RESUMO

BACKGROUND: This study aims to (1) determine the reliability and validity of the interRAI Chinese Self-reported Carer Needs (SCaN) assessment among informal Chinese caregivers of older adults, (2) identify predictors of caregiving distress in Asian regions with long-standing Confucian values of filial piety and family responsibility. METHODS: This cross-sectional study recruited 531 informal Chinese caregivers of older adults in Hong Kong, Shanghai, Taiwan, and Singapore. The scale reliability was examined using Cronbach's alphas (α) and McDonald's omega coefficient (ω). The concurrent validity and discriminant validity were assessed using Spearman rank correlations (rho). To examine the predictors of caregiving distress among informal caregivers of older adults, we employed hierarchical linear regression analyses informed by the Model of Carer Stress and Burden and categorized the predictors into six domains. RESULTS: Results revealed good internal consistency reliability (α = 0.83-0.96) and concurrent validity (rho = 0.45-0.74) of the interRAI Chinese SCaN assessment. Hierarchical linear regression analysis revealed that entering the background factors, primary stressors, secondary stressors, appraisal, and exacerbating factors all significantly enhanced the model's predictability, indicating that the source of caregiving distress is multidimensional. In the full model, caregivers with longer informal care time, lack of support from family and friends, have unmet needs, experience role overload, have sleep problems, and low IADL functioning are at a higher risk of caregiving distress. CONCLUSIONS: The interRAI Chinese SCaN Assessment was found to be a reliable and valid tool among the Chinese informal caregivers of older adults. It would be useful for determining family caregivers' strengths, needs, and challenges, and tailoring interventions that address the potentially modifiable factors associated with caregiving distress and maximize support. Healthcare providers working in home and community settings should be aware of the early identification of caregiving distress and routine assessment of their needs and empower them to continue taking care of their needs and providing adequate care to the care recipient.

RESUMO

China has always adhered to the strategy of sustainable development. It is prevalent the public want a good living environment, which requires local governments and businesses to enhance their environmental governance capabilities. Using the panel data from Chinese cities from 2012 to 2019 and econometrics models, we examine the impact mechanisms of public environmental appeals (PEA) on efficiency of collaborative governance in pollution reduction and carbon mitigation (GPC). Results indicate that there is a positive spatial clustering of GPC across cities, with high-high clustering is notably concentrated in the southern regions of China and low-low clustering is prevalent in the northern regions. Spatial econometrics model results reveal that the stronger PEA, the higher GPC. The result of mechanism analysis shows the mediation of environmentally friendly technological innovation is crucial. Subsequent inquiry uncovers that the digital economy positively moderates the impact of PEA on GPC. The Belt and Road policy region exhibits heightened sensitivity to PEA, thereby enhancing the positive impact of PEA on GPC.

RESUMO

Background: High prices of anticancer drugs have raised concerns due to their financial impact on patients and healthcare systems. This study aimed to assess the initial and latest list prices and clinical value of reimbursed anticancer drugs in China, Japan, and South Korea. Methods: We identified anticancer drugs newly approved by the National Medical Products Administration of China from January 2012 to June 2022, and by the Pharmaceuticals and Medical Devices Agency of Japan and the Ministry of Food and Drug Safety of South Korea up until June 2022. We compared initial and latest treatment prices between countries and assessed clinical value using patients' survival, quality of life (QoL), and European Society for Medical Oncology Magnitude of Clinical Benefit Scale (ESMO-MCBS). We calculated Spearman rank correlation coefficients of treatment prices with clinical value for individual countries and employed regression analyses to investigate whether the relationship between prices and clinical value was modified by the country setting. Findings: Our cohort included 91 anticancer drug indications, with 60 listed for reimbursement in China, 91 in Japan, and 87 in South Korea. Median treatment prices were highest in Japan, followed by South Korea, and lowest in China, both for initial prices (US$64082 vs. US$45529 vs. US$19144, p < 0.0001) and latest prices (US$50859 vs. US$31611 vs. US$18666, p < 0.0001). Over time, China (ß = -0.047, p < 0.0001) and South Korea (ß = -0.049, p < 0.0001) witnessed more significant price reductions compared to Japan (ß = -0.013, p = 0.011). The correlations between both initial and latest treatment prices and clinical value (QoL and ESMO-MCBS) were more significant and stronger in China and South Korea than in Japan, although Japan exhibited slightly stronger correlations in terms of survival compared to China and South Korea. The relationship between clinical value and treatment prices may not be modified by the country setting. Interpretation: In comparison, South Korea's list prices and their correlations with clinical value appear reasonable. Policymakers in Japan could enhance efficiency by controlling prices and aligning them with clinical value, while China would need to take substantial steps to expand anticancer drug coverage. RESUMO

Stroke survivors frequently experience difficulties in daily activities, such as bathing, feeding, and mobility. This study aimed to evaluate the reliability and validity of a computer-adaptive test-Longshi scale (CAT-LS) for assessing activities of daily living (ADL) in stroke survivors. This cross-sectional study collected data using an electronic application. The ADL function of stroke survivors in rehabilitation departments of hospitals was assessed using both the CAT-LS and BI. Correlations between the CAT-LS and Barthel index (BI) and concurrent validity were evaluated using Pearson's correlation test and multiple linear regression. Interrater reliability was evaluated using the intraclass correlation coefficient based on a two-way random effect. The internal consistency of the CAT-LS was assessed using Cronbach's coefficient (α) and corrected item-total correlations. Overall, 103 medical institutions in China were used in this study. In total, 7151 patients with stroke were included in this study. The CAT-LS classified patients into three ADL groups (bedridden, domestic, and community) with significantly different BI scores (P < 0.05). The CAT-LS results obtained using the decision-tree scoring model were consistent with the scores for each BI item. A strong correlation was observed between CAT-LS and BI (Pearson's r: 0.6-0.894, P < 0.001). The CAT-LS demonstrated good internal consistency (Cronbach's α, 0.803-0.894) and interrater reliability (ICC, 0.928-0.979). CAT-LS is time-efficient and requires < 1 min to administer. The CAT-LS is a reliable and valid tool for assessing ADL function in stroke survivors and can provide rapid and accurate assessments that reduce the burden on healthcare professionals. Further validation of this tool in other populations and settings is necessary.Study registration number: No.: ChiCTR2000034067; http://www.chictr.org.cn/showproj.aspx?proj=54770 .

RESUMO

BACKGROUND: The potential role played by launch price and clinical value in reimbursement decisions has not been sufficiently established in China. This study aimed to investigate the association of launch price and clinical value with reimbursement decisions for anticancer drugs after the implementation of reimbursement-linked price negotiation in China. METHODS: Anticancer drugs approved by the National Medical Products Administration (NMPA) of China from January 2017 to June 2022 were eligible for inclusion. Approval and reimbursement dates of included drug indications were retrieved from publicly available resources. We collected measures of clinical value, including survival, quality of life (QoL), and overall response rate from pivotal clinical trials and calculated treatment price at launch. Univariate and multivariate Cox proportional hazards models were employed to estimate the association between launch price, clinical value, and reimbursement decisions of anticancer drugs in China. RESULTS: The median reimbursement lag was 579 days (interquartile range [IQR]: 402-936) for 93 indications supported by randomized controlled trials and 637 days (IQR: 373-858) for 42 indications supported by single-arm clinical trials. Reimbursement was granted to 60 (65%) and 23 (55%) indications supported by randomized controlled and single-arm clinical trials, respectively. The launch price of anticancer drugs was not associated with reimbursement decisions in multivariate regression analyses. Indications supported by randomized controlled trials with higher clinical value were more likely to be reimbursed (hazard ratio [HR] for survival=1.07, 95% CI: 1.00-1.15, P=.037), while the overall response rate of indications supported by single-arm clinical trials was not associated with the likelihood of being reimbursed (HR=2.09, 95% CI: 0.14-32.28, P=.595). CONCLUSION: The launch price of anticancer drugs may not have a significant impact on reimbursement decisions, while the implementation of reimbursement-linked price negotiation in China has prioritized anticancer drugs with higher clinical value, but only for indications supported by randomized controlled trials. Efforts are needed to prioritize indications supported by single-arm clinical trials that have higher value during the process of price negotiation.

RESUMO

Objectives: Socioeconomic disparities in obesity have been observed in both childhood and adulthood. However, it remains unclear how the role of risk factors influencing these inequalities has evolved over time. Methods: Longitudinal data on 2,866 children and adolescents (6-17 years old) from the China Health and Nutrition Survey were used to track their BMI during childhood, adolescence, and adulthood. Concentration Index was utilized to measure socioeconomic inequalities in obesity, while Oaxaca decomposition was employed to determine the share of different determinants of inequality. Results: The concentration index for obesity during childhood and adulthood were 0.107 (95% CI: 0.023, 0.211) and 0.279 (95% CI: 0.203, 0.355), respectively. Changes in baseline BMI (24.6%), parental BMI (10.4%) and socioeconomic factors (6.7%) were found to be largely responsible for the increasing inequality in obesity between childhood and adulthood. Additionally, mother's education (-7.4%) was found to contribute the most to reducing these inequalities. Conclusion: Inequalities in obesity during childhood and adulthood are significant and growing. Interventions targeting individuals with higher BMI, especially those who are wealthy, can significantly reduce the gap.

RESUMO

BACKGROUND: While China has implemented reimbursement-linked drug price negotiation annually since 2017, emphasizing value-based pricing to achieve a value-based strategic purchase of medical insurance, whether drug prices became better aligned with clinical value after price negotiation has not been sufficiently established. This study aimed to assess the changes in prices and their relationship with the clinical value of anticancer drugs after the implementation of price negotiations in China. METHODS AND FINDINGS: In this observational study, anticancer drug indications that were negotiated successfully between 2017 and 2022 were identified through National Reimbursement Drug Lists (NRDL) of China. We excluded extensions of indications for drugs already listed in the NRDL, indications for pediatric use, and indications lacking corresponding clinical trials. We identified pivotal clinical trials for included indications by consulting review reports or drug labels issued by the Center for Drug Evaluation, National Medical Products Administration. We calculated treatment costs as outcome measures based on publicly available prices and collected data on clinical value including safety, survival, quality of life, and overall response rate (ORR) from publications of pivotal clinical trials. The associations between drug costs and clinical value, both before and after negotiation, were analyzed using regression analyses. We also examined whether price negotiation has led to a reduction in the variation of treatment costs for a given value. We included 103 anticancer drug indications, primarily for the treatment of blood cancer, lung cancer, and breast cancer, with 76 supported by randomized controlled trials and 27 supported by single-arm clinical trials. The median treatment costs over the entire sample have been reduced from US$34,460.72 (interquartile range (IQR): 19,990.49 to 55,441.66) to US$13,688.79 (IQR: 7,746.97 to 21,750.97) after price negotiation (P < 0.001). Before price negotiation, each additional month of survival gained was associated with an increase in treatment costs of 3.4% (95% confidence interval (CI) [2.1, 4.8], P < 0.001) for indications supported by randomized controlled trials, and a 10% increase in ORR was associated with a 6.0% (95% CI [1.6, 10.3], P = 0.009) increase in treatment costs for indications supported by single-arm clinical trials. After price negotiation, the associations between costs and clinical value may not have changed significantly, but the variation of drug costs for a given value was reduced. Study limitations include the lack of transparency in official data, missing data on clinical value, and a limited sample size. CONCLUSIONS: In this study, we found that the implementation of price negotiation in China has led to drug pricing better aligned with clinical value for anticancer drugs even after substantial price reductions. The achievements made in China could shed light on the price regulation in other countries, particularly those with limited resources and increasing drug expenditures.

RESUMO

The exceptional rise in overall economic activities has deteriorated environmental sustainability around the world. However, countries around the globe are implementing strategies for reaching the global climate objective. For this purpose, OECD countries committed many efforts, although their pledges and results are not parallel to the level of the Paris Agreement's ambition. This study examines the impact of eco-innovation, environmental taxes, and renewable energy consumption on the environmental performance of selected OECD countries over the period of 2006 to 2020. This study uses the generalized method of moments (GMM) and instrumental variables 2 stage least square (2SLS) methods. For robustness checks, this study uses a quantile regression approach. We conclude that an increase in the adoption of renewable energy and green innovation has a statistically significant impact on controlling CO2 emissions. Moreover, the empirical model is expanded by incorporating environmental taxes as an explanatory variable. The expanded model showed that the imposition of environmental taxes has a detrimental impact on the reduction of CO2 emissions. Moreover, on the contrary, an increase in economic activities, measured by GDP, is responsible for rising CO2 emissions in OECD countries. In light of the results we obtained, policy recommendations are provided.

RESUMO

The morphology control of metal phosphonates is always a difficulty because there are many challenges derived from the complexity of crystallization and the multivariable synthesis system. Responding to challenges, we propose a synthesis strategy guided by a decision tree for morphology control of metal phosphonates, through which directional design of the morphology-controlled synthesis can be realized. Specifically, any one synthetic condition involving the synthesis of metal phosphonates can be regarded as a decision problem to construct a binary decision tree. By means of the classification principle of the binary decision tree, the samples synthesized under the boundary value of each synthesis condition are classified based on crystal phase and morphology. The key synthetic conditions determining crystal phase and morphology can be precisely screened out to serve as decision nodes for the binary decision tree and are also rapidly optimized by the recursion level by level, whereas others cannot. Here, the ß-polymorph of copper phenylphosphonate (ß-CuPP) is selected as an example to elaborate the decision-tree-guided synthesis strategy for morphology control of metal phosphonates. From the constructed binary decision tree, it is clear that the right amount of methanol in the solvent is vital to obtain ß-phase of CuPP, whereas the reactant concentration, pH value, and reaction time are important for morphology and phase transformation. Under the optimal synthetic conditions screened out by the binary decision tree, ß-CuPP can thus be controlled to be hierarchically flower-like microsphere morphology through either the direct synthesis route or the solid-to-solid phase transformation route. This research work confirms that the decision-tree-guided synthesis is highly efficacious for the morphology control of metal phosphonates. Furthermore, the morphology-controlled synthesis guided by a decision tree may provide some valuable inspiration for morphology control of metal-organic frameworks (MOFs) and even coordinate compounds.

RESUMO

BACKGROUND: The social disparities in obesity may originate in early life or in adulthood, and the associations of socioeconomic position (SEP) with obesity could alter over time. It is unclear how lifetime-specific and life-course SEP influence adult obesity development in China. METHODS: Based on the China Health and Nutrition Survey (CHNS), three SEP-related indicators, including the father's occupational position and the participant's education and occupational position, were obtained. The life-course socioeconomic changes and a cumulative SEP score were established to represent the life-course SEP of the participants in the study. The growth mixture modeling was used to identify BMI trajectories in adulthood. Multinomial logistic regression was adopted to assess the associations between SEP and adult BMI trajectories. RESULTS: A total of 3,138 participants were included in the study. A positive correlation was found between the paternal occupational position, the participants' occupational position, education, and obesity in males, whereas an inverse correlation was observed among females. Males who experienced social upward mobility or remained stable high SEP during the follow-up had 2.31 and 2.52-fold risks of progressive obesity compared to those with a stable-low SEP. Among females, stable high SEP in both childhood and adulthood was associated with lower risks of progressive obesity (OR = 0.63, 95% CI: 0.43-0.94). Higher risks of obesity were associated with the life-course cumulative SEP score among males, while the opposite relationship was observed among females. CONCLUSIONS: The associations between life-course SEP and BMI development trajectories differed significantly by gender. Special emphasis should be placed on males experiencing upward and stable high socioeconomic change.

RESUMO

BACKGROUND: Diabetes mellitus (DM) imposes a significant disease burden in economically transitioning regions. Most transitioning regions share similar experience in urbanization processes. Shanghai's Pudong district serves as a representative area of such regions. OBJECTIVE: We aimed to assess the burden of and trends in DM mortality in Shanghai's Pudong district and analyze the impact of aging and multimorbidity. METHODS: A longitudinal, population-based study was conducted to analyze DM mortality in Pudong from 2005 to 2020. We used joinpoint regression to analyze epidemiological features and long-term trends in crude mortality rate (CMR), age-standardized mortality rate worldwide (ASMRW), and years of life lost (YLL). Furthermore, the decomposition method was used to evaluate the contribution of demographic and nondemographic factors associated with mortality. RESULTS: There were 49,414 deaths among individuals with DM, including 15,512 deaths due to DM. The CMR and ASMRW were 109.55/105 and 38.01/105 person-years, respectively. Among the mortality associated with and due to DM, the total annual ASMRW increased by 3.65% (95% CI 3.25%-4.06%) and 1.38% (95% CI 0.74%-2.02%), respectively. Additionally, the total annual YLL rate increased by 4.98% (95% CI 3.92%-6.05%) and 2.68% (95% CI 1.34%-4.04%). The rates of YLL increase in persons aged 30 to 44 years (3.98%, 95% CI 0.32%-7.78%) and 45 to 59 years (4.31%, 95% CI 2.95%-5.69%) were followed by the increase in persons aged 80 years and older (10.53%, 95% CI 9.45%-11.62%) for deaths associated with DM. The annual CMR attributable to demographic factors increased by 41.9% (95% CI 17.73%-71.04%) and 36.72% (95% CI 16.69%-60.2%) for deaths associated with and due to DM, respectively. Hypertension, cerebrovascular disease, and ischemic heart disease were the top 3 comorbidities. CONCLUSIONS: Aging and multimorbidity played essential roles in changing the burden of DM in an urbanizing and transitioning region. There is an increasing disease burden among young and middle-aged people, emphasizing the need for greater attention to these groups. Health management is an emerging method that holds important implications for alleviating the future burden of DM.

RESUMO

Plant lodging is one of the most essential phenotypes for soybean breeding programs. Soybean lodging is conventionally evaluated visually by breeders, which is time-consuming and subject to human errors. This study aimed to investigate the potential of unmanned aerial vehicle (UAV)-based imagery and machine learning in assessing the lodging conditions of soybean breeding lines. A UAV imaging system equipped with an RGB (red-green-blue) camera was used to collect the imagery data of 1266 four-row plots in a soybean breeding field at the reproductive stage. Soybean lodging scores were visually assessed by experienced breeders, and the scores were grouped into four classes, i.e., non-lodging, moderate lodging, high lodging, and severe lodging. UAV images were stitched to build orthomosaics, and soybean plots were segmented using a grid method. Twelve image features were extracted from the collected images to assess the lodging scores of each breeding line. Four models, i.e., extreme gradient boosting (XGBoost), random forest (RF), K-nearest neighbor (KNN) and artificial neural network (ANN), were evaluated to classify soybean lodging classes. Five data preprocessing methods were used to treat the imbalanced dataset to improve classification accuracy. Results indicate that the preprocessing method SMOTE-ENN consistently performs well for all four (XGBoost, RF, KNN, and ANN) classifiers, achieving the highest overall accuracy (OA), lowest misclassification, higher F1-score, and higher Kappa coefficient. This suggests that Synthetic Minority Oversampling-Edited Nearest Neighbor (SMOTE-ENN) may be a good preprocessing method for using unbalanced datasets and the classification task. Furthermore, an overall accuracy of 96% was obtained using the SMOTE-ENN dataset and ANN classifier. The study indicated that an imagery-based classification model could be implemented in a breeding program to differentiate soybean lodging phenotype and classify lodging scores effectively.

RESUMO

The prevalence of sleep disorders has increased because of the fast-paced and stressful modern lifestyle, negatively impacting the quality of human life and work efficiency. It is crucial to address sleep problems. However, the current practice of diagnosing sleep disorders using polysomnography (PSG) has limitations such as complexity, large equipment, and low portability, hindering its practicality for daily use. To overcome these challenges, in this article an optical fiber sensor is proposed as a viable solution for sleep monitoring. This device offers benefits like low power consumption, non-invasiveness, absence of interference, and real-time health monitoring. We introduce the sensor with an optical fiber interferometer to capture ballistocardiography (BCG) and electrocardiogram (ECG) signals from the human body. Furthermore, a new machine learning method is proposed for sleep condition detection. Experimental results demonstrate the superior performance of this architecture and the proposed model in monitoring and assessing sleep quality.

RESUMO

Capacity distribution is a challenging issue for an airline under financial budget and resource consideration. It is a large-scale optimization problem covering both long-term planning and short-term operating arrangements. This study investigates on the airline capacity distribution problem with financial budget and resource consideration. It contains subproblems of financial budget arrangement, fleet introduction, and fleet assignment. Among them, financial budget is arranged in multiple decision periods, fleet introduction is decided under fixed time points, while fleet assignment is decided under all available time points. To tackle this problem, an integer programming model is formulated for descriptions. Then, an integrated algorithm of modified Variable Neighborhood Search (VNS) and Branch-and-bound (B&B) strategy is developed to find solutions. In detail, a greedy heuristic approach is utilized to generate an initial solution for fleet introduction, the modified B&B strategy is utilized to generate the optimal solution for fleet assignment and the modified VNS is applied to update current solution for a new one with better quality. In addition, budget limit checks are added for financial budget arrangements. Finally, the hybrid algorithm is tested on efficiency and stability. It is also compared to other algorithms which replace the modified VNS by basic VNS, differential evolution and genetic algorithm. Computational results show that performance of our approach is powerful in terms of objective value, convergence speed and stability.

RESUMO

BACKGROUND: Patient global assessment (PtGA) has been recommended as one of the core domains in psoriasis clinical trials. Among multiple versions of PtGA, the single-question, 11-point PtGA numeric rating scale (NRS) remains to be validated in patients with plaque psoriasis. OBJECTIVES: To evaluate the psychometric characteristics of an 11-point PtGA NRS for disease severity in patients with moderate-to-severe plaque psoriasis. METHODS: Data were analysed from 759 patients with moderate-to-severe psoriasis in the Shanghai Psoriasis Effectiveness Evaluation CoHort (SPEECH), a prospective, multicentre and observational registry assessing the comparative effectiveness and safety of biologics (adalimumab, ustekinumab, secukinumab or ixekizumab), conventional systemic therapies (acitretin or methotrexate) and phototherapy. RESULTS: The test-retest reliability of the PtGA NRS showed good agreement (intraclass correlation coefficient range 0.79-0.83). No floor or ceiling effects of PtGA NRS were observed. The PtGA NRS was significantly correlated with the Psoriasis Area and Severity Index (PASI), static Physician Global Assessment (sPGA), body surface area, Dermatology Quality of Life Index (DLQI) and Hospital Anxiety and Depression Scale. Relatively large correlations of PtGA NRS with PASI and the DLQI 'symptoms and feelings' domain (all correlations ≥ 0.4 except at baseline) supported convergent validity. The presence of psoriatic arthritis or joint symptoms had no significant association with the PtGA NRS. In multivariate regression analyses, the PtGA NRS at baseline was predicted by age, lesion extent, lesion intensity, patients' symptoms and feelings, and impact on work or school. The PtGA NRS displayed known-groups validity with the PASI, sPGA and DLQI score bands. The PtGA NRS was responsive to change in PASI and DLQI after treatment. Anchor- and distribution-based approaches supported -3 as the minimal important difference for PtGA NRS. An absolute PtGA NRS ≤ 2 during follow-up was concordant with the state of minimal disease activity based on a 90% reduction in PASI (PASI 90) or PASI 90 plus a DLQI of 0/1. Sensitivity analysis using subgroup comparison and multiple imputation model yielded consistent conclusions. CONCLUSIONS: The PtGA NRS showed good reliability, validity and responsiveness in patients with psoriasis, and was feasible in clinical trials and daily practice.

RESUMO

Background: The study explored sources of meaning in older adults and the action path among family care, meaning in life, quality of life, and depression. Materials and methods: We investigated 627 older adults using the Sources of Meaning in Life Scale for the Elderly (SMSE), the Family Care Index (APGAR), the Center for Epidemiological Studies Depression Scale-10 (CES-D-10), and the EuroqOL-5 Dimensions (EQ-5D). Results: Scores categorized 454 older adults with good family function, 99 with moderate, and 47 with severe family dysfunction; 110 older adults had depression. The structural equation model showed that family care affected the quality of life and depression by influencing meaning, and depression had a significant negative effect on the quality of life (P < 0.05). The model was a good fit for the data (χ2/df = 3.300, SRMR = 0.0291, GFI = 0.975, IFI = 0.971, TLI = 0.952, CFI = 0.971, RMSEA = 0.062). Conclusion: Meaning in life is an intermediary factor that affects depression and quality of life in older adults. Family care had a significant positive impact on SMSE and a negative influence on depression. The SMSE effectively clarifies the sources of meaning in life and can be used to improve meaning and promote mental health in older adults.

RESUMO

BACKGROUND: As lymphovascular space invasion (LVSI) was closely related to lymph node metastasis and prognosis, the preoperative assessment of LVSI in early-stage cervical cancer is crucial for patients. PURPOSE: To develop and validate nomogram based on multimodal MR radiomics to assess LVSI status in cervical cancer patients. STUDY TYPE: Retrospective. POPULATION: The study included 168 cervical cancer patients, of whom 129 cases (age 51.36 ± 9.99 years) from institution 1 were included as the training cohort and 39 cases (age 52.59 ± 10.23 years) from institution 2 were included as the external test cohort. FIELD STRENGTH/SEQUENCE: There were 1.5 T and 3.0 T MRI scans (T1-weighted imaging [T1WI], fat-saturated T2-weighted imaging [FS-T2WI], and contrast-enhanced [CE]). ASSESSMENT: Six machine learning models were built and selected to construct the radiomics signature. The nomogram model was constructed by combining the radiomics signature with the clinical signature, which was then validated for discrimination, calibration, and clinical usefulness. STATISTICAL TESTS: The clinical characteristics were compared using t-tests, Mann-Whitney U tests, or chi-square tests. The Spearman and LASSO methods were used to select radiomics features. The receiver operating characteristic (ROC) analysis was performed, and the area under the curve (AUC), accuracy, sensitivity, and specificity were calculated. RESULTS: The logistic regression (LR) model performed best in each sequence. The AUC of CE-T1-T2WI-combined was the highest in the LR model, with an AUC of 0.775 (95% CI: 0.570-0.979) in external test cohort. The nomogram showed high predictive performance in the training (AUC: 0.883 [95% CI: 0.823-0.943]) and test cohort (AUC: 0.830 [95% CI: 0.657-1.000]) for predicting LVSI. Decision curve analysis demonstrated that the nomogram was clinically useful. DATA CONCLUSION: Our findings suggest that the proposed nomogram model based on multimodal MRI of CE T1WI-T2WI-combined could be used to assess LVSI status in early cervical cancer. EVIDENCE LEVEL: 4. TECHNICAL EFFICACY: Stage 2.

RESUMO

Introduction: In the context of an aging population and age-related conditions increasing, the increasing number of middle-aged and older adults are involved in grandchildren care. This study aimed to 1) explore the association between caring for grandchildren based on living arrangements and cognitive function among Chinese middle-aged and older adults; and 2) investigate the mediating roles of social activities and depressive symptoms in the aforementioned association. Methods: This study selected 5490 Chinese people (≥45 years old) from the 2018 China Health and Retirement Longitudinal Study (CHARLS). Participants answered questions related to socio-demographics, the Mini-mental State Examination, the intensity of grandchildren care, the Center for Epidemiological Studies Depression Scale, and social activity. Results: The results showed that caring for grandchildren and cohabiting with a spouse was positively associated with cognitive function among Chinese middle-aged and older adults (B = 0.829, p < 0.001). Furthermore, there was a positive association between providing intensive or no-intensive grandchildren care and cognitive function. In contrast, caring for grandchildren but not cohabiting with a spouse was negatively associated with cognitive function (B = -0.545, p < 0.05). Moreover, directly and indirectly, caring for grandchildren was significantly associated with cognitive function among Chinese middle-aged and older adults, as mediated by social activities and depressive symptoms. Discussion: The findings emphasize that living arrangements, social engagement, and psychological health could be considered when encouraging grandparent care as formal care.

RESUMO

In the current times, the global economies and international organizations declared that pollution is one of the prominent causes of declined human health. Still, most literature is biased toward economic sustainability and ignores such vital issues. The current study tends to identify the factors affecting public health in the Group of Seven economies except for Italy (G6). Specifically, this study aims to investigate the influence of household waste (HHW), bureaucratic quality (BQ), democratic accountability (DA), urbanization growth (URP), GDP per capita, and renewable energy use (EPR) on public health, throughout 1996-2020. This study uses advanced panel data approaches and finds the heterogeneity of slope coefficients, the dependence of cross-sections, and the persistence of cointegration between the variables. The asymmetric distribution of data leads to employing the novel method of moment quantile regression. The estimated results reveal that URP, GDPPC, and EPR significantly increase domestic general government health expenditures, improving public health. However, HHW and BQ adversely affect public health by reducing health expenditures. The robustness of the results is tested via utilizing the panel quantile regression. Based on the empirical findings, this study suggests policies regarding the improvement in public health expenditure, R&D investment, spending in renewable energy sector, and strengthening of the institutional quality.

RESUMO

The disaster and devastation from abuse of Methamphetamine (MAMP) have a serious impact on people's mental and physical health. Developing a rapid and accurate method to screen drug suspects and thus control MAMP abuse is essential to social security. Hair analysis for MAMP detection is considered to be one of the most potential methods for monitoring drug abuse due to its convenient sample collection, easy for storage and long traceability period. However, the current accurate detection of MAMP in hair primarily utilizes hyphenated mass spectrometry (MS) techniques, but it is not suitable for field-based detection due to the bulky instrument. Hence, developing alternative portable detection techniques for rapid on-site detection of MAMP in hair is an urgent problem to be solved. Here, the high-performance Au nanocakes (Au NCs) were constructed as surface-enhanced Raman spectroscopy (SERS) substrates to detect MAMP in hair, realizing 5 min ultrafast and ultrasensitive detection utilizing a portable Raman spectrometer. Experiments and finite-difference time-domain (FDTD) simulations show that Au NCs have stronger enhancement than Au nanospheres (Au NPs), and 0.5 ppb (3.35 × 10-9 M) MAMP standard is stably detected by Au NCs as an enhanced substrate. A strategy of liquid-liquid microextraction was exploited to eliminate the interference of complex matrices in hair. This method exhibited excellent reproducibility and temporal stability across different drug addicts (relative standard deviation was 5.14% within 160 s). Our approach shows great promise in public safety, providing a rapid and accurate method to detect in hair by SERS.
