RESUMO

The aim of our study was multi-faceted study of status the psychological of young people suffering from bruxism. A total of 101 people (44 men and 57 women) divided into 4 groups. The study of mental states and personality traits included the Freiburg Personality Inventory (FPI). The majority of subjects characterized by high reactivity and emotional lability, the surveyed with day and combined bruxism is more pronounced symptoms of irritability, tension, and increased excitability. The best approach when examining young patients with bruxism should be based on accounting data and anamnestic examination not only dentists, but also psychologists and psychotherapists to identify the "background" of mental states and personality traits and correction of sharp variations by special methods.

A symposium was held in Toronto, 2008, in which research progress regarding the biobehavioural dimension of the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was presented. An extended workshop was held in April 2009 in which further recommendations were made from an expert panel, using the 2008 symposium material as a base. This paper is a summary of the 2008 symposium proceedings with elaborations based on further developments. Seven studies were conducted between 2001 and 2008, in which the following were investigated: (i) basic properties of Axis II instruments, (ii) reliability and criterion validity of Axis II instruments, (iii) expansion of predictors, (iv) metric equivalence of the depression and non-specific physical symptoms subscales in the RDC/TMD, (v) laboratory investigation of oral behaviours, (vi) field data collection of oral behaviours, and (vii) functional limitation of the jaw. Methods and results for each of these studies are described. Based on the results of these studies that have been published, as well as the direction of interim results from the few studies that await completion and publication, the biobehavioural domain of the RDC/TMD, as published in 1992, is reliable and valid. These results also provide strong evidence supporting the future growth of the biobehavioural domain as the RDC/TMD matures into subsequent protocols for both clinical and research applications.

The aim of the study was to assess the structure of perception in people with bruxism. 50 people with a diagnosis of bruxism (B), in whom primary evidence of oclusal disharmony and temporomandibular disorders were excluded, were included in the study. The control group (C) consisted of 50 people with no evidence of parafunctional abnormalities of the chewing mechanism. Shalit's diagram was used to assess the structure of perception. The primary indices: discriminability (D), articulation (A), affective load+ (AL+) and affective load- (AL-) and secondary indices: reduction (R), emotionality (E), intensity (I) were derived from Shalit's diagram. A significant reduction in the mean value of the primary indices D and AL+ was noted, as well as in the secondary indices E and I in people with bruxism, in comparison to the control group. Assessment of the structure of perception in people with bruxism through the analysis of the primary and secondary indices, makes it possible to predict behavior which has a negative effect on the individual's own health and provides the opportunity for changing such behavior to be more pro-health oriented.

The authors review the most updated information concerning the etiopathogenesis of nocturnal bruxism, stressing the co. destructive role of psychosomatic factors and of traumatogenic ones, of the type of premature dental contacts, and of occluding interferences, with evident direct implications in the buccal cavity and at a distance, giving rise to hypnic masticatory automatism, with painful hypercontraction of the masticatory muscles, and stress exerted on the temporomandibular articulations. On the basis of original studies of the interdisciplinary type (prosthetic gnathology and neurophysiology), with the aid of polygraphic investigations during sleep carried out in subjects with nocturnal bruxism it was possible to demonstrate that this oral parafunction may determine sleep disturbances at a central level which makes necessary a complex stomatologic and neuropsychiatric medical therapy.
