RESUMO

Background: Rapidly growing healthcare demand associated with global population aging has spurred the development of new digital tools for the assessment of cognitive performance in older adults. Objective: To develop a fully automated Mini-Mental State Examination (MMSE) assessment model and validate the model's rating consistency. Methods: The Automated Assessment Model for MMSE (AAM-MMSE) was an about 10-min computerized cognitive screening tool containing the same questions as the traditional paper-based Chinese MMSE. The validity of the AAM-MMSE was assessed in term of the consistency between the AAM-MMSE rating and physician rating. Results: A total of 427 participants were recruited for this study. The average age of these participants was 60.6 years old (ranging from 19 to 104 years old). According to the intraclass correlation coefficient (ICC), the interrater reliability between physicians and the AAM-MMSE for the full MMSE scale AAM-MMSE was high [ICC (2,1)=0.952; with its 95% CI of (0.883,0.974)]. According to the weighted kappa coefficients results the interrater agreement level for audio-related items showed high, but for items "Reading and obey", "Three-stage command", and "Writing complete sentence" were slight to fair. The AAM-MMSE rating accuracy was 87%. A Bland-Altman plot showed that the bias between the two total scores was 1.48 points with the upper and lower limits of agreement equal to 6.23 points and -3.26 points. Conclusions: Our work offers a promising fully automated MMSE assessment system for cognitive screening with pretty good accuracy.

RESUMO

The temporal and spatial difference between the urban ecological environment and economic development refers to the unbalanced and insufficient contradiction between the quality of urban ecological environment and the development of economic strength. Based on the relevant theories of urban ecological environment and economic development, this paper explores the development laws of urban ecology and economic development and uses graph neural network algorithm to model the spatial and temporal dependence of the city's ecological environment in a province. The quality data and economic development strength data are analyzed in detail. The analysis results show that the ecological benefit index and economic benefit index of each city in the province have reached above 0.6 after 5 years of development. The level of coordinated urban development has improved significantly compared with 2017. However, in the process of the development of the market economy, it is necessary to rationally adjust the proportion of the secondary industry and the tertiary industry in the urban production structure and continuously promote the balanced development of the economy and ecology.

RESUMO

Fish and other aquatic foods (blue foods) present an opportunity for more sustainable diets1,2. Yet comprehensive comparison has been limited due to sparse inclusion of blue foods in environmental impact studies3,4 relative to the vast diversity of production5. Here we provide standardized estimates of greenhouse gas, nitrogen, phosphorus, freshwater and land stressors for species groups covering nearly three quarters of global production. We find that across all blue foods, farmed bivalves and seaweeds generate the lowest stressors. Capture fisheries predominantly generate greenhouse gas emissions, with small pelagic fishes generating lower emissions than all fed aquaculture, but flatfish and crustaceans generating the highest. Among farmed finfish and crustaceans, silver and bighead carps have the lowest greenhouse gas, nitrogen and phosphorus emissions, but highest water use, while farmed salmon and trout use the least land and water. Finally, we model intervention scenarios and find improving feed conversion ratios reduces stressors across all fed groups, increasing fish yield reduces land and water use by up to half, and optimizing gears reduces capture fishery emissions by more than half for some groups. Collectively, our analysis identifies high-performing blue foods, highlights opportunities to improve environmental performance, advances data-poor environmental assessments, and informs sustainable diets.

RESUMO

Finding a suitable mechanism to finance long-term care (LTC) is a pressing policy concern for many countries. Using Qingdao city in China as a case study, this article presents an initial assessment of a newly piloted LTC insurance by evaluating its effects on equity and efficiency in financing. Drawing data from 47 in-depth interviews conducted in 2016, this study found that there remain sizable disparities in financial burden among insurance participants, despite an emphasis on ensuring equitable access to care. Although the insurance brought cost savings to the health care sector, the LTC providers are incentivized to provide care at the least cost, even when such care is deemed inadequate due to the fixed payment for their services. This article offers critical insights into the potentials and challenges of applying the LTC insurance model in a developing country, where critical lessons can be drawn for public LTC insurance in other countries.

RESUMO

China's 13th Five-Year Plan, launched in March 2016, provides a sound policy platform for the protection of marine ecosystems and the restoration of capture fisheries within China's exclusive economic zone. What distinguishes China among many other countries striving for marine fisheries reform is its size-accounting for almost one-fifth of global catch volume-and the unique cultural context of its economic and resource management. In this paper, we trace the history of Chinese government priorities, policies, and outcomes related to marine fisheries since the 1978 Economic Reform, and examine how the current leadership's agenda for "ecological civilization" could successfully transform marine resource management in the coming years. We show how China, like many other countries, has experienced a decline in the average trophic level of its capture fisheries during the past few decades, and how its policy design, implementation, and enforcement have influenced the status of its wild fish stocks. To reverse the trend in declining fish stocks, the government is introducing a series of new programs for sustainable fisheries and aquaculture, with greater traceability and accountability in marine resource management and area controls on coastal development. As impressive as these new plans are on paper, we conclude that serious institutional reforms will be needed to achieve a true paradigm shift in marine fisheries management in China. In particular, we recommend new institutions for science-based fisheries management, secure fishing access, policy consistency across provinces, educational programs for fisheries managers, and increasing public access to scientific data.

RESUMO

We investigated aquaculture production of Asian tiger shrimp, whiteleg shrimp, giant river prawn, tilapia, and pangasius catfish in Bangladesh, China, Thailand, and Vietnam by using life cycle assessments (LCAs), with the purpose of evaluating the comparative eco-efficiency of producing different aquatic food products. Our starting hypothesis was that different production systems are associated with significantly different environmental impacts, as the production of these aquatic species differs in intensity and management practices. In order to test this hypothesis, we estimated each system's global warming, eutrophication, and freshwater ecotoxicity impacts. The contribution to these impacts and the overall dispersions relative to results were propagated by Monte Carlo simulations and dependent sampling. Paired testing showed significant (p < 0.05) differences between the median impacts of most production systems in the intraspecies comparisons, even after a Bonferroni correction. For the full distributions instead of only the median, only for Asian tiger shrimp did more than 95% of the propagated Monte Carlo results favor certain farming systems. The major environmental hot-spots driving the differences in environmental performance among systems were fishmeal from mixed fisheries for global warming, pond runoff and sediment discards for eutrophication, and agricultural pesticides, metals, benzalkonium chloride, and other chlorine-releasing compounds for freshwater ecotoxicity. The Asian aquaculture industry should therefore strive toward farming systems relying upon pelleted species-specific feeds, where the fishmeal inclusion is limited and sourced sustainably. Also, excessive nutrients should be recycled in integrated organic agriculture together with efficient aeration solutions powered by renewable energy sources.

RESUMO

While the cerebral cortex in the human brain is of functional importance, functions defined on this structure are difficult to analyze spatially due to its highly convoluted irregular geometry. This study developed a novel L1-norm regularization method using a newly proposed multi-resolution face-based wavelet method to estimate cortical electrical activities in electroencephalography (EEG) and magnetoencephalography (MEG) inverse problems. The proposed wavelets were developed based on multi-resolution models built from irregular cortical surface meshes, which were realized in this study too. The multi-resolution wavelet analysis was used to seek sparse representation of cortical current densities in transformed domains, which was expected due to the compressibility of wavelets, and evaluated using Monte Carlo simulations. The EEG/MEG inverse problems were solved with the use of the novel L1-norm regularization method exploring the sparseness in the wavelet domain. The inverse solutions obtained from the new method using MEG data were evaluated by Monte Carlo simulations too. The present results indicated that cortical current densities could be efficiently compressed using the proposed face-based wavelet method, which exhibited better performance than the vertex-based wavelet method. In both simulations and auditory experimental data analysis, the proposed L1-norm regularization method showed better source detection accuracy and less estimation errors than other two classic methods, i.e. weighted minimum norm (wMNE) and cortical low-resolution electromagnetic tomography (cLORETA). This study suggests that the L1-norm regularization method with the use of face-based wavelets is a promising tool for studying functional activations of the human brain.
