RESUMO

OBJECTIVES: Electroconvulsive therapy (ECT) is an effective somatic treatment, but it may be limited by cognitive adverse effects. The existing cognitive screening instruments often lack specificity to ECT-associated cognitive deficits. The ElectroConvulsive Therapy Cognitive Assessment was developed and validated in a clinical setting, but the reliability and validity of the Chinese version of ElectroConvulsive Therapy Cognitive Assessment (ECCA-C) have not been studied in a large clinical sample. METHODS: The ECCA-C and the Montreal Cognitive Assessment (MoCA) were administered to patients with major depressive disorder (MDD) undergoing ECT at 3 time points: pretreatment (baseline), before the fifth treatment, and 1 week posttreatment. The instruments were also administered to a sample of healthy subjects. RESULTS: Sixty-five patients with MDD and 50 age- and sex-matched healthy controls were recruited in this study. Overall, the patient group had statistically significantly lower MoCA and ECCA-C scores than the control group (both P values <0.001). The Cronbach α of the ECCA-C was 0.88 at baseline. Statistically significant decreases over time were observed in ECCA-C: pre-ECT (23.9 ± 4.0) > mid-ECT (21.3 ± 3.4) > post-ECT (18.7 ± 4.8) (all P values <0.001), whereas no statistically significant changes in MoCA scores were found at these 3 time points (F = 1.86, P = 0.165). A cutoff score of 26.5 on the ECCA-C was found to best differentiate between MDD patients and healthy controls. CONCLUSIONS: The ECCA-C showed satisfactory psychometric properties and may be a more sensitive instrument than the MoCA to assess cognitive impairment associated with ECT.

RESUMO

Precision medicine demands accurate assessment of tumor response to therapies with the purpose of timely optimization or adjustment of the given treatment regimens. Chemoradiation remains the standard of care in advanced lung cancers and imaging-based noninvasive response evaluation could improve therapeutic efficacy and reduce treatment-related severe side effects. In this review, we overviewed the applications and pitfalls of major imaging modalities in response evaluation in lung cancer from a quantitative perspective. The development of radiomics will provide more precise information for treatment predicting, evaluation and prognosis.

OBJECTIVE: The objective of this study was to investigate the feasibility of using monochromatic spectral computed tomography (CT) imaging to assess fatty infiltration in liver. MATERIALS: With spectral CT imaging, phantoms with known fat concentrations were studied for scanning parameter optimization, then 52 patients enrolled into 4 groups (healthy, mild, moderate, and severe fatty infiltration) received abdominal scanning. Based on reconstructed monochromatic images, hepatic attenuation was analyzed, and dual-energy subtraction imaging (DESI) was created for quantifying fat infiltration. RESULTS: Corresponding to various hepatic fat infiltrations, 4 characteristic CT attenuation curve patterns were described. In DESI images, only fat and fatty components appeared bright. For livers without abnormal fat deposition, isolated bright pixels were visualized (% area = 0.5% ± 0.3%). With hepatic fat accumulation increasing, more bright pixels appeared in subtraction images with percentages of total liver area involved in 2.5%, 6.7%, and 13.4% of mild, moderate, and severe fat infiltration cases, respectively (P < 0.05). The corresponding CT values were as follows: 1.33, 2.53, 8.69, and 16.4 Hounsfield units (P < 0.01), which correlated with the % DESI area values (r = 0.9811). CONCLUSIONS: Spectral CT imaging is a promising method to quantitatively assess hepatic fat content and fatty infiltration with advantages compared with conventional CT imaging.

A simple, fast and sensitive method for the simultaneous determination of cnidilin and its two metabolites (M1 and M2) in rat bile and stool using HPLC coupled with electrospray ionization tandem mass spectrometry (HPLC-ESI-MS/MS) has been developed. The sample pretreatment was simple, because methanol was the only additive used for dilution of bile and ultrasound of stool. Pimpinellin was used as internal standard (IS). The separation was performed on a reverse phase C18 column with gradient elution consisting of 0.5‰ aqueous formic acid and methanol (containing 0.5‰ formic acid). The detection was in the multiple-reaction monitoring mode within 7 min. All the analytes were in accordance with the requirement of the validation of the method in vivo (linearity, precision, accuracy, limit of detection and limit of quantification). After oral administrating 24 mg/kg of the prototype drug cnidilin, M1 and M2 were determined in bile within 36 h, and in stool within 60 h. Cnidilin in bile was completely excreted in 24 h, and the main excretive amount of cnidilin was 80% in the first 6 h, but the drug recovery in bile within 24 h was <1.95%. In stool, the main excretive amount of cnidilin was 95.8% in the first 24 h, and the drug recovery within 48 h was lower than 1.48%.
