OBJECTIVE: The functional assessment of cancer therapy-bone marrow transplant (FACT-BMT) is a widely used instrument to assess quality of life (QOL) in hematopoietic stem cell transplant (HSCT) patients, but there is little evidence of its validity in Latin American populations. This study evaluated the psychometric properties of the Spanish language version of the FACT-BMT in Mexican patients. METHOD: First, the original version was piloted with 15 HSCT patients to obtain an adequate cultural version, resulting in the adaptation of one item. After that, the new version was completed by 139 HSCT patients. RESULTS: The results showed a FACT factor structure that explains 70.84% of the total variance, a factor structure similar to the original FACT structure, and with a high internal consistency (Cronbach's alpha = 0.867). For the BMT subscale, the best factor structure included 17 items which explain 61.65% of the total variance with an adequate internal consistency (Cronbach's alpha = 0.696). SIGNIFICANCE OF THE RESULTS: The FACT-BMT was found to be a valid and reliable instrument to evaluate QOL in Mexican patients. Our results constitute new FACT-BMT empirical evidence that supports its clinical and research uses.

OBJECTIVE: Describe and analyze the mental health service and medical needs of a group of transgender men from the point when they changed their gender identity to male. MATERIAL AND METHOD: Transgender men volunteers who attended a specialized care center in Mexico City answered a qualitative semi-structured interview to explore their needs as well as the adversities they unnecessarily face due to the lack of information and care for their condition. Interviews were audio-recorded and transcribed for thematic analysis. RESULTS: Respondents began to develop their transgender identity in childhood; however, during that time, almost none of them had any helpful contact with a health professional. Those who did receive some form of care received mistreatment rather than positive feedback. It was not until adulthood that they obtained facts about a specialized care center. CONCLUSION: It is necessary for health professionals to have information about sexual diversity and be trained to meet the needs of transgender children, in order to have information on places and professionals who accompany them and advise on available treatments such as hormone blockers or treatments. The study includes a small but important sample. However, the currently hostile, discriminatory environment, significantly exposes transgender people to developing mental health problems.

Resumen: Objetivo: Analizar las demandas de atención de los trastornos mentales graves (TMG) y factores asociados con la utilización de servicios en México. Material y métodos: Se llevó a cabo un estudio analítico transversal en dos fases: la primera con una base de datos nacional de servicios disponibles y su utilización; la segunda, una muestra de registros médicos de un hospital psiquiátrico. Resultados: La esquizofrenia es el TMG más prevalente; más de 50% de hospitalizados fueron hombres, con edad promedio 37 años. La utilización de servicios estuvo asociada con la edad (β=1.062; p=.000), ingreso familiar (β=1.000, p=.000) y no tener ocupación (β=3.407; p=.000). La población con esquizofrenia tiene cuatro veces más la probabilidad de requerir estar exenta de pago (β=4.158; p=.000). Conclusiones: La población con TMG es más vulnerable por la discapacidad funcional y social asociada; requiere de intervenciones específicas de salud acompañadas de una política de protección financiera adaptada a sus necesidades de atención.

Objective: To compare the presence of criteria listed in the DSM-5 and ICD-10 diagnostic manuals in a Brazilian sample of transgender persons seeking health services specifically for physical transition. Methods: This multicenter cross-sectional study included a sample of 103 subjects who sought services for gender identity disorder in two main reference centers in Brazil. The method involved a structured interview encompassing the diagnostic criteria in the two manuals. Results: The results revealed that despite theoretical disagreement about the criteria, the manuals overlap regarding diagnosis confirmation; the DSM-5 was more inclusive (97.1%) than the ICD-10 (93.2%) in this population. Conclusions: Although there is no consensus on diagnostic criteria on transgenderism in the diversity of social and cultural contexts, more comprehensive diagnostic criteria are evolving due to society's increasing inclusivity.

INTRODUCTION: Assessing dangerousness to gauge the likelihood of future violent behaviour has become an integral part of clinical mental health practice in forensic and non-forensic psychiatric settings, one of the most effective instruments for this being the Historical, Clinical and Risk Management-20 (HCR-20). OBJECTIVE: To examine the HCR-20 factor structure in Mexican psychiatric inpatients and to obtain its predictive validity and reliability for use in this population. METHOD: In total, 225 patients diagnosed with psychotic, affective or personality disorders were included. The HCR-20 was applied at hospital admission and violent behaviours were assessed during psychiatric hospitalization using the Overt Aggression Scale (OAS). Construct validity, predictive validity and internal consistency were determined. RESULTS: Violent behaviour remains more severe in patients classified in the high-risk group during hospitalization. Fifteen items displayed adequate communalities in the original designated domains of the HCR-20 and internal consistency of the instruments was high. CONCLUSION: The HCR-20 is a suitable instrument for predicting violence risk in Mexican psychiatric inpatients.

ANTECEDENTES: La teoría sistémica de la familia plantea que en ésta existen tres subsistemas: conyugal, parental y filial, los cuales responden, según su particular función, cuando uno de los miembros en edad pediátrica enfrenta alguna enfermedad crónica que amenaza su vida. La evaluación de las familias en un contexto hospitalario institucional presenta distintos desafíos debido al tiempo mínimo necesario para obtener un diagnóstico preciso y por la escasez de recursos humanos preparados para dichas tareas.OBJETIVO: Desarrollar y validar un instrumento para la evaluación de los subsistemas familiares durante la hospitalización a causa de una enfermedad crónica pediátrica (eSisFam).MÉTODO: A partir de la bibliografía y de instrumentos conocidos de funcionamiento familiar, se conformó uno nuevo de cuatro secciones: sistema general y subsistemas conyugal, parental y filial. Participaron 312 cuidadores primarios de pacientes crónicos internados en un hospital público pediátrico. Los reactivos tipo Likert con cuatro opciones de respuesta, fueron contestados en un programa de cómputo. Se determinó la validez de contenido, de constructo y la confiabilidad.RESULTADOS: La validez de contenido arrojó más del 80% de acuerdo interjueces. La validez de constructo por análisis factorial exploratorio identificó tres causas del sistema general (cohesión, comunicación y adaptación), dos del subsistema conyugal (relación afectivo-sexual y distanciamiento-infidelidad), tres del parental (bienestar, responsabilidad compartida y cuidado de los hermanos) y tres del filial (bienestar, conocimiento de la enfermedad y aceptación-participación) con coeficientes alfa de Cronbach superiores a 0.80.DISCUSIÓN Y CONCLUSIÓN: El instrumento desarrollado (eSisFam) es válido y confiable para una evaluación general del sistema familiar y de los subsistemas conyugal, parental y filial. La medida podrá ser utilizada en el hospital de pediatría para la detección temprana de situaciones familiares que pudieran interferir con la atención médica del niño o adolescente.


BACKGROUND: According to the family systems model, the family consists of three main subsystems: marital, parental and filial, all of which perform specific functions. A chronic pediatric illness presents challenges to these subsystems leading family members to make the necessary adjustments in order to maintain system balance. The evaluation of families in a public hospital setting is hampered by time constraints and limited human resources to carry out a precise evaluation.OBJECTIVE: The aim of this study was to develop and validate a practical instrument for, evaluation of family subsystems during the hospitalization of a pediatric patient with a chronic illness (eSisFam).METHOD: Based on a review of scientific literature and well-known family functioning instruments, a new measure with four sections was developed: the general system and the marital, parental and filial subsystems. This was applied to 312 primary caregivers of chronically ill hospitalized pediatric patients in a public pediatric hospital. Likert-type items with four response options were self-administered with a computer program. Content validity, construct validity and reliability were assessed.RESULTS: Content validity was obtained by 80% agreement between judges. Construct validity was evaluated by exploratory factor analysis, resulting in three factors for the general system (cohesion, communication and adaptation); two for the marital subsystem (affective-sexual relationship and distancing-infidelity); three for the parental subsystem (well-being, shared responsibility and care of siblings), and three for the filial subsystem (well-being, illness knowledge and acceptation-participation). We assessed the internal consistency with Cronbach's alpha coefficients; these were higher than 0.80 in all subscales.DISCUSSION AND CONCLUSION: The instrument (eSisfam) that was developed is a valid and reliable measure to assess general family system functioning and marital, parental and filial subsystems. This measure could be used in pediatric hospitals for the early detection of family situations that might interfere with child or adolescent medical attention.

Lacking in the literature are data addressing the extent to which changes in reproductive and lifestyle factors predispose women in developing nations to higher breast cancer rates, and the degree to which these are due to globalization influences. This article describes the development and psychometric assessment of an instrument intended to measure global, predominantly U.S., influences on breast cancer risk profile among women residing in Mexico. Using investigator consensus and a focus group methodology, the Measure of Globalization Influence on Health Risk (MGIHR) was developed and completed by 341 women. Psychometric analysis support the use of an 11-item Consumerism and Modernity scale and 7-item Reproductive Control and Gender Role scale. The MGIHR is a valid and reliable instrument for understanding changing lifestyle and reproductive factors for breast cancer risk and may provide a more complete understanding of breast cancer development and needed interventions.

Background Psychological disturbances are common in patients with chronic renal failure (CRF). From the cognitive-behavioral theoretical perspective, they may be due largely to specific cognitive distortions (CD) that are likely to be modified by brief interventions. The aim of this study was to develop and validate an instrument for evaluation of specific CD among patients with CRF. Method A questionnaire with an encouraging phrase for each of five categories of CD (catastrophizing, dichotomous thinking, outside self-worth, negative self-labeling, and perfectionism) was developed. This was applied to 21 patients with CRF (11 women and 10 men). Based on the technique of natural semantic networks, a set of responses (defining words) with greater semantic weight to each sentence stimulus (core network) were identified and a pilot instrument was developed. This one was applied along with Beck's inventories of anxiety and depression among 255 patients with CRF (118 women, 137 men) attended in four different hospitals. Results The pilot questionnaire was composed of 46 items with greater semantic weight between 343 original defining words. Sixteen items were discarded for lack of ability to discriminate (1), low internal reliability (8) and duplicated in more than one factor (7). The final instrument of 30 items had an internal consistency of 0.93 for the total scale and more than 0.75 in all subscales. The five factors correlated positively and significantly with each other and with symptoms of anxiety and depression. Conclusions The instrument that is presented is a valid and reliable measure to assess five CD associated with depression and anxiety in Mexican patients with CRF. It is suggested as a useful tool for the design and evaluation of cognitive behavioral interventions for depression and anxiety among patients with CRF.


Antecedentes Las alteraciones psicológicas son frecuentes en los pacientes con insuficiencia renal crónica terminal (IRC). Desde la perspectiva teórica cognitivo-conductual, éstas pueden deberse en gran medida a distorsiones cognitivas específicas (DC), que son susceptibles de ser modificadas mediante intervenciones breves. El objetivo del presente estudio fue desarrollar y validar un instrumento de evaluación de las DC de pacientes con IRC. Método Se integró un cuestionario con una frase estímulo para cada una de cinco categorías de DC: catastrofismo, pensamiento dicotómico, autovalorización externa, autoetiquetización negativa y perfeccionismo. Éste se aplicó a 21 pacientes con IRC (11 mujeres y 10 hombres). Con base en la técnica de redes semánticas naturales, se identificó el conjunto de respuestas (palabras definidoras) con mayor peso semántico para cada frase estímulo (núcleo de la red), con las que se construyó el instrumento piloto. Éste se aplicó junto con los Inventarios de Ansiedad y Depresión de Beck a 255 pacientes con IRC (118 mujeres, 137 hombres) de cuatro centros hospitalarios. Resultados El cuestionario piloto se integró con 46 reactivos con mayor peso semántico de entre 343 palabras definidoras originales. Se descartaron 16 reactivos por falta de capacidad de discriminación (1), baja confiabilidad interna (8) y duplicidad en más de un factor (7). El instrumento final de 30 reactivos tuvo una consistencia interna de 0.93 para el total y mayor a 0.75 en todas las subescalas. Los cinco factores se correlacionaron positiva y significativamente entre sí y con los síntomas de ansiedad y depresión. Conclusiones El instrumento que se presenta es una medida válida y confiable para evaluar cinco DC asociadas con depresión y ansiedad en pacientes mexicanos con IRC. La medida se sugiere útil para el diseño y valoración de intervenciones cognitivo-conductuales para la depresión y ansiedad de los pacientes con IRC.

Introduction Depression is a public health problem that carries substantial costs for the individual and the society. In order to establish evidence-based priorities for resource allocation in mental health care, it is necessary to integrate the costs and effectiveness of interventions and specify the essential packages for their treatment. The following are pioneering studies of cost-effectiveness for the treatment of depression: 1. compared psychopharmacology options (fluoxetine, imipramine and desipramine) to found no difference between drugs in terms of clinical efficacy, effect on quality of life and costs, and 2. evaluated cost-effectiveness of collaborative program of stepped care in primary care of persistent depression, to demonstrate a substantial increase in the effectiveness and additional moderate cost increase in comparison with usual treatment. Introduction Schizophrenia is a disorder that causes significant disability. In addition, its treatment is expensive because the increased prescription of atypical antipsychotics with associated high costs. In a recent 14-country study on disability associated with physical and mental conditions, active psychosis was ranked the third most disabling condition in general population, more than paraplegia and blindness. In the global burden of disease study, schizophrenia accounted for 1.1% of the total Disability-adjusted life years (DALYs) and 2.8% of Years of lived with disability (YLDs). The economic cost of schizophrenia for society is also high. The study of the burden of schizophrenia for society, whether expressed in epidemiological or costs terms, is an insufficient basis for setting priorities for resources allocation. Thus, increasingly sophisticated economic models have been developed. Such is the case of cost-effectiveness studies, which show the relationship between resources used (costs) and benefit achieved (effectiveness) of an intervention compared with others. In Mexico, there is only one study that evaluated the cost-effectiveness of different antipsychotics to treat schizophrenia, but it was a specific approach (not generalized), and did not include psychological interventions. The present study is part of a World Health Organization's initiative labeled WHO-CHOICE: CHOosing Interventions that are Cost-Effective. WHO-CHOICE methodology involves the evaluation of interventions based on a generalized measure: DALYs, which allows carrying out several and important comparisons. The main objective was to determine the cost-effectiveness of different interventions for the treatment of schizophrenia in Mexican communitarian settings. Method Schizophrenia was modeled as a serious chronic disorder with a high level of disability, excess mortality from natural and unnatural causes, and a low rate of remission. The incidence, prevalence, and the fatality rate were estimated based on the study of the Global Burden of Disease and a review of the epidemiological literature. As the first episode of schizophrenia is currently not preventable, the occurrence represents how the epidemiological situation would be without intervention. In relation to the referral and the fatality, we did not found evidence that these rates change by a specific effect of the treatment; thus, they were kept as constants for the scenarios with or without treatment. Community-level interventions assessed were: 1. typical traditional antipsychotics (haloperidol), 2. new atypical antipsychotics (risperidone), 3. traditional antipsychotics + psychosocial treatment (family therapy, social skills training and cognitive behavioral therapy), 4. new antipsychotics + psychosocial treatment, 5. traditional antipsychotics + psychosocial treatment + case management, and 6. new antipsychotics + psychosocial treatment + case management. The effectiveness of the treatments referred to the control of positive and negative symptoms and associated levels of disability. To calculate the improvement in disability compared with natural history (when the disease is not treated), the effect sizes reported in controlled clinical trials were converted to a weight change of disability. Efficacy and extrapyramidal effects of typical and atypical antipsychotics compared to placebo were estimated from the meta-analysis of controlled clinical trials, with the score of the BPRS severity scale and the need anti-Parkinson drugs as efficacy measures. From another meta-analysis we obtained an estimate of the magnitude of the effect by adding psychosocial interventions. As an ad hoc Cochrane systematic review that found case management did not had a significant impact on clinical or psychosocial outcomes, only a minimal addition effect size when added to the combination of pharmacologic-psychosocial treatment was observed. Costs included those of the patient, the program and the training required to implement the intervention. The provision of community-based services, daily administration of antipsychotics and anticholinergics, and laboratory tests were taken into account. For psychological interventions were envisaged from 6 to 12 sessions: in primary care from 6 to 12 visits, in outpatients services a visit per month for 20-50% of cases, and in day care communitarian attention from 1-2 times a week for 20-50% of cases. A 3% discount by the process of converting future values to present ones and an age adjustment giving less weight to year lived by young were included. Finally, the cost of DALYs averted for each intervention was estimated to determine their cost-effectiveness. Results The main findings of the study are, in relation to the costs of interventions: 1. the largest share corresponds to those generated by medication, 2. the current intervention is the cheapest, and 3. the combination of new atypical antipsychotics, psychological treatment and proactive case management is the most expensive intervention. Concerning the effectiveness of interventions, the one available today, with a coverage of 50%, prevents 68 222 DALYs. Increasing coverage to 80%, the number of DALYs averted is almost doubled with the use of typical antipsychotics. The effect of psychological interventions makes the number of DALYs averted three to four times higher. Finally, in regard to cost effectiveness, the combination of typical antipsychotics, psychosocial intervention and proactive case management was the treatment with the best relation. The cost per DALY averted was $390,892 Mexican pesos, which corresponds to one third of the cost of DALY averted in the current scenario ($1,313,120 Mexican pesos). Conclusions The resources for the attention of a public health issue involve a social investment rather than an expense budget, but they are also finite and must be chosen properly to be allocated. Cost-effectiveness studies of available interventions are an essential tool for making such important decisions. Our Mexican study of cost-effectiveness of interventions to treat schizophrenia in communitarian settings suggests, in general terms: 1. That while the current situation is the one with the lowest cost, it is the least efficient, 2. all alternatives involve an additional cost to the current situation because they assume an expansion of coverage; however, the extra cost in not excessive, and 3) that within a model of community-based care, the least expensive option is treatment with typical antipsychotics combined with psychological intervention. Thus, for a modest extra cost it is possible to yield a major impact on disability. Recently, the Mexican Federal Government has included schizophrenia in the catalog of diseases covered by the program called <>, that provides a health insurance to general population, especially to the poorest and unemployed ones. The planned actions include four specialty consultations in an interval of two months and annual psychopharmacological treatment. Clearly the addition of haloperidol, trifluoperazine and risperidone to the list of available medications should be considered a success. However, schizophrenia also requires a proactive case monitoring of long-term for best control of symptoms and a successful rehabilitation. Moreover, consistent with our findings, case management has proven to be cost-effective when compared with routine care in the community. Among the limitations of the study it is important to note that it was based on modeled parameters obtained from the international literature. In this sense, the challenge is the data generation directly from studies in Mexico.


Introducción La esquizofrenia es un trastorno que produce una importante discapacidad, y su costo para la sociedad es muy elevado. El estudio de la carga que impone la esquizofrenia sobre la sociedad, ya sea expresada en términos epidemiológicos o de costos, es una base insuficiente para establecer las prioridades para la asignación de recursos, por lo que se han desarrollado modelos económicos cada vez más elaborados. Tal es el caso de los estudios de costo-efectividad, que muestran la relación entre los recursos empleados (costos) y los beneficios logrados (efectividad) de una intervención comparada con otra(s). El presente trabajo forma parte de la iniciativa de la Organización Mundial de la Salud (OMS) para la elección basada en evidencia de intervenciones costo-efectivas denominada WHO-CHOICE, Choosing Interventions that are Cost-Effective, que implica la determinación de una medida general que posibilita comparaciones internacionales: los años de vida vividos con discapacidad (AVISAs o DALYs, de las siglas de Disability-adjusted life years). Objetivo Fue determinar la relación costo-efectividad en México de diferentes intervenciones que han demostrado ser efectivas para el tratamiento comunitario de la esquizofrenia. Método Se evaluaron las siguientes intervenciones: 1. Antipsicóticos tradicionales típicos (haloperidol), 2. Antipsicóticos nuevos atípicos (risperidona), 3. Antipsicóticos tradicionales + tratamiento psicosocial (terapia familiar, entrenamiento en habilidades sociales y terapia cognitivo conductual), 4. Antipsicóticos nuevos + tratamiento psicosocial, 5. Antipsicóticos tradicionales + tratamiento psicosocial + manejo de caso, y 6. Antipsicóticos nuevos + tratamiento psicosocial + manejo de caso. La efectividad relativa de los tratamientos se refirió al control tanto de los síntomas positivos y negativos así como de los niveles asociados de discapacidad. Se consideraron los costos del paciente, del programa y del entrenamiento, así como un descuento de 3% por el proceso de convertir valores futuros a presentes, y un ajuste de edad, otorgando menos peso a los años vividos por los jóvenes. Finalmente, se calculó el costo por AVISA evitado por cada intervención para determinar su costo-efectividad. Resultados Los principales hallazgos del estudio son, en relación con los costos de las intervenciones: 1. que la mayor proporción corresponde a los generados por los medicamentos; 2. que la intervención actual resulta la más barata; y 3. que la combinación de antipsicótico nuevo o atípico con intervención psicosocial y manejo proactivo de caso es la más cara. En cuanto a la efectividad de las intervenciones, la disponible en la actualidad, con cobertura de 50%, evita 68 222 AVISAs. Aumentando la cobertura a 80%, el número de AVISAs evitados casi se duplica con el empleo de antipsicóticos típicos. El efecto de las intervenciones psicosociales hace que el número de AVISAs evitados sea tres a cuatro veces mayor. Finalmente, en lo que respecta a costo-efectividad, la combinación de antipsicóticos típicos, intervención psicosocial y manejo proactivo de caso fue el tratamiento con la mejor relación. El costo por AVISA evitado fue de $390,892, que corresponde a un tercio del costo de AVISA evitado en el escenario actual ($1,313,120). Conclusiones Los hallazgos del estudio sugieren, en términos generales: 1. Que aunque la situación actual es la de menor costo, es la menos eficiente; 2. Que todas las alternativas implican un costo adicional a la situación actual porque asumen una ampliación de la cobertura, sin embargo, ese costo extra no es excesivo; y 3. Que dentro de un modelo de atención basado en la comunidad, la opción menos costosa es el tratamiento con antipsicóticos típicos combinados con intervención psicosocial. Por un costo extra modesto se obtendría un impacto importante en la discapacidad. Entre las limitaciones del estudio destaca que fue producto de un modelado con parámetros obtenidos de la bibliografía internacional. En este sentido, el reto es la generación de datos derivados directamente de estudios realizados en México.
