Background: Despite international efforts to improve reproductive health indicators, little attention is paid to the contributions of contextual factors to modern contraceptive coverage, especially in the Latin America and the Caribbean (LAC) region. This study aimed to identify the association between country-level Gender Inequality and Health Expenditure with demand for family planning satisfied by modern contraceptive methods (DFPSm) in Latin American sexually active women. Methods: Our analyses included data from the most recent (post-2010) Demographic and Health Survey or Multiple Indicator Cluster Survey from 14 LAC countries. Descriptive analyses and multilevel logistic regressions were performed. Six individual-level factors were included. The effect of the country-level factors Gender Inequality Index (GII) and Current Health Expenditure on DFPSm was investigated. Findings: DFPSm ranged from 41.8% (95% CI: 40.2-43.5) in Haiti to 85.6% (95% CI: 84.9-86.3) in Colombia, with an overall median coverage of 77.8%. A direct association between the odds of DFPSm and woman's education, wealth index, and the number of children was identified. Women from countries in the highest GII tertile were less likely (OR: 0.32, 95% CI: 0.13-0.76) to have DFPSm than those living in countries in the lowest tertile. Interpretation: Understanding the contribution of country-level factors to modern contraception may allow macro-level actions focused on the population's reproductive needs. In this sense, country-level gender inequalities play an important role, as well as individual factors such as wealth and education. ABSTRACT OBJECTIVE To determine the total demand and unmet need for contraception with modern methods and their determinants among mothers participating in the 2015 Pelotas Birth Cohort. METHODS Data from the 48-month follow-up of mothers participating in the 2015 Pelotas Birth Cohort were analyzed. Only biological mothers (aged up to 49 years) of children belonging to the 2015 Birth Cohort and who answered the 48-month questionnaire were included in the study sample. Logistic regression and respective 95% confidence intervals were used to determine associated factors. RESULTS The study sample consisted of 3577 biological mothers. The prevalence of use of any contraceptive and of modern contraceptives was 86.0% (95%CI: 84.8-87.1) and 84.9% (95%CI: 83.7-86.1), respectively. The prevalence of unmet need for modern contraceptives was 10.7% (95%CI: 9.7-11.7), and the total demand for contraceptives was 95.6%. The factors associated with an unmet need for modern contraception were being over 34 years of age (OR = 0.6, 95%CI: 0.5-0.8), not having a husband or partner (OR = 1.9, 95%CI: 1.4-2.6), not being the head of the household (OR = 0.6, 95%CI: 0.4-0.9), having had three or more pregnancies (OR = 1.9, 95%CI: 1.3-2.6), and having had an abortion at least once after the birth of the child participating in the cohort (OR = 1.9, 95%CI: 1.0-3.6). CONCLUSIONS Despite the high prevalence of modern contraceptive use, one in ten women had an unmet need for modern contraception and was at risk of unplanned pregnancy.

Indiscriminate use of anti-bacterial agents during pregnancy can increase antimicrobial resistance and endanger both the mother's and the children's health. Currently, Brazil has the Collegiate Directive Resolution n. 20/2011, which controls prescription and dispensation of anti-bacterial agents. Given this scenario, this study compared the use of anti-bacterial agents by pregnant women participating in the 2004 and 2015 Pelotas (Brazil) birth cohorts, in Rio Grande do Sul, Brazil, considering the regulation issued between the two cohorts. Data were collected in the perinatal period of the two studies. The main outcome was the use of anti-bacterial agents during pregnancy. Prevalence scans were described based on independent variables and differences in percentage points (p.p.) between the two cohorts. The prevalence of anti-bacterial use was 41.9% (95%CI: 40.4; 43.3) in 2004 and 39.2% (95%CI: 37.7; 40.6) in 2015. Considering the pregnant women who reported having infection during pregnancy, a greater reduction in use was observed in 2015, when compared to 2004, in poor women (-15.4p.p., 95%CI: 9.59; 21.20) and in those who had less consultations (-17.1p.p., 95%CI: 2.81; 31.36). Considering total medications, the proportion of anti-bacterial used dropped from 20.6% (95%CI: 19.9; 21.4) in 2004 to 12.6% (95%CI: 12.1; 13.1) in 2015. The reductions found in both the prevalence of use and the proportion of anti-bacterial agents over total medications used may be a reflection of the regulatory policy implemented in 2011.


A utilização indiscriminada de antibacterianos no período gestacional pode aumentar a resistência antimicrobiana e colocar em risco a saúde da gestante e da criança. Atualmente, está em vigência no Brasil a Resolução da Diretoria Colegiada nº 20/2011, que controla a prescrição e fornecimento de antibacterianos. O objetivo deste estudo foi comparar o uso de antibacterianos pelas gestantes participantes das coortes de nascimentos de Pelotas, Rio Grande do Sul, Brasil, de 2004 e 2015, considerando a regulamentação implementada entre as duas coortes. Foram utilizados dados coletados no período perinatal dos dois estudos. O desfecho principal foi o uso de antibacterianos na gestação. As prevalências de uso foram descritas a partir de variáveis independentes e diferenças em pontos percentuais (p.p.) entre as duas coortes. A prevalência do uso de antibacterianos foi de 41,9% (IC95%: 40,4; 43,3) em 2004 e 39,2% (IC95%: 37,7; 40,6) em 2015. El uso indiscriminado de antibacterianos durante el embarazo puede aumentar la resistencia a los antimicrobianos y poner en riesgo la salud de la gestante y del niño. Actualmente, está vigente en Brasil la Resolución de la Dirección Colegiada nº 20/2011, que controla la prescripción y dispensación de antibacterianos. El objetivo de este estudio fue comparar el uso de antibacterianos por gestantes participantes de las cohortes de nacimientos de Pelotas, Rio Grande do Sul, Brasil, del 2004 y del 2015, considerando la regulación implementada entre las dos cohortes. Se utilizaron los datos recopilados en el período perinatal de los dos estudios. El resultado principal fue el uso de antibacterianos durante el embarazo. Las prevalencias de uso se describieron con base en las variables independientes y diferencias en puntos porcentuales (p.p.) entre las dos cohortes. Hubo una reducción en la proporción de antibacterianos usados, considerando la cantidad total de medicamentos del 20,6% (IC95%: 19,9; 21,4) en el 2004 al 12,6% (IC95%: 12,1; 13,1) en el 2015. Las reducciones encontradas, tanto en las prevalencias de uso como en la proporción de antibacterianos sobre la cantidad total de medicamentos utilizados, pueden ser reflejo de la política regulatoria implementada en el 2011.

The aim of this study was to investigate the association of sociodemographic factors, lifestyle, maternal reproductive profile and prenatal and childbirth care with neonatal near miss (NNM) morbidity in four birth cohorts. This study involved four population-based birth cohorts: Ribeirão Preto (RP) and São Luís (SL) (2010), Pelotas 2004 (PEL04) and 2015 (PEL15). NNM was defined when one or more of the following conditions were present: birthweight <1,500 g, 5-minute Apgar score <7, gestational age <32 weeks, and report of congenital malformations. The covariates were obtained with questionnaires applied to the puerperal women. Some particularities between cohorts were identified. In the RP and SL cohorts, factors of the more distal levels (sociodemographic, lifestyle, and reproductive profile) were associated with NNM. On the other hand, proximal factors related to healthcare were more significant for the occurrence of NNM in PEL. Only the absence of prenatal care was associated with NNM in all cohorts: RP (OR=4.27, 95%CI 2.16-8.45), SL (OR=2.32, 95%CI 1.09-4.94), PEL04 (OR=4.79, 95%CI 1.59-14.46), and PEL15 (OR=5.10, 95%CI 2.60-9.97).

[ABSTRACT]. Objectives. To estimate inequalities in demand for family planning satisfied with modern methods among women in Latin America and the Caribbean, with an emphasis on Brazil and Mexico, and to calculate the scenario for recovery of modern contraceptive coverage by expanding access to long-acting contraceptives (LARC) after the COVID-19 pandemic. Methods. National health surveys from 2006 to 2018 were used to estimate the demand for family planning satisfied with modern methods and how it was affected by the COVID-19 pandemic. The scenario included three variables: coverage, health outcomes, and costs. Considering coverage, United Nations Population Fund data were used to estimate the impact of COVID-19 on access to contraception in Latin America and the Caribbean. Health outcomes were assessed with the Impact 2 tool. Direct investment was used to evaluate cost-effectiveness. Results. Substantial inequalities were found in the use of modern contraceptive methods before the pandemic. We showed the potential cost-effectiveness of avoiding maternal deaths by introducing LARCs. Conclusions. In the scenario predicted for Brazil and Mexico, the costs of modern family planning and averted disability-adjusted life years are modest. Governments in Latin America and the Caribbean should consider promoting LARCs as a highly efficient and cost-effective intervention.


[RESUMEN]. Objetivos. Estimar las desigualdades en la demanda de planificación familiar satisfecha con métodos anticonceptivos modernos entre las mujeres de América Latina y el Caribe, especialmente en Brasil y México, y analizar el escenario de recuperación de la cobertura de los anticonceptivos modernos mediante la ampliación del acceso a los anticonceptivos de acción prolongada tras la pandemia de COVID-19. Métodos. Se emplearon encuestas nacionales de salud desde el año 2006 hasta el año 2018 para estimar la demanda de planificación familiar satisfecha con métodos modernos y el impacto de la pandemia de COVID-19. El escenario comprendía tres variables: cobertura, resultados en materia de salud y costos. En lo respectivo a la cobertura, se emplearon datos del Fondo de Población de las Naciones Unidas para evaluar la repercusión de la COVID-19 en el acceso a los anticonceptivos en América Latina y el Caribe. Los resultados en materia de salud se examinaron con la herramienta Impact 2. Se empleó la inversión directa para evaluar la costo-efectividad. Resultados. Se encontraron desigualdades sustanciales en el uso de métodos anticonceptivos modernos antes de la pandemia. Se demostró la posible costo-efectividad de evitar muertes maternas mediante la introducción de anticonceptivos de acción prolongada. Conclusiones. De acuerdo con el escenario previsto para Brasil y México, los costos de la planificación familiar moderna y los años de vida ajustados en función de la discapacidad evitados son moderados. Estimar as desigualdades na demanda por planejamento familiar atendida por métodos contraceptivos modernos em mulheres da América Latina e do Caribe, com ênfase no Brasil e no México, e calcular o cenário de recuperação da cobertura por métodos contraceptivos modernos por meio da ampliação do acesso a métodos contraceptivos reversíveis de longa duração (LARC) após a pandemia de COVID-19. Métodos. Foram usadas pesquisas nacionais de saúde de 2006 a 2018 para estimar a demanda por planejamento familiar atendida por métodos contraceptivos modernos e como ela foi afetada pela pandemia de COVID-19. O cenário incluiu três variáveis: cobertura, desfechos de saúde e custos. Para cobertura, os dados do Fundo de População das Nações Unidas foram usados para estimar o impacto da COVID-19 no acesso à contracepção na América Latina e no Caribe. Desfechos de saúde foram avaliados com a ferramenta Impact 2. O investimento direto foi usado para avaliar a relação custo-benefício. Resultados. Foram constatadas desigualdades importantes no uso de métodos contraceptivos modernos antes da pandemia. ABSTRACT OBJECTIVE Assessing the regular consumption of ultra-processed foods by children at 24 months of age from the 2015 Pelotas Birth Cohort and the main demographic, socioeconomic, and behavioral factors related to the consumption of these products. METHODS Population-based cohort in the city of Pelotas, RS, where 4,275 children were assessed at birth and 95.4% of them were followed up until 24 months of age. Food consumption was assessed by a questionnaire on regular consumption of ultra-processed foods, which collected information regarding sex, household income, maternal skin color, schooling level, and age, the child attending day care and having siblings, breastfeeding status, and obesity. The outcome was the sum of ultra-processed foods regularly consumed by a child. A multivariate Poisson regression analysis was used to calculate the association between the regular consumption of ultra-processed foods and exposure variables. RESULTS The mean number of ultra-processed foods consumed was 4.8 (SD = 2.3). The regular consumption of ultra-processed foods was positively associated with black skin color and having siblings, and negatively associated with household income and maternal schooling level and age. CONCLUSION The mean regular consumption of ultra-processed foods by children from the 2015 Pelotas Birth Cohort is high, which can negatively affect the children's diet. The risk of consuming this kind of food was higher among children from families of lower socioeconomic status, whose mothers present lower education level, black skin color, and younger age.


RESUMO OBJETIVO Avaliar o consumo habitual de alimentos ultraprocessados aos 24 meses de idade por crianças pertencentes à Coorte de Nascimentos de Pelotas de 2015 e os principais fatores demográficos, socioeconômicos e comportamentais relacionados ao consumo desses produtos. MÉTODOS Coorte de base populacional na cidade de Pelotas-RS, onde foram avaliadas 4.275 crianças ao nascimento, das quais 95,4% foram acompanhadas até os 24 meses. O consumo alimentar foi avaliado por meio de um questionário de consumo habitual de alimentos ultraprocessados, coletando informações sobre sexo, renda familiar, cor da pele, escolaridade e idade da mãe, frequentar creche, ter irmãos, status de amamentação e obesidade. O desfecho foi a somatória de alimentos ultraprocessados consumidos habitualmente pela criança. Análise multivariada por regressão de Poisson foi utilizada para estimar a associação entre consumo habitual de alimentos ultraprocessados e as variáveis de exposição. RESULTADOS O número médio de alimentos ultraprocessados consumidos habitualmente foi de 4,8 (DP = 2,3). O consumo habitual de alimentos ultraprocessados foi associado positivamente à cor da pele preta e ter irmãos e negativamente associado com renda familiar, escolaridade e idade materna. CONCLUSÕES A média de consumo habitual de alimentos ultraprocessados por crianças pertencentes à Coorte de Nascimentos de 2015 da cidade de Pelotas é elevada, o que pode causar um efeito negativo na dieta das crianças. O risco de consumo desses alimentos foi maior entre crianças de famílias de menor posição socioeconômica, filhas de mães de baixa escolaridade, de cor da pele preta, mais jovens e de baixa renda.

Monitoring trends of contraceptive use and identifying the groups with less coverage are needed to guide public policies and make them more efficient. But, in Brazil, recent data about these aspects are limited. This study aimed to investigate the prevalence of contraceptive use and its inequalities during adolescence and early adulthood. Data from the 1993 Pelotas birth cohort, Rio Grande do Sul State, Brazil, were used. At 15, 18 and 22 years, respectively, 335, 1,458 and 1,711 women reported having started their sexual lives and were included in analysis. Prevalence and 95% confidence intervals were obtained to describe the most used contraceptive methods. Inequalities in modern contraceptive use were evaluated according to wealth index, scholastic backwardness and ethnicity. In all follow-ups, more than 80% of women used at least one modern method. The use of barrier methods decreased with age; at 22 this prevalence was 36.3%. Such use concomitant with other modern methods was lower than 50% in all follow-ups. We observed inequalities in the use of modern contraceptive methods, mainly in barrier methods used with other modern methods. These findings may contribute and improve the public policies in family planning.

BACKGROUND: Inadequate antenatal care (ANC) has been associated with adverse pregnancy outcomes. ANC quality is considered a key component of the right to health and a route to equity and dignity for women and their children. Although ANC coverage is relatively high in Brazil, there are revealed some health disparities when coverage is examined by socio-demographic determinants. In this study we evaluated ANC quality and its socio-demographic determinants using data from the 2015 Pelotas birth cohort, Rio Grande do Sul, Brazil. METHODS: This study is part of the 2015 Pelotas population-based birth cohort (n = 3923 pregnant women) conducted in southern Brazil. ANC quality was assessed through 19 content and service utilization indicators recommended by the Brazilian Ministry of Health. Descriptive analyses and associations of each of the ANC indicators and independent variables were performed using the chi-square and linear trend test. ANC indicators were analyzed individually and aggregated as a score. Associations between ANC score quality and socio-demographic variables were assessed with ordinal regressions. Mediation analysis with G-computation was performed to estimate direct and indirect effect of mother's level of education on ANC quality mediated by the number of consultations and timing of ANC initiation. Base and post confounders were included. RESULTS: The results showed that except for breast examination, height measurement, tetanus toxoid vaccination and ANC starting at the first trimester, all ANC indicators showed more than 80% coverage during ANC visits. In the adjusted analysis, inadequate quality ANC was associated with lower maternal education level, not having a partner, being multiparous, being attended by a private provider and by the same professional in all consultations. In the mediation analyses, 6.8% of the association between ANC quality and mother's education was mediated by the trimester in which ANC started, while 12.8% was mediated by the number of ANC visits. CONCLUSIONS: ANC quality is associated with pregnant women's socio-demographic characteristics. Significant efforts are needed to improve the quality of facility-based maternity care.

Fertility reduction is a phenomenon observed in demographic transition. The demographic changes noted in female fertility represent a need for adjustment on health services regarding female health and family planning support. Thus, this study aimed to perform a descriptive analysis by tracing the sociodemographic profile of primiparous mothers belonging to nine Brazilian birth cohorts, in three cities from different states. Standardized questionnaires were applied to assess reproductive characteristics and covariables. Primiparous mothers were defined as women whose child included in birth cohorts was their firstborn child. Sample description was performed using analysis of variance (continuous variables) and chi-square (categorical variables). In total, 44,615 women were included in the analyses and 41.8% (95%CI: 41.3; 42.2) were categorized as primiparous. The primiparity rates were the lowest in Ribeirão Preto (São Paulo State) 1978 (32%; 95%CI: 30.9; 33.1) and the highest in most recent cohorts, reaching up to 50% of the participants (São Luís - Maranhão State 2010: 47.2%; 95%CI: 45.8; 48.6; Ribeirão Preto 2010: 50.2%; 95%CI: 49.1; 51.4); Pelotas (Rio Grande do Sul State) 2015: 49.4% (95%CI: 47.9; 50.9). Primiparous mothers' age and schooling increased over the years in all cohorts. Maternal age at the first childbirth behaved similarly in the three studied cities. There was an increase in the proportion of first-time mothers that were older, higher educated and belonged to richer income groups. Also, the proportion of teenage mothers (aged 15 years or younger) increased until the early 2000's and started decreasing around the years 2010, especially among women in the poorer income groups.

This paper describes the history, objectives and methods used by the nine Brazilian cohorts of the RPS Brazilian Birth Cohorts Consortium (Ribeirão Preto, Pelotas and São Luís) Common thematic axes are identified and the objectives, baseline periods, follow-up stages and representativity of the population studied are presented. The Consortium includes three birth cohorts from Ribeirão Preto, São Paulo State (1978/1979, 1994 and 2010), four from Pelotas, Rio Grande do Sul State (1982, 1993, 2004 and 2015), and two from São Luís, Maranhão State (1997 and 2010). The cohorts cover three regions of Brazil, from three distinct states, with marked socioeconomic, cultural and infrastructure differences. The cohorts were started at birth, except for the most recent one in each municipality, where mothers were recruited during pregnancy. The instruments for data collection have been refined in order to approach different exposures during the early phases of life and their long-term influence on the health-disease process. The investigators of the nine cohorts carried out perinatal studies and later studied human capital, mental health, nutrition and precursor signs of noncommunicable diseases. A total of 17,636 liveborns were recruited in Ribeirão Preto, 19,669 in Pelotas, and 7,659 in São Luís. In the studies starting during pregnancy, 1,400 pregnant women were interviewed in Ribeirão Preto, 3,199 in Pelotas, and 1,447 in São Luís. Different strategies were employed to reduce losses to follow-up. This research network allows the analysis of the incidence of diseases and the establishment of possible causal relations that might explain the health outcomes of these populations in order to contribute to the development of governmental actions and health policies more consistent with reality.

Monitoring trends of contraceptive use and identifying the groups with less coverage are needed to guide public policies and make them more efficient. But, in Brazil, recent data about these aspects are limited. This study aimed to investigate the prevalence of contraceptive use and its inequalities during adolescence and early adulthood. Data from the 1993 Pelotas birth cohort, Rio Grande do Sul State, Brazil, were used. At 15, 18 and 22 years, respectively, 335, 1,458 and 1,711 women reported having started their sexual lives and were included in analysis. Prevalence and 95% confidence intervals were obtained to describe the most used contraceptive methods. Inequalities in modern contraceptive use were evaluated according to wealth index, scholastic backwardness and ethnicity. In all follow-ups, more than 80% of women used at least one modern method. The use of barrier methods decreased with age; at 22 this prevalence was 36.3%. Such use concomitant with other modern methods was lower than 50% in all follow-ups. We observed inequalities in the use of modern contraceptive methods, mainly in barrier methods used with other modern methods. These findings may contribute and improve the public policies in family planning.


É necessário monitorar as tendências no uso de métodos contraceptivos e identificar os grupos com menor cobertura, a fim de orientar as políticas públicas e torná-las mais eficientes. Entretanto, no Brasil, são limitados os dados recentes sobre a cobertura dos métodos contraceptivos. O estudo buscou investigar a prevalência do uso de métodos contraceptivos e as desigualdades no uso durante a adolescência e início da vida adulta. Foram usados dados da coorte de nascimentos de Pelotas de 1993, Rio Grande do Sul, Brasil. Aos 15, 18 e 22 anos, respectivamente, 335, 1.458 e 1.711 mulheres informaram já terem iniciado a vida sexual e foram incluídas na análise. Foram obtidas as prevalências e intervalos de 95% de confiança para descrever os métodos contraceptivos mais utilizados. As desigualdades no uso de métodos contraceptivos modernos foram avaliadas de acordo com o índice de riqueza, atraso escolar e cor da pele. Em todos os seguimentos, mais de 80% das mulheres informavam usar pelo menos um método moderno. O uso de métodos de barreira diminuiu com a idade (prevalência de 36,3% aos 22 anos). Esse uso junto com outro método moderno era menos de 50% em todos os seguimentos. Foram observadas desigualdades no uso de métodos contraceptivos modernos, principalmente no uso de método de barreira junto com outro método moderno. Os achados podem contribuir para melhorar as políticas públicas em planejamento familiar.

El estudio buscó investigar la prevalencia del uso de métodos contraceptivos y las desigualdades en su uso, durante la adolescencia e inicio de la vida adulta. Se usaron datos de la cohorte de nacimientos de 1993 en Pelotas, Río Grande do Sul, Brasil. A los 15, 18 y 22 años, respectivamente, 335, 1.458 y 1.711 mujeres informaron ya haber iniciado la vida sexual y fueron incluidas en el análisis. Se obtuvieron las prevalencias e intervalos de 95% de confianza para describir los métodos contraceptivos más utilizados. Las desigualdades en el uso de métodos contraceptivos modernos fueron evaluadas de acuerdo con el índice de riqueza, atraso escolar y color de piel. En todos los seguimientos, más de un 80% de las mujeres informaban usar por lo menos un método moderno. El uso de métodos de barrera disminuía con la edad (prevalencia de 36,3% a los 22 años). Ese uso junto con otro método moderno era menos de un 50% en todos los seguimientos. Resumo: O artigo descreve a história, objetivos e métodos utilizados pelas nove coortes do Consórcio RPS de Coortes de Nascimento. São identificados eixos temáticos comuns, com apresentação dos objetivos, anos de linha de base, fases de seguimento e representatividade das populações de estudo. O Consórcio inclui três coortes de nascimento de Ribeirão Preto, Estado de São Paulo (1978/1979, 1994 e 2010), quatro de Pelotas, Estado do Rio Grande do Sul (1982, 1993, 2004 e 2015) e duas de São Luís, Estado do Maranhão (1997 e 2010). As coortes provêm de três regiões do Brasil, de três estados diferentes, com importantes diferenças socioeconômicas, culturais e de infraestrutura. As coortes foram iniciadas ao nascer dos participantes, exceto a mais recente em cada município, onde as mães foram recrutadas durante a gestação. Os instrumentos para a coleta de dados foram refinados para aproximar diferentes exposições na primeira infância e a influência, a longo prazo, no processo saúde-doença. Os investigadores das nove coortes realizaram estudos perinatais e depois examinaram o capital humano, saúde mental, nutrição e sinais percursores de doenças crônicas. Um total de 17.636 nascidos vivos foram recrutados em Ribeirão Preto, 19.669 em Pelotas e 7.659 em São Luís. Nas coortes que foram iniciadas durante a gestação, foram entrevistadas 1.400 gestantes em Ribeirão Preto, 3.199 em Pelotas e 1.447 em São Luís. Foram utilizadas diferentes estratégias para reduzir as perdas de seguimento. A rede de pesquisa do Consórcio permite analisar a incidência de doenças e identificar possíveis relações causais que podem explicar os desfechos de saúde nessas populações e contribuir para o desenvolvimento de medidas públicas e políticas de saúde que estejam mais de acordo com as respectivas realidades.

Se identificaron los ejes temáticos comunes y los objetivos, así como los periodos de referencia, la presentación del estadio de seguimiento y representatividad de la población estudiada. El consorcio incluye tres cohortes de nacimiento de Ribeirão Preto, Estado de São Paulo (1978/1979, 1994 y 2010), cuatro de Pelotas, Estado del Rio Grande do Sul (1982, 1993, 2004 y 2015), y dos de São Luís, Estado del Maranhão (1997 y 2010). Las cohortes cubren tres regiones de Brasil, de tres estados distintos, con marcadas diferencias socioeconómicas, culturales y de infraestructura. Las cohortes comenzaron con el nacimiento, excepto para la más reciente en cada municipio, donde las madres fueron reclutadas durante la gestación. Los instrumentos para la recogida de datos han sido depurados, con el fin de realizar una aproximación a diferentes exposiciones durante las fases tempranas de la vida y su influencia a largo plazo en el proceso de salud-enfermedad. Se incluyeron a los investigadores de las nueve cohortes, donde se llevaron a cabo estudios perinatales, así como los recursos humanos analizados posteriormente, al igual que la salud mental, nutrición y signos precursores de enfermedades no comunicables. Un total de 17.636 nacidos vivos fueron reclutados en Ribeirão Preto, 19.669 en Pelotas, y 7.659 en São Luís. A redução na taxa de fertilidade é um fenômeno observado na transição demográfica. As mudanças demográficas na fertilidade feminina criam a necessidade de ajustes nos serviços de saúde em termos de saúde da mulher e planejamento familiar. Assim, o estudo buscou realizar uma análise descritiva, traçando o perfil sociodemográfico de mães primíparas em nove coortes de nascimentos, de três cidades em estados diferentes do Brasil. Foram aplicados questionários padronizados para avaliar as características reprodutivas e covariáveis. As mães primíparas foram definidas como mulheres cujos filhos incluídos nas coortes de nascimentos eram os seus primogênitos. A descrição da amostra foi realizada com a análise de variância (variáveis contínuas) e qui-quadrado (variáveis categóricas). Um total de 44.615 mulheres foram incluídas nas análises, e 41,8% (IC95%: 41,3; 42,2) foram categorizadas como primíparas. As taxas de primiparidade foram mais baixas em Ribeirão Preto (São Paulo) em 1978 (32%; IC95%: 30,9; 33,1), e as mais altas foram nas coortes mais recentes, atingindo 50% das participantes (São Luís - Maranhão, 2010: 47,2%; IC95%: 45,8; 48,6; Ribeirão Preto, 2010: 50,2%; IC95%: 49,1; 51,4); Pelotas (Rio Grande do Sul) 2015: 49,4% (IC95%: 47,9; 50,9). A idade e escolaridade das primíparas aumentaram ao longo dos anos em todas as coortes. A idade materna no primeiro parto mostrou perfil semelhante nas três cidades. Houve um aumento na proporção de primíparas mais velhas, com maior escolaridade e pertencentes aos estratos econômicos mais altos. Além disso, a proporção de mães adolescentes (até 15 anos de idade) aumentou até o início da década de 2000 e começou a diminuir em torno dos anos 2010, especialmente entre mulheres nos grupos de menor renda.


La reducción de la fertilidad es un fenómeno observado en la transición demográfica. Los cambios demográficos observados en la fertilidad femenina representan la necesidad de un ajuste en los servicios de salud, en términos de salud femenina y asistencia a la planificación familiar. Teniendo esto en consideración, este estudio tuvo como objetivo realizar un análisis descriptivo, mediante el seguimiento del perfil sociodemográfico de las madres primíparas, que pertenecen a nueve cohortes de brasileñas nacimiento, en tres ciudades de diferentes estados. Se aplicaron cuestionarios estandarizados para evaluar las características reproductivas y covariables. Las madres primíparas fueron definidas como mujeres cuyos hijos incluidos en las cohortes de nacimiento eran sus primogénitos. La descripción de la muestra se realizó usando un análisis de variancia (variables continuas) y chi-cuadrado (variables categóricas). Un total de 44.615 mujeres estuvieron incluidas en el análisis y un 41,8% (IC95%: 41,3; 42,2) fueron categorizadas como primíparas. La tasas de primiparidad fueron las más bajas en Ribeirão Preto (Estado de São Paulo) 1978 (32%; 95%CI: 30,9; 33,1) y las más altas en las cohortes más recientes, alcanzando hasta a un 50% de los participantes (São Luís - Estado de Maranhão 2010: 47,2%; IC95%: 45,8; 48,6; Ribeirão Preto 2010: 50,2%; IC95%: 49,1; 51,4); Pelotas (Estado de Rio Grande do Sul) 2015: 49,4% (IC95%: 47,9; 50,9). La edad de las madres primíparas y escolaridad se incrementó a lo largo de los años en todas las cohortes. La edad maternal en el momento del nacimiento del primer hijo se comportó de forma similar en las tres ciudades estudiadas. Hubo un incremento en la proporción de las madres por primera vez, que eran más viejas, con más formación educativa, y pertenecientes a los grupos con ingresos más altos. Asimismo, la proporción de las madres adolescentes (15 años o más jóvenes) se incrementó hasta principios del año 2000 y empezó a decrecer a partir del año 2010, especialmente entre mujeres en los grupos con los ingresos más bajos.

OBJECTIVES: To assess time trends in unplanned pregnancy, stratified by sociodemographic status, reproductive history, and inequalities in family income and women's skin color, in Pelotas, Brazil. METHODS: Data from mothers of participants of the 1993 (N = 5264), 2004 (N = 4243), and 2015 (N = 4268) Pelotas birth cohorts were analyzed. Unplanned pregnancy was investigated in the perinatal period, with tests to assess changes over time among different sociodemographic and reproductive history subgroups and inequalities as a function of family income and skin color. RESULTS: The prevalence of unplanned pregnancy was 62.7% (3299/ 5264), 65.9% (2794/ 4243), and 52.2% (2226/ 4268) in the 1993, 2004, and 2015 cohorts, respectively. Black or brown women and women of lower socioeconomic status had a higher prevalence of unplanned pregnancy in all cohorts. The overall rate of unplanned pregnancy decreased over time in most subgroups. Inequality as a function of family income and skin color increased during the time frame of assessment. CONCLUSIONS: The prevalence of unplanned pregnancies decreased in the period analyzed, but it is still unjustifiably high. Efforts aimed at reducing unplanned pregnancy are vital and will require special attention to the most vulnerable groups.

Although most childbirth care in Brazil is financed by the Brazilian Unified National Health System (SUS), there are out-of-pocket expenditures (private personal costs) involved in births. This study aims to compare maternal out-of-pocket expenditures in births of children from the Pelotas Birth Cohorts of 2004 and 2015. The study drew on information collected right after birth and at three months of age. The target variables include sociodemographic and economic data, private health plan coverage, and expenditures related to the birth. Values from 2004 were adjusted to 2015 by the general price index. There was an increase in private health plan coverage from 33.4% (95%CI: 31.9-34.9) to 45.1% (95%IC: 43.6-46.7) in the target period, directly associated with the families' socioeconomic status (p < 0.001). There was an increase in mean expenditures on hospitalization for the birth, from BRL 60.38 (SD = 288.66) to BRL 171.15 (SD = 957.07), and in additional medical expenditures, from BRL 191.60 (SD = 612.86) to BRL 1,424.80 (SD = 4,459.16) among mothers admitted to hospital under their private health plans (and there was no significant difference in these expenditures for mothers that opted for direct payment). There was an important increase in expenditures for childbirth care, especially among mothers admitted to hospital under private health plans.

Resumo: Apesar de a maioria dos partos no Brasil ser financiada pelo Sistema Único de Saúde (SUS), existem gastos diretos (pessoais privados) envolvidos no nascimento. Este estudo visa a comparar o desembolso materno para financiar os partos das crianças pertencentes às coortes de nascimento de Pelotas de 2004 e 2015. Foram utilizadas informações coletadas logo após o nascimento e aos três meses de idade. As variáveis analisadas incluem informações sociodemográficas, econômicas, cobertura por plano privado de saúde e despesas relacionadas ao parto. Os valores de 2004 foram ajustados pelo Índice Nacional de Preços ao Consumidor Amplo (IPCA). Observou-se aumento na posse de planos de saúde de 33,4% (IC95%: 31,9-34,9) para 45,1% (IC95%: 43,6-46,7) no período analisado e este esteve diretamente associado à posição econômica das famílias (p < 0,001). Ocorreu um aumento na média dos gastos com hospitalização para o parto de R$ 60,38 (DP = 288,66) para R$ 171,15 (DP = 957,07), e nos gastos adicionais com médicos de R$ 191,60 (DP = 612,86) para R$ 1.424,80 (DP = 4.459,16) entre as mães que se internaram pelo plano privado de saúde (e não houve diferença significativa nestes gastos entre as mães que optaram pelo parto particular). Houve aumento importante no gasto com a assistência ao parto principalmente entre as mães que se internaram pelo plano privado de saúde.


Abstract: Although most childbirth care in Brazil is financed by the Brazilian Unified National Health System (SUS), there are out-of-pocket expenditures (private personal costs) involved in births. This study aims to compare maternal out-of-pocket expenditures in births of children from the Pelotas Birth Cohorts of 2004 and 2015. The study drew on information collected right after birth and at three months of age. The target variables include sociodemographic and economic data, private health plan coverage, and expenditures related to the birth. Values from 2004 were adjusted to 2015 by the general price index. There was an increase in private health plan coverage from 33.4% (95%CI: 31.9-34.9) to 45.1% (95%IC: 43.6-46.7) in the target period, directly associated with the families' socioeconomic status (p < 0.001). There was an increase in mean expenditures on hospitalization for the birth, from BRL 60.38 (SD = 288.66) to BRL 171.15 (SD = 957.07), and in additional medical expenditures, from BRL 191.60 (SD = 612.86) to BRL 1,424.80 (SD = 4,459.16) among mothers admitted to hospital under their private health plans (and there was no significant difference in these expenditures for mothers that opted for direct payment). There was an important increase in expenditures for childbirth care, especially among mothers admitted to hospital under private health plans.


Resumen: A pesar de que la mayoría de los partos en Brasil esté financiado por el Sistema Único de Salud, existen gastos directos (personales privados) implicados en el nacimiento. Este estudio tiene como objetivo comparar el desembolso materno para financiar los partos de los niños, pertenecientes a las cohortes de nacimientos de Pelotas desde el 2004 al 2015. Se utilizó información recogida tras el nacimiento y a los tres meses de edad. Las variables analizadas incluyen información sociodemográfica, económica, cobertura con plan privado de salud y gastos relacionados con el parto. Los valores de 2004 se ajustaron por el Índice Nacional de Precios al Consumidor Amplio. Se observó un aumento en la posesión de planes de salud de un 33,4% (IC95%: 31,9-34,9) a un 45,1% (IC95%: 43,6-46,7) durante el período analizado y este se mostró directamente asociado a la posición económica de las familias (p < 0,001). Se produjo un aumento en la media de los gastos con hospitalización para el parto de BRL 60,38 (DE = 288,66) a BRL 171,15 (DE = 957,07), y en los gastos adicionales con médicos, de BRL 191,60 (DE = 612,86) a BRL 1.424,80 (DE = 4.459,16) entre las madres que estaban internadas por el plan privado de salud (y no hubo diferencia significativa en estos gastos entre las madres que optaron por el parto particular). Hubo un aumento importante en el gasto con asistencia al parto, principalmente, entre madres que estuvieron internadas para el parto mediante un plan privado de salud.

Importance: Poor sleep during early childhood is associated with adverse outcomes, including obesity, cognitive impairment, and mental and behavioral disorders. Objective: To assess the efficacy of an educational intervention in the promotion of nighttime sleep duration. Design, Setting, and Participants: This single-blind, intent-to-treat randomized clinical trial included participants in Pelotas, Brazil, aged 3 months who were followed up until age 24 months. Eligibility criteria included healthy infants aged approximately 3 months who slept less than 15 hours per 24 hours. Infants were randomized to the intervention group or control group. Interventions: Information on sleep characteristics, improvements in the environment, establishment of a nighttime sleep routine, and waiting before attending nocturnal awakenings was delivered to mothers in the intervention group by trained home-visitors at baseline. The intervention group received a telephone call on the first and second day after the intervention and a home visit on the third day after the intervention. The intervention's content was reinforced at health care visits for ages 6 months and 12 months. Mothers allocated to the control group were counseled on the benefits of breastfeeding for the mother's and child's health and given written material with content on breastfeeding. Main Outcomes and Measures: Nighttime sleep duration was measured by interview and actigraphy at baseline and ages 6, 12, and 24 months and diaries at baseline and age 6 months. At ages 3 and 6 months, nighttime sleep self-regulation was calculated by subtracting nighttime sleep duration recorded by actigraphy from nighttime sleep duration recorded in the diaries and at ages 12 and 24 months by subtracting nighttime sleep duration recorded by actigraphy from nighttime sleep duration obtained by interview. Results: Among 1812 mother-infant dyads invited to participate, 798 met the inclusion criteria and 586 agreed to participate. The intervention group included 298 infants (154 [52.9%] boys), and the control group included 288 infants (164 [58.2%] boys). At age 6 months, mean (SD) nighttime sleep duration recorded in diaries was 9.80 (1.85) hours in the intervention group and 9.49 (2.07) hours in the control group, a difference of 19 minutes longer for the intervention group. At age 12 months, mean (SD) nighttime sleep duration based on the Brief Infant Sleep Questionnaire was 8.43 (1.35) hours in the intervention group and 8.52 (1.35) hours in the control group, a difference of 5 minutes shorter for the intervention group. At age 24 months, compared with information from the interview, actigraphy records showed that children in the intervention group stayed awake at night without signalizing for a mean (SD) of 0.52 (2.52) hours, whereas children in the control group stayed awake at night without signalizing for a mean (SD) of 0.23 (2.43) hours. There were no statistically significant difference between groups in any of the sleep parameters investigated. Conclusions and Relevance: This randomized clinical trial found that the educational intervention did not achieve longer nighttime sleep duration among infants in the intervention group.
