RESUMO

BACKGROUND: Bladder, kidney and prostate cancers make significant contributors to cancer burdens. Exploring their cross-country inequalities may inform equitable strategies to meet the 17 sustainable development goals before 2030. METHODS: We analyzed age-standardized disability-adjusted life-years (ASDALY) rates for the three cancers based on Global Burden of Diseases Study 2019. We quantified the inequalities using slope index of inequality (SII, absolute measure) and concentration index (relative measure) associated with national sociodemographic index. RESULTS: Varied ASDALY rates were observed in the three cancers across 204 regions. The SII decreased from 35.15 (95% confidence interval, CI: 29.34 to 39.17) in 1990 to 15.81 (95% CI: 7.99 to 21.79) in 2019 for bladder cancers, from 78.94 (95% CI: 75.97 to 81.31) in 1990 to 59.79 (95% CI: 55.32 to 63.83) in 2019 for kidney cancer, and from 192.27 (95% CI: 137.00 to 241.05) in 1990 to - 103.99 (95% CI: - 183.82 to 51.75) in 2019 for prostate cancer. Moreover, the concentration index changed from 12.44 (95% CI, 11.86 to 12.74) in 1990 to 15.72 (95% CI, 15.14 to 16.01) in 2019 for bladder cancer, from 33.88 (95% CI: 33.35 to 34.17) in 1990 to 31.13 (95% CI: 30.36 to 31.43) in 2019 for kidney cancer, and from 14.61 (95% CI: 13.89 to 14.84) in 1990 to 5.89 (95% CI: 5.16 to 6.26) in 2019 for prostate cancer. Notably, the males presented higher inequality than females in both bladder and kidney cancer from 1990 to 2019. CONCLUSIONS: Different patterns of inequality were observed in the three cancers, necessitating tailored national cancer control strategies to mitigate disparities. Priority interventions for bladder and kidney cancer should target higher socioeconomic regions, whereas interventions for prostate cancer should prioritize the lowest socioeconomic regions. Additionally, addressing higher inequality in males requires more intensive interventions among males from higher socioeconomic regions.

RESUMO

BACKGROUND: Children with Autism spectrum disorder (ASD) was frequently experienced dental anxiety and uncooperative behaviors during dental treatment. Oral health care was necessary because of the poor oral hygiene and prevalent dental diseases in this population. AIM: In this systematic review, we evaluated the effectiveness and feasibility for pediatric dentist to manage the dental anxiety in children with ASD. DESIGN: PubMed, Embase, and Cochrane Library were systematically performed on the literature search. The date of eligible publications was from inception to January 2023. After that, the quality of eligible studies was assessed by the Newcastle Ottawa Scale (NOS). Review findings were summarized using the PRISMA Statement for reporting. RESULTS: A total of six studies were systematically evaluated according to the inclusion and exclusion criteria. Five studies were conducted to evaluate ASD Children's anxiety and uncooperative performance in the progressive oral examination, oral disease prophylaxis and fluoride application. The other one study evaluated the success rate of treatment in decayed permanent tooth treatment. In the included studies, four studies indicated that it was extremely necessary to reduce dental anxiety of ASD children to increase the cooperation in sensory-adapted dental environment (SADE). CONCLUSION: It is not always effective and feasible for pediatric dentist to manage the dental anxiety in children with autism during routine oral examination. Meanwhile, it is necessary for ASD children to conduct preoperative psychological assessment, to investigate parents' expectations and cooperation, and to determine whether to start corresponding dental treatment.

RESUMO

Effective CO2 hydrogenation has recently attracted quite some attention for producing more valuable chemical oxygenates (such as methanol, formate) in mild conditions. However, the influence of the metal center on the CO2 activation remains unclear. First, electrospray ionization mass spectrometry (ESI-MS) was employed to explore the direct CO2 hydrogenation to formic acid mediated by [Cp2MH]+ (M = Zr, Hf) in the gas phase at room temperature. The key formate intermediate [Cp2M(O2CH)]+ (M = Zr, Hf) was confirmed by traveling wave ion mobility spectrometry (TWIMS). Second, to gain insights into the metal effect, the CO2 hydrogenation process involving Group 4 (i.e., Ti, Zr, Hf) transition metals was calculated along with Group 5 (i.e., V, Nb, Ta) by density functional theory (DFT) methods. The CO2 insertion process was found to be the rate-limiting step. For [Cp2TiH]+, [Cp2ZrH]+, [Cp2HfH]+, [Cp2VH]+, [Cp2NbH]+, and [Cp2TaH]+, the barriers are +7.7, +6.5, +5.9, +9.2, +8.0, and +6.3 kcal/mol, respectively. [Cp2HfH]+-mediated CO2 hydrogenation occurs the most rapidly, as revealed by MS. According to the orbital analysis on the CO2 insertion transition state, the electron-deficient metal center resulting in a low-lying lowest unoccupied molecular orbital (LUMO) could interact more favorably with the π bond of deformed CO2, which was also consistent with the natural bond orbital (NBO) results. Last but not the least, NBO charges on the metal centers were found to correlate linearly well with the CO2 insertion barriers rather than hydride affinity. Thus, the reactivity of different metal hydride complexes with CO2 to produce a formate could be estimated by the NBO charge on metals. Our findings might provide a series of candidates for the catalyst as well as guidance for catalyst design in mild CO2 hydrogenation.

RESUMO

OBJECTIVES: To develop and validate an ultrasound elastography radiomics nomogram for preoperative evaluation of the axillary lymph node (ALN) burden in early-stage breast cancer. METHODS: Data of 303 patients from hospital #1 (training cohort) and 130 cases from hospital #2 (external validation cohort) between Jun 2016 and May 2019 were enrolled. Radiomics features were extracted from shear-wave elastography (SWE) and corresponding B-mode ultrasound (BMUS) images. The minimum redundancy maximum relevance and least absolute shrinkage and selection operator algorithms were used to select ALN status-related features. Proportional odds ordinal logistic regression was performed using the radiomics signature together with clinical data, and an ordinal nomogram was subsequently developed. We evaluated its performance using C-index and calibration. RESULTS: SWE signature, US-reported LN status, and molecular subtype were independent risk factors associated with ALN status. The nomogram based on these variables showed good discrimination in the training (overall C-index: 0.842; 95%CI, 0.773-0.879) and the validation set (overall C-index: 0.822; 95%CI, 0.765-0.838). For discriminating between disease-free axilla (N0) and any axillary metastasis (N + (≥ 1)), it achieved a C-index of 0.845 (95%CI, 0.777-0.914) for the training cohort and 0.817 (95%CI, 0.769-0.865) for the validation cohort. The tool could also discriminate between low (N + (1-2)) and heavy metastatic ALN burden (N + (≥ 3)), with a C-index of 0.827 (95%CI, 0.742-0.913) in the training cohort and 0.810 (95%CI, 0.755-0.864) in the validation cohort. CONCLUSION: The radiomics model shows favourable predictive ability for ALN staging in patients with early-stage breast cancer, which could provide incremental information for decision-making. KEY POINTS: • Radiomics analysis helps radiologists to evaluate the axillary lymph node status of breast cancer with accuracy. • This multicentre retrospective study showed that radiomics nomogram based on shear-wave elastography provides incremental information for risk stratification. • Treatment can be given with more precision based on the model.

RESUMO

OBJECTIVES: To evaluate the prediction performance of deep convolutional neural network (DCNN) based on ultrasound (US) images for the assessment of breast cancer molecular subtypes. METHODS: A dataset of 4828 US images from 1275 patients with primary breast cancer were used as the training samples. DCNN models were constructed primarily to predict the four St. Gallen molecular subtypes and secondarily to identify luminal disease from non-luminal disease based on the ground truth from immunohistochemical of whole tumor surgical specimen. US images from two other institutions were retained as independent test sets to validate the system. The models' performance was analyzed using per-class accuracy, positive predictive value (PPV), and Matthews correlation coefficient (MCC). RESULTS: The model achieved good performance in identifying the four breast cancer molecular subtypes in the two test sets, with accuracy ranging from 80.07% (95% CI, 76.49-83.23%) to 97.02% (95% CI, 95.22-98.16%) and 87.94% (95% CI, 85.08-90.31%) to 98.83% (95% CI, 97.60-99.43) for the two test cohorts for each sub-category, respectively. In terms of 4-class weighted average MCC, the model achieved 0.59 for test cohort A and 0.79 for test cohort B. Specifically, the DCNN also yielded good diagnostic performance in discriminating luminal disease from non-luminal disease, with a PPV of 93.29% (95% CI, 90.63-95.23%) and 88.21% (95% CI, 85.12-90.73%) for the two test cohorts, respectively. CONCLUSION: Using pretreatment US images of the breast cancer, deep learning model enables the assessment of molecular subtypes with high diagnostic accuracy. TRIAL REGISTRATION: Clinical trial number: ChiCTR1900027676 KEY POINTS: • Deep convolutional neural network (DCNN) helps clinicians assess tumor features with accuracy. • Multicenter retrospective study shows that DCNN derived from pretreatment ultrasound imagine improves the prediction of breast cancer molecular subtypes. • Management of patients becomes more precise based on the DCNN model.

RESUMO

INTRODUCTION: Monitoring of laboratory indicators is important for predicting changes in disease severity and clinical outcomes. We aimed to identify the critical predictors that can effectively assess the disease conditions of patients with COVID-19 by analyzing the clinical characteristics and laboratory findings of patients with SARS-CoV-2 infection. METHODS: All consecutive patients (n = 294) with confirmed SARS-CoV-2 infection admitted to the General Hospital of Central Theater Command of the PLA from February 6 to February 21, 2020, were enrolled. These patients were divided into the severe group and the nonsevere group according to disease severity during hospitalization. RESULTS: The median neutrophil-to-lymphocyte ratio (NLR) value of the severe patients was dramatically higher than that of the nonsevere patients (10.4 vs 2.6; P < .001). The NLR value equal to 5 was a boundary value worthy of reference, because more than 80% severe patients had an NLR value greater than 5 and over 80% nonsevere patients had an NLR value less than 5. The NLR value of these COVID-19 patients was positively and respectively correlated with the values of C-reactive protein (R = .5921, P < .001), lactate dehydrogenase (R = .4509, P < .001), procalcitonin (R = .5504, P < .001), fibrinogen (R = .4710, P < .001), and D-dimers (R = .4425, P < .001). However, the NLR value was merely and positively correlated with the interleukin-6 value (R = .3594, P < .05), but had no correlations with the values of interleukin-10, interleukin-4, interleukin-17, interferon-γ, and tumor necrosis factor-α (P > .05). DISCUSSION: Neutrophil-to-lymphocyte ratio is a critical predictor for assessment of disease severity in patients with COVID-19, and it has a close relation with the laboratory indicators related to disease conditions.

RESUMO

BACKGROUND: The neural mechanism of amblyopia and its impact on the adult brain remain unclear. This hinders effective treatment for adults with this disease. PURPOSE: To investigate neuroanatomical differences in cortical morphometry between amblyopic adults and healthy controls, and to explore the structural covariance of abnormal morphometric changes. STUDY TYPE: Prospective. POPULATION: Twenty-one amblyopic adults and 34 healthy controls. FIELD STRENGTH/SEQUENCE: 3T MRI, T1 -weighted, MPRAGE sequence. ASSESSMENT: All participants completed ophthalmologic exams to confirm the diagnosis of amblyopia or its absence in the healthy controls, including tests of ocular motility and dilation, fundus exam, autorefraction and synoptophore tests. Cortical volume, thickness, and surface area measurements were obtained using FreeSurfer software. STATISTICAL TESTS: Statistical differences of MRI measures between amblyopic adults and healthy controls were identified using a general linear model with intracranial volume and age as covariates. Monte Carlo simulations were used to correct for multiple comparisons. The structural covariance of abnormal morphometric changes and the relationship between morphometric abnormalities and visual acuity of the amblyopic eye were examined. RESULTS: Compared with healthy controls, amblyopic adults showed reduced cortical volume in left lateral occipital cortex, and decreased cortical thickness in bilateral inferior temporal gyrus and left precentral gyrus (P < 0.05; Monte Carlo corrected). Structural covariance between cortical volume of left lateral occipital cortex and cortical thickness of right inferior temporal gyrus in amblyopic adults was significantly less than in healthy controls (z = 1.73; P < 0.05). DATA CONCLUSION: Our study identified morphological abnormalities in occipital cortex and in temporal and frontal cortex which are projection fields of visual cortex important for processing of visual form and object location information, and disrupted structural covariance of visual cortex with other brain regions in amblyopic patients. RESUMO

Comparative studies of subspecies under different ecological environments offer insights into intraspecies evolutionary adaptive mechanisms. Golden snub-nosed monkeys (Rhinopithecus roxellana) include three subspecies in China classified mainly by their morphological variations: R. r. roxellana (Sichuan and Gansu province), R. r. qinlingensis (Shaanxi province) and R. r. hubeiensis (Hubei province). These three subspecies live in three isolated area with different environments. Past works focused on the last two subspecies, but little information of habitat and behaviors of the nominated subspecies (R. r. roxellana) is available to date. We conducted a two-year study on the diet, activity budget, home range and social organization of 4 herds of R. r. roxellana, based on a total of 106 days' observation in Laohegou (LHG) Nature Reserve, Sichuan province. By using scan sampling method, our results suggest that the R. r roxellana feeds predominantly on leaves (77.5%), and spends more time feeding (40.0%) and resting (27.0%) while compared to the other two subspecies. Kernel Density Estimation Method based on GPS technology confirms that R. r roxellana has relatively larger home ranges (49.1 km2). The unit size (8.3 ± 3.5 individuals) of R. r roxellana is also smaller. Therefore, it is possible that differences in food availability in relation to habitats have important impacts on the feeding strategy and social system of the golden snub-nosed monkey. These results provide data to further explore intraspecific adaptations of living primates.

RESUMO

This work presents a simple, low-cost method to fabricate semi-circular channels using solder paste, which can amalgamate the cooper surface to form a half-cylinder mold using the surface tension of Sn-Pd alloy (the main component in solder paste). This technique enables semi-circular channels to be manufactured with different dimensions. These semi-circular channels will then be integrated with a polymethylmethacrylate frame and machine screws to create miniaturized, portable microfluidic valves for sequential liquid delivery and particle synthesis. This approach avoids complicated fabrication processes and expensive facilities and thus has the potential to be a useful tool for lab-on-a-chip applications.

RESUMO

Based on the past years vegetation cover, annual maximal grass yield and June-September mean modified soil-adjusted vegetation index (MSAVI) that were inversely deduced from the 10-day composite data of NOAA/AVHRR channels 1 and 2 and NDVI in 1982-2000, this paper analyzed the recent 20 years dynamics of grassland desertification in Naqu of northern Tibet. The results showed that in recent 20 years, the area of degraded grassland in Naqu was averagely 43.1% of the total land area. It was decreased in the former ten years while increased in the latter ten years, but overall, had a decreasing trend. The degraded area was larger in west part of Naqu than in its other regions. Among the eight climatic factors including temperature, precipitation, potential evapotranspiration, vapor pressure, wind velocity, sunshine hour, ratio of precipitation to evapotranspiration, and ratio of temperature to precipitation, the most remarkable factor affecting the dynamic of grassland degradation was the potential evapotranspiration.

RESUMO

By summarizing the state of application of "syndrome differentiation through formula effect assessment", it was pointed out that several problems exist in this kind of study, mainly the irrational setting of experimental control group, over simplified pharmacodynamic indexes, and negligence of study on "correspondence between drug and disease", etc. The authors proposed accordingly the thinking methods and strategy in the study on "syndrome differentiation through formula effect assessment" should be setting control properly, taken "correspondence between disease and syndrome" as the key point, and to define clearly the criteria for evaluating the efficacy of formula as the premise.
