RESUMO

The concentration of potentially toxic elements (PTEs) in soils of different land-use types varies depending on climatic conditions and human. Topsoil samples were collected in Northwest China to investigate PTE pollution and risk in different land uses, and thereby estimate the risk of various pollution sources. The results showed that human activity had an impact on PTE concentrations in the study area across all land use types, with farmland, grassland, woodland, and the gobi at moderate pollution levels and the desert at light pollution levels. Different PTE sources pose different risks depending on the land-use type. Apart from deserts, children are exposed to carcinogenic risk from a variety of sources. A mixed natural and agricultural source was the main source of public health risk in the study area, contributing 38.7% and 39.0% of the non-carcinogenic and 40.7% and 35.5% of the carcinogenic risks, respectively. Monte Carlo simulations showed children were at a higher health risk from PTEs than adult s under all land uses, which ranked in severity as farmland > woodland > grassland > gobi > desert. As and Ni has a higher probability of posing both a non-carcinogenic and a carcinogenic risk to children. Sensitivity analysis showed that the contribution of parameters to the assessment model of PTEs exhibited the following contribution pattern: concentration > average body weight > ingestion rate > other parameters. The PTEs affecting the risk assessment model were not common among different land use types, where the importance distribution pattern of each parameter was basically the same in woodland, grassland, and farmland, and Ni contributed the most to carcinogenic risk. However, Cr contributed the most to the carcinogenic risk in the desert and gobi.

Background: To promote a comprehensive understanding of global trends and burden of type 2 diabetes attributable to physical inactivity. Methods: We utilized data regarding mortality, disability-adjusted life years (DALYs), as well as age-standardized mortality rates (ASMR) and DALYs rates (ASDR) derived from the global burden of disease study 2019 to evaluate the impact of physical inactivity on the prevalence of type 2 diabetes in 204 countries and territories over the period from 1990 to 2019. This method facilitated the analysis of the diabetes burden across different ages, genders, and regions. To determine the long-term progression of type 2 diabetes prevalence, we computed the estimated annual percentage change (EAPC) in burden rates. Results: Globally, the number of deaths and DALYs from type 2 diabetes due to physical inactivity more than doubled between 1990 and 2019. Concurrently, there was an increase in the ASMR and ASDR, with EAPC of 0.26 (95% CI: 0.13-0.39) and 0.84 (95% CI: 0.78-0.89), respectively. As of 2019, the global ASMR and ASDR for physical inactivity stood at 1.6 (95% UI: 0.8-2.7) per 100 000 and 55.9 (95% UI: 27.2-97.6) per 100 000, respectively. Notable disparities were observed in the type 2 diabetes burden associated with physical inactivity worldwide, with higher sociodemographic index (SDI) countries experiencing lower ASDR and ASMR compared to lower SDI countries. Initially, females exhibited higher ASMR and ASDR than males, but this gender disparity in ASMR and ASDR has lessened in recent years. The mortality and DALYs rates associated with physical inactivity exhibit an inverted V-shaped pattern across various age groups, predominantly affecting the elderly population. Conclusion: Between 1990 and 2019, there was a marked rise in the worldwide burden of type 2 diabetes associated with physical inactivity, underscoring the role of physical inactivity as a key changeable risk factor in the global landscape of this disease. This necessitates additional research to explore the variables contributing to the varying levels of disease burden across different countries and between sexes. Furthermore, it calls for the formulation of public health policies aimed at guiding prevention tactics, promoting early detection, and enhancing the management of type 2 diabetes.

BACKGROUND: Fatigue is a common symptom of long COVID syndrome. Compared to male survivors, females have a higher incidence of post-COVID fatigue. Therefore, long-term follow-up is necessary to understand which groups of females are more vulnerable to post-COVID fatigue. METHODS: This is a nested case-control study of female COVID-19 survivors who were discharged from two designated hospitals in Wuhan, China in 2020, and received 2-year follow-up from March 1 to April 6, 2022. All patients completed the Checklist Individual Strength-subscale subjective fatigue (CIS-fatigue), a chronic obstructive pulmonary disease (COPD) assessment test (CAT), and the Hospital Anxiety and Depression Scale (HADS; including the HADS-Anxiety [HADS-A] and the HADS-Depression [HADS-D]). Individuals with CIS-fatigue scores of 27 or higher were classified as cases. The risk factors for fatigue was analysed with multivariable logistic regression analysis. RESULTS: A total of 899 female COVID-19 survivors were enrolled for analysis, including 47 cases and 852 controls. Compared with controls, cases had higher CAT, HADS-A and HADS-D scores, and showed a higher prevalence of symptoms, including anxiety (cases vs. controls, 44.7% vs. 4.0%, p < 0.001), chest tightness (21.2% vs. 2.3%, p < 0.001), dyspnoea (19.1% vs. 0.8%, p < 0.001) and so on. In multivariable logistic regression analysis, age (OR, 1.03; 95% CI, 1.01-1.06; p = 0.02) and cerebrovascular disease (OR, 11.32; 95% CI, 2.87-43.00; p < 0.001) were risk factors for fatigue. Fatigue had a statistically significant moderate correlation with depression (r = 0.44, p < 0.001), but not with CAT ≥ 10. CONCLUSION: Female COVID-19 patients who had cerebrovascular disease and older age have higher risk of fatigue. Patients with fatigue have higher CAT scores, and are more likely to have concurrent depression.

Nitrogen dioxide (NO2) is associated with mortality and many other adverse health outcomes. In 2021, the World Health Organization established a new NO2 air quality guideline (AQG) (annual average <10 µg/m3). However, the burden of diseases attributable to long-term NO2 exposure above the AQG is unknown in China. Nitrogen oxide is a major air pollutant in populous cities, which are disproportionately impacted by NO2; this represents a form of environmental inequality. We conducted a nationwide risk assessment of premature deaths attributable to long-term NO2 exposure from 2013 to 2020 based on the exposure-response relationship, high-resolution annual NO2 concentrations, and gridded population data (considering sex, age, and residence [urban vs rural]). We calculated health metrics including attributable deaths, years of life lost (YLL), and loss of life expectancy (LLE). Inequality in the distribution of attributable deaths and YLLs was evaluated by the Lorenz curve and Gini index. According to the health impact assessments, in 2013, long-term NO2 exposure contributed to 315,847 (95% confidence interval [CI]: 306,709-319,269) premature deaths, 7.90 (7.68-7.99) million YLLs, and an LLE of 0.51 (0.50-0.52) years. The high-risk subgroup (top 20%) accounted for 85.7% of all NO2-related deaths and 85.2% of YLLs, resulting in Gini index values of 0.81 and 0.67, respectively. From 2013 to 2020, the estimated health impact from NO2 exposure was significantly reduced, but inequality displayed a slightly increasing trend. Our study revealed a considerable burden of NO2-related deaths in China, which were disproportionally frequent in a small high-risk subgroup. Future clean air initiatives should focus not only on reducing the average level of NO2 exposure but also minimizing inequality.

BACKGROUND: Inflammation has a significant role in the onset and progression of inflammatory bowel disease (IBD). Increasing attention has been paid to the use of acupuncture in IBD patients; however, its regulatory effects on inflammatory factors in IBD still require validation. Here, we systematically evaluated the effects of acupuncture on inflammatory factors in IBD patients. METHODS: Eight electronic databases were searched for studies that met the inclusion criteria. After evaluating the quality of the studies selected by two reviewers, the meta-analysis was performed to assess the efficacy of acupuncture in IBD patients and the impact on inflammatory factors (TNF-α, IL-1, IL-8 and IL-10). RESULTS: Four randomized controlled trials with a total of 228 patients satisfied the inclusion criteria. Acupuncture has a positive therapeutic impact on IBD (MD = 1.22, 95% CI [1.07, 1.39], P = 0.003). Moreover, it regulates the levels of TNF-α (MD =-60.58, 95% CI [-100.30, -20.89], P = 0.003), IL-8 (MD =-56.40, 95% CI [-60.02, -52.14], P < 0.00001) and IL-10 (MD =35.96, 95% CI [11.02, 60.91], P = 0.005) in IBD patients. However, the P value of meta-analysis in IL-1 great than 0.05.(MD =-27.90, 95% CI [-97.82, 42.02], P = 0.11). CONCLUSION: Acupuncture has a positive therapeutic impact on IBD and can effectively regulate inflammatory factors in IBD patients. TNF-α, IL-8 and IL-10 are more appropriate inflammatory indicators for clinically evaluating the anti-inflammatory response in the blood of IBD patients by acupuncture.

N-nitrosodimethylamine (NDMA) is one of the emerging nitrogenous disinfection by-products (DBPs) with probable cytotoxicity, genotoxicity, and carcinogenesis. Its potential toxicological effects have drawn considerable attention but remain inadequately known. Here two methods, including developmental toxicity evaluation and neurobehavioral toxicity evaluation, were used and compared to investigate the safety of low-dose (0, 0.1, 1, 10, 100 µg/L) NDMA exposure to embryo-larval stage of zebrafish. Results showed that the survival and malformation rate of larvae at 6-day post fertilization (dpf) and the hatching rates at 48-h post fertilization (hpf) and 72 hpf in treatment groups had no significant difference with the control group, indicating that the tested NDMA doses were not developmentally toxic. Differently, the changes of neurobehavioral indicators performed more sensitivity. For example, 100 µg/L NDMA exposure induced locomotor hyperactivity at 7 dpf and induced an increasing effect on the relative path angle value. Further, relative value of path angle had more consistency with locomotion results compared with absolute value, indicating that relative value of path angle may be more suitable for the safety evaluation of low-dose NDMA exposure. This work supported that zebrafish neurobehavioral test is a powerful tool in the safety assessment of low-dose NDMA exposure and also has the potential to evaluate the safety of other DBPs.

BACKGROUND: Agrobacterium rhizogenes-mediated hairy root transformation provides a powerful tool for investigating the functions of plant genes involved in rhizobia-legume symbiosis. However, in the traditional identification methods of transgenic hairy roots based on reporter genes, an expensive chemical substrate or equipment is required. RESULTS: Here, we report a novel, low cost, and robust reporter for convenient, non-destructive, and directly visual selection of transgenic hairy roots by naked eye, which can be used in the study of rhizobia-legume symbiosis. The reporter gene AtMyb75 in Arabidopsis, encoding an R2R3 type MYB transcription factor, was ectopically expressed in hairy roots-mediated by A. rhizogenes, which induced purple/red colored anthocyanin accumulation in crop species like soybean (Glycine max (L.) Merr.) and two model legume species, Lotus japonicas and Medicago truncatula. Transgenic hairy roots of legumes containing anthocyanin can establish effective symbiosis with rhizobia. We also demonstrated the reliability of AtMyb75 as a reporter gene by CRISPR/Cas9-targeted mutagenesis of the soybean resistance to nodulation Rfg1 gene in the soybean PI377578 (Nod-) inoculated with Sinorhizobium fredii USDA193. Without exception, mature nitrogen-fixation nodules, were formed on purple transgenic hairy roots containing anthocyanin. CONCLUSIONS: Anthocyanin is a reliable, user-friendly, convenient, non-destructive, low cost, directly visual reporter for studying symbiotic nitrogen-fixing nodule development and could be widely applied in broad leguminous plants.

BACKGROUND: The aim of this study is to evaluate the microstructure and microcirculation of regional lymph nodes (LNs) in rectal cancer by using non-invasive intravoxel incoherent motion MRI (IVIM-MRI), and to distinguish metastatic from non-metastatic LNs by quantitative parameters. METHODS: All recruited patients underwent IVIM-MRI (b = 0, 5, 10, 20, 30, 40, 60, 80, 100, 150, 200, 400, 600, 1000, 1500 and 2000 s/mm2) on a 3.0 T MRI system. One hundred sixty-eight regional LNs with a short-axis diameter equal to or greater than 5 mm from 116 patients were evaluated by two radiologists independently, including 78 malignant LNs and 90 benign LNs. The following parameters were assessed: the short-axis diameter (S), long-axis diameter (L), short- to long-axis diameter ratio (S/L), pure diffusion coefficient (D), pseudo-diffusion coefficient (D*), and perfusion factor (f). Intraclass correlation coefficients (ICCs) were calculated to assess the interobserver agreement between two readers. Receiver operating characteristic curves were applied for analyzing statistically significant parameters. RESULTS: Interobserver agreement of IVIM-MRI parameters between two readers was excellent (ICCs> 0.75). The metastatic group exhibited higher S, L and D (P < 0.001), but lower f (P < 0.001) than the non-metastatic group. The area under the curve (95% CI, sensitivity, specificity) of the multi-parameter combined equation for D, f and S was 0.811 (0.744~0.868, 62.82%, 87.78%). The diagnostic performance of the multi-parameter model was better than that of an individual parameter (P < 0.05). CONCLUSION: IVIM-MRI parameters provided information about the microstructure and microcirculation of regional LNs in rectal cancer, also improved diagnostic performance in identifying metastatic LNs.

OBJECTIVE: To evaluate the diagnostic value of 3.0T high-resolution MRI in mesorectal lymph node metastasis of rectal cancer. METHODS: The images and postoperative pathological data of patients with pathologically diagnosed rectal cancer who underwent prospective 3.0T two dimensional high-resolution MRI rectal examinations and surgery within two weeks after MRI examination at the First Affiliated Hospital, Sun Yat-sen University from November 2015 to November 2016 were retrospectively collected. Patients who received preoperative neoadjuvant therapy and those who did not undergo operation after MRI examination were excluded. The MRI sequences included high-resolution sagittal, coronal and oblique axial T2 weighted image (T2WI) (repetition time/echo time, 3000-4000 ms/77-87 ms; slice thickness/gap, 3 mm/0 mm; field of view, 18-22 cm). Two abdominal MRI radiologists independently assessed the morphology, margin, signal of all visible mesorectal nodes, measured their minor axes (three times for each radiologist) and gave estimation of the malignancy. The criteria of metastatic nodes on high-resolution MRI T2WI were nodes with irregular shape, ill-defined border and/or heterogeneous signal. The results of MRI diagnosis were compared with postoperative pathology. The sensitivity, specificity, accuracy, positive predictive value(PPV) and negative predictive value(NPV) of mesorectal nodes and nodes with different short-axis diameter ranges were calculated to evaluate the diagnostic efficiency of high-resolution MRI. Kappa statistics was used to evaluate the agreement for per node and for per patient between high-resolution MRI and pathological results. A Kappa value of 0-0.20 indicated poor agreement; 0.21-0.40 fair agreement; 0.41-0.60 moderate agreement; 0.61-0.80 good agreement; and 0.81-1.00 excellent agreement. RESULTS: A total of 81 patients were enrolled in the retrospective cohort study, including 50 males and 31 females with age of (59.3±11.1) years. Histopathology showed 1 case of well differentiated adenocarcinoma, 63 of moderately differentiated adenocarcinoma, 9 of moderately to poorly differentiated adenocarcinoma, 2 of poorly differentiated adenocarcinoma, 3 of mucinous adenocarcinoma and 3 of tubulovillous adenocarcinoma. Histopathological staging showed 2 cases in T1 stage, 20 in T2 stage, 45 in T3 stage and 14 in T4 stage; 34 in N0 stage, 40 in N1 stage and 7 in N2 stage; 76 in M0 stage and 5 in M1 stage. A total of 377 nodes were included in the node-by-node evaluation, of which 168 (44.6%) nodes were metastatic from 58.0% (47/81) patients. The median short-axis diameter was 5.4(2.4-18.6) mm in metastatic nodes, which was significantly larger than 3.8 (2.0-8.7) mm in non-metastatic nodes[Z=10.586, P=0.000]. The sensitivity, specificity, accuracy, PPV and NPV were 74.4% (125/168), 94.7% (198/209), 85.7% (323/377), 91.9% (125/136) and 82.2% (198/241), respectively. The Kappa values between high-resolution MRI and histopathological diagnosis for node-by-node and patient-by-patient were 0.71 and 0.70 respectively, indicating good agreements. Fourteen nodes >10 mm were all metastatic. The results of high-resolution MRI for nodal status were consistent with the results of histopathological diagnosis, and the sensitivity, accuracy and PPV were all 100.0%. Among 124 nodes with short-axis diameter of 5-10 mm, 95 (76.6%) were metastatic, and the sensitivity, specificity, accuracy, PPV and NPV were 78.9% (75/95), 86.2% (25/29), 80.6% (100/124), 94.9% (75/79) and 55.6% (25/45), respectively. The agreement was fair (Kappa value 0.55) between high-resolution MRI and histopathological diagnosis. Among 239 nodes with short-axis diameter ≤5 mm, 59(24.7%) were metastatic, and the sensitivity, specificity, accuracy, PPV and NPV were 61.0% (36/59), 96.1%(173/180), 87.4%(209/239), 83.7%(36/43) and 88.3%(173/196), respectively. The agreement was good (Kappa value 0.63) between high-resolution MRI and histopathological diagnosis. CONCLUSION: Rectal high-resolution MRI has good diagnostic value for estimating metastatic mesorectal nodes by evaluating the morphology, margin and signal of nodes.
