RESUMO

As people age, there is an increase in chronic illness and disability and thus a growing need for long-term care. Unfortunately, the concept of long-term care can be very complex and confusing. Elderly individuals and their family members frequently have many questions, such as the following: What is long-term care, and who needs it? What long-term care services are available? Who will provide the services? Where will the services be provided? How much will the services cost, and who will pay for the expenses? Indeed, the cost of providing long-term care, whether in an institution or in one's own home, may represent an enormous burden for individuals and their families. Nurses, especially community health nurses, are in a prime position to address these questions and concerns to ensure appropriate care and services for their clients. This article addresses the questions clients frequently ask by defining long-term care and discussing long-term care services, settings, and providers. In addition, 4 methods of financing long-term care will be presented.

RESUMO

Nurses must frequently make arduous decisions when faced with ethical dilemmas that occur in clinical practice. Utilizing ethical principles for analyzing and reflecting on the issues may ease this difficult task. In addition, the nurse involved may experience less anxiety and uncertainty over whether or not the correct decision was made. The purpose of this article is to present a case study that involves the allocation of a scarce resource that posed an ethical dilemma for a nursing instructor and her students in a community health setting. Toulmin's framework is utilized for analyzing this dilemma.

The ethical (prescription) pharmaceutical industry depends on a stream of innovative, patent-protected products to maintain growth and profitability. With Sterling Drug's entry into a new therapeutic area (oncology, the study and treatment of cancer), licensing and research collaborations have become an important part of the process of developing a pipeline of potential new drugs. The application of the analytic hierarchy process has led to improved articulation of the relevant criteria and better definition of organizational procedures, thereby benefiting Sterling's decision-making process.
