these investigators in scientific meetings or to contribute for the arrangements of such meetings or even subsidise the acquisition of the equipment for the hospital services, etc.--measures which in countries with poor investments in research sector, such as Portugal, may constitute the only possibility to be carried out.(ABSTRACT TRUNCATED AT 250 WORDS)
