RESUMO

OBJECTIVE: To compare the permeability of the enamel of primary teeth from individuals free of Early Childhood Caries (ECC) with that from individuals affected with ECC by assessment of dye penetration using Laser Scanning Confocal Microscope (LSCM). STUDY DESIGN: Experimental in vitro study. Exfoliated primary maxillary anterior teeth (n = 44) were collected and divided into two groups (n=22 per group): samples with ECC (Group 1) and without ECC (Group 2). The samples were immersed in Rhodamine B dye solution for 1 day, cut longitudinally into 3 sections, observed using LSCM. Dye penetration depths in the incisal, middle, cervical thirds and on labial, lingual surfaces were recorded. Data were analyzed by the Mann-Whitney test (α = 5%, p < .005). RESULTS: The overall mean penetration depth for group 1 (100.6 µm ± 58.48 µm) was significantly higher than that of group 2 (31.55 µm ± 23.40 µm, p < .000). Mean penetration depth in the incisal, middle, and cervical thirds and on the labial and lingual surfaces of group 1 also presented significantly higher scores than in group 2 (p < .005). CONCLUSION: There was significantly more dye penetration in the ECC group than in the non-ECC group. This could be related to a higher level of enamel permeability in teeth affected with ECC.

BACKGROUND: Over the years, certain primary teeth have been shown to be highly sensitive to dental caries, while others have remained caries-free. It has been hypothesized that this may be attributed to differences in the permeability of the enamel surface. AIM: The aim of the study was to evaluate the hypothesized differences in the permeability of primary tooth enamel in children with and those without Severe Early Childhood Caries (S-ECC) using scanning electron microscopy. MATERIALS AND METHODS: Sixteen children between 3 and 6 years of age were randomly selected and divided into two groups: Group 1, children without S-ECC (n = 8), and Group 2, children with S-ECC (n = 8). In each child, 4 teeth (the maxillary right and left central and lateral incisors) were subjected to evaluation. An impression was made with polyvinylsiloxane impression material, and scanning electron microscopy was used to inspect the negative replicas for droplets. RESULTS: The results indicated higher significance when individual regions (cervical, middle, and incisal thirds) in the two groups were evaluated and compared. Similarly, the overall results showed high statistical significance between S-ECC and non-S-ECC teeth. CONCLUSION: There could be a positive relationship between the permeability of tooth enamel and the development of caries, which needs further research.

Establishing reproducible methodologies for assessment of early enamel lesions using optical coherence tomography (OCT) appears to be challenging. This in vitro study longitudinally evaluated the subsurface enamel lesion progression after 3, 9 and 15 days by cross-sectional scanning using 1310 nm centered swept-source OCT (SS-OCT) under hydrated and dry conditions. The positive difference between the depth-integrated OCT signals at dry and hydrated conditions were calculated and adopted as dehydration parameter (DH). A linear regression was found between DH and the square root of demineralization time (R(2) = 0.99). Significant differences were found in DH between sound and demineralized enamel, and between different periods of demineralization (p < 0.001). Hydration state affects the reflectivity of demineralized porous enamel, and the effect can be potentially used for assessment of early enamel lesion using OCT.

AIMS: To investigate the relationship between caries risk and glucosylated haemoglobin and to evaluate the use of a computerized caries risk assessment program as a predictor of metabolic control in schoolchildren with Type 1 diabetes mellitus. METHODS: The material consisted of 64 young Type 1 diabetes mellitus patients (8-16 years) attending an outpatient paediatric clinic. Within 2 weeks after the onset of diabetes, a caries risk assessment was carried out with the aid of a computer-based program (Cariogram) and data on the level of metabolic control were collected from the medical records at the 3-year check-up. Caries increment was registered at recall clinical examinations. RESULTS: A statistically significant positive relationship between caries risk and metabolic control was found (r = 0.51; P < 0.01), with a sevenfold increased risk of impaired metabolic control after 3 years in those assessed with high caries risk at onset (OR 7.3; P < 0.01). When the Cariogram was used as a predictor for the metabolic state of the disease, the sensitivity and specificity was 75% and 71%, respectively. The negative predictive value was 91%. CONCLUSIONS: Our data suggested that a caries risk assessment at the diagnosis of diabetes mellitus in children may be a good indicator of overall health care that can provide useful prognostic information on the level of metabolic control after 3 years.

OBJECTIVES: It was the purpose of this study to investigate the effect of milk and fluoridated milk on experimentally induced caries-like-lesions. MATERIALS AND METHODS: Twelve extracted impacted human molars were covered with wax leaving two 3 mm x 3 mm windows on the buccal and lingual surfaces and incubated alternating in demineralizing solution at pH 4.85 and in milk, F-milk, saline, and remineralizing solution, respectively. Afterwards serial ground sections were cut and investigated by polarization-light-microscopy (PLM) and SEM using EDX element analysis. RESULTS: The results showed increased thickness of the superficial layer in the F-milk samples. Quantitative element analysis revealed a significant increase in the fluoride content of the superficial layer and of the body of the lesion of the F-milk group. The body of the lesion of the F-milk group was less demineralized than in the other groups. CONCLUSION: From the results we conclude, that the parallel investigation by PLM, SEM combined with EDX quantitative element analysis is a powerful method to assess caries-like-lesion formation and that fluoridated milk has protective properties in inhibiting demineralization.

Recent studies show that methacrylate-based composites with amorphous calcium phosphate (ACP) as a filler can release supersaturating levels of calcium and phosphate ions in proportions favorable for apatite formation. These findings suggest that such composites could be effectively used as coatings for remineralizing teeth damaged by tooth decay. To examine this hypothesis, we tested composites in vitro for their efficacy to remineralize artificially formed caries-like lesions in extracted bovine incisors. Single 120-microns-thick sagittal tooth sections were placed in holders that exposed only the carious enamel surface. The exposed surfaces were coated with a 1-mm- to 1.5-mm-thick layer of the composite containing, by mass, 40% apatite, silica, or P2O7(-4)-stabilized ACP and 60% photoactivated resin comprised of Bis-GMA, TEGDMA, HEMA, and ZrM. The photocured composite-coated sections were immersed either in a remineralizing solution for 4 weeks at 37 degrees C (static model) or cyclically immersed in demineralizing (0.5 h) and remineralizing solutions (11.5 h) for 2 weeks (dynamic model). Quantitative digital image analysis of matched 102 microns x 220 microns areas from contact microradiographs taken of the sections before and after immersion showed that lesions coated with ACP-filled composites fractionally recovered 71% +/- 33% of their lost mineral compared with 14% +/- 13% for apatite controls in the static model and 38% +/- 16% compared with -6% +/- 24% in the dynamic model. The results suggest that sealants based on ACP-filled methacrylate composites have the potential to remineralize carious enamel lesions.

This paper discusses key points made during the symposium in the light of work carried out in other laboratories. It is emphasized that the unique importance of plaque fluid is that the net result of chemical changes induced by microbial activity is reflected in this medium, which is in intimate contact with the enamel surface, and that this medium is accessible to chemical and biochemical analyses. However, in order to assess the cariogenic potential of plaque, we must consider the properties of both whole plaque and plaque fluid together. Although it is apparent that results of plaque fluid composition are sensitive to both isolation and the storage procedures utilized, plaque fluid appears to be a distinct entity within the oral cavity. Technical advances have been made which allow for the determination of the activity of selected ions (hydrogen, calcium, phosphate, potassium, fluoride) in plaque fluid obtained from a single site within the mouth. It appears, however, that such data alone may be insufficient to define the cariogenic potential of plaque appropriately. Evidence is presented from which it can be concluded that, with use of pooled samples of plaque obtained from individuals with clear differences in caries experience, results on plaque and plaque fluid composition can be obtained which are consistent with noted differences in caries susceptibility. The importance of base production is also discussed, and it is noted that few studies have been carried out to elucidate the role of proteins found in plaque fluid. In conclusion, recent advances in the study of plaque fluid have provided new insights into the mechanism of caries formation which are also germane to the formation of dental calculi.
