RESUMO

The purpose of this study was to evaluate the mechanism of action and the inhibiting effects of two types of desensitizers against dentin demineralization using pre-demineralized hypersensitivity tooth model in vitro. In this study, we confirmed that a hypersensitivity tooth model from our preliminary experiment could be prepared by immersing dentin discs in an acetic acid-based solution with pH 5.0 for three days. Dentin discs with three days of demineralization were prepared and applied by one of the desensitizers containing calcium fluoro-alumino-silicate glass (Nanoseal, NS) or fluoro-zinc-silicate glass (Caredyne Shield, CS), followed by an additional three days of demineralization. Dentin discs for three days of demineralization (de3) and six days of demineralization (de6) without the desensitizers were also prepared. The dentin discs after the experimental protocol were scanned using swept-source optical coherence tomography (SS-OCT) to image the cross-sectional (2D) view of the samples and evaluate the SS-OCT signal. The signal intensity profiles of SS-OCT from the region of interest of 300, 500, and 700 µm in depth were obtained to calculate the integrated signal intensity and signal attenuation coefficient. The morphological differences and remaining chemical elements of the dentin discs were also analyzed using scanning electron microscopy and energy-dispersive X-ray spectroscopy. SS-OCT images of CS and NS groups showed no obvious differences between the groups. However, SS-OCT signal profiles for both the CS and NS groups showed smaller attenuation coefficients and larger integrated signal intensities than those of the de6 group. Reactional deposits of the desensitizers even after the additional three days of demineralization were observed on the dentin surface in NS group, whereas remnants containing Zn were detected within the dentinal tubules in CS group. Consequently, both CS and NS groups showed inhibition effects against the additional three days of demineralization in this study. Our findings demonstrate that SS-OCT signal analysis can be used to monitor the dentin demineralization and inhibition effects of desensitizers against dentin demineralization in vitro.

This study evaluated the ability of different types of silver diammine fluoride (SDF) to inhibit dentin demineralization using micro-focused X-ray computed-tomography (µCT). Dentin specimens were divided into five groups (n=10); no-treatment (control), 3.8% SDF (RC), 38% SDF, 38% SDF with potassium iodide (SDF/KI), and potassium fluoride (KF). The treated-dentin surfaces were subjected to demineralization for 7-days and assessed using µCT to determine mineral loss (ML) values. Specimens were also analyzed with scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDS). The ML values of the SDF and KF groups were significantly lower than those of the RC and SDF/KI groups. EDS detected fluoride ions in the SDF and KF groups but not in the RC and SDF/KI groups. It was concluded that 38% SDF demonstrated a high ability to inhibit dentin demineralization while additional application of KI may diminish the inhibitory effect of SDF. The amount of dentin demineralization with SDF treatments was material dependent.

Evaluation of gap formation at the interfaces of a two-step self-etching adhesive with/without pre-etching was performed using sweptsource optical coherence tomography (SS-OCT). Round cavities were prepared in bovine incisors at the middle (MC) and cervical (CC) thirds of the crown and the cervical third of the root (CR). Clearfil SE bond was directly applied to one group (SE) and another (PA) was pretreated with K-etchant gel. Following restoration by flowable composite resin, the teeth were thermally challenged and stored for 2 months. Interfacial gaps observed in the cross-sectional OCT images were analyzed and the bottom cavities exhibited increased gaps compared to the margin and dentin-enamel junction (DEJ). The CR site had a larger gap than at MC and CC in the SE group. DEJ separation at the MC was significantly smaller than that at CC in both groups. Therefore, gap formation depends on the cavity region, location, and bonding protocol.

OBJECTIVE: The purpose of this study was to detect and investigate the association of enamel microcracks with demineralization at proximal contact areas of premolars, using 3D swept-source optical coherence tomography (SS-OCT). METHODS: Extracted maxillary and mandibular premolars (n=50 each), without any visible tooth cracks, were examined for demineralization of interproximal contact areas, using the International Caries Detection and Assessment System (ICDAS). SS-OCT was used to evaluate demineralization and detect microcracks. Demineralization progression was divided into 4 types by depth: 0 for sound enamel and Type I, II, and III for enamel demineralization penetrating into the outer third, the middle third, and the inner third of the enamel thickness, respectively. Enamel microcracks were classified according to the predominant crack pattern: Type O, no cracks; Type A, subsurface microdefects; Type B, enamel microcrack running along the direction of the enamel prism; Type C, enamel microcrack running transverse to the enamel prism direction; and Type D, a combination of patterns A, B, and C. SS-OCT findings were confirmed by confocal laser scanning microscopy observation. RESULTS: Microcracks confined within the superficial enamel in proximal contact areas were seen as bright lines on SS-OCT. There were significant positive correlations among ICDAS codes, demineralization levels, and microcrack distribution (p≤0.001). The mesial side of maxillary premolars showed significantly more demineralization (n=36) and microcracks (n=27) than that of the mandibular premolars (n=20 and n=14, respectively; p<0.001). SIGNIFICANCE: The presence of microcracks within the superficial enamel in proximal contact areas could be determined using SS-OCT, and correlated with the level of demineralization. 3D SS-OCT is a valuable diagnostic tool for comprehensive assessment of microstructural changes related to enamel demineralization and crack development.

Using micro-computed tomography (micro-CT), this study assessed the inhibitory effect of three different types of restorations on root-caries formation under a cariogenic challenge. Bovine-root dentin blocks with a cylindrical cavity were divided into three restoration groups: a fluoride-free self-etch adhesive and a resin composite (SE-ES); a self-etch adhesive and a resin composite with multi-ion release (FL-BF); and a glass ionomer cement (Fuji-VII). After the restorative procedures, the specimens were stored in artificial saliva for 1 d, then subjected to a demineralization solution for 4 d and a remineralization solution for 28 d. Mineral density and mean mineral loss of dentin around the restorations were measured using micro-CT. The mean mineral loss values of dentin around the restorations were highest among SE-ES restorations and lowest among Fuji-VII restorations. Fuji-VII showed the highest mineral density and the lowest mineral loss after 28 d of remineralization. The observations made by scanning electron microscopy demonstrated that Fuji-VII created the smallest outer lesion followed by FL-BF and SE-ES. Fuji-VII has a larger inhibitory effect on root caries around the restorations and enhances remineralization more effectively than either FL-BF or SE-ES. Multi-ion and fluoride release from the restorative materials may be beneficial for inhibition of root-dentin caries around the restorations.

The purpose of this study was to compare optical properties of root caries under two observing conditions using swept-source optical coherence tomography (SS-OCT). In vitro and natural root caries were observed by SS-OCT under wet and dry conditions, followed by confocal laser scanning microscope (CLSM) and transverse microradiography (TMR). Signal intensity (SI), distance between SI peaks (SI-distance) and optical lesion depth were obtained from OCT. Lesion depth was measured from CLSM; lesion depth (LDTMR) and mineral loss (ML) were obtained from TMR. In vitro root caries under wet and dry conditions showed different OCT images and SI patterns. Lesion depth of OCT and that of CLSM, SI-distance and LDTMR, LDTMR and ML significantly correlated. Under dry conditions, half natural root caries showed similar OCT images and SI patterns as in vitro root caries. The base of demineralized dentin could be detected more clearly under dry conditions than under wet conditions.

This study was performed to evaluate the comparative radiopacity of adhesive/resin composite materials in cylindrical cavities using micro-computed X-ray tomography (µCT). The two-step self-etch adhesive systems, Clearfil SE Bond (SE) and FL-Bond II (FL), and flowable resin composites, Beautifil Flow F10 (BF) and Clearfil Majesty ES Flow High (MJ), were used. The radiopacity of bovine tooth structures and restorative materials was measured by µCT. In addition, cylindrical cavities prepared in bovine teeth were restored with the following adhesive/composite combinations: SE-BF, SE-MJ, FL-BF, and FL-MJ. The mean gray values of the composite restorations were calculated. The threshold values of the µCT images were evaluated using the Otsu's thresholding method. The current results show that the comparative radiopacity of the materials and tooth structure varied, which affected distinguishing the µCT images of the composite restorations in the cylindrical cavity. The proper combination of restorative materials should be considered when conducting in vitro µCT assessments of composite restorations.

OBJECTIVES: Swept-source optical coherence tomography (SS-OCT) can construct cross-sectional images of internal biological structures. The aim of this study was to evaluate enamel cracks at the cavosurface margin of composite restorations using SS-OCT. METHODS: Bowl-shaped cavities were prepared at two locations (mid-coronal and cervical regions) on the enamel surface of 60 bovine teeth. Half of the cavities (30) were treated with phosphoric acid gel. A two-step self-etch adhesive (Clearfil SE Bond) was applied to all cavities and a flowable composite was placed in bulk. After 7days in water at 37°C, three-dimensional (3D) images of the specimens were obtained using SS-OCT, and cross-sectional views of the cavosurface margin were examined. Presence and extent of enamel cracks along the cavosurface margin circumference were evaluated using a 5-point scale. The results were statistically compared with Wilcoxon rank sum test with Bonferroni correction. RESULTS: 3D SS-OCT could detect enamel cracks at the cavosurface margin of composite restorations. Cervical regions caused more enamel cracking than mid-coronal regions. Phosphoric acid etching increased the incidence of enamel cracks compared with the preparations without etching. CONCLUSION: SS-OCT can be used to detect enamel cracks at the margins of composite restorations noninvasively. Presence and extent of enamel cracks depended on the enamel region and bonding protocol. CLINICAL SIGNIFICANCE: SS-OCT can be used to detect enamel cracks at the margins of composite restorations noninvasively. Selective phosphoric acid etching of the enamel significantly increased the incidence of marginal cracks, especially in cervical preparation.

OBJECTIVE: To observe the bacterial demineralization of the enamel and dentin around composite restorations bonded with one-step and two-step self-etch adhesive systems using swept-source optical coherence tomography (SS-OCT). METHODS: Forty class V cavities (2.5-mm surface diameter, 2.0-mm maximum depth) were prepared on cervical areas of 20 human molars. The specimens were either treated with one-step adhesive (Clearfil Tri-S Bond ND Quick; TS) or two-step adhesive (Clearfil SE Bond; SE), restored with a flowable resin composite (Estelite Flow Quick). Specimens in the demineralized group were incubated for 2 weeks after Streptococcus mutans biofilm formation, while specimens in the control group were incubated for 2 weeks without biofilms. After SS-OCT observation, specimens were cut and examined under confocal laser scanning microscope (CLSM). The lesion depth (LD), interfacial gap depth (GD) and gap length (GL) obtained from SS-OCT and CLSM were analyzed by Pearson's correlation, LD by Independent-samples t-test, GD and GL by Welch t-test, the frequency of specimens with or without gap by Fisher's exact test at the 95% significant level. RESULTS: Specimens with interfacial gaps in the demineralized group showed significantly higher frequency than that in the control group (p<0.05). There was a significant correlation between SS-OCT and CLSM values of LD, GD and GL (p<0.05). SE showed significantly larger LD of dentin, but less GD and GL of enamel than TS (p<0.05). SIGNIFICANCE: SS-OCT nondestructively detected demineralization around composite restorations and interfacial gaps created by S. mutans biofilm in this in vitro model.

Swept source optical coherence tomography (SS-OCT) is an interferometric imaging technique. This study aimed to assess SS-OCT as a diagnostic tool for the evaluation of occlusal fissure depth and sealant penetration into fissure. Seventy investigation sites of occlusal fissures without visible evidence of caries were categorized into four groups (smooth, shallow, intermediate, and deep fissures). Categorization utilized two methods: 1) visually, using a computer screen, and 2) using SS-OCT images. After sealant placement, penetration was observed in SS-OCT. The results obtained from SS-OCT and visual inspections were compared with those of confocal laser scanning microscope (CLSM). The diagnostic power of SS-OCT was higher than that of visual inspection for fissure depth. Additionally, clear cross-sectional images of sealant penetration into fissures were observed with SS-OCT. SS-OCT can be used to evaluate fissure depth and monitor sealant penetration.

Exposed root surfaces due to gingival recession are subject to biofilm stagnation that can result in caries formation. Cervical enamel and dentin demineralization induced by a cariogenic biofilm was evaluated using swept-source optical coherence tomography (SS-OCT). The cementoenamel junction (CEJ) sections of extracted human teeth were subjected to demineralization for 1, 2, or 3 weeks. A suspension of Streptococcus mutans was applied to form a cariogenic biofilm using an oral biofilm reactor. After incubation, demineralization was observed by SS-OCT. For the analysis of SS-OCT signal, the value of the area under the curve (AUC) of the signal profile was measured. Statistical analyses were performed with 95% level of confidence. Cervical demineralization was displayed as a bright zone in SS-OCT. The demineralization depth of dentin was significantly deeper than that of enamel ([Formula: see text]). Enamel near the CEJ demonstrated a significant increase of AUC over the other enamel region after the demineralization. The gaps along the dentinoenamel junction were additionally observed in SS-OCT. SS-OCT was capable of monitoring the cervical demineralization induced by a cariogenic biofilm and is considered to be a promising modality for the diagnosis of cervical demineralization.

Shear bond strength (SBS) and the interfacial adaptation (IA) of self-adhesive resin (SAR) composites to dentin were evaluated. Two SARs [Vertise Flow (VTF) and Fusio Liquid Dentin (FLD)] were evaluated and compared with a conventional restorative system [adhesive: OptiBond FL and composite: Herculite Précis (OBF/HP)]. Human third molars were used for SBS testing and IA imaging (n=7) using optical coherence tomography (OCT). Flattened dentin disks were prepared and the composites were applied into molds (2.4 mm diameter) that were positioned on dentin. Samples were subjected to SBS testing and OCT analysis, which considered an increase in signal intensity at the bonded interface as evidence of internal gaps. SBS data were analyzed by one-way analysis of variance and Tukey's test and IA data (% distribution of high brightness values) by Kruskal-Wallis and Dunn's test (p≤0.05). No statistically significant difference in SBS was observed between VTF (13.9±3.6 MPa) and FLD (11.3±3.2 MPa), whereas OBF/HP showed higher average strength (27.3±6.1 MPa). However, there was a statistically significant difference in IA when VTF (33.3%) was compared with FLD (1.2%) and OBF/HP (1.5%). The conventional restorative system exhibited superior SBS performance compared with SARs. However, the IA of FLD to dentin had values that were not significantly different from OBF/HP.

A technology to characterize early enamel lesions is needed in dentistry. Optical coherence tomography (OCT) is a noninvasive method that provides high-resolution cross-sectional images. The aim of this study is to compare OCT with microfocus x-ray computed tomography ([Formula: see text]) for assessment of natural enamel lesions in vitro. Ten human teeth with visible white spot-like changes on the enamel smooth surface and no cavitation (ICDAS code 2) were subjected to imaging by µCT (SMX-100CT, Shimadzu) and 1300-nm swept-source OCT (Dental SS-OCT, Panasonic Health Care). In [Formula: see text], the lesions appeared as radiolucent dark areas, while in SS-OCT, they appeared as areas of increased signal intensity beneath the surface. An SS-OCT attenuation coefficient based on Beer-Lambert law could discriminate lesions from sound enamel. Lesion depth ranged from 175 to [Formula: see text] in SS-OCT. A correlation between [Formula: see text] and SS-OCT was found regarding lesion depth ([Formula: see text], [Formula: see text]) and also surface layer thickness ([Formula: see text], [Formula: see text]). The images obtained clinically in real time using the dental SS-OCT system are suitable for the assessment of natural subsurface lesions and their surface layer, providing comparable images to a laboratory high-resolution [Formula: see text] without the use of x-ray.

OBJECTIVES: To evaluate the sealing ability and the microtensile bond strength (MTBS) of different adhesive systems bonded to dentin in class I cavities. METHODS: Round tapered dentin cavities (3-mm diameter, 1.5-mm height) prepared in extracted human molars were restored using composite resin (Clearfil Majesty Posterior) with two-step etch-and-rinse adhesive system (Adper Single Bond 2: ASB2), two-step self-etch adhesive (Clearfil SE Bond: CSEB), all-in-one adhesives (G-Bond Plus: GBP; Tri-S Bond Plus: TSBP), or no adhesive (Control), or bonded using low-shrinkage composite with its proper adhesive (Filtek Silorane, Silorane Adhesive System: FSS). After 24-h water storage or 10,000 cycles of thermal stress, the specimens were immersed into a contrast agent. Two and three-dimensional images were obtained using optical coherence tomography (OCT). The mean percentage of high brightness (HB%) at the interfacial zone in cross-sectional images was calculated as an indicator of contrast agent or gap at the interface. The specimens were then sectioned into beams and the MTBS measured. RESULTS: The HB% (ASB2=TSBP=CSEBTSBP=GBP=FSS, ASB2>FSS) differed significantly among the adhesives. After aging, HB% increased for GBP and FSS specimens, and the MTBS decreased for FSS specimens (ANOVA, Tukey's post hoc, p<0.05). The HB% and MTBS were significantly and negatively correlated (p=0.002). Confocal laser scanning and scanning electron micrographs confirmed contrast agent infiltration within the gap. SIGNIFICANCE: There was a significant correlation between sealing performance and bond strength of the adhesives in the whole cavity. After aging, the two-step systems showed equal or superior performance to the all-in-one and Silorane systems.

Non-carious cervical lesions (NCCLs) involve various forms of tooth loss with different etiologies. This study aimed to utilize swept-source optical coherence tomography (SS-OCT) at 1300 nm wavelength range in vitro and in vivo to evaluate and clarify the mechanism of NCCLs. In the in vitro phase, a dentin attenuation coefficient (µt ) derived from the SS-OCT signal at NCCL was compared with mineral loss obtained from transverse microradiography (TMR) to determine a µt threshold to discriminate demineralization of cervical dentin in vivo. In the clinical study, 242 buccal surfaces were investigated in 35 subjects. Presence and dimensions of NCCLs, cervical cracking and the degree of demineralization at the exposed cervical dentin were determined using SS-OCT. Dentin demineralization was observed in 69% of NCCLs. SS-OCT results confirm that dentin mineral loss and occlusal attrition were associated with larger NCCLs, and can be considered as an etiological factor in formation and progress of these lesions. This study evaluated the effect of two desensitizers on inhibition of dentin demineralization, after immersion in artificial saliva using micro-computed tomography (µCT). Dentin blocks cut from bovine incisors were treated with deionized water (DW, a negative control) or one of three desensitizers: a fluoride varnish (Duraphat, a positive control), a calcium phosphate desensitizer (Teethmate Desensitizer), and a fluoro-alumino-calcium silicate-based desensitizer (Nanoseal). After each treatment, the specimens in Duraphat, Nanoseal, and Teethmate Desensitizer groups were pre-immersed in artificial saliva (pH 6.5) for either 1 d or 1 wk. The mineral loss of the specimens after demineralization (pH 5.0, 3 h) was evaluated by µCT. The treated surface was investigated with scanning electron microscopy. Mineral loss in all treatment groups was significantly lower than that in DW. Duraphat was the most effective treatment against demineralization, followed by Nanoseal. Nanoseal showed significantly better reduction in mineral loss following immersion for 1 wk in artificial saliva than for 1 d. However, Teethmate Desensitizer and Duraphat did not exhibit enhanced inhibition of demineralization over a longer period of immersion in artificial saliva. Scanning electron microscopy images showed deposition of particles on the dentin in both Teethmate Desensitizer. The application of Teethmate Desensitizer and Nanoseal to the exposed dentin surface resulted in inhibition of demineralization, with Nanoseal resulting in improved inhibition after prolonged immersion in artificial saliva.

ABSTRACT. This study aimed to nondestructively evaluate sealing performance of eight one-step self-etch adhesives (1-SEAs) using optical coherence tomography (OCT). The two-step self-etch adhesive (2-SEA) served as the control. Round tapered class-I cavities (D=4 mm, H=2 mm) were prepared in bovine incisors, treated with each adhesive (n=5), and restored with a flowable resin composite. Cross-sections were obtained from each restoration using swept-source OCT with 1310-nm laser. The average percentage of the sealed interface (SI%) for each adhesive was calculated using image analysis software, considering increased signal intensity at the interface as gap. Samples were then sectioned and observed under confocal laser scanning microscope (CLSM). Significantly different SI% values were found among different adhesives (analysis of variance, Bonferroni, p<0.05). There was also a significant correlation in SI% between OCT and CLSM (p<0.0001, r=0.96). Additionally, microscopic analysis revealed that the gaps in 1-SEAs occurred not only at dentine-adhesive interfaces but also frequently at adhesive-composite interfaces. Some recent 1-SEAs could achieve reliable short-term sealing comparable to 2-SEA. OCT is a unique tool to nondestructively evaluate the sealing performance of the restoratives through the cavity, provided that cavity walls have a certain minimum inclination with respect to the beam.

This study aimed to evaluate the potential of a new device (Cariotester) for monitoring of incipient carious lesion remineralization in root dentin by topical fluoride in vitro. Demineralized bovine dentin specimens were treated by fluoride solutions (APF or neutral NaF) and remineralized for 4 weeks. Cariotester was used to measure penetration depth (CTR depth) of the indenter into the de- and remineralized specimen surface. The specimens were assessed by transverse microradiography (TMR) to determine lesion parameters (depth: LD, mineral loss: ΔZ). Pearson's correlation analysis showed an overall significant relationship between CTR depth and both TMR parameters. CTR depth appeared to distinguish the positive effect that topical fluoride application had on the remineralization of the outer zone of dentin lesions. Cariotester had the potential to serve as a quantitative tool for monitoring of incipient carious lesion remineralization in root dentin.

PURPOSE: To investigate the effect of composite type and cavity size on gap and void formation using optical coherence tomography (OCT). MATERIALS AND METHODS: Class I cavities of two depths (2 mm or 4 mm; 4 mm diameter) were prepared, treated with Tri-S Bond (Kuraray Medical), and bulk filled with either Surefil SDR Flow (SF; Dentsply) or Clearfil Majesty LV (MJ; Kuraray Medical) to form four groups. After 24 h, the specimens were 3D scanned using swept-source OCT (Santec) with 1310 nm laser at a 20 kHz sweep rate. In OCT tomograms, the bonding interface and the bulk of the restorations were evaluated. The percentage values of sealed interfaces (SP) and void volume (VP) for all groups were calculated and statistically analyzed using two- and one-way ANOVA and Tukey's post-hoc test. Selected specimens were cross sectioned and observed using a scanning electron microscope (SEM) and a confocal laser scanning microscope to confirm OCT findings. RESULTS: The following values were obtained for SP and VP, respectively: SF-2mm: 92% and 0.08%; SF-4mm: 66% and 0.13%; MJ-2mm: 86% and 1.79%; MJ-4mm: 33% and 1.96%. Both composites showed a significant increase in gap formation at 4-mm cavity depth (p < 0.001). While SF showed a rather homogeneous bulk compared to MJ, cavity depth did not significantly affect the void volume fraction (p = 0.08). CONCLUSION: The flowable composite with SDR (stress-decreasing resin) technology performed better than the conventional composite; however, bulk filling a 4-mm-deep cavity will compromise the sealing of the bonding interface regardless of the type of composite. OCT is a unique method of characterizing materials and their behaviors nondestructively and precisely.

Establishing reproducible methodologies for assessment of early enamel lesions using optical coherence tomography (OCT) appears to be challenging. This in vitro study longitudinally evaluated the subsurface enamel lesion progression after 3, 9 and 15 days by cross-sectional scanning using 1310 nm centered swept-source OCT (SS-OCT) under hydrated and dry conditions. The positive difference between the depth-integrated OCT signals at dry and hydrated conditions were calculated and adopted as dehydration parameter (DH). A linear regression was found between DH and the square root of demineralization time (R(2) = 0.99). Significant differences were found in DH between sound and demineralized enamel, and between different periods of demineralization (p < 0.001). Hydration state affects the reflectivity of demineralized porous enamel, and the effect can be potentially used for assessment of early enamel lesion using OCT.
