BACKGROUND: Mild cognitive deficits (MCD) emerge before the first episode of psychosis (FEP) and persist in the clinical high-risk (CHR) stage. This study aims to refine risk prediction by developing MCD models optimized for specific early psychosis stages and target populations. METHODS: A comprehensive neuropsychological battery assessed 1059 individuals with FEP, 794 CHR, and 774 matched healthy controls (HCs). CHR subjects, followed up for 2 years, were categorized into converters (CHR-C) and non-converters (CHR-NC). The MATRICS Consensus Cognitive Battery standardized neurocognitive tests were employed. RESULTS: Both the CHR and FEP groups exhibited significantly poorer performance compared to the HC group across all neurocognitive tests (all p < 0.001). The CHR-C group demonstrated poorer performance compared to the CHR-NC group on three sub-tests: visuospatial memory (p < 0.001), mazes (p = 0.005), and symbol coding (p = 0.023) tests. Upon adjusting for sex and age, the performance of the MCD model was excellent in differentiating FEP from HC, as evidenced by an Area Under the Receiver Operating Characteristic Curve (AUC) of 0.895 (p < 0.001). However, when applied in the CHR group for predicting CHR-C (AUC = 0.581, p = 0.008), the performance was not satisfactory. To optimize the efficiency of psychotic risk assessment, three distinct MCD models were developed to distinguish FEP from HC, predict CHR-C from CHR-NC, and identify CHR from HC, achieving accuracies of 89.3%, 65.6%, and 80.2%, respectively. CONCLUSIONS: The MCD exhibits variations in domains, patterns, and weights across different stages of early psychosis and diverse target populations. Emphasizing precise risk assessment, our findings highlight the importance of tailored MCD models for different stages and risk levels.

Deficiencies of selenium (Se), a necessary microelement for humans, can be remedied by appropriately supplying Se-enriched rice. However, overconsumption of Se-enriched rice poses a potential risk. To accurately assess Se human health risks associated with Se-enriched rice consumption, we developed a rat in vivo model to systematically explore the relative bioavailability of Se (Se-RBA) from Se-enriched rice from a wide geographic range. Se concentrations were in the range of 0.06 ± 0.05 to 0.15 ± 0.15 mg kg-1, averaging 0.12 ± 0.11 mg kg-1, in 196 rice samples from 21 Chinese provinces, and selenomethionine (SeMet) was the dominant Se fraction (58.0-96.5%). The Se-RBA of Se-enriched rice calculated from urine ranged from 34.86% to 102.29%, averaging 62.27% (n = 12), and was positively correlated with the proportion of SeMet in rice (p < 0.05, R2 = 0.51). Furthermore, the Se intake calculated based on the Se-RBA indicated that the Se intake of consumers of Se-enriched rice was far less than the tolerable upper intake level. Thus, the limits established by law assume overestimates of the actual nutritional value of the Se content in Se-enriched rice, and it is important to consider Se bioavailability. The current study offers suggestions for future research and provides methods to reduce the uncertainty in estimating the health risks associated with Se intake from rice.

Arsenic and cadmium pose a potential health risk to human beings via rice grain consumption. In the current study, a pot experiment was conducted to evaluate the effect of Br (5 mM and 20 mM) and Se (1 mM) at rice tillering and filling stages on Cd and As accumulation in rice grain and their health risk indices. The results showed that Br or Se applications at different stages of rice improved the photosynthesis, reduce MDA content in flag leaves by 17.41%-38.65%, increased rice biomass and grain yield by 10.50%-29.94% and 10.50%-36.56%, and enhanced grain N and P uptake by 3.25%-34.90%, and 22.98%-72.05%, respectively. Applications of Br and Se effectively decreased Cd and As concentration in rice grain by 31.74%-86.97% and 16.42%-81.13% respectively. Compared to the individual treatment, combined 20 mM Br and 1 mM Se at the filling stage showed the lowest accumulation of As (0.149 mg·kg-1) and Cd (0.105 mg·kg-1) in grain, and its health risk index was below the acceptable limits (HRI < 1). This implies that application of Br and Se at the filling stage is a promising strategy for the safe production of rice in As and Cd co-contaminated regions.


In this study, foliar applications of Br and Se at the grain filling and tillering stage demonstrate their effect on As and Cd accumulation. The findings showed that Br and Se resulted in the Se concentration in grains reaching the Se-enriched level, and the accumulation of As and Cd was the lowest. Furthermore, the application of Br and Se decreased lipid peroxidation, promoted N and P uptake, and increased the rate of photosynthesis in the rice plants, which resulted in increasing rice growth and grain yield. The HRI of heavy metals was below the acceptable limits after application of Br and Se.

Arsenic (As) contamination in vegetables is a severe threat to human health. However, the evaluation of As relative bioavailability (As-RBA) or bioaccessibility in vegetables is still unexplored. The study sought to evaluate the As-RBA in commonly consumed ten leaf vegetables collected from As-polluted farmlands. Additionally, the As-RBA was determined using rat bioassay and compared with As bioaccessibility through five commonly used in vitro methods, including UBM (Unified BARGE Method), SBRC (Solubility Bioavailability Research Consortium), DIN (Deutsches Institut für Normung e.V.), IVG (In Vitro Gastrointestinal), and PBET (Physiologically Based Extraction Test). Results showed that the As-RBA values were 14.3-54.0% among different vegetables. Notably, significant in vivo-in vitro correlations (IVIVC) were observed between the As-RBA and the As bioaccessibility determined by the PBET assay (r2 = 0.763-0.847). However, the other assays (r2 = 0.417-0.788) showed a comparatively weaker relationship. The estimation of As-RBA using derived IVIVC to assess As exposure risk via vegetable consumption confirmed that As exposure risk based on As-RBA was lower than that the total As concentrations. Therefore, it was concluded that PBET could better predict the As-RBA in vegetables than other in vitro assays. Furthermore, As-RBA values should be considered for accurate health risk assessment of As in vegetables.

Rice (Oryza sativa L.) consumption represents a major route for the exposure to cadmium (Cd) and arsenic (As) in many countries. Two varieties of rice that were grown in soils contaminated with Cd and As were evaluated for the accumulation of these toxins in rice grains and the risks of exposure of local residents to Cd and As when treated with different amounts of silkworm excrement and types of water management. Silkworm excrement, water management and the variety of rice significantly affected the accumulation of Cd and As in rice. The combination of multiple measures can be more effective at reducing heavy metals than the use of single measure, i.e., silkworm excrement management, water management, and the selection of low accumulation variety. The use of a variety that accumulates low amounts of Cd combined with 1% silkworm excrement management can effectively increase the soil pH and electrical conductivity (EC) and decrease the contents of soil available Cd and the transfer coefficients of Cd in rice, subsequently reducing the concentrations of Cd in rice grains and lowering the health risks of the intake of Cd. Similarly, the use of a conventional rice variety combined with alternating periods of drying and wetting in the three weeks before and after the heading stage decreased the contents of soil available As and the transfer coefficient of As in rice, subsequently reducing the accumulation of As in the grains and lowering the health risk of the intake of As. The significantly lower concentrations of Cd and As in rice grains and the risk of intake of Cd and As from rice was observed using a conventional rice variety combined with alternating drying-wetting in the three weeks before and after the heading stage and 1% silkworm excrement management. Thus, the combination of multiple measures in the coexistence of Cd and As in contaminated soils can be a promising strategy to avoid serious health risks and ensure the safety of food for local residents.

The correlation of in vitro and in vivo assays for determining bioavailable Cd amounts in vegetables is limited. Herein, the correlations between Cd relative bioavailability (Cd-RBA) in rat models and Cd bioaccessibility in four in vitro assays were examined in vegetables. Results showed that the combined liver plus kidney data provided the appropriate endpoint and was used as a biomarker to estimate Cd-RBA. The Cd-RBA was negatively correlated with the mole ratio of Ca/Cd and Fe/Cd in vegetables. Strong in vivo-in vitro correlations were found from physiologically based extraction test (PBET) and in vitro gastrointestinal (IVG) (R2 = 0.66-0.69). We concluded that PBET and IVG were optimal models for Cd-RBA determination in vegetables. The nutritional elements in the vegetables could affect Cd absorption. Furthermore, the Cd bioavailability in vegetables should be considered because risk estimates solely based on the total Cd concentration in vegetables would overestimate Cd intake.

OBJECTIVE: The argument surrounding the safety and effectiveness of interventions for the population of individuals at a clinical high risk of developing psychosis has been ongoing for the past 30 years. However, few studies have assessed the needs of this special young population, who are struggling with the recent onset of psychotic symptoms. METHOD: The sample consisted of 171 family members of 108 clinical high-risk individuals included from the ShangHai at Risk for Psychosis research programme. A 'WeChat' group was established to provide mutual support. There were 22,007 valid messages sent within the group between 1 April 2015 and 27 June 2016. Chat records were subsequently analysed to determine the needs of families during intervention at the early stages of psychosis. RESULTS: Families of clinical high-risk individuals were highly involved in the entire medical process, and the major concerns of the families of clinical high-risk individuals focused on both functional recovery and medication. The themes of 'take medication', 'go to school' and 'study in school' were often discussed within the group. CONCLUSION: A family-focused intervention targeting functional recovery and real-time professional explanations of medication would meet the major needs of families of Chinese clinical high-risk individuals.

In many countries cadmium (Cd) and arsenic (As) commonly coexist in soils contaminated by mining activities, and can easily enter the human body via consumption of leafy vegetables, like the popularly consumed pakchoi (Brassica chinensis L.), causing major health concerns. In the present study, bioaccessibility and human exposure of Cd and As were assessed in twenty genotypes of pakchoi cultured at two different levels of co-contamination to identify low health risk genotypes. The bioaccessibilities of Cd and As represent a fraction of the total metals content could be bioaccessible for human, in the present study, significant differences in pakchoi Cd and As bioaccessibility were observed among all tested genotypes and co-contaminated levels. Cd and As bioaccessibility of pakchoi were in the ranges of 24.0-87.6% and 20.1-82.5%, respectively, for in the high level co-contaminated soils, which was significantly higher than for low level co-contaminated soils with 7.9-71.8% for Cd bioaccessibility and 16.1-59.0% for As bioaccessibility. The values of bioaccessible established daily intakes (BEDI) and the total bioaccessible target hazard quotients (TBTHQ) of Cd and As were also considerably higher in high level co-contaminated soils than in low level co-contaminated soils. Two genotypes (Meiguanqinggengcai and Zhenqing60F1) contained relatively low concentrations and bioaccessible Cd and As and, their BEDI and TBTHQ for Cd and As ranged below the tolerable limits set by the FAO/WHO (BEDI of Cd < 0.83 µg kg-1 bw day-1, BEDI of As < 3 µg kg-1 bw day-1) and United States Environmental Protection Agency (TBTHQ for Cd and As < 1), this applied for both levels of co-contaminated soils for adults and children. Consequently, these findings suggest identification of safe genotypes in leafy vegetable with low health risk via genotypic screening and breeding methods could be a useful strategy to ensure the safety of food crops grown in those Cd and As co-contaminated fields due to mining activities.

Previous published methods for the analysis of folates are time consuming because of lengthy sample extraction, clean-up and total running time. This study details the development and validation of a rapid, sensitive and robust method that combines a simple extraction step with ultra-performance liquid chromatography coupled to tandem quadrupole mass spectrometry. Here, we reported application of a tandem quadrupole mass spectrometer to analyze maximum seven vitamers of folate from plant origin. The analytical performance was evaluated by linearity, sensitivity, precision, recovery test and analysis of certified reference materials. The limit of detection and limit of quantification ranged between 0.003 and 0.021µg/100g FW and between 0.011 and 0.041µg/100g FW, respectively; the recovery and precession ranged from 71.27 to 99. 01% and from 1.7 to 7.8% RSD, respectively, depending upon folate vitamers. This newly developed and validated method is rapid (a chromatographic run time of 5min), easy to be performed (no laborious and time consuming clean-up) and can be used to simultaneously analyze seven vitamers of folate from plant sources.
