PURPOSE: To introduce a user-friendly protocol for implant-retained prosthetic rehabilitation of fully edentulous patients based on facial profile and the cephalometric intermaxillary relationship. MATERIALS AND METHODS: The prosthetic rehabilitation of edentulous patients with implants based on facial profile assessment protocol commences with a clinical and cephalometric analysis, followed by a preliminary projection to obtain a harmonious interarch relationship, eventually stabilized by the preplanned prosthesis. The protocol is divided into the following 5 phases: esthetic evaluation; cephalometric-occlusal assessment; surgery; provisionalization; and definitive prosthesis. Evaluation of patient's satisfaction from the treatment provided was performed through a preoperative and postoperative visual analogue scale scores. RESULTS: The patient was granted immediate function and superior esthetics at day 1 of the treatment. These results were re-evaluated on an interval of 18 months and continued to be stable. The overall treatment time was significantly reduced. CONCLUSION: Our recommended protocol gives the clinician a chance to grasp results beyond the offered by traditional protocols by using a wider perspective of rehabilitation, involving the patient's facial profile, thus fulfilling the integration of both occlusal and cephalometric parameters in a unified surgical-prosthetic approach.

PURPOSE: Edentulous patients benefit significantly from implant-supported overdenture prostheses. The purpose of this systematic review was to evaluate the cost-effectiveness of implant-supported overdentures (IODs) for edentulous patients. MATERIALS AND METHODS: The search was limited to studies written in English and included an electronic and manual search through MEDLINE (Ovid, 1946 to November 2015), Embase (Ovid, 1966 to November 2015), Cochrane Central Register of Controlled Trials (CENTRAL) (to November 2015), and PubMed (to November 2015). Two investigators extracted the data and assessed the studies independently. No meta-analysis was conducted due to the high heterogeneity within the literature. RESULTS: Of the initial 583 selected articles, 10 studies involving 802 participants were included. Of these, 6 studies had a high risk of bias and the rest had an unclear risk of bias. Implant-supported prostheses were more cost-effective when compared to conventional dentures and fixed implant-supported prostheses. Overdentures supported by two implants and magnet attachment were reported as cost-effective. CONCLUSION: Implant-supported overdentures are a cost-effective treatment for edentulous patients. More clinical studies with appropriate scientific vigor are required to further assess the cost-effectiveness of implant-supported overdentures.

PURPOSE: The aim of this study was to correlate the location of the mental foramen and overlying teeth and apply these observations to posterior prosthetic teeth selection in a group of Egyptian edentulous patients. MATERIALS AND METHODS: Retrospective cone beam computed tomography data from 40 dentate and 40 edentulous patients scanned with radiopaque denture duplicates were compared in relation to mental foramen position. RESULTS: The probability of locating the mental foramen opposite the first premolar was higher in dentate patients, while finding it opposite the first molar was higher in edentulous patients. CONCLUSION: Selected posterior teeth size for the selected Egyptian edentulous patients might be slightly smaller than for dentate patients.

BACKGROUND: Some relation exists between oral and general health with progressive aging. Certain risk factors are common between oral and systemic diseases. Absence of teeth also affects the oral health by altering the quality of life. Hence, the nutritional changes occurring in elderly patients following prosthodontic rehabilitation are evaluated. MATERIALS AND METHODS: A total of 250 patients who underwent prosthodontic treatment for missing teeth were included for the study. Twice measurement of nutritional parameters was done: Initially at the time of diagnosis and then 5 months following commencement of the prosthodontic treatment. Dental analysis, evaluation of the diet, anthropometric assessment, and analysis of serum biochemical values were done in all the patients and tabulated records were maintained. Independent Student's t test and Tukey's test were done to assess the level of significance. RESULTS: A total of 250 patients were included for the study. The complete denture (CD) group showed the highest alteration in the mean values of the nutritional parameters followed by the removable partial denture group. A significant change was seen in the body mass index, protein, carbohydrate, and iron levels among the different patients who were grouped based on the mode of treatment modality. The CD group showed significantly higher mean change in carbohydrates value compared with mean change in patients receiving fixed treatment. CONCLUSION: Both nutrition and diet form an integral part of the prosthodontic treatment to maintain the health of elderly population. CLINICAL SIGNIFICANCE: With the advancement in the level of edentulism, rehabilitation by prosthetic treatment has become progressively important to restore and improve dietary parameters.

BACKGROUND: Full fixed prosthesis on a limited number of implants (FFP) are a viable treatment option for edentulous patients with a reduced amount of residual bone. Jaw muscular function in FFP patients has been evaluated in several studies, however heterogeneous data emerge from literature. PURPOSE: The aim of this review of the literature was to assess the function of jaw muscles in edentulous patients restored with full fixed prostheses on a limited number (≤ 6) of implants, as compared to dentate subjects and edentulous subjects wearing dentures, implant-supported overdentures or full fixed prostheses supported by more than six implants. MATERIALS AND METHODS: An electronic search of databases up to December 2013 was performed. The articles were selected using specific inclusion criteria, independent of the study design. RESULTS: A total of 1598 records were identified. After removing the duplicates and excluding records based on title and abstract, only 37 eligible records were identified. After full-text review, seventeen studies were selected for analysis according to the inclusion criteria. From the included studies, only one evaluated masseter muscle thickness in a cross sectional study by means of ultrasound, while the 16 remaining papers evaluated muscular function by using electromyography (EMG). Those studies analysed several heterogeneous parameters throughout the execution of five functional tests and were therefore described and pooled according to the following task categories: clenching; swallowing; reflex and fatigue for statics; and chewing for dynamics. CONCLUSIONS: The results of selected studies seem to indicate that, compared to dentate controls, FFP patients display a global satisfactory neuromuscular equilibrium in static activities, but still have some impairment during chewing.

OBJECTIVE: The objective is to compare, in a prospective randomized clinical trial, three types of attachment systems for mandibular implant overdenture, focusing on costs, maintenance requirements and complications from baseline to the end of 5-year follow-up period. MATERIALS AND METHODS: Sixty-nine fully mandibular and fully/partially maxillary edentulous patients received two screw-type Straumann implants, in the mandibular canine region. New overdentures with three types of attachment systems were inserted according to an early-loading protocol: Group B (balls, divided into Subgroup B.1 - retentive anchor with gold matrix and Subgroup B.2 - retentive anchor with titanium matrix) (n = 23), Group M (magnets) (n = 23) and Group L (locator) (n = 23). RESULTS: The highest maintenance event number (195) was observed in Group B vs. 31 in Group L and 15 in Group M. Significantly more complications were recorded in Subgroup B.1 than in Subgroup B.2, Group M and Group L (P < 0.05). Group M registered the highest prosthetic success (82.6%) in the 5 years, followed by Group L (78.2%). Subgroup B.1 had the lowest success rate (50%). The magnet group recorded statistically significant higher costs, comparing with the other two groups (P < 0.05). CONCLUSIONS: The three attachment systems functioned well after 5 years. The magnets had a low maintenance requirement and high success rate, despite the relatively increased initial costs. Retentive anchor with titanium matrix and locator may be a better choice from a financial point of view, taking into consideration the initial low cost of the components and also the reduced number of complications.

The aim of this study was to investigate the production of mandibular implant-supported fixed and removable prostheses among prosthodontic specialists in Sweden and to compare the results with findings in a similar study made in 2001 (17). Questionnaires regarding treatment with mandibular implant prostheses during 20011 were mailed to all specialists in prosthodontics in Sweden (n = 156, according to available data). Of the 156 questionnaires, 129 (83%) were returned and of those 114 were completed. The reported number of treatments with mandibular implant-supported prostheses varied much among the specialists. Fixed implant prostheses were more common than overdentures (means 11 and 3, median values 8 and 2, respectively). However, the range was large for both alternatives. Ten (9%) of the specialists reported no treatment with fixed implant prostheses while 29 (25%) had not made any implant overdenture during 2011. The most common anchorage system for overdentures in 2011 (as well as in 2001) was two un-splinted implants with ball attachments or Locator abutments. The most common reasons for choosing overdenture treatment instead of a fixed implant prosthesis in 2011 were the reduced cost and the patient's main wish to improve denture retention. A majority of the prosthodontists (58%) reported that patients with implant overdentures were as satisfied as those with fixed implant-supported prostheses, whereas 40% claimed they were less satisfied. Two respondents (2%) considered that overdenture patients were more satisfied than those with a fixed prosthesis. It can be concluded that the general attitude among Swedish prosthodontists towards implant overdentures has not changed much during the 10-year period between the present and the previous investigation.An overdenture is still a seldom-used option in implant treatment of patients with edentulous mandibles in Sweden. Instead, a fixed implant-supported prosthesis continues to be the preferred option,

PURPOSE: Conventionally, in patients with completely edentulous mandibles, fixed prostheses have been supported by four or more implants. However, an alternative protocol employing three implants and immediate loading has been developed. The objective of the present study was to assess the rehabilitation of edentulous patients treated with a complete fixed mandibular prosthesis with immediate loading and supported by three implants. MATERIALS AND METHODS: In this observational study, a total of 99 implants placed in 33 patients was evaluated after 18 months in use. Panoramic radiographs were digitized to measure bone loss at the mesial and distal surfaces of each implant. RESULTS: Mean peri-implant bone loss was 0.66 ± 0.51 mm for the left implant, 0.92 ± 0.61 mm for the central implant, and 0.82 ± 0.53 mm for the right implant. The bone loss observed around the distal implants was similar to that seen around the central implant, and there were no statistically significant differences in peri-implant bone loss between the three implant locations. There was no significant correlation between implant length and bone loss. The results were compatible with the peri-implant bone loss that has been described for prostheses of the same type supported by larger numbers of implants. CONCLUSIONS: The use of a complete fixed mandibular prosthesis with immediate loading supported by three implants proved to be an adequate option for the rehabilitation of edentulous patients. The protocol allowed simplification of treatment while maintaining similar bone loss to that reported for the same type of treatment supported by a larger number of implants.

RESUMO

When treating an edentulous patient with complaints of unsatisfactory retention of his complete mandibular denture, various treatment options are available: a new conventional denture, an implant-supported overdenture, and an implant-supported fixed partial denture. Important considerations during the decision-making process are patient satisfaction, cost of denture fabrication, and cost of maintenance. The outcome of the diagnostic procedure will have financial consequences. On that basis, a satisfactory treatment plan can be established and carried out. The need exists for a protocol which takes into account cost-effectiveness and is useful in selecting the appropriate treatment option, adjusted to individual requirements.

Patient well-being is always the goal of rehabilitation of edentulism; however, evaluations of treatment success often overlook the patient's subjective feelings about comfort, function, speech, social image, social inhibitions, psychological discomfort, and/or disabilities. The purpose of this study was to assess these patient responses using an oral health questionnaire. To assess such feelings, a self-administered 20-question multiple-choice patient-reported Edentulous Patient Impact Questionnaire was developed, based upon the previously validated Oral Health Impact Profile patient-assessment tool. Responses were solicited from randomly selected patients treated with an implant-supported, fixed, immediately loaded full arch prosthesis. The questionnaires were completed by 250 patients. Of the respondents, 95% described themselves as being either extremely satisfied (74%) or satisfied (21%) with their new teeth, and 98% said they would definitely recommend similar treatment (88%) or consider recommending it (10%) to a friend or colleague. Based upon an oral health impact survey completed by 250 patients treated with full-arch implant-supported, immediately loaded fixed dental prostheses, it appears that patient satisfaction is high and that treated patients would generally be willing to recommend this treatment to others.

The objective was to evaluate the development of oral health-related quality of life (OHRQoL) in patients with complete dentures and the association between OHRQoL and patient satisfaction. Fifty-two patients (mean age, 66.3, 48.1% male) received dentures in at least one jaw. The analysis was conducted on participants with dentures in both jaws (CD-Both; n = 22) or in the upper jaw only (CD-Max; n = 28). Data were collected 4 weeks, 6 months, and 1 and 2 years after insertion. OHRQoL was measured by use of the OHIP-EDENT. Self-rated patient satisfaction was assessed on a scale of 0-10. To prove the hypothesis that patient satisfaction would be a meaningful predictor of OHRQoL, and not vice versa, multilevel analysis and cross-lagged correlation analysis were performed for both groups separately. OHRQoL improved from 22.9 (SD, 20.7) to 12.1 (SD, 14.5) for CD-Both and from 20.3 (SD, 17.2) to 14.7 (SD, 15.1) for CD-Max. Multilevel analysis revealed that patient satisfaction and OHRQoL were significantly associated (p < 0.0001) for both groups. Differences between the groups were found with regard to the effect of time after insertion and the interaction between time and satisfaction with OHRQoL which were significant only for the group CD-Both; however, no evidence was found for the causality of this association in the cross-lagged analysis for both groups (ZPF test, p > 0.016). Patient satisfaction and OHRQoL were associated for wearers of complete dentures. Within the limitations of the study, however, the causality that patient satisfaction predicts OHRQoL, and not vice versa, could not be proven.

The McGill consensus statement on overdentures (14) was published following a symposium held at McGill University in Montreal, Canada in 2002. A panel of relevant experts in the field stated that: The evidence currently available suggests that the restoration of the edentulous mandible with a conventional denture is no longer the most appropriate first choice prosthodontic treatment. There is now overwhelming evidence that a two-implant overdenture should become the first choice of treatment for the edentulous mandible (14). In 2009, a further consensus statement was released as a support and follow-up to the McGill consensus statement. This report was jointly created by members of the BSSPD (British Society for the Study of Prosthetic Dentistry) Council and the panel of presenters at the BSSPD conference in York, UK in April 2009 (15). This report also highlighted that since the McGill statement in 2002, uptake by dentists of implant technology for complete denture wearers has been slow. The York statement concluded that 'a substantial body of evidence is now available demonstrating that patients' satisfaction and quality of life with ISOD mandibular overdentures is significantly greater than for conventional dentures. Much of this data comes from randomised controlled trials (15). Whilst it is accepted that the two-implant overdenture is not the gold standard of implant therapy it is the minimum standard that should be sufficient for most people, taking into account performance, patient satisfaction, cost and clinical time.

RESUMO

Resorption of the alveolar ridge is a common problem in edentulous patients and can compromise the stability and function of dentures. Resorption and its consequences can be minimized when strategically placed implants are used; however, this option is financially out of reach for many patients. The article discusses a more cost-effective alternative (metalbased dentures) for patients with ridge resorption. In certain environments, like a dental school, where patients are looking for solutions to their dental problems at a reasonable price, cast metal bases can be a feasible economical alternative for edentulous patients. Both cases presented here demonstrated a significant improvement in stability, phonation, and mastication.

RESUMO

Immediate load protocols for the edentulous mandible offer the patient many advantages in terms of decreased number of visits, improved early function, and reduction of surgical exposure; however, this treatment modality is not universally appropriate for all patients. The available evidence will assist the clinician in developing a customized and comprehensive informed consent. Patient selection and patient-mediated factors will dictate the suitability of not only a fixed or removable prosthesis, but also whether immediate loading would enhance the cost/benefit ratio. The indications, objective and subjective patient considerations, and design strategies are discussed for the immediate load scenario.

PURPOSE: To assess and compare patient satisfaction and oral health-related quality of life (OHQOL) in patients treated with implant-supported overdentures and complete implant fixed prostheses. MATERIALS AND METHODS: From a database of patients who had undergone implant treatment over a 6-year period, a study population of 62 patients was identified; every patient had at least four implants placed in one edentulous arch and was restored with either an overdenture or a fixed prosthesis. Patients were examined and a self-administered, structured multiple-response questionnaire, including the Oral Health Impact Profile-14 measurement tool and a patient satisfaction survey, was used to evaluate patient-centered treatment outcomes. RESULTS: Generally, patient satisfaction was very high in both the implant overdenture and fixed prosthesis groups, although the subjects in the overdenture group, who had mostly maxillary prostheses, reported significantly lower overall satisfaction and lower satisfaction with chewing capacity and esthetics. In just three categories--cost, satisfaction with treating doctor, and ability to perform oral hygiene measures--the fixed prosthesis group was less satisfied than the removable overdenture group, but the difference was not significant. Similarly, the overall OHQOL was high, although patients receiving a fixed prosthesis demonstrated significantly lower psychologic discomfort and psychological disability compared to the overdenture group. CONCLUSIONS: Among all patients who had similar numbers of implants placed, those who received an implant overdenture were less satisfied and had lower OHQOL than the patients who had a fixed prosthesis. Since patient and dentist preferences influenced the type of prosthesis provided, it is likely that subjective, patient-related factors are major determinants of satisfaction and treatment outcomes.

PURPOSE: The purpose of this retrospective study was to review and assess the outcomes of subjects treated with dental implants at the University of Washington Graduate Prosthodontic Program from 1988 to 2000. MATERIALS AND METHODS: A retrospective review and an outcomes assessment were conducted of patients treated with osseointegrated implants between 1988 and 2000 at the University of Washington Graduate Prosthodontic Program. Patients were treated by graduate students. All included patients had a prosthesis that had been in service for at least 5 years. Implants and prostheses were assessed by chart review, phone survey, and clinical review. Clinical review consisted of mobility testing, soft tissue evaluation, and occlusal evaluation. Specific success criteria were used and life table analyses of implants and prostheses were performed. RESULTS: Of a total of 114 patients treated, 69 subjects (60.5% of those treated) participated in the assessment; 103 prostheses supported by 273 implants were reviewed clinically. The estimated cumulative survival rate was 96.3% for all implants and 85.4% for all prostheses. CONCLUSIONS: A retrospective review of 69 subjects who were treated in the University of Washington Graduate Prosthodontic Program between the years 1988 and 2000 showed an implant survival rate of 96.3% and a prosthesis survival rate of 85.4%. Prostheses showed higher failure rates than implants.

RESUMO

The Annual Conference of the BSSPD (British Society for the Study of Prosthetic Dentistry) was held in York on 6 and 7 April 2009. At the symposium on mandibular overdentures, presenters offered a synopsis of the research available on the efficacy of implant-supported mandibular overdentures in the edentulous mandible. Emphasis was given to both qualitative and quantitative research based on patient-centred outcomes of treatment. A draft consensus was circulated to all presenters and to the Council members of the BSSPD and to BSSPD members on the Society's website. The statement was modified in the light of their comments, audience feedback following the presentations and members' feedback. We hope that this consensus statement will be a useful guide for patients and clinicians and that it will act to stimulate wider debate. We also hope that it will prove useful to other patient and professional organisations and will inform discussions with providers of national healthcare and with independent funders.

PURPOSE: This randomized clinical trial tested hypotheses that there are no differences in patient satisfaction, component costs, or treatment and maintenance times when mandibular overdentures are retained by one or two implants. MATERIALS AND METHODS: Subjects wearing conventional complete dentures were randomized to receive either one midline or two bilateral mandibular implants followed by a mandibular denture reline to incorporate implant retention. They indicated on a visual analog scale satisfaction with their dentures before implants and at 2 months and 1 year after implant retention. Satisfaction outcomes between the two groups were compared using the Wilcoxon/Mann-Whitney nonparametric rank test, while changes within each group were analyzed using signed-rank tests. Component costs and times for surgery, prosthodontic treatment, and maintenance were compared using nonparametric and t tests. RESULTS: Eighty-six subjects enrolled in this study and 85 completed the 1-year follow-up, at which median satisfaction was 93 (maximum 100) in the single-implant group and 94 in the two-implant group (P > .5). Within each group, median improvement in satisfaction was similarly dramatic (approximately 44) and significant (P < .001). Prosthodontic maintenance time was similar for both groups (P > .37), but the single-implant group had significantly lower component costs (P < .001) and lower times for surgery (P = .002), postsurgical denture maintenance (P = .021), and denture reline (P < .001). Five implants failed in four subjects, all in the two-implant group and all before denture reline. CONCLUSION: Lower component costs and treatment times, with comparable satisfaction and maintenance time over the first year, indicate that a mandibular overdenture retained by a single midline implant may be an alternative to the customary two-implant overdenture for maladaptive denture patients.
