RESUMO

The growing demand for renewable energy systems is driven by climate change concerns, government support, technological advancements, economic viability, and energy security. These factors combine to create a strong momentum towards a clean and sustainable energy future. Governments, governments, and individuals are increasingly aware of the environmental impacts of traditional energy sources and adopting renewable energy solutions. Hybrid Renewable Energy Systems (HRES) are developed as an effective way of meeting the energy demands in remote locations. The complexity of the system components and the fluctuation of renewable energy sources make it difficult to design an economical and effective HRES. In this study, the Improved Aquila Optimization (IAO) approach has been suggested as a powerful tool to optimize the HRES design. The study addresses the implementation of the IAO approach in the design of HRES and emphasizes its advantages over other optimization techniques. Through extensive simulations and analyses, our findings demonstrate the superior performance of the IAO algorithm in improving the efficiency and cost-effectiveness of HRES. The optimization process using IAO resulted in a significant reduction in overall system costs, achieving an estimated Net Present Cost (NPC) of $201,973. It translates to a cost reduction of 25% compared to conventional optimization techniques. Furthermore, our analysis reveals that the IAO approach enhances the utilization of renewable energy sources, leading to a 15% increase in overall energy generation efficiency. These results highlight the effectiveness of the IAO approach in addressing the challenges associated with designing HRES. By significantly reducing costs and improving efficiency, it facilitates the adoption of sustainable energy systems in remote areas. The outcomes of this study emphasize the importance of utilizing advanced optimization techniques, such as IAO, to ensure the economic viability and environmental sustainability of HRES.

Groundwater contamination risk assessment is an effective tool for groundwater pollution prevention and control. The evaluation system mainly includes three parts:groundwater contamination source load assessment, groundwater vulnerability assessment, and groundwater function value evaluation. Taking the plain area of southern Turpan Basin as an example, based on the survey data and land use data, point source pollution and non-point source pollution were divided to evaluate the load of groundwater pollution sources, the classical DRASTIC model was selected to evaluate the vulnerability of groundwater, and the functional value of groundwater was evaluated from the point of view of water quality and quantity. The three factors were weighted and superimposed via GIS platform to generate the risk zoning map of groundwater contamination. The results showed that the overall risk of groundwater contamination in the study area was low. The area of high-risk and relatively high-risk areas accounted for 15.5% of the total study area, which were mainly distributed in L1, L2, and L3 of the study area. L1 was mainly affected by high pollution source load and high groundwater vulnerability. L2 was mainly the result of the joint action of high groundwater function value and domestic non-point source pollution. Non-point source pollution dominated by agricultural activities and high functional value of groundwater were the main reasons for the high risk of groundwater pollution in the L3 area. The results of the groundwater contamination risk assessment serve as an important reference for decision-makers to delineate the prevention and control area of groundwater pollution.

Potentially toxic elements in soils (SPTEs) from industrial and mining sites (IMSs) often cause public health issues. However, previous studies have either focused on SPTEs in agricultural or urban areas, or in a single or few IMSs. A systematic assessment of the pollution and risk levels of SPTEs from IMS at the national scale is lacking. Here, we obtained SPTE (As, Cd, Cr, Cu, Hg, Ni, Pb, and Zn) concentrations from IMSs across China based on 188 peer-reviewed articles published between 2004 and 2022 and quantified their pollution and risk levels using the pollution index and risk assessment model, respectively. The results indicated that the average concentrations of the eight SPTEs were 4.42-270.50 times the corresponding background values, and 19.58% of As, 14.39% of Zn, 12.79% of Pb, and 8.03% of Cd exceeded the corresponding soil risk screening values in these IMSs. In addition, 27.13% of the examined IMS had one or more SPTE pollution, mainly distributed in the southwest and south central China. On the examined IMSs, 81.91% had moderate or severe ecological risks, which were mainly caused by Cd, Hg, As, and Pb; 23.40% showed non-carcinogenic risk and 11.70% demonstrated carcinogenic risk. The primary exposure pathways of the former were ingestion and inhalation, while that for the latter was ingestion. A Monte Carlo simulation also confirmed the health risk assessment results. As, Cd, Hg, and Pb were identified as priority control SPTEs, and Hunan, Guangxi, Guangdong, Yunnan, and Guizhou were selected as the key control provinces. Our results provide valuable information for public health and soil environment management in China.

OBJECTIVES: To explore the diagnostic potential of texture analysis applied to native T1 maps obtained from cardiac magnetic resonance (CMR) images for the assessment of heart failure with preserved ejection fraction (HFpEF) among patients with end-stage renal disease (ESRD). METHODS: This study, conducted from June 2018 to November 2020, included 119 patients (35 on hemodialysis, 55 on peritoneal dialysis, and 29 with kidney transplants) in Renji Hospital. Native T1 maps were assessed with texture analysis, using a freely available software package, in participants who underwent cardiac MRI at 3.0 T. Four texture features, selected by dimension reduction specific to the diagnosis of HFpEF, were analyzed. Multivariate logistic regression was performed to examine the independent association between the selected features and HFpEF in ESRD patients. RESULTS: Seventy-six of 119 patients were diagnosed with HFpEF. Demographic, laboratory, cardiac MRI, and echocardiogram characteristics were compared between HFpEF and non-HFpEF groups. The four texture features that were analyzed showed statistically significant differences between groups. In multivariate analysis, age, left atrial volume index (LAVI), and sum average 4 (SA4) turned out to be independent predictors for HFpEF in ESRD patients. Combining the texture feature, SA4, with typical predictive factors resulted in higher C-index (0.923 vs. 0.898, p = 0.045) and a sensitivity and specificity of 79.2% and 95.2%, respectively. CONCLUSIONS: Texture analysis of T1 maps adds diagnostic value to typical clinical parameters for the assessment of heart failure with preserved ejection fraction in patients with end-stage renal disease. KEY POINTS: • Non-invasive assessment of HFpEF can help predict prognosis in ESRD patients and help them take timely preventative measures. • Texture analysis of native T1 maps adds diagnostic value to the typical clinical parameters for the assessment of HFpEF in patients with ESRD.

China's carbon emissions have developed swiftly in recent decades, which will not only affect the nation's own sustainable development, but have a potentially negative impact on global climate stability. Given that socioeconomic development is susceptible to regional heterogeneity and geographic scales, a systematic exploration of spatiotemporal variations of carbon emission intensity (CEI) and their drivers across different levels is conducive to enacting more reasonable and efficient measures for emission reduction. However, there is still a lack of comprehensive analysis of these issues. In this paper, we attempted to quantify and compare the spatiotemporal evolution and spatial spillover effects of impact factors on CEI from nighttime light imagery and socioeconomic data at two China's administrative levels by utilizing the variation coefficient, spatial autocorrelation model and spatial econometric methods. The results showed that the spatiotemporal variations of CEI were greater at the prefecture level compared to the provincial level during 2000-2017. There were significant positive spatial autocorrelation of CEI at two administrative levels, and self-reinforcing agglomeration was more substantial at the prefectural level than that provincial level. While the local spatial clustering of CEI of each administrative level altered with scale dependence, the binary spatial structure (High-High and Low-Low) of CEI remained relatively steady in China. Various driver factors not only had direct effects on local CEI, but had spatial spillover effects on neighboring areas. Our findings illustrate that China's CEI is sensitive to the space-time hierarchy of multi-mechanisms, and suggest that "proceed in the light of local conditions" strategies can assist the Chinese government for CEI mitigation.

OBJECTIVE: This study aimed to investigate the oral health service needs and dental treatment among 3-year-old to 5-year-old preschool children in Jiangxi province and provide data to support the development of relevant oral health policies, enhance the oral health service utilization and visit rate, and optimize oral manpower and service resources. METHODS: A total of 2 880 preschool children were selected via stratified cluster sampling. Dental examination methods and criteria according to the fourth national oral health epidemiological survey were adopted. Excel 2007 and SPSS 19.0 softwares were used for statistics analysis. Chi-square test and logistic regression analysis were performed. RESULTS: The caries prevalence was 49.13% (1 415/2 880) among the 2 880 cases of 3-
year-old to 5-year-old preschool children in Jiangxi province, approximately 53.37% (1 537/2 880) of which possessed oral health service needs. The parents of younger children assessed the oral situation better in older age than in their younger years (P<0.05), which was better in urban children than in rural children (P<0.05). The visiting rate of all the respondents was 12.33% (355/2 880), while the visiting rate of children with oral diseases was 17.31% (266/1 537). The top three reasons why patients did not visit the dentist were as follows: children's teeth have no problem, the teeth damage in teeth was considerably minimal, and primary teeth would be replaced without the need for treatment. The influencing factors included lower visiting rate in females than in males (OR=0.499, 95%CI: 0.411-0.606) and lower visiting rates in rural areas than in urban areas (OR=0.428, 95%CI: 0.353-0.519). Subjects with poor oral health and high oral knowledge scores possessed high dental service visit rates. CONCLUSIONS: The prevalence of caries in 3-year-old to 5-year-old preschool children in Jiangxi province is at a lower level than that in the national level. The proportion of the population actively seeking medical services is low, and their main purpose of treatment is to treat rather than prevention. Oral health education should be strengthened, and it should be incorporated to the contents of the kindergarten teacher training program to improve residents' awareness regarding children's oral health.

Many patients with cancer are diagnosed through an emergency presentation, which is associated with inferior clinical and patient-reported outcomes compared with those of patients who are diagnosed electively or through screening. Reducing the proportion of patients with cancer who are diagnosed as emergencies is, therefore, desirable; however, the optimal means of achieving this aim are uncertain owing to the involvement of different tumour, patient and health-care factors, often in combination. Most relevant evidence relates to patients with colorectal or lung cancer in a few economically developed countries, and defines emergency presentations contextually (that is, whether patients presented to emergency health-care services and/or received emergency treatment shortly before their diagnosis) as opposed to clinically (whether patients presented with life-threatening manifestations of their cancer). Consistent inequalities in the risk of emergency presentations by patient characteristics and cancer type have been described, but limited evidence is available on whether, and how, such presentations can be prevented. Evidence on patients' symptoms and health-care use before presentation as an emergency is sparse. In this Review, we describe the extent, causes and implications of a diagnosis of cancer following an emergency presentation, and provide recommendations for public health and health-care interventions, and research efforts aimed at addressing this under-researched aspect of cancer diagnosis.

BACKGROUND: H7N9 continues to cause human infections and remains a pandemic concern. Understanding the economic impacts of this novel disease is important for making decisions on health resource allocation, including infectious disease prevention and control investment. However, there are limited data on such impacts. METHODS: Hospitalized laboratory-confirmed H7N9 patients or their families in Jiangsu Province of China were interviewed. Patients' direct medical costs of hospitalization were derived from their hospital bills. A generalized linear model was employed to estimate the mean direct medical costs of patients with different characteristics. RESULTS: The mean direct cost of hospitalization for H7N9 was estimated to be ¥ 71 060 (95 % CI, 48 180-104 820), i.e., US$ 10 996 (95 % CI, 7 455-16 220), and was ¥12 060 (US$ 1 861), ¥136 120 (US$ 21 001) and ¥218 610 (US$ 33 728) for those who had mild or severe symptoms or who died, respectively. The principal components of the total fees differed among patients with different disease severity, although medication fees were always the largest contributors. Disease severity, proportion of reimbursement and family member monthly average income were identified as the key factors that contributed to a patient's direct medical cost of hospitalization. CONCLUSIONS: The direct medical costs of hospitalized patients with H7N9 are significant, and far surpass the annual per capita income of Jiangsu Province, China. The influencing factors identified should be taken into account when developing related health insurance policies and making health resource allocation. TRIAL REGISTRATION: Not applicable. This is a survey study with no health care intervention implemented on human participants.

BACKGROUND: Truancy has been linked to risky sexual behaviours in teenagers. However, no studies in England have examined the association between truancy and teenage pregnancy, and the use of truancy as a marker of teenagers at risk of pregnancy. METHODS: Using logistic regression, we investigated the association between truancy at age 15 and the likelihood of teenage pregnancy by age 19 among 3837 female teenagers who participated in the Longitudinal Study of Young People of England. We calculated the areas under the ROC curves of four models to determine how useful truancy would be as a marker of future teenage pregnancy. RESULTS: Truancy showed a dose-response association with teenage pregnancy after adjusting for ethnicity, educational intentions at age 16, parental socioeconomic status and family composition ('several days at a time' versus 'none', odds ratio 3.48 95% confidence interval 1.90-6.36, P < 0.001). Inclusion of risk behaviours improved the accuracy of predictive models only marginally (area under the ROC curve 0.76 full model versus 0.71 sociodemographic characteristics only). CONCLUSIONS: Truancy is independently associated with teenage pregnancy among English adolescent girls. However, the discriminatory powers of models were low, suggesting that interventions addressing the whole population, rather than targeting high-risk individuals, might be more effective in reducing teenage pregnancy rates.
