RESUMO

Bridge operational accident analysis is a critical process in bridge operational risk management. It provides valuable knowledge support for responding to newly occurring accidents. However, there are three issues: (1) research specifically focused on the past bridge operational accidents is relatively scarce; (2) there is a lack of mature research findings regarding the bridge operational accidents knowledge representation; and (3) in similar case retrieval, while case-based reasoning (CBR) is a valuable approach, there are still some challenges and limitations associated with its usage. To tackle these problems, this research proposed an automated analysis approach for bridge operational accidents based on a knowledge graph and CBR. The approach includes case representation and case retrieval, leveraging advancements in computer science and artificial intelligence. In the proposed approach, the case representation involves the adoption of a knowledge graph to construct multi-dimensional networks. The knowledge graph captures the relationships between various factors and entities, allowing for a comprehensive representation of accidents domain knowledge. In the case retrieval, a multi-circle layer retrieval strategy was innovatively proposed to enhance retrieval efficiency. Three target cases were randomly selected to verify the validity of the proposed methodology. The combination of a knowledge graph and CBR can indeed provide useful tools for the automated analysis of bridge operational accidents. Additionally, the proposed methodology can serve as a reference for intelligent risk management in other types of infrastructures.

BACKGROUND: Patients' subjective experiences during clinical interactions may affect their engagement in healthcare, and better understanding of the issues patients consider most important may help improve service quality and patient-staff relationships. While diagnostic imaging is a growing component of healthcare utilization, few studies have quantitatively and systematically assessed what patients deem most relevant in radiology settings. To elucidate factors driving patient satisfaction in outpatient radiology, we derived quantitative models to identify items most predictive of patients' overall assessment of radiology encounters. METHODS: Press-Ganey survey data (N = 69,319) collected over a 9-year period at a single institution were retrospectively analyzed, with each item response dichotomized as "favorable" or "unfavorable." Multiple logistic regression analyses were performed on 18 binarized Likert items to compute odds ratios (OR) for those question items significantly predicting Overall Rating of Care or Likelihood of Recommending. In a secondary analysis to identify topics more relevant to radiology than other encounter types, items significantly more predictive of concordant ratings in radiology compared to non-radiology visits were also identified. RESULTS: Among radiology survey respondents, top predictors of Overall Rating and Likelihood of Recommending were items addressing patient concerns or complaints (OR 6.8 and 4.9, respectively) and sensitivity to patient needs (OR 4.7 and 4.5, respectively). When comparing radiology and non-radiology visits, the top items more predictive for radiology included unfavorable responses to helpfulness of registration desk personnel (OR 1.4-1.6), comfort of waiting areas (OR 1.4), and ease of obtaining an appointment at the desired time (OR 1.4). CONCLUSIONS: Items related to patient-centered empathic communication were the most predictive of favorable overall ratings among radiology outpatients, while underperformance in logistical issues related to registration, scheduling, and waiting areas may have greater adverse impact on radiology than non-radiology encounters. Findings may offer potential targets for future quality improvement efforts.

In order to reveal the pollution characteristics and risk levels of DBPs in typical drinking water sources in Wuhan under the COVID-19 pandemic, 26 sampling sites were selected in typical drinking water sources in Wuhan. N,N-diethyl-1,4-phenylenediamine spectrophotometry and gas chromatograph-micro-cell electron capture detector (GC-µECD) methods were used to detect residual chlorine disinfectants and DBPs in water, respectively, and their health and ecology risks were assessed. The results showed that free chlorine or total residual chlorine were detected in 16 of the 26 water samples, and the maximum concentration was 0.04 mg·L-1, which exceeded the limit of the surface water standard in China. The concentration of residual chlorine was higher in sampling sites near the outfall of a municipal sewage plant. There were 34 types of DBPs measured in 10 sampling sites, and 24 types of substances were detected with the detection rate of 10.00%-100.00%. The ρ (total DBPs) was in the range of 0.11-104.73 µg·L-1, with an average value of 7.26 µg·L-1. The concentration of chloroform was the highest among all the DBPs, ranging from 9.98 µg·L-1 to 11.15 µg·L-1, with an average value of 10.47 µg·L-1. The concentration of 2-bromo-2-iodoacetamide was the lowest, ranging from ND-0.11 µg·L-1, with an average value of 0.01 µg·L-1. The overall detection level of the DBPs area was low in this study area, and the result of the health risk assessment showed that the DBPs had no carcinogenic or non-carcinogenic health risks to human body. However, the results of the ecological risk assessment showed that chloroform presented a high ecological risk to aquatic organisms.

Objective: To characterize predictors of patient satisfaction in outpatient radiology, we examined whether patient satisfaction differs across radiology modalities and demographic groups. Methods: A random sampling of Press-Ganey outpatient services surveys for radiology and non-radiology visits from September 2008 to September 2017 were retrospectively analyzed. Composite scores averaged across all Likert items were analyzed as both a continuous variable and a dichotomous variable of dissatisfaction (defined as ≤3 on the 5-point scale). Results: Among 9983 radiology surveys, mammography had higher composite scores than MRI, CT, radiography, US, and NM/PET (p < 0.001) and lower dissatisfaction (3.9%) than CT (6.7%), MRI (7.3%), and radiography (8.2%). Low-scoring responses were most common in the Facilities domain (7.8%) and least common in Overall Assessment (3.8%). Satisfaction metrics were lowest for ages 20-29 and highest for ages 70-79. Lower dissatisfaction rates were seen among Hispanics (3%) and whites (6%), compared to blacks (10%) and Asians (18%). Conclusion: Significant differences in patient satisfaction were found across imaging modalities and demographic variables. Further investigations to identify contributing factors may help improve patient experiences.

Solid phase extraction (SPE) and high-performance liquid chromatography-mass spectrometry (HPLC-MS) were used to detect and analyze the distribution of 10 antibiotics including sulfonamides, fluoroquinolones, and macrolides in the Harbin section of Songhua River basin and Ashe River, a tributary of Songhua River. The correlation between the antibiotic concentration and water quality indexes was analyzed and the ecological risks were evaluated. The results showed that only six antibiotics were detected in the entry section of Harbin city on the Songhua River and the concentration was relatively low. However, nine antibiotics were detected in the exit section and only sulfamethazine (SM1) was not detected. The concentrations of macrolide antibiotics increased most significantly, followed by those of sulfonamides and fluoroquinolones. The inflow of three tributaries in Harbin city was the direct cause of the increase in antibiotic concentration in the Songhua River. Only sulfapyridine (SMPD) was not detected in the upper section of Ashe River. Ten antibiotics were detected in the section where the Ashe River enters the Songhua River. The other nine antibiotics were the highest except norfloxacin (NOR). Wastewater discharged from four sewage treatment plants along Ashe River is an important factor affecting the concentration of antibiotics in the Ashe River. Correlation analysis shows that three kinds of antibiotics in the Songhua River have certain positive correlations with ammonia nitrogen, total phosphorus, and total organic carbon. There is a significant positive correlation between the three kinds of antibiotics and ammonia nitrogen and total phosphorus in the Ashe River system, indicating that the water quality indexes of the Harbin section of Songhua River and Ashe River are closely related to their antibiotic concentrations. The results of ecological risk assessment showed that macrolide antibiotics in the Harbin section of Songhua and Ashe Rivers had certain ecological risks.

This study was conducted to investigate the mechanism of action and extent of selective dopaminergic neurodegeneration caused by exposure to trichloroethylene (TCE) leading to the endogenous formation of the neurotoxin 1-trichloromethyl-1,2,3,4-tetrahydro-ß-carboline (TaClo) in rodents. Beginning at 3 months of age, male C57BL/6 mice received oral TCE dissolved in vehicle for 8 months. Dopaminergic neuronal loss was assessed by nigral tyrosine hydroxylase (TH) immunoreactivity. Selective dopaminergic neurodegeneration was determined based on histological analysis of non-dopaminergic neurons in the brain. Behavioral assays were evaluated using open field activity and rotarod tests. Mitochondrial complex I activity, oxidative stress markers, and microglial activation were also examined in the substantia nigra. The level of TaClo was detected using HPLC-electrospray ionization tandem mass spectrometry. Dopaminergic neurotoxicity of TaClo was determined in midbrain organotypic cultures from rat pups. Following 8 months of TCE treatment, there was a progressive and selective loss of 50% of the dopaminergic neurons in mouse substantia nigra (SN) and about 50% loss of dopamine and 72% loss of 3,4-dihydroxyphenylacetic acid in the striatum, respectively. In addition, motor deficits, mitochondrial impairment, oxidative stress, and inflammation were measured. TaClo content was quantified in the brain after TCE treatment. In organotypic cultures, TaClo rather than TCE induced dopaminergic neuronal loss, similar to MPP+. TCE exposure may stimulate the endogenous formation of TaClo, which is responsible for dopaminergic neurodegeneration. However, even prolonged administration of TCE was insufficient for producing a greater than 50% loss of nigral dopamine neurons, indicating that additional co-morbid factors would be needed for mimicking the profound loss of dopamine neurons seen in Parkinson's disease.
