BACKGROUND: Digital technology offers individuals the opportunity to monitor their symptoms. Information gathered from apps, devices, and web platforms may be used to direct clinical care and to support research. AIM: Using this survey, we aim to explore the views of people attending the Anne Rowling Regenerative Neurology Clinic (ARRNC) and their relatives/caregivers regarding the use of digital health technologies to monitor health. METHOD: People attending the ARRNC were invited to complete a structured 18-item questionnaire evaluating their experience and attitudes to using technology for monitoring health. People with neurodegenerative disease (pwND) and their caregivers completed a mix of closed and open-ended questions. RESULTS: 249 people responded, 51 relatives/caregivers and 198 pwND. 67.1% (n= 167) of respondents do not use technology for monitoring their health, but 98.2% (n = 164) of these are interested in their future use. 29.7% (n = 74) respondents currently use a smartphone for health monitoring, 20.9% (n = 52) use a wearable device, and 13.3% (n = 33) use a tablet. 79.3% (n = 65) of users use their technology for monitoring physical activity, 37.8% (n = 31) use it for assisting with self-management, and 41.5% (n = 34) use it for tracking sleep. Factors which would encourage use of technology are ease of access to devices and ability to monitor health. Respondents reported data security concerns and difficulty using technology as potential barriers. CONCLUSION: People attending a neurology clinic, and their relatives/caregivers, support the use of digital technologies as an adjunct to routine care. There is a need for coordinated digital strategies for development and delivery of validated measures.

BACKGROUND: Schizophrenia is a complex mental disorder characterized by significant cognitive and neurobiological alterations. Impairments in cognitive function and eye movement have been known to be promising biomarkers for schizophrenia. However, cognitive assessment methods require specialized expertise. To date, data on simplified measurement tools for assessing both cognitive function and eye movement in patients with schizophrenia are lacking. OBJECTIVE: This study aims to assess the efficacy of a novel tablet-based platform combining cognitive and eye movement measures for classifying schizophrenia. METHODS: Forty-four patients with schizophrenia, 67 healthy controls, and 41 patients with other psychiatric diagnoses participated in this study from 10 sites across Japan. A free-viewing eye movement task and 2 cognitive assessment tools (Codebreaker task from the THINC-integrated tool and the CognitiveFunctionTest app) were used for conducting assessments in a 12.9-inch iPad Pro. We performed comparative group and logistic regression analyses for evaluating the diagnostic efficacy of the 3 measures of interest. RESULTS: Cognitive and eye movement measures differed significantly between patients with schizophrenia and healthy controls (all 3 measures; P<.001). The Codebreaker task showed the highest classification effectiveness in distinguishing schizophrenia with an area under the receiver operating characteristic curve of 0.90. Combining cognitive and eye movement measures further improved accuracy with a maximum area under the receiver operating characteristic curve of 0.94. Cognitive measures were more effective in differentiating patients with schizophrenia from healthy controls, whereas eye movement measures better differentiated schizophrenia from other psychiatric conditions. CONCLUSIONS: This multisite study demonstrates the feasibility and effectiveness of a tablet-based app for assessing cognitive functioning and eye movements in patients with schizophrenia. Our results suggest the potential of tablet-based assessments of cognitive function and eye movement as simple and accessible evaluation tools, which may be useful for future clinical implementation.

BACKGROUND: Limited resources often hinder regular cognitive assessment of people with multiple sclerosis (pwMS) in standard clinical care. A self-administered iPad®-based cognitive screening-tool (Processing Speed Test; PST) might mitigate this problem. OBJECTIVE: To evaluate the PST in clinical routine. METHODS: We investigated the feasibility of the PST in both a quiet and a waiting room setting. We assessed the validity of the PST in comparison with the established Symbol Digit Modalities Test (SDMT). We explored associations between processing speed assessments and the Brief International Cognitive Assessment for MS (BICAMS), magnetic resonance imaging (MRI) parameters, and psychological factors. Additionally, we explored the ability of the PST to detect impairment in processing speed compared to the SDMT. RESULTS: The PST was feasible in the waiting room setting. PST and SDMT correlated comparably with the BICAMS, MRI parameters, and psychological variables. Of 172 pwMS, 50 (30.8%) showed cognitive impairment according to the BICAMS; respective values were 47 (27.3%) for the SDMT and 9 (5.2%) for the PST. CONCLUSIONS: The PST performed in a waiting room setting correlates strongly with established cognitive tests. It thus may be used to assess processing speed in a resource-efficient manner and complement cognitive assessment in clinical routine. Despite comparable validity of the PST and SDMT, we identified more pwMS with impaired processing speed using normative data of the SDMT compared to the PST and advise caution, that the common cut-off score of - 1.5 SD from the current PST is not appropriate in Europe.

PURPOSE: In the current age of greater digital delivery of services, it is important to examine the validity and differences between spoken and digital delivery of materials. The current study is a practice-based research partnership between school-based speech-language pathologists (SLPs) and researchers, evaluating presentation effects and validity of a narrative retell assessment tool created by SLPs. METHOD: Fifty-one children across kindergarten, Grade 1, and Grade 2 completed the narrative retell task, retelling One Frog Too Many and Frog Goes to Dinner in three in-person story presentation conditions administered 1 week apart: spoken, iPad with audio-recorded natural rate of speech, and iPad with slow rate of speech. This was followed by 10 comprehension questions related to story events. Children also completed the Story Retelling subtests from the Test of Integrated Language and Literacy Skills (TILLS). RESULTS: Children recalled significantly fewer events in the spoken condition compared to audio-recorded iPad conditions. No significant effect of speaking rate was found. Presentation condition and rate did not affect performance on comprehension questions. Correlations among retell measures and corresponding subtests on a standardized language test ranged from weak to strong, providing some evidence of concurrent validity. CONCLUSIONS: This practice-based research partnership provided valuable insight into differences in delivery modality as well as the validity of a school-based SLP created narrative retell assessment tool. This study found that rate did not impact recall of events or performance on comprehension questions. Additionally, children performed better on narrative retell measures when stories were told using an iPad. This highlights the potential for iPad delivery as an option in narrative retell tasks. Finally, this study provided an initial examination of the Narrative Evaluation Tool's validity, finding the tool captures ability to recall narrative events; however, future studies are needed to examine the tool's validity as a measure of narrative comprehension. BACKGROUND: Mobile devices have been used by many older adults and have the potential to assist individuals with subjective cognitive complaints (SCCs) in daily living tasks. Financial management is one of the most complex daily activity for older adults, as it is easily impaired in the prodromal stage of dementia and cognitive impairment. AIM: To investigate financial management ability among older adults from SCCs and mobile device proficiency. METHODS: A self-administered questionnaire was sent to 529 participants who were ≥ 65 years and regularly use mobile devices. Participants were divided into four groups based on SCC prevalence and scores of the Mobile Device Proficiency Questionnaire (MDPQ-16). Financial management abilities were compared between groups using the Process Analysis of Daily Activities for Dementia subscale. Regression model and crosstabulation table were used to investigate associations in detail. RESULTS: A significant difference in financial management ability was observed among the four groups (p < 0.001), with the dual impairment group showed significantly lower than the robust and SCC groups (p < 0.001). Educational history, sex, and MDPQ-16 score were significantly associated with participants' financial management ability (p ≤ 0.01). The proportion of participants who could use ATMs and electronic money independently was significantly lower among those with low proficiency in mobile devices (LPM), regardless of SCC (p < 0.05). CONCLUSION: The LPM group showed an impaired ability to manage their finances, particularly in situations where they would use information devices. Therefore, healthcare professionals should assess not only the SCC but also their proficiency with mobile devices to predict their impairment in activities of daily living.

As mobile devices have become a central part of our daily lives, they are also becoming increasingly important in research. In the medical context, for example, smartphones are used to collect ecologically valid and longitudinal data using Ecological Momentary Assessment (EMA), which is mostly implemented through questionnaires delivered via smart notifications. This type of data collection is intended to capture a patient's condition on a moment-to-moment and longer-term basis. To collect more objective and contextual data and to understand patients even better, researchers can not only use patients' input via EMA, but also use sensors as part of the Mobile Crowdsensing (MCS) approach. In this paper, we examine how researchers have embraced the topic of MCS in the context of EMA through a systematic literature review. This PRISMA-guided review is based on the databases PubMed, Web of Science, and EBSCOhost. It is shown through the results that both EMA research in general and the use of sensors in EMA research are steadily increasing. In addition, most of the studies reviewed used mobile apps to deliver EMA to participants, used a fixed-time prompting strategy, and used signal-contingent or interval-contingent self-assessment as sampling/assessment strategies. The most commonly used sensors in EMA studies are the accelerometer and GPS. In most studies, these sensors are used for simple data collection, but sensor data are also commonly used to verify study participant responses and, less commonly, to trigger EMA prompts. Security and privacy aspects are addressed in only a subset of mHealth EMA publications. Moreover, we found that EMA adherence was negatively correlated with the total number of prompts and was higher in studies using a microinteraction-based EMA (µEMA) approach as well as in studies utilizing sensors. Overall, we envision that the potential of the technological capabilities of smartphones and sensors could be better exploited in future, more automated approaches.

It includes technologies, platforms, and systems that engage consumers in all aspects of life. It makes health and healthcare be people-centered and personalized. Digital health requires total interoperability - standards, common data elements, and the integration of data from all sources. It demands data sharing. Digital Health brings together a wide range of stakeholders for similar goals using the same resources. Digital Health uses mobile devices and wearable sensors and uses Artificial Intelligence and Machine Learning to handle the vast amount of data Digital Health engages. Finally, Digital Health has the potential to open the gap between the different social and economic classes that must be addressed.

Objective: Evaluate the inter- and intrarater reliability of a wound assessment tool in iPhone 12 and 13 mini modalities against a validated iPad mini/Structure Sensor configuration. Approach: We assessed a wound measurement application (eKare inSight®) for result consistency in patients presenting with wounds. Assessments were analyzed using a two-way analysis of variance. Intraclass correlation coefficient (ICC) was computed for intrarater (ICC1,1) and inter-rater (ICC2,1) analysis using a two-way random effects model. Paired t-test assessed the statistical difference between measurement methods. Results: Forty-two lesions were analyzed with surface areas ranging from 0.2 to 23 cm2 (average 4.33 ± 5.44 cm2). A high level of reliability was observed for repeat wound area measurements by the same examiner (ICC1,1 = 0.997) and between examiners with iPhone 13 mini (ICC2,1 = 0.998). There was no significant difference between iPhone 12 and iPad mini/Structure Sensor (p = 0.78) or between iPhone 13 mini and iPhone 12 (p = 0.22). Minimal difference existed between iPhone 13 mini and iPad mini/Structure Sensor (p = 0.049, Cohen's d = 0.01). Innovation: Increased pervasiveness of smartphones in clinical care, coupled with advances in smartphone imaging and machine learning, allows for a potential solution to the problem of fast and accurate wound measurements. The application investigated produces wound measurement results quickly and with demonstrated accuracy. It does not require a calibration sticker or reference marker and allows for automatic wound boundary delineation. Conclusion: The results of this study suggest that a digital planimetry mobile application may offer high levels of reliability across devices and users.

Spontaneous retinal Venous Pulsations (SVP) are rhythmic changes in the caliber of the central retinal vein and are observed in the optic disc region (ODR) of the retina. Its absence is a critical indicator of various ocular or neurological abnormalities. Recent advances in imaging technology have enabled the development of portable smartphone-based devices for observing the retina and assessment of SVPs. However, the quality of smartphone-based retinal videos is often poor due to noise and image jitting, which in return, can severely obstruct the observation of SVPs. In this work, we developed a fully automated retinal video stabilization method that enables the examination of SVPs captured by various mobile devices. Specifically, we first propose an ODR Spatio-Temporal Localization (ODR-STL) module to localize visible ODR and remove noisy and jittering frames. Then, we introduce a Noise-Aware Template Matching (NATM) module to stabilize high-quality video segments at a fixed position in the field of view. After the processing, the SVPs can be easily observed in the stabilized videos, significantly facilitating user observations. Furthermore, our method is cost-effective and has been tested in both subjective and objective evaluations. Both of the evaluations support its effectiveness in facilitating the observation of SVPs. This can improve the timely diagnosis and treatment of associated diseases, making it a valuable tool for eye health professionals.

BACKGROUND: There is growing interest in enhancing stroke self-management support using mobile health (mHealth) technology (eg, smartphones and apps). Despite this growing interest, "self-management support" is inconsistently defined and applied in the poststroke mHealth intervention literature, which limits efforts to synthesize and compare evidence. To address this gap in conceptual clarity, a scoping review was conducted. OBJECTIVE: The objectives were to (1) identify and describe the types of poststroke mHealth interventions evaluated using a randomized controlled trial design, (2) determine whether (and how) such interventions align with well-accepted conceptualizations of self-management support (the theory by Lorig and Holman and the Practical Reviews in Self-Management Support [PRISMS] taxonomy by Pearce and colleagues), and (3) identify the mHealth functions that facilitate self-management. METHODS: A scoping review was conducted according to the methodology by Arksey and O'Malley and Levac et al. In total, 7 databases were searched. Article screening and data extraction were performed by 2 reviewers. The data were analyzed using descriptive statistics and content analysis. RESULTS: A total of 29 studies (26 interventions) were included. The interventions addressed 7 focal areas (physical exercise, risk factor management, linguistic exercise, activities of daily living training, medication adherence, stroke education, and weight management), 5 types of mobile devices (mobile phones or smartphones, tablets, wearable sensors, wireless monitoring devices, and laptops), and 7 mHealth functions (educating, communicating, goal setting, monitoring, providing feedback, reminding, and motivating). Collectively, the interventions aligned well with the concept of self-management support. However, on an individual basis (per intervention), the alignment was less strong. CONCLUSIONS: On the basis of the results, it is recommended that future research on poststroke mHealth interventions be more theoretically driven, more multidisciplinary, and larger in scale.

The assessment of cranial deformation is relevant in the field of medicine dealing with infants, especially in paediatric neurosurgery and paediatrics. To address this demand, the smartphone-based solution PhotoMeDAS has been developed, harnessing mobile devices to create three-dimensional (3D) models of infants' heads and, from them, automatic cranial deformation reports. Therefore, it is crucial to examine the accuracy achievable with different mobile devices under similar conditions so prospective users can consider this aspect when using the smartphone-based solution. This study compares the linear accuracy obtained from three smartphone models (Samsung Galaxy S22 Ultra, S22, and S22+). Twelve measurements are taken with each mobile device using a coded cap on a head mannequin. For processing, three different bundle adjustment implementations are tested with and without self-calibration. After photogrammetric processing, the 3D coordinates are obtained. A comparison is made among spatially distributed distances across the head with PhotoMeDAS vs. ground truth established with a Creaform ACADEMIA 50 while-light 3D scanner. With a homogeneous scale factor for all the smartphones, the results showed that the average accuracy for the S22 smartphone is -1.15 ± 0.53 mm, for the S22+, 0.95 ± 0.40 mm, and for the S22 Ultra, -1.8 ± 0.45 mm. Worth noticing is that a substantial improvement is achieved regardless of whether the scale factor is introduced per device.

Introduction. Extended focused assessment with sonography for trauma (E-FAST) can be performed with minimal training and achieve ideal results. It allows easy transport and use in austere environments such as the Colombian Caribbean, where many centers do not have 24-hour radiology services. The objective of this study was to determine the performance of the use of E-FAST in the evaluation of trauma by second-year general surgery residents in the emergency department. Methods. Retrospective observational study that evaluated the diagnostic performance of E-FAST with Butterfly IQ, in patients with thoracoabdominal trauma, who attended a referral center in the Colombian Caribbean between November 2021 and July 2022. Sensitivity, specificity, and positive and negative predictive values were evaluated, compared with intraoperative findings or conventional imaging. Results. A total of 46 patients were included, with a mean age of 31.2 ± 13.8 years, 87.4% (n=39) were male. The main mechanism of trauma was penetrating (n=32; 69.5%). It was found that 80.4% (n=37) of the patients had a positive E-FAST result, and of these, 97% (n=35) had a positive intraoperative finding. Sensitivity, specificity, positive predictive value and negative predictive value were 92.1%, 75%, 94.6%, and 66.6%, respectively. The positive likelihood ratio was 3.68, while the negative likelihood ratio was 0.10. Conclusion. General surgery residents have the competence to perform accurate E-FAST scans. The hand-held ultrasound device is an effective diagnostic tool for trauma and acute care surgery patients.


Introducción. La evaluación enfocada extendida con ecografía en trauma (E-FAST, extended focused assessment with sonography for trauma) puede realizarse con entrenamiento mínimo y lograr resultados ideales. Su fácil transporte permite usarla en entornos austeros, como el Caribe colombiano, donde muchos centros no disponen de servicio radiológico las 24 horas. El objetivo de este estudio fue determinar el rendimiento del uso de E-FAST por residentes de cirugía general de segundo año en la evaluación del paciente con trauma en urgencias. Métodos. Estudio observacional retrospectivo que evaluó el rendimiento diagnóstico de E-FAST con Butterfly IQ, en pacientes con trauma toracoabdominal que acudieron a un centro de referencia del Caribe colombiano, entre noviembre de 2021 y julio de 2022. Se evaluaron sensibilidad, especificidad, valores predictivos positivo y negativo, comparando la descripción de la ecografía con los hallazgos intraoperatorios o imagenología convencional. Resultados. Se incluyeron un total de 46 pacientes, con una media de edad de 31,2 ± 13,8 años, siendo el 87,4 % (n=39) hombres. El principal mecanismo de trauma fue penetrante (n=32; 69,5 %). Se encontró que el 80,4 % (n=37) de los pacientes tuvo resultado E-FAST positivo, y que, de estos, el 97 % (n=35) tuvo un hallazgo positivo intraoperatorio. Se calculó una sensibilidad de 92,1 %, especificidad de 75 %, valor predictivo positivo de 94,6 % y negativo de 66,6 %; la razón de verosimilitud positiva fue de 3,68 y la negativa de 0,10. Conclusión. Los residentes de cirugía general están capacitados para realizar exploraciones E-FAST precisas. El ecógrafo portátil es una herramienta de diagnóstico eficaz para pacientes traumatizados.

CONTEXT: Progress in mobile technology, especially the use of applications for mobile devices, can support the process of monitoring patients in palliative care (therapeutics), controlling symptoms, or providing self-care guidelines for the user, namely patients or caregivers. OBJECTIVES: To map the available knowledge regarding the use of applications for mobile devices to support adult patients in palliative care at home. METHODS: Literature review, based on the Joanna Briggs Institute model(s) for Scoping Review. All articles published until October 27, 2022, were identified in the electronic databases MEDLINE®, CINAHL®, Psychology and Behavioral Sciences Collection, Cochrane Library, and Scopus using the respective Boolean logical operators and key terms. RESULTS: A total of 634 articles were identified, and a final 24 studies were included. Eleven mobile device applications were identified, demonstrating different aspects of design, use, and technological development. These have incorporated the most recent technology in their functionalities. CONCLUSION: Mobile applications can be considered a viable and effective means of monitoring patients in palliative care. However, these applications must go beyond the academic scenario in which they were developed and move toward widespread use in practice, allowing the evaluation of the impact of this "new" intervention modality to understand their effectiveness and the application of best practices.

Continuous ergonomic risk assessment of the human body is critical to avoid various musculoskeletal disorders (MSDs) for people involved in physical jobs. This paper presents a digital upper limb assessment (DULA) system that automatically performs rapid upper limb assessment (RULA) in real-time for the timely intervention and prevention of MSDs. While existing approaches require human resources for computing the RULA score, which is highly subjective and untimely, the proposed DULA achieves automatic and objective assessment of musculoskeletal risks using a wireless sensor band embedded with multi-modal sensors. The system continuously tracks and records upper limb movements and muscle activation levels and automatically generates musculoskeletal risk levels. Moreover, it stores the data in a cloud database for in-depth analysis by a healthcare expert. Limb movements and muscle fatigue levels can also be visually seen using any tablet/computer in real-time. In the paper, algorithms of robust limb motion detection are developed, and an explanation of the system is provided along with the presentation of preliminary results, which validate the effectiveness of the new technology.

Smart wearables advance to reliably and continuously measure vital signs. Analyzing the produced data requires complex algorithms, which would unreasonably increase the energy consumption of mobile devices and exceed their computing power. Fifth-generation (5G) mobile networks provide low latencies, high bandwidth, and many connected devices and introduced multi-access edge computing, which brings high computation power close to the clients. We propose an architecture for evaluating smart wearables in real-time and evaluate it exemplary with electrocardiography signals and binary classification of myocardial infarctions. Our solution shows that real-time infarct classification is feasible with 44 clients and secured transmissions. Future releases of 5G will increase real-time capability and enable capacity for more data.

OBJECTIVE: Accurate measurement of adult and child screen media use are needed to robustly assess its impact on health outcomes. Our objective was to systematically review screen media use measurement tools that have been validated against an objective "gold standard" tool. METHODS: The search strategy was initially conducted in Medline Ovid and translated to Embase, Web of Science, PsychInfo and Cochrane. A modified natural language search was conducted in Google Scholar and IEEE. The initial search was conducted in March 2021, and an updated search was conducted in June 2022. Additional studies were included from the references. Studies had to describe the validation of a tool to measure screen media use on participants of any age against a 'gold standard' or comparable objective measure. The COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) was used to assess the criterion validity. Four authors reviewed the titles in two rounds and extracted data. RESULTS: Twenty-nine articles were included in the review. Studies measured TV, computer, mobile device and social media site screen media use through: self or parent report, direct or video observation, computer and mobile device use tracking programs, and through other novel devices such as wearable devices and cameras. Correlations of self or parent report of screen media with the gold standard were lower than correlations of technology-based measures, and video observation with the gold standard. The COSMIN criterion validity ratings ranged from poor to excellent; most of the studies received a global score of fair or poor. CONCLUSIONS: Technology based validated tools that more directly measure screen use are emerging that have been validated against a gold standard for measuring screen use. However, practical, objective measures of diverse types of screen media use that have been tested on diverse populations are needed to better understand the impact of screen media use on the development and physical and mental health of children and adults.

A construction site has specific risks and organizational conditions requiring on-site safety inspections. Paperwork inspections have important limitations that can be overcome by substituting paper records with digital registers and using new information and communication technologies. Although academic literature has provided several tools to carry out on-site safety inspections adopting new technologies, most construction sites are not currently ready to adopt them. This paper covers this need of on-site control by providing an application that uses a simple technology accessible to most construction companies. The main objective and contribution of this paper is to design, develop, and implement a mobile device application (App), named "RisGES". It is based on the model of risk that grounds the Construction Site Risk Assessment Tool (CONSRAT) and on the related models that connect risk with specific organizational and safety resources. This proposed application is aimed to assess the on-site risk and organizational structure by using new technologies and considering all relevant resources and material safety conditions. The paper includes practical examples of how to use RisGES in real settings. Evidence for the discriminant validity of CONSRAT is also provided. The RisGES tool is at once preventive and predictive since it yields a specific set of criteria for interventions intended to decrease the levels of risks on-site, as well as to detect improvement needs in the site structure and resources for increasing the safety levels.

Mobile health (mHealth)-that is, use of mobile devices, such as mobile phones, monitoring devices, personal digital assistants, and other wireless devices, in medical care-is a promising approach to the provision of support services. mHealth may aid in facilitating monitoring of mental health conditions, offering peer support, providing psychoeducation (i.e., information about mental health conditions), and delivering evidence-based practices. However, some groups may fail to benefit from mHealth despite a high need for mental health services, including people from racially and ethnically disadvantaged groups, rural residents, individuals who are socioeconomically disadvantaged, and people with disabilities. A well-designed mHealth ecosystem that considers multiple elements of design, development, and implementation can afford disadvantaged populations the opportunity to address inequities and facilitate access to and uptake of mHealth. This article proposes inclusion of the following principles and standards in the development of an mHealth ecosystem of equity: use a human-centered design, reduce bias in machine-learning analytical techniques, promote inclusivity via mHealth design features, facilitate informed decision making in technology selection, embrace adaptive technology, promote digital literacy through mHealth by teaching patients how to use the technology, and facilitate access to mHealth to improve health outcomes.

Disparities in patient portal use are impacted by individuals' access to technology and the internet as well as their skills and health behaviors. An individual's geographic location may impact these factors as well as contribute to their decision to use a portal, their choice of device to access the portal, and their use of portal functions. This study evaluated patient portal use by geographic location according to three comparators: proximity to the medical center offering the portal, urban/rural classification, and degree of digital distress. Patients residing farther from the medical center, in rural areas, or in areas of higher digital distress were less likely to be active portal users. Patients in areas of higher digital distress were more likely to use the mobile portal application instead of the desktop portal website alone. Users of the mobile portal application used portal functions more frequently, and being a mobile user had a greater impact on the use of some portal functions by patients residing in areas of higher digital distress. Mobile patient portal applications have the potential to increase portal use, but work is needed to ensure equitable internet access, to promote mobile patient portal applications, and to cultivate individuals' skills to use portals.

In recent years, skin spectral information has been gradually applied in various fields, such as the cosmetics industry and clinical medicine. However, the high price and the huge size of the skin spectrum measurement device make the related applications of the skin spectrum unable to be widely used in practical applications. We used convolutional neural network (CNN) to achieve a satisfying accuracy of the Fitzpatrick skin-type classification by using a simple self-developed device in 2018. Leveraging on the hardware, firmware, and software app-developing experience, a low-cost miniature skin spectrum measurement system (LMSSMS) using deep neural network (DNN) technology was further studied, and the feasibility of the system is verified in this paper. The developed LMSSMS is divided into three parts: (1) miniature skin spectrum measurement device (MSSMD), (2) DNN model, and (3) mobile app. The MSSMD was developed with innovative low-cost MSSC, 3D printing, and a simple LED light source. The DNN model is designed to enhance measurement accuracy. Finally, the mobile app is used to control and show the measurement results. The developed app also includes a variety of skin-spectrum-related applications, such as erythema index and melanin index (EI/MI) measurement, Fitzpatrick skin-type classification, Pantone SkinTone classification, sun-exposure estimation, and body-fat measurement. In order to verify the feasibility of LMSSMS, we used the standard instrumentation device as a reference. The results show that the accuracy of the LMSSMS can reach 94.7%, which also confirms that this development idea has much potential for further development.
