RESUMO

A workshop organized by the University Medical Center Groningen addressed various current issues regarding nutrient status of individuals and populations, tools and strategies for its assessment, and opportunities to intervene. The importance of nutrient deficiencies and information on nutrient status for health has been illustrated, in particular for elderly and specific patient groups. The nutrient profile of individuals can be connected to phenotypes, like hypertension or obesity, as well as to socio-economic data. This approach provides information on the relationship between nutrition (nutrient intake and status) and health outcomes and, for instance, allows us to use the findings to communicate and advocate a healthy lifestyle. Nutrition is complex: a broader profile of nutrients should be considered rather than focusing solely on a single nutrient. Evaluating food patterns instead of intake of individual nutrients provides better insight into relationships between nutrition and health and disease. This approach would allow us to provide feedback to individuals about their status and ways to improve their nutritional habits. In addition, it would provide tools for scientists and health authorities to update and develop public health recommendations.

BACKGROUND: Currently, assessment of symptoms associated with Parkinson's disease is mainly performed in the clinic. However, these assessments have limitations because they provide only a snapshot of the condition. METHODS: The feasibility and usability of an objective, continuous and relatively unobtrusive system (SENSE-PARK System), which consists of wearable sensors (three worn during the day and one worn at night), a smartphone-based App, a balance board and computer software, was tested 24/7 over 12 weeks in a study including 22 PD patients. During the first four weeks of the study, patients did not get feedback about their performance, during the last eight weeks they did. The study included seven clinical visits with standardized interviews, and regular phone contact. The primary outcome was the number of drop-outs during the study. As secondary outcomes, the Post-Study System Usability Questionnaire (PSSUQ), score and information obtained from the standardized interviews were used to evaluate the usability of the system. RESULTS: All patients completed the study. The participants rated the usability of the SENSE-PARK System with a mean score of 2.67 (±0.49) on the PSSUQ. The interviews revealed that most participants liked using the system and appreciated that it signaled changes in their health condition. CONCLUSIONS: This 12 week controlled study demonstrates that the acceptance level of PD patients using the SENSE-PARK System as a home-based 24/7 assessment is very good. Particular emphasis should be given to a user-friendly design. Motivation to wear such a system can be increased by providing direct feedback about the individual health condition.

BACKGROUND: Parkinson's disease (PD) is a neurodegenerative disorder with fluctuating symptoms. To aid the development of a system to evaluate people with PD (PwP) at home (SENSE-PARK system) there was a need to define parameters and tools to be applied in the assessment of 6 domains: gait, bradykinesia/hypokinesia, tremor, sleep, balance and cognition. OBJECTIVE: To identify relevant parameters and assessment tools of the 6 domains, from the perspective of PwP, caregivers and movement disorders specialists. METHODS: A 2-round Delphi study was conducted to select a core of parameters and assessment tools to be applied. This process included PwP, caregivers and movement disorders specialists. RESULTS: Two hundred and thirty-three PwP, caregivers and physicians completed the first round questionnaire, and 50 the second. Results allowed the identification of parameters and assessment tools to be added to the SENSE-PARK system. The most consensual parameters were: Falls and Near Falls; Capability to Perform Activities of Daily Living; Interference with Activities of Daily Living; Capability to Process Tasks; and Capability to Recall and Retrieve Information. The most cited assessment strategies included Walkers; the Evaluation of Performance Doing Fine Motor Movements; Capability to Eat; Assessment of Sleep Quality; Identification of Circumstances and Triggers for Loose of Balance and Memory Assessment. CONCLUSIONS: An agreed set of measuring parameters, tests, tools and devices was achieved to be part of a system to evaluate PwP at home. A pattern of different perspectives was identified for each stakeholder.

Worldwide approximately two billion people have a diet insufficient in micronutrients. Even in the developed world, an increasing number of people consume nutrient-poor food on a regular basis. Recent surveys in Western countries consistently indicate inadequate intake of nutrients such as vitamins and minerals, compared to recommendations. The International Osteoporosis Foundation's (IOF) latest figures show that globally about 88% of the population does not have an optimal vitamin D status. The Lancet's "Global Burden of Disease Study 2010" demonstrates a continued growth in life expectancy for populations around the world; however, the last decade of life is often disabled by the burden of partly preventable health issues. Compelling evidence suggests that improving nutrition protects health, prevents disability, boosts economic productivity and saves lives. Investments to improve nutrition make a positive contribution to long-term national and global health, economic productivity and stability, and societal resilience.

The link between a sufficient intake of vitamins and long term health, cognition, healthy development and aging is increasingly supported by experimental animal, human and epidemiology studies. In low income countries billions of people still suffer from the burden of malnutrition and micronutrient deficiencies. However, inadequate micronutrient status might also be an issue in industrialized countries. Recent results from nutritional surveys in countries like the United States, Germany, and Great Britain indicate that the recommended intake of micronutrients is not reached. This notably concerns certain vulnerable population groups, such as pregnant women, young children and the elderly, but also greatly influences the general healthcare costs. An overview is provided on the gap that exists between current vitamin intakes and requirements, even in countries where diverse foods are plentiful. Folic acid and vitamin D intake and status are evaluated in more detail, providing insight on health and potential impact on health care systems.

RESUMO

Biomarkers and their role in evaluating efficacy and safety were the topic of the 23rd Hohenheim Consensus Meeting at the University of Hohenheim in Stuttgart. Scientists who had published and reviewed scientific and regulatory papers on the topic were invited, among them basic researchers, toxicologists, clinicians, and nutritionists. The participants were presented with 11 questions (in bold font), which were discussed and answered (in italic font) at the workshop, with the aim of summarizing the current state of knowledge on the subject. The explicatory text accompanying the short answers was produced and agreed on after the conference and was backed up by corresponding references.

OBJECTIVE: To examine differences between subtypes of children with attention-deficit/hyperactivity disorder (ADHD), simple attentional difficulties and situational ADHD during an initial assessment. METHOD: In a single-centre study examination data of 205 children who had an initial assessment between January 1, 2002 and December 31, 2003 were retrospectively analyzed. The groups of children were compared with respect to their history features, neuromotor deficits, neuropsychological performance, and comorbid behavioural and developmental deficits by standardized test procedures and questionnaires. RESULTS: Children with ADHD more frequently have a positive family history of attentional symptoms than non-ADHD children. There is no difference between these groups with respect to the associated behavioural problems. Children classified as ADHD show more neuromotor problems and neuropsychological deficits than children with non-ADHD attentional problems. Children with ADHD subtype hyperactive/impulsive demonstrate better neuropsychological performance than children with subtypes inattentive and combined. Children with situational home-only or school-only ADHD show no clear differences in neuromotor behaviour, comorbid problems or neuropsychological performance. CONCLUSIONS: There are clear neuropsychological differences between children who fulfil the classification criteria of The Diagnostic and Statistical Manual of Disease, Fourth Revision (DSM-IV) for ADHD and children who are confronted daily with attentional difficulties, but do not fulfil the criteria for ADHD. In contrast, differences in behavioural comorbidities were not found, nor were differences found between home-only and school-only ADHD children.

In eight right-handed healthy males aged 9.6-12.9 years, changes in the frontal cerebral concentrations of oxygenated, deoxygenated, and total hemoglobin, as well as in tissue oxygenation index and cytochrome oxidase aa(3) were measured by near-infrared spectroscopy. The males were evaluated using a trail-making test with four test cycles interrupted by relax breaks. During the first cycle of this task, a significant increase in deoxygenated hemoglobin was documented in the left frontal region. In the extended course of the test, a significant increase in oxygenated and total hemoglobin was observed on both sides, indicating an increase in cerebral blood volume. However, only on the left side was an increase in deoxygenated hemoglobin documented. In children, changes in cerebral oxygenation induced by a cognitive task could be measured by near-infrared spectroscopy. In a short and extended attention task, a lateralized increase in oxygen consumption was documented by an increase in deoxygenated hemoglobin. In an extended attention task, an increase in oxygenated and total hemoglobin was recorded additionally in both frontal brain regions, indicating a compensatory increase in cerebral perfusion. Further studies are required to examine the significance of this technique in testing differences in children with neurobehavioral disabilities.
