BACKGROUND: Tinea pedis, commonly known as athlete's foot, is a fungal infection affecting the skin of the feet, primarily between the toes. Despite being a common condition, there may be gaps in knowledge and practices regarding its transmission, risk factors, and treatment options among the general population. METHODS: This study adopted a cross-sectional research design. The study involved 2371 adult population in Saudi Arabia. The questionnaire was distributed online through social media means. Data was analyzed using IBM SPSS Statistics for Windows, Version 27 (Released 2020; IBM Corp., Armonk, New York, United States). RESULTS: A significant portion (66.1%) of respondents reported not examining their feet regularly. The majority (80.7%) of the respondents reported regular use of nail scissors as the common practice. The results further revealed that the majority of respondents (71.7%) were not aware of tinea pedis. Similarly, a large proportion (77.3%) of respondents were unaware of the risk factors associated with tinea pedis. However, among those aware, the most recognized risk factor was diabetes mellitus (82.3%), followed by peripheral arterial disease (37.1%), and immunocompromised conditions like HIV (31.3%). There was a significant association (p = 0.001) between regular foot examination practices and awareness of tinea pedis. CONCLUSION: In conclusion, there is low awareness regarding tinea pedis among the Saudi Arabian population. Only a small proportion demonstrated good knowledge of the condition and its symptoms. However, there is a relatively higher awareness of specific risk factors such as diabetes mellitus and peripheral arterial disease. Hence, it is imperative to enhance education and awareness campaigns to address the gaps in understanding tinea pedis, its associated risk factors, and symptoms, particularly among individuals engaged in physical activities and those who regularly wear sports shoes.

A facile procedure for extracting and determining seven neonicotinoids was developed. Water was the only extraction solvent without phase separation and cleanup steps. The method was validated according to European Union standards, and the values obtained were compared with the criteria. The accuracy values were between 99.8% (thiamethoxam) and 106.8% (clothianidin) at the spiking levels of 0.01, 0.1, and 1 mg/kg in the tested matrices. The precision as pooled RSD values was ≤6.1% (intra-day) and ≤6.9% (inter-day). The limit of quantification was set and tested at 0.01 mg/kg. The matrix effect was evaluated, and all matrices had a suppressive effect. The matrix of the cucumber was the most effective, with -20.9% for dinotefuran and an average of -9.8% for all compounds, while the tomato matrix had the slightest effect. Real marketed samples were analyzed using the developed and QuEChERS (Quick, Easy, Cheap, Effective, Rugged, and Safe) methods; the results were not significantly different. A supervised field trial was conducted in the open field to study the dissipation patterns of imidacloprid and dinotefuran in tomatoes. The dissipation of both compounds followed first-order kinetics. The half-life (T½) values were 3.4 and 2.5 days, with dissipation rates k of 0.2013 and 0.2781 days, respectively. Following the EU-MRL database, the calculated pre-harvest interval (PHI) values were 7 and 14 days for imidacloprid and dinotefuran, respectively, and 3 days for both compounds following Codex Alimentarius regulations. The risk of imidacloprid and dinotefuran residues was estimated from chronic and acute perspectives. The risk factors of dinotefuran were lower than those of imidacloprid. Nonetheless, the highest expected residues of both compounds were below the tolerance limits.

Objective: Depression is a common comorbidity in patients with asthma with a significant impact on clinical management. However, little information is available about physicians' perceptions and current practices in identifying and managing depression in individuals with asthma in Saudi Arabia. Thus, this study aims to assess physicians' views and current practices of identifying and managing depression in patients with asthma in Saudi Arabia. Methods: A cross-sectional study was employed. Between September 2022 and February 2023, an online survey was distributed to physicians (general practitioners and family, internal and pulmonary medicine specialists) in Saudi Arabia. Descriptive statistics were performed to analyze the collected responses. Results: Out of 1800 invited participants, a total of 1162 physicians completed the online survey. Nearly 40% of the respondents received adequate training for managing depression. More than 60% of physicians reported that depression interferes with self-management and worsens asthma symptoms, while 50% viewed the importance of regular screening for depression. Less than 40% (n=443) aim to identify depression during patients' visits. Of those, only 20% always screen for depression in asthma patients. Physicians show a low level of confidence when asking patients about their feelings (30%), being able to recognize depression (23%), and knowing if patients have depression (23%). The most common barriers linked to recognizing depression are high workload (50%), lack of time to screen for depression (46%), limited knowledge about depression (42%), and poor training (41%). Conclusion: The rate of recognizing and confidently managing depression in asthmatic patients is significantly low. This is attributed to high workload, poor training, and limited knowledge about depression. There is a need to support psychiatric training and implement a systematic approach to depression detection in clinical settings.

BACKGROUND: Multifocal (MFBC)/multicentric (MCBC) breast cancer is being more recognized due to the improved imaging modalities and the greater orientation with this form of breast cancer, however, optimal surgical treatment, still poses a challenge. The standard surgical treatment is mastectomy, however, breast-conserving surgeries (BCS) may be appropriate in certain situations. METHODS: A total of 464 cases of MF/MCBC out of 4798 cases of breast cancer were retrospectively analyzed from the database of the Oncology Center, Mansoura University (OCMU), between January 2008 and December 2019. RESULTS: Radiologic involvement of multiple quadrants was reported in 27.9% by ultrasonography, 19% by mammography, and 59.1% by magnetic resonance imaging. BCS was performed in 32 cases (6.9%) while 432 cases underwent a mastectomy. Postoperative pathology revealed infiltration of other quadrants grossly in 23.5%, and under the microscope in 63.6% of the examined cases. Mean disease-free and overall survival were 95.5 and 164.6 months, respectively. When compared with MFBC, MCBC showed higher pathologic tumor size (p < 0.001), higher stages (p < 0.001), higher recurrence rates (p = 0.006), and lower DFS (P = 0.009) but with similar OS (P = 0.8). CONCLUSION: Mastectomy is still the primary treatment option for MCBC with higher recurrence rates compared with MFBC. However, BCS for properly selected MFBC is considered oncologically safe, following the same rules of breast conservation for unifocal disease.

Water quality index (WQI) of Narora channel and health of endemic fish Bagarius bagarius and plant Eichhornia crassipes, district Bulandshahar, Uttar Pradesh, India were studied. Among the physicochemical properties of water, pH, D.O, Cr, Fe, Ni, and Cd were above the recommended standards. These factors lead to high WQI (4124.83), indicating poor quality and not suitable for drinking and domestic usage. In fish tissues, the highest metal load was reported in the liver (58.29) and the lowest in the kidney (33.73). Heavy metals also cause a lowering of condition indices. As expected, decreased serum protein (- 63.41%) and liver glycogen (- 79.10%) were recorded in the exposed fish. However, blood glucose (47.22%) and serum glycogen (74.69%) showed elevation. In the plant, roots (21.50) contained the highest, and leaves (16.87) had the lowest heavy metal load. Bioaccumulation factor (BAF) > 1, indicates hyperaccumulation of all metals. E. crassipes roots showed the highest translocation factor (TF) > 1 for Ni (1.57) and Zn (1.30). The high mobility factor (MF) reflected the suitability of E. crassipes for phytoextraction of Mn, Cd, Zn, Fe, Ni, and Cu. Moreover, Bagarius sp. consumption could not pose any non-cancer risk. Although, lower cancer risk can be expected from Ni and Cr.

Background: Despite recent advancements in the respiratory care (RC) profession, no single institution in the Kingdom of Saudi Arabia (KSA) offers a master's degree program in RC. Methods: A nationwide and validated survey was used to explore the current needs and interests in establishing RC master's degree programs in the KSA. The process included representatives from the healthcare industry, universities, and professional societies. Results: A total of 1250 stakeholders across the KSA completed the survey. The sample includes 722 (58%) males, 504 (40%) respiratory therapists, 547 (44%) students, 138 (11%) leaders, and 61 (5%) were academic respondents. Most respondents were from Central 491 (39%) and Eastern 307 (25%) regions, with 1003 (80%) of the total sample worked or studied in governmental sectors. A total of 574 (82%) of the leaders and RTs had Bachelor degree and 430 (61%) of them had 1-5 years working experience. According to 80% of the employers and employees, only 0-5% of the RTs in their organization had a master's degree. The calculated mean % of the agreement (agree/strongly agree) on the needs was 83% in all needs' assessment items, which shows a great support for establishing a master's in RC to meet the personal, professional and society needs. The mean % of the agreement for the level of interests among all participants was 86%, indicating a great level of interests in establishing a master degree in RC. The agreement % on the needs assessment and level of interests in establishing a master degree in RC in KSA were ≥80% in each stakeholder group. Conclusion: There are obvious needs and interests in establishing master's degree programs in RC in the KSA. A master's degree in RC should be established to address the growing needs for advanced RC services throughout the nation and enhance RC research.

BACKGROUND: Autism spectrum disorder (ASD) is a multifaceted neurodevelopmental disorder requiring multimodal intervention and an army of multidisciplinary teams for a proper rehabilitation plan. Accordingly, multiple practice guidelines have been published for different disciplines. However, systematic evidence to detect and intervene must be updated regularly. Our main objective is to compare and summarize the recommendations made in the clinical practice guidelines (CPGs) for ASD in children released from November 2015 to March 2022. METHODS: CPGs were subjected to a systematic review. We developed the inclusion and exclusion criteria and health-related questions, then searched and screened for CPGs utilizing bibliographic and CPG databases. Each of the CPGs used in the study were critically evaluated using the Appraisal of Guidelines for REsearch and Evaluation II (AGREE II) instrument. In a realistic comparison table, we summarized the recommendations. RESULTS: Four eligible CPGs were appraised: Australian Autism CRC (ACRC); Ministry of Health New Zealand (NZ); National Institute for Health and Care Excellence (NICE); and Scottish Intercollegiate Guidelines Network, Healthcare Improvement Scotland (SIGN-HIS). The overall assessments of all four CPGs scored greater than 80%; these findings were consistent with the high scores in the six domains of AGREE II, including: (1) scope and purpose, (2) stakeholder involvement, (3) rigor of development, (4) clarity of presentation, (5) applicability, and (6) editorial independence domains. Domain (3) scored 84%, 93%, 86%, and 85%; domain (5) 92%, 89%, 54%, and 85%; and domain (6) 92%, 96%, 88%, and 92% for ACRC, NICE, NZ, and SIGN-HIS, respectively. Overall, there were no serious conflicts between the clinical recommendations of the four CPGs, but some were more comprehensive and elaborative than others. CONCLUSIONS: All four assessed evidence-based CPGs demonstrated high methodological quality and relevance for use in practice.

Despite the acknowledged injustice and widespread existence of parachute research studies conducted in low- or middle-income countries by researchers from institutions in high-income countries, there is currently no pragmatic guidance for how academic journals should evaluate manuscript submissions and challenge this practice. We assembled a multidisciplinary group of editors and researchers with expertise in international health research to develop this consensus statement. We reviewed relevant existing literature and held three workshops to present research data and holistically discuss the concept of equitable authorship and the role of academic journals in the context of international health research partnerships. We subsequently developed statements to guide prospective authors and journal editors as to how they should address this issue. We recommend that for manuscripts that report research conducted in low- or middle-income countries by collaborations including partners from one or more high-income countries, authors should submit accompanying structured reflexivity statements. We provide specific questions that these statements should address and suggest that journals should transparently publish reflexivity statements with accepted manuscripts. We also provide guidance to journal editors about how they should assess the structured statements when making decisions on whether to accept or reject submitted manuscripts. We urge journals across disciplines to adopt these recommendations to accelerate the changes needed to halt the practice of parachute research.

Diabetes is a life-threatening disease, and chronic diabetes affects parts of the body including the liver, kidney, and pancreas. The root cause of diabetes is mainly associated with oxidative stress produced by reactive oxygen species. Minocycline is a drug with a multi-substituted phenol ring and has shown excellent antioxidant activities. The objective of the present study was to investigate the antidiabetic potential of minocycline-modified silver nanoparticles (mino/AgNPs) against alloxan-induced diabetic mice. The mino/AgNPs were synthesized using minocycline as reducing and stabilizing agents. UV-visible, FT-IR, X-ray diffraction (XRD), and transmission electron microscopy (TEM) were applied for the characterization of mino/AgNPs. A 2,2-diphenyl-1-picrylhydrazyl free radical scavenging assay was conducted to determine the antioxidant potential of newly synthesized mino/AgNPs. The results revealed that the mino/AgNPs showed higher radical scavenging activity (IC50 = 19.7 µg/mL) compared to the minocycline (IC50 = 26.0 µg/mL) and ascorbic acid (IC50 = 25.2 µg/mL). Further, mino/AgNPs were successfully employed to examine their antidiabetic potential against alloxan-induced diabetic mice. Hematological results showed that the mice treated with mino/AgNPs demonstrated a significant decrease in fasting blood glucose level and lipid profile compared to the untreated diabetic group. A histopathological examination confirmed that the diabetic mice treated with mino/AgNPs showed significant recovery and revival of the histo-morphology of the kidney, central vein of the liver, and islet cells of the pancreas compared to the untreated diabetic mice. Hence, mino/AgNPs have good antidiabetic potential and could be an appropriate nanomedicine to prevent the development of diabetes.

Despite the extensive use of carbon steel in all industrial sectors, particularly in the petroleum industry, its low corrosion resistance is an ongoing problem for these industries. In the current work, two malonyl dihydrazide derivatives, namely 2,2'-malonylbis (N-phenylhydrazine-1-carbothiamide (MBC) and N'1, N'3-bis(-2-hydroxybenzylidene) malonohydrazide (HBM), were examined as inhibitors for the carbon steel corrosion in 1.0 M HCl. Both MBC and HBM were characterised using thin-layer chromatography, elemental analysis, infrared spectroscopy, and nuclear magnetic resonance techniques. The corrosion tests were performed using mass loss measurements, polarisation curves, and electrochemical impedance spectroscopy. It is obtained from the mass loss studies that the optimal concentration for both inhibitors is 2.0 × 10-5 mol/L, and the inhibition efficiencies reached up to 90.7% and 84.5% for MBC and HBM, respectively. Electrochemical impedance spectroscopy (EIS) and potentiodynamic polarisation (PDP) indicate an increased impedance in the presence of both MBC and HBM and mixed-type inhibitors, respectively. Both inhibitors can mitigate corrosion in the range of 298-328 K. Values of free energy changes obtained from the Langmuir model suggest that the inhibitors suppress the corrosion process principally by chemisorption. The computational investigations were conducted to identify the factors connected with the anti-corrosive properties of the examined inhibitors.

Triazole fungicides may potentially harm human health. The 'quick, easy, cheap, effective, rugged, and safe' approach has become popular for extraction and cleanup during pesticide residue analysis. We aimed to (a) validate a method for the simultaneous determination of myclobutanil, penconazole, tebuconazole, and triadimenol in squash using LC-MS/MS and (b) determine the pre-harvest intervals (PHIs) and assess the related risk of consuming squash cultivated under open-field conditions in Saudi Arabia. Using acetonitrile as the extraction solvent and fourfold dilution in deionized water led to weak signal suppression (<-6.1%). The limits of quantitation ranged from 10 to 40 µg/kg. Mean recovery and relative standard deviation ranged from 81.7 to 96.3% and from 3.6 to 11.4%. The half-lives ranged from 2.22 to 3.83 days, and the dissipation followed first-order kinetics. The terminal residues of myclobutanil, penconazole, tebuconazole, and triadimenol were <0.771, <0.307, <0.459, and <0.954 mg/kg, respectively, 7 days after two or three applications of recommended dosages. The PHIs of 7.1-11.4, 8.7-13.1, 3.8-5.3, and 11.3-14.3 days are suggested after the application of the recommended dose and double the recommended dose. A consumer risk assessment based on estimated dietary intake indicated that the consumption of squash treated with the recommended doses does not pose a significant health risk.

This study aimed to examine the association between the payer mix and the financial performance of public and private hospitals in Lebanon. The sample comprised 24 hospitals, representing the variety of hospital characteristics in Lebanon. The distribution of the payer mix revealed that the main sources of revenue were public sources (61.1%), out-of-pocket (18.4%) and private insurance (18.2%). Increases in the percentage of revenue from public sources were associated with lower total costs and revenues, but not profit margins. An inverse association was noted between increased revenue from private insurance and profitability, attributed to increased costs. Increased percentage of out of- pocket payments was associated with lower costs and higher profitability. The study provides evidence that payer mix is associated with hospital costs, revenues and profitability. This should initiate/inform discussions between public and private payers and hospitals about the level of payment and its association with hospital sector financial viability.

لقد هدفت هذه الدراسة إلى فحص الارتباط بين خليط الدافعين وبين الأداء المالي للمستشفيات العامة والخاصة في لبنان. ولقد شملت العينة 24 مستشفى تمثل مختلف أنواع المستشفيات في لبنان. فأظهر توزيع خليط الدافعين أن المصادر الرئيسية للإيرادات كانت إما مصادر عامة [%61.1]، أو دفع من الجيب [18.4%]، أو تأمين خاص [18.2%]. وكانت الزيادة في النسبة المئوية للإيرادات من مصادر عامة مرتبطة بانخفاض إجمالي التكاليف والإيرادات، لا بهوامش الربح. كما لوحظ وجود ارتباط عكسي بين زيادة الإيرادات من التأمين الخاص وبين الأرباح، تعزى إلى زيادة التكاليف. وكانت زيادة النسبة المئوية للمدفوعات من الجيب مرتبطة بانخفاض التكاليف وارتفاع الأرباح. إن هذه الدراسة تقدم دليلاً على أن خليط الدافعين يرتبط بتكاليف المستشفيات وإيراداتها وأرباحها. وينبغي أن تسهم هذه النتائج في بدء/إثراء مناقشات بين الدافعين من القطاعين العام والخاص وبين المستشفيات حول مستوى المدفوعات وارتباط ذلك بالجدوى المالية لقطاع المستشفيات

La présente étude visait à examiner l'association entre la pluralité des payeurs et la performance financière des hôpitaux publics et privés au Liban. L'échantillon portait sur 24 hôpitaux, représentant la diversité des caractéristiques des établissements au Liban. La répartition des différents payeurs a révélé que parmi lesprincipales sources de revenus figuraient le secteur public (61,1 %), les paiements directs (18,4 %) et les assurances privées (18,2 %). Des augmentations du pourcentage des revenus provenant du secteur public étaient associéesà des coûts et à des revenus totaux plus faibles, mais non toutefois aux marges bénéficiaires. Une association inverse a été observée entre l'augmentation de revenus issus des assurances privées et la rentabilité, attribuée à des coûts plus élevés. Une hausse du pourcentage de paiements directs était associée à des coûts moindres et à une meilleure rentabilité. La présente étude fournit des élèments de preuve montrant que la pluralité de payeurs est associée aux coûts, aux revenus et à la rentabilité des hôpitaux. Cette situation devrait non seulement lancer des discussions entre les payeurs publics et privés et les hôpitaux sur le niveau de paiement et son association à la viabilité financière du secteur hospitalier, mais également contribuer aux débats à ce sujet.

RESUMO

This mixed design study explored attitudes of dental students towards use of computers. It employed quantitative analyses of a questionnaire answered by 979 students and qualitative analysis of suggestions by a subgroup of 339. Positive attitudes towards computers were predicted by "computer use for > 1 year" and "year of study". Qualitative analysis of students' suggestions confirmed these findings and brought up new issues such as the need for establishing a website for the faculty. The results indicate that careful planning is needed to improve students' skills and incorporate computer applications in educational curricula.

In a prospective controlled study, we aimed to determine the prevalence of Chlamydia trachomatis infection in Jordanian women attending an infertility clinic and whether screening is useful as part of routine investigations for infertility. Two groups of patients (152 infertile patients and 146 control patients) had endocervical swab testing by polymerase chain reaction for the presence of C. trachomatis infection. A total of 6/152 patients in the infertility group tested positive for C. trachomatis (3.9%), compared with 1/146 patients in the control group (0.7%), a difference that was not statistically significant. In view of the tendency toward increased prevalence of C. trachomatis infection, screening women for chlamydial infection as part of routine investigations for infertility is recommended.

BACKGROUND: Occupational injuries and illnesses are a major preventable public health problem. METHODS: This study evaluated the incidence, nature, and cause of awarded workers' compensation claims for a large state university and teaching hospital. Rates and types of injury were compared across age and gender. RESULTS: Rates of injury varied over twofold with age, with those 16-25 having the lowest rates of injury and those 36-45 having the highest rates. Claims rates were 1.36-fold higher for women than men. Women had higher rates for injury resulting from lifting, falling, noxious exposures, repetitive motion, and carpal tunnel syndrome. Similarly women had significantly higher rates of claims for pain, sprains, bruises, burns, concussion, and inhalation injury; with lower rates of cuts, ligament injury, and jammed joints. CONCLUSIONS: These group differences suggest the need to examine age and gender job distributions and relevant ergonomic and environmental causative factors.

This research examined the performance of rural hospitals engaged in different levels of managed care activities and identified factors related to performance and competition that affected rural hospitals' likelihood of pursuing managed care as a strategy. The sample studied consisted of 139 rural hospitals in Iowa and Nebraska. Results showed that a relatively high percentage of hospitals were engaged in managed care activities, mainly through contractual arrangements. The study found that high competition in the marketplace increased the likelihood of hospitals pursuing managed care strategies, while high demand markets had a negative association with the likelihood of pursuing a managed care strategy. No significant relationship was detected between poor performance and pursuing a managed care strategy.

PIP: Full histories, clinical examinations, and anthropometric measurements were taken on 392 male and 352 female preschool children 1-71 months of age in Bani Ahmed Village, El-Minia Governorate, to assess their growth, the prevalence of protein-energy malnutrition, and the influence of selected risk factors upon subjects' nutritional status. The children were selected for inclusion in the study through systematic random sampling. They were found to be both lighter and shorter than children of similar ages studied in Assiut, Lower Egypt, some developing countries, and the developed countries. The weight, height, and head circumference of males were not significantly different than those of females. There was a 72.2% prevalence of malnutrition, with the highest prevalence of 83.7% being in the age group 6-23 months. The prevalence of wasting was 31.6%, with the highest prevalence of 62% also being in the age group 6-23 months. The prevalence of stunting was 76.5%, highest among children aged 24-35 months at 87.4%. Low socioeconomic status, increased birth order, recurrent diarrhea, and non-breastfeeding were high risk factors for the prevalence of malnutrition in the sample.^ieng

Twenty-three percent of deaths to women of reproductive age (15-49 years) in Bali, Indonesia and Menoufia, Egypt were due to maternal causes. Among the younger women, the percentage was even higher. In both areas complications of pregnancy and childbirth were a leading cause of death (the first cause in Bali, the second in Menoufia). In both sites, postpartum hemorrhage was the most common cause of maternal death. Relative to the United States, the number of maternal deaths per 100,000 live births was 20 times higher in Menoufia and 78 times higher in Bali. Families of women of reproductive age who died were interviewed about the conditions leading to death and other characteristics of the deceased. Completed histories were reviewed by a Medical Panel who were able to assign a cause of death in more than 90% of cases. Two-thirds of the maternal deaths occurred to women who were over 30 and/or who had 3 children--the usual targets of family planning programs. Other possible intervention strategies include antenatal outreach programs, training of traditional birth attendants, and better hospital management of obstetric emergencies.
