BACKGROUND: Psychosocial rehabilitation in forensic psychiatric services requires sound measurement of patient and staff perceptions of psychosocial function. The recommended World Health Organisation Disability Assessment Schedule 2.0 (WHODAS), designed for this, has not been examined with offender patients. AIMS: To examine patient and staff WHODAS ratings of secure hospital inpatients with psychosis, any differences between them and explore associations with other clinical factors. METHODS: Seventy-three patients self-rated on the WHODAS after 3 months as inpatients. An occupational therapist interviewed the patient's primary nurse and care team at about the same time (staff ratings). Scores were calculated according to the WHODAS manual. WHODAS scores and interview-rated symptom severity, cognitive measures, daily antipsychotic dose and duration of care were compared. RESULTS: Patient ratings indicated less disability than staff ratings for total score and for the domains of understanding and communicating, getting along and life activities. Self-care and participation ratings were similar. Patients were more likely to rate themselves as disabled in getting around (mobility). Only one-fifth of patient- and staff- ratings (16, 22%) were similar, while for nearly a third of the patients (23, 32%) self-ratings were higher than staff ratings. More severe positive symptoms were associated with higher self-rated WHODAS disability after accounting for treatment duration, negative symptoms, cognitive score and antipsychotic dose. No variable accounted for the staff/patient differences in ratings. CONCLUSION: Our mean WHODAS score findings echoed those in other patient samples-of patient underestimation of disability, linked to severity of symptoms. In this study using the WHODAS for the first time in a forensic mental health secure inpatient service, however, we found that, by comparing individuals, half of the patients reported equivalent or greater disability than did staff. Future research should focus on elucidating from patients what contributes to their self-ratings. Understanding their thought processes in rating may enhance rehabilitation planning.

The number of women involved with forensic mental health systems internationally is rising, however, limited research has explored the characteristics of those assessed for criminal responsibility. We investigated the demographic, psychiatric, and criminological characteristics of women recommended as eligible or ineligible for the defence of Not Criminally Responsible (NCR) on account of mental disorder following a criminal responsibility assessment in Central Canada. Data were collected through retrospective chart reviews of court-ordered criminal responsibility assessments for 109 women referred for evaluations between 2003 and 2019. Accused were an average age of 34.55 years, predominately identified as Indigenous (37.7%) or Caucasian (20.8%), and had often been charged with assault (47.7%). Women identified in the reports as NCR-eligible were significantly more likely to be employed, experience delusions during the index offence, and have expert reports linking their mental health symptoms to NCR legal criteria. They were also significantly less likely to have a personality disorder, substance-related diagnosis, or have used substances during the index offence. Delusions during the index offence significantly predicted assessment recommendations when controlling for age at assessment order, current substance-related diagnosis, and whether the expert report linked mental health symptoms to NCR legal criteria. Findings indicate the key factors considered by forensic mental health professionals when conducting criminal responsibility assessments with women. Meaningful differences exist between women identified as NCR-eligible and ineligible, with findings illustrating who may be more likely to receive services within the Canadian forensic mental health system.

BACKGROUND: Young adults who commit low-level offences commonly have a range of health and social needs and are significantly over-represented in the criminal justice system. These young adults may need to attend court and potentially receive penalties including imprisonment. Alternative routes exist, which can help address the underlying causes of offending. Some feel more should be done to help young adults entering the criminal justice system. The Gateway programme was a type of out-of-court disposal developed by Hampshire Constabulary, which aimed to address the complex needs of young adults who commit low-level crimes. This study aimed to evaluate the effectiveness and cost-effectiveness of the Gateway programme, issued as a conditional caution, compared with usual process. METHODS: The Gateway study was a pragmatic, parallel-group, superiority randomised controlled trial that recruited young adults who had committed a low-level offence from four sites covering Hampshire and Isle of Wight. The primary outcome was mental health and well-being measured using the Warwick-Edinburgh Mental Well-being Scale. Secondary outcomes were quality of life, alcohol and drug use, and recidivism. Outcomes were measured at 4, 16 and 52 weeks postrandomisation. RESULTS: Due to issues with retention of participants and low data collection rates, recruitment ended early, with 191 eligible participants randomised (Gateway 109; usual process 82). The primary outcome was obtained for 93 (48.7%) participants at 4 weeks, 93 (48.7%) at 16 weeks and 43 (22.5%) at 1 year. The high attrition rates meant that effectiveness could not be assessed as planned. CONCLUSIONS: Gateway is the first trial in a UK police setting to have a health-related primary outcome requiring individual data collection, rather than focusing solely on recidivism. We demonstrated that it is possible to recruit and randomise from the study population, however follow-up rates were low. Further work is needed to identify ways to facilitate engagement between researchers and vulnerable populations to collect data. TRIAL REGISTRATION NUMBER: ISRCTN11888938.

BACKGROUND: To date, there is no evidence supporting the existence of an association between Autism Spectrum Disorder (ASD) and extremism in the general population. However, there is increasing recognition that several features of ASD may provide the context of vulnerability to engage in extremist behaviour. AIMS: This paper sets out the case for a dedicated clinical approach to better integrate clinical risk appraisal processes with an assessment of ASD individuals' vulnerabilities within the Criminal Justice System. METHODS AND RESULTS: In this paper the Framework for the Assessment of Risk & Protection in Offenders on the Autistic Spectrum (FARAS): A Guide for Risk Assessors Working with Offenders on the Autistic Spectrum is explored. In developing the FARAS, Al-Attar proposed seven facets of ASD that 'may have different functional links with push and pull factors to terrorism' (p. 928), which include circumscribed interests; rich vivid fantasy and impaired social imagination; need for order, rules, rituals, routine and predictability; obsessionality, repetition and collecting; social interaction and communication difficulties; cognitive styles and Sensory processing. DISCUSSION AND CONCLUSION: We describe the FARAS within the context of the most widely used clinical risk appraisal 'aide memoire' instruments integral to the Structured Professional Judgement of risk process, namely the HCR20v3.

Resumo Este artigo discute limites e possibilidades da prática de um grupo reflexivo junto a homens autores de violência contra mulheres. Apoia-se em um estudo etnográfico que incluiu observação-participante e entrevistas com os facilitadores de um grupo realizado em uma organização feminista não governamental em parceria com a justiça criminal. A partir da discussão de uma cena emblemática, argumentamos a favor de um processo de trabalho dialógico como uma alternativa que pode expandir os limites da abordagem judicial: para dialogar com o grupo de pares, os facilitadores precisam levar em consideração a posição minoritária dos pontos de vista feministas no campo das masculinidades. Só esse passo permite cooperar com os participantes para que, a partir do compartilhamento espontâneo e genuíno de experiências, reconheçam o caráter inadequado de suas expectativas em relação às mulheres. Deste modo instaura-se uma dinâmica de cuidado entre homens, favorável à mitigação das violências e complementar da promoção da equidade de gênero.


Abstract This article discusses limits and possibilities of the practice of a reflection group together with male perpetrators of violence against women. It is based on an ethnographic study, which included participant observation and interviews with the facilitators of a group held in a feminist non-governmental organization in partnership with the criminal justice system. Starting with the discussion of an emblematic scene, we argue in favor of this dialogue process as an alternative that could expand the limits of the judiciary approach. In order to enter into a dialogue with the peer group, the facilitators must consider the minority position of feminist viewpoints in the field of masculinity. This step alone makes it possible to cooperate with the participants such that, as they spontaneously and genuinely share their experiences, they come to acknowledge the inadequacy of their expectations regarding women. This way, a dynamic of care takes place between men, which is conducive to the mitigation of violence and serves as a complement to the promotion of gender equality.

Objective: To analyze the differences in psychopathic traits between offender and non-offender youths with similar socioeconomic backgrounds. Method: The Psychopathy Checklist Revised (PCL-R) scale was used to identify whether 39 young offenders with no history of mental disorders or criteria for psychopathy exhibited differences in its total score, and specifically for factor 1 or factor 2 of this scale, when compared with 32 other young people, living in similar socioeconomic conditions, who had not committed offenses. Results: We observed statistically significant between-group differences (p < 0.01) in mean PCL-R scores, with a mean score of 13.4 in the offender group vs. 2.1 in the non-offender group. We also detected significant between-group differences when we analyzed mean factor 1 (p < 0.01) and factor 2 (p < 0.01) scores separately. Although the groups exhibited statistically significant difference in educational attainment, between-group comparison of mean PCL-R scores controlling for educational attainment by analysis of covariance (ANCOVA) showed that the difference in PCL-R scores remained statistically significant (p < 0.01). Conclusions: We conclude that, in this sample, the presence of both primary (interpersonal/affective characteristics) and secondary (lifestyle/antisocial behavior) psychopathic traits differed between offender and non-offender youths, even when excluding psychopathy and other mental disorders from the assessments. These results suggest a need for wide-ranging interventions, not restricted to socioeconomic aspects, for the management of juvenile delinquency. .

O presente artigo é uma revisão da literatura sobre o papel da religião na promoção da saúde, na prevenção da violência e na reabilitação e recuperação de pessoas envolvidas com a criminalidade. Pesquisa de artigos nas bases Medline via PubMed, Lilacs, Scopus, Social Index With Full Text, Sociological Abstracts e Social Services Abstracts selecionou e analisou 29 estudos entre 1990 e 2011 nas línguas portuguesa, inglesa e espanhola. Os resultados apontam a função do grupo e o papel da religião na reabilitação de pessoas, na prevenção do envolvimento com a criminalidade, em orientações correcionais, e em territórios de iniquidade social. É realizado um debate acerca dos principais temas encontrados, destacando a multiplicidade de papéis da religião, sua atuação junto às pessoas encarceradas, como base de apoio e controle social e sua influência na promoção da saúde mental e da qualidade de vida. A relação entre religião e esfera pública é discutida, assinalando o papel de grupos religiosos e a fragilidade do Estado em fornecer políticas públicas básicas à população. Apesar de apontarem o papel exitoso da religião na prevenção da violência e na reabilitação de pessoas, alguns estudos põem em xeque esta relação e destacam conflitos entre os conceitos utilizados e os resultados obtidos.


This is a review of the literature on the role of religion in the promotion of health, the prevention of violence and the rehabilitation of individuals involved in criminal activities. Research was conducted in the Medline through PubMed, Lilacs, Scopus, Social Index with full text, Sociological abstracts and Social services abstracts databases. Twenty-nine essays in Portuguese, English and Spanish were selected and analyzed between 1990 and 2011. The results indicate the function of the religious group and the role of religion in the rehabilitation of individuals, the prevention of criminal involvement, corrective orientations and in social iniquity environments. A debate is presented on the main themes revealed, highlighting the multiplicity of religious roles, their influence over prisoners, as a social control and support base and its influence on mental health promotion and quality of life. The relation between religion and the public sphere is discussed, emphasizing the role of religious groups and the fragility of the State in providing basic public policies. Despite revealing the successful role of religion in violence prevention and in rehabilitation, some studies question this relation and highlight the conflicts between the concepts employed and the final results.

INTRODUCTION: Risk-taking behaviors, family criminality, poverty, and poor parenting have been frequently associated with an earlier onset of criminal activities and a longer criminal career among male convicts. OBJECTIVE: This study aims to identify factors related to the onset and recurrence of criminal behavior among female robbers in the State of São Paulo - Brazil. METHOD: It was a cross-sectional study carried out inside a feminine penitentiary in São Paulo. From June 2006 to June 2010, 175 inmates convicted only for robbery were recruited to be evaluated about family antecedents of criminal conviction, alcohol and drug misuse, impulsiveness, depressive symptoms, and psychosocial features. RESULTS Having family antecedents of criminal conviction consistently predicted an earlier onset of criminal activities and a longer criminal career among female robbers. Drug use in youth and the severity of drug misuse were significantly related to the initiation and recurrence of criminal behavior, respectively. DISCUSSION: Prisons must systematically screen detainees and provide treatments for those with health problems in general. Children of inmates should obtain help to modify the negative consequences of their parents' incarceration in order to mitigate the negative consequences of pursuing this 'static' factor.


INTRODUÇÃO:Comportamentos de correr riscos, criminalidade familiar, pobreza e pais inadequados foram frequentemente associados a um início mais precoce de atividades criminais e a uma carreira criminal mais longa em presos do sexo masculino. OBJETIVO:Esse estudo visa identificar os fatores relacionados ao início e à recorrência do comportamento criminal em mulheres assaltantes no estado de São Paulo, Brasil. MÉTODO: Este foi um estudo em corte transversal realizado dentro de uma penitenciária feminina em São Paulo. De junho de 2006 a junho de 2010, 175 internas condenadas apenas por roubo foram recrutadas para avaliação quanto a antecedentes familiares de condenação por crimes, uso inadequado de drogas e álcool, impulsividade, sintomas depressivos e características psicossociais. RESULTADOS: Ter antecedentes familiares de condenação por crimes predisse consistentemente um início mais precoce de atividades criminais e uma carreira criminal mais longa em mulheres assaltantes. O uso de drogas na juventude e a gravidade do uso de drogas estavam significativamente relacionados respectivamente ao início e à recorrência do comportamento criminal. DISCUSSÃO: As prisões deveriam avaliar sistematicamente os presos e proporcionar tratamento para aqueles com problemas de saúde em geral. Os filhos dos internos deveriam obter ajuda para modificar as consequências do encarceramento de seus pais e reduzir as consequências negativas da evolução desse fator 'estático.'.
