It is well known that women bear the greatest burden of health, work time, and labor supply due to gender disparity in many developing countries. In this study, we analyze the health inequality in rural China caused by indoor air pollution from traditional energy use. Specifically, we study the effect of clean energy access on woman health outcome by exploiting a nationwide rollout of clean cooking fuel program in 2014. Based on interviews with rural women in 2014 and 2016, this study analyzes the impact of clean energy use on women's health by using the propensity score matching method with the difference-in-differences model (PSM-DID). We also analyze the heterogeneous health effects of clean energy uptake on rural women with different characteristics. The results show that clean energy applications can significantly improve the health of rural women. The positive health effects are substantial for middle-aged and older women, illiterate women, and those women lived in northeastern China. The results highlight the role of clean energy in reducing gender disparities in health inequality.

The purpose of this study was to investigate the effects of e-Cooker® and moderate electric field (MEF) cooking on physical and chemical changes occurring during the cooking of meat. Beef muscle samples (38.86 ± 0.08 g) were cooked in saline solution (0.5% w/w NaCl) to a target temperature of 72 °C, followed by a 2 min holding time. The experimental results revealed that e-Cooker® and MEF significantly reduced the come-up time required to achieve a target temperature of 72 °C to 1.16 ± 0.02 min and 0.86 ± 0.02 min, respectively compared to 14.12 ± 0.55 min in conventional cooking. The colour and instrumental texture of cooked meat by e-Cooker® and MEF systems were not significantly different (P > 0.05) from conventionally cooked ones. Overall, the results obtained demonstrated that e-Cooker® and MEF can be used to cook meat in a shorter time and reasonably low energy input while producing a product which is comparable in quality to conventionally cooked meat.

Cooking exhaust gas includes polycyclic aromatic hydrocarbons (PAHs) that are unintentionally generated during cooking, which exposes the cook and others in the vicinity to these toxic compounds. However, information on the occurrence of PAHs, particularly their chlorinated derivatives (ClPAHs), in cooking exhaust gas is limited. Here, we determined the concentrations of 12 PAHs and 20 ClPAHs in cooking exhaust gas emitted during gas-grilling of a Pacific saury using a typical Japanese fish grill in an indoor kitchen. The total concentrations of PAHs and ClPAHs in the cooking exhaust gas were 3400 and 19 ng m-3, respectively. All 12 PAHs were detected in the cooking exhaust gas, with phenanthrene (2100 ng m-3), fluorene (630 ng m-3), and anthracene (200 ng m-3) detected at the highest concentrations. Four of the 20 ClPAHs were detected, with 9-monochlorinated phenanthrene detected at the highest concentration (12 ng m-3). The exposure rates for the cook to the PAHs and ClPAHs in the cooking exhaust gas, estimated using the National Institute of Advanced Industrial Science and Technology - Indoor Consumer Exposure Assessment Tool (AIST-ICET), were in the range of 7.2-72 ng-BaPeq kg-1 day-1 (toxic equivalent concentrations relative to the toxicity of benzo[a]pyrene), which was comparable with that for dietary ingestion of cooked foods (54 ng-BaPeq kg-1 day-1). A risk assessment of exposure to PAHs and ClPAHs in cooking exhaust gas in the indoor environment revealed that this gas may pose a health risk to the cook (incremental lifetime cancer risk: 2.1 × 10-6 to 2.1 × 10-5), indicating that further investigations are warranted.

BACKGROUND: High quality personal exposure data is fundamental to understanding the health implications of household energy interventions, interpreting analyses across assigned study arms, and characterizing exposure-response relationships for household air pollution. This paper describes the exposure data collection for the Household Air Pollution Intervention Network (HAPIN), a multicountry randomized controlled trial of liquefied petroleum gas stoves and fuel among 3,200 households in India, Rwanda, Guatemala, and Peru. OBJECTIVES: The primary objectives of the exposure assessment are to estimate the exposure contrast achieved following a clean fuel intervention and to provide data for analyses of exposure-response relationships across a range of personal exposures. METHODS: Exposure measurements are being conducted over the 3-y time frame of the field study. We are measuring fine particulate matter [PM < 2.5µm in aerodynamic diameter (PM2.5)] with the Enhanced Children's MicroPEM™ (RTI International), carbon monoxide (CO) with the USB-EL-CO (Lascar Electronics), and black carbon with the OT21 transmissometer (Magee Scientific) in pregnant women, adult women, and children <1 year of age, primarily via multiple 24-h personal assessments (three, six, and three measurements, respectively) over the course of the 18-month follow-up period using lightweight monitors. For children we are using an indirect measurement approach, combining data from area monitors and locator devices worn by the child. For a subsample (up to 10%) of the study population, we are doubling the frequency of measurements in order to estimate the accuracy of subject-specific typical exposure estimates. In addition, we are conducting ambient air monitoring to help characterize potential contributions of PM2.5 exposure from background concentration. Stove use monitors (Geocene) are being used to assess compliance with the intervention, given that stove stacking (use of traditional stoves in addition to the intervention gas stove) may occur. CONCLUSIONS: The tools and approaches being used for HAPIN to estimate personal exposures build on previous efforts and take advantage of new technologies. In addition to providing key personal exposure data for this study, we hope the application and learnings from our exposure assessment will help inform future efforts to characterize exposure to household air pollution and for other contexts. https://doi.org/10.1289/EHP6422.

Biomass is primary source of energy for household in rural communities. Developing countries are focusing on increasing utilization of indigenous energy resources for energy security and to achieve sustainable development goal. Combustion of solid biomass is the primary approach for utilizing biomass to generate electricity and heat. Sixty-eight percent of population of Pakistan is living in rural areas while 30% population is still without electricity. The traditional household appliances used for cooking and heating are less efficient, more hazardous to users, and more damaging to the environment. Low carbon energy system prerequisites access to modern energy services. This paper presents an assessment of biomass resources potential in Pakistan as renewable energy resources and reviews potentials to adopt efficient use of biomass for cooking, heating, and small decentralized electricity generation. Objective of this study is to increase the sustainability of the use of biomass as source of energy in developing countries like Pakistan by an integrating energy-efficient and modern appliances and technologies that fit into a sustainable development path. Promotion of cleaner technologies and efficient use of biomass energy constitute appropriate strategies to mitigate global climate, health risks, and help in attending the targets set by sustainable development goal (SDG) to confirm worldwide access to reliable, affordable, and modern energy services by 2030.

Household air pollution (HAP) secondary to the burning of solid fuels is a major risk factor for the development of COPD. Our study seeks to examine the impact of a clean cookstove, liquid petroleum gas (LPG), on respiratory outcomes. Women (n = 200) from neighboring Indian communities, one cooking with LPG and one with biomass, were enrolled. Spirometry was performed. Relationships between primary cooking fuel and spirometry measures, as raw values, Global Lung Initiative (GLI) percent predicted (pp), and GLI z-scores, were examined using linear regression. Effect modification by age was explored. Women were young (average age 33.3 years), with low education (median 5.0 years), and the majority had multiple sources of air pollution exposures. Overall, the lung function in both groups was poor [FEV1 z-score median -2.05, IQR (-2.64, -1.41). Biomass was associated with lower FEV1/FVC (raw values -7.0, p = 0.04; GLI pp -7.62, p = 0.05, and z-score -0.86, p = 0.05) and FEF25-75 (GLI pp -25.78, p = 0.05, z-score -1.24, p = 0.05), after adjusting for confounders. Increasing impairment in lung function with age was found among biomass users (p-interaction = 0.01). In conclusion, use of a clean fuel cookstove may improve lung function. These findings have broad implications for research and public policy.

Most of the health hazards in fried foods are related to unqualified frying oil and excessive oil content. In this study, the feasibility of using low-field nuclear magnetic resonance techniques (LF-NMR) for analysis of the water and oil contents in French fries, as well as simultaneous evaluation of frying oil quality during deep frying, was investigated. Three proton populations were identified and successfully assigned to water and oil relaxation signals. Significant correlation between the T2 relaxation parameters ( Awater and RCoil) and the water and oil content was acquired. MRI could visualize the changes of signal intensity and spatial distribution, as well as the internal structural changes during frying. Using the correlation model built by multiple regression analysis, the total polar compounds content of the frying oil could be successfully predicted by LF-NMR relaxation characteristics, which indicates that LF-NMR was an effective method to monitor the quality of frying oil.

Roadside food-vending shanties using coal cookstoves may be an important source of carbon monoxide (CO) exposure in megacities in India. The shanties are often small, congested and poorly ventilated, and very little is known about the level of human exposure to CO. Here, we assessed the level of exposure to CO in 25 roadside food-vending shanties using coal cookstoves in Kolkata, India. Portable electrochemical CO monitors were used to measure CO concentrations during peak and non-peak customer-periods in closed (blocked from three sides) and semi-closed (blocked from two sides) shanties. Measurements were taken where customers sit indoor about 5-7 ft away from the cookstoves. The shanties' ventilation rates were measured using tracer gas concentration-decay technique. Levels of blood carboxyhaemoglobin (COHb) and exhaled CO were estimated using regression models. The 1-hr time weighted average (TWA) indoor CO exposure levels ranged from 7.8 to 18.1 ppm during peak-periods, and 0.7-3.1 ppm during non-peak-periods. The exposure levels during peak-periods exceeded the USEPA's reference limit of 9 ppm in all cases in the closed shanties, and in 71% of cases in the semi-closed shanties. The ventilation rates ranged from 5.5 to 23.4 and 14.8 to 32.5 cubic feet per minute (cfm) per person for the closed and semi-closed shanties, respectively, indicating poor ventilation in some shanties. There was significant variation (p = 0.01) in the level of indoor CO exposure between peak and non-peak periods, and between shanty types. The estimated levels of blood COHb during peak and non-peak hours were 0.78 ±â€¯0.7% and 0.35 ±â€¯0.07%, respectively, that were within the normal physiological values in non-smokers.

Conditional cash transfers (CCTs)-cash payments provided to households or specific household members who meet defined conditions or fulfill certain behaviors-have been extensively used in India to encourage antenatal care, institutional delivery, and vaccination. This paper describes the social design and technical development of a low-cost, meal-counting stove use monitor (the Pink Key) that enables a CCT based on liquefied petroleum gas (LPG) usage and presents pilot data from its testing and the initial deployment. The system consists of a sensing harness attached to a two-burner LPG stove and an easily removable datalogger. For each cooking event with LPG, households receive 2 rupees-less than the cost of fuel, but enough to partially defray LPG refill costs. The system could enable innovative "self-monitoring" at a large scale-participants initiate the CCT by bringing their Pink Key to antenatal clinic visits, where care providers download data and initiate payments, and participants return the sensor to their stove at home. The system aligns with existing Indian programs to improve health among poor, pregnant women, and contributes a new method to encourage the use of clean cooking technologies.

BACKGROUND: Despite their potential health and social benefits, adoption and use of improved cookstoves has been low throughout much of the world. Explanations for low adoption rates of these technologies include prices that are not affordable for the target populations, limited opportunities for households to learn about cookstoves through peers, and perceptions that these technologies are not appropriate for local cooking needs. The P3 project employs a novel experimental design to explore each of these factors and their interactive effects on cookstove demand, adoption, use and exposure outcomes. METHODS: The P3 study is being conducted in the Kassena-Nankana Districts of Northern Ghana. Leveraging an earlier improved cookstove study that was conducted in this area, the central design of the P3 biomass stove experiment involves offering stoves at randomly varying prices to peers and non-peers of households that had previously received stoves for free. Using household surveys, electronic stove use monitors, and low-cost, portable monitoring equipment, we measure how prices and peers' experience affect perceptions of stove quality, the decision to purchase a stove, use of improved and traditional stoves over time, and personal exposure to air pollutants from the stoves. DISCUSSION: The challenges that public health and development communities have faced in spreading adoption of potentially welfare-enhancing technologies, like improved cookstoves, have highlighted the need for interdisciplinary, multisectoral approaches. The design of the P3 project draws on economic theory, public health practice, engineering, and environmental sciences, to more fully grasp the drivers and barriers to expanding access to and uptake of cleaner stoves. Our partnership between academic institutions, in the US and Ghana, and a local environmental non-governmental organization creates unique opportunities to disseminate and scale up lessons learned. Within the domain of public health, commonalities exist between the sanitation and cookstove sectors. Despite these commonalities and the grounds established for cross-learning between both sectors, however, there has not been much evidence of knowledge exchange across them to date. Our paper frames this as a missed opportunity for the cookstove sector, given the capacity for user-centred innovation and multi-scale approaches demonstrated in the sanitation sector. The paper highlights points of convergence and divergence in the approaches used in both sectors, with particular focus on behaviour change approaches that go beyond the level of the individual. The analysis highlights the importance of the enabling environment, community-focused approaches and locally specific contextual factors in promoting behavioural change in the sanitation sector. Our paper makes a case for the application of such approaches to cookstove interventions, especially in light of their ability to drive sustained change by matching demand-side motivations with supply-side opportunities. BACKGROUND: Air pollution from cooking with solid fuels is a potentially modifiable risk factor for increased blood pressure and may lead to eye irritation. OBJECTIVES: To evaluate whether a climate motivated cookstove intervention reduced blood pressure and eye irritation symptoms in Indian women. METHODS: Households using traditional stoves were randomized to receive a rocket stove or continue using traditional stoves. Systolic (SBP) and diastolic blood pressure (DBP), and self-reported eye symptoms were measured twice, pre-intervention and at least 124 days post-intervention in women > 25 years old in control (N = 111) and intervention (N = 111) groups in rural Karnataka, India. Daily (24-h) fine particle (PM2.5) mass and absorbance (Abs) were measured in cooking areas at each visit. Mixed-effect models were used to estimate before-and-after differences in SBP, DBP and eye symptoms. RESULTS: We observed a lower SBP (-2.0 (-4.5, 0.5) mmHg) and DBP (-1.1 (-2.9, 0.6) mmHg) among exclusive users of intervention stove, although confidence intervals included zero. Stacking or mixed use of intervention and traditional stoves contributed to a small increase in SBP 2.6 (-0.4, 5.7) mmHg) and DBP (1.2 (-0.9, 3.3) mmHg). Exclusive and mixed stove users experienced higher post-intervention reductions, on average, in self-reported eye irritation symptoms for burning sensation in eyes, and eyes look red often compared to control. Median air pollutant concentrations increased post-intervention in all stove groups, with the lowest median PM2.5 increase in the exclusive intervention stove group. CONCLUSIONS: Health benefits were limited due to stacking and lower-than-predicted efficiency of the intervention stove in the field. Stove adoption and use behavior, in addition to stove technology, affects achievement of health co-benefits. Carbon-financing schemes need to align with international guidelines that have been set based on health outcomes to maximize health co-benefits from cookstove interventions.

Both poor water, sanitation, hygiene (WaSH) and household air pollution (HAP) adversely affect the health of millions of people each year around the globe and specifically in developing countries. The objective of current work is to highlight the importance of HAP in parallel to WaSH for decision making to achieve better health specially in developing countries. There are examples, where developing countries are strengthening efforts to tackle the issue of poor water and sanitation such as 'Clean India Mission' was recently launched by the Government of India. However, there is lack of actions to address the issue related to HAP-to extend the coverage of clean fuel, efficient stoves and ventilated kitchens to the deprived population under the 'Clean India Mission'. Most of the rural household and urban slums in developing countries have only a single room, where people cook and sleep. This leads them to exposure to toxic HAP, which can be minimized by developing country specific indoor air quality guidelines and action framework. Hence, there should be policies to provide them not only the subsidy for clean fuel but also to build properly ventilated kitchens along with the promotion of clean toilets and water supplies. There is a need to strengthen global efforts, to jointly address the challenges associated with the risks related to WaSH and HAP in order to efficiently reduce the global burden of disease. Further, this will also help to timely attain the sustainable development goals for better health and environment.

Pneumonia is the leading cause of mortality for children under five years in sub-Saharan Africa. Household air pollution has been found to increase risk of pneumonia, especially due to exposure from dirty burning biomass fuels. It has been suggested that advanced stoves, which burn fuel more efficiently and reduce smoke emissions, may help to reduce household air pollution in poor, rural settings. This qualitative study aims to provide an insight into the household costs and perceived benefits from use of the stove in Malawi. It was conducted alongside The Cooking and Pneumonia Study (CAPS), the largest village cluster-level randomised controlled trial of an advanced combustion cookstove intervention to prevent pneumonia in children under five to date. In 2015, using 100 semi-structured interviews this study assessed household time use and perceptions of the stove from both control and intervention participants taking part in the CAPS trial in Chilumba. Household direct and indirect costs associated with the intervention were calculated. Users overwhelming liked using the stove. The main reported benefits were reduced cooking times and reduced fuel consumption. In most interviews, the health benefits were not initially identified as advantages of the stove, although when prompted, respondents stated that reduced smoke emissions contributed to a reduction in respiratory symptoms. The cost of the stove was much higher than most respondents said they would be willing to pay. The stoves were not primarily seen as health products. Perceptions of limited impact on health was subsequently supported by the CAPS trial data which showed no significant effect on pneumonia. While the findings are encouraging from the perspective of acceptability, without innovative financing mechanisms, general uptake and sustained use of the stove may not be possible in this setting. The findings also raise the question of whether the stoves should be marketed and championed as 'health interventions'.

Burning coal for winter heating has been considered a major contributor to northern China's winter haze, with the district heating boilers holding the balance. However a decade of intensive efforts on district heating boilers brought few improvements to northern China's winter air quality, arousing a speculation that the household heating stoves mainly in rural area rather than the district heating boilers mainly in urban area dominate coal emissions in winter. This implies an extreme underestimation of rural household coal consumption by the China Energy Statistical Yearbooks (CESYs), although direct evidence supporting this speculation is lacking. A village energy survey campaign was launched to gather the firsthand information on household coal consumption in the rural areas of two cities, Baoding (in Hebei province) and Beijing (the capital of China). The survey data show that the rural raw coal consumption in Baoding (5.04 × 103 kt) was approximately 6.5 times the value listed in the official CESY 2013 and exceeded the rural total of whole Hebei Province (4668 kt), revealing a huge amount of raw coal missing from the current statistical system. More importantly, rural emissions of particulate matter (PM) and SO2 from raw coal, which had never been included in widely distributing environmental statistical reports, were found higher than those from industrial and urban household sectors in the two cities in 2013, which highlights the importance of rural coal burning in creating northern China's heavy haze and helps to explain why a number of modeling predictions on ambient pollutant concentrations based on normal emission inventories were more bias-prone in winter season than in other seasons. We therefore recommend placing greater emphasis on the "missing" rural raw coal to help China in its long-term ambition to achieve clean air in the context of rapid economic development.

Many households use solid fuels for cooking and heating purposes. There is currently a knowledge gap in our understanding of the variations in indoor air quality throughout the household as most of the studies focus on the areas in the close proximity of the cookstove. A low-cost wireless particulate matter (PM) sensor network was developed and deployed in households in Raipur, India to establish the spatio-temporal variation of PM concentrations. The data from multiple sensors were acquired in real-time with a wireless system. Data collected from the sensors agreed well (R2 =0.713) with the reference data collected from a commercially available instrument. Low spatial variability was observed within the kitchen due to its small size and poor ventilation - a common feature of most rural Indian kitchens. Due to insufficient ventilation from open doors and windows, high PM concentrations similar to those found in the kitchen were also found in the adjoining rooms. The same household showed significantly different post-extinguished cookstove PM concentration decay rates (0.26mg/m3-min and 0.87mg/m3-min) on different days, owing to varying natural air exchange rates (7.68m3/min and 37.40m3/min).

BACKGROUND: Over 40% of the world's population relies on solid fuels for heating and cooking. Use of improved biomass cookstoves (ICS) has the potential to reduce household air pollution (HAP). OBJECTIVES: As part of an evaluation to identify ICS for use in Kenya, we collected indoor air and personal air samples to assess differences between traditional cookstoves (TCS) and ICS. METHODS: We conducted a cross-over study in 2012 in two Kenyan villages; up to six different ICS were installed in 45 households during six two-week periods. Forty-eight hour kitchen measurements of fine particulate matter (PM2.5) and carbon monoxide (CO) were collected for the TCS and ICS. Concurrent personal CO measurements were conducted on the mother and one child in each household. We performed descriptive analysis and compared paired measurements between baseline (TCS only) and each ICS. RESULTS: The geometric mean of 48-hour baseline PM2.5 and CO concentrations in the kitchen was 586µg/m3 (95% CI: 460, 747) and 4.9ppm (95% CI: 4.3, 5.5), respectively. For each ICS, the geometric mean kitchen air pollutant concentration was lower than the TCS: median reductions were 38.8% (95% CI: 29.5, 45.2) for PM2.5 and 27.1% (95% CI: 17.4, 40.3) for CO, with statistically significant relationships for four ICS. We also observed a reduction in personal exposures to CO with ICS use. CONCLUSIONS: We observed a reduction in mean 48-hour PM2.5 and CO concentrations compared to the TCS; however, concentrations for both pollutants were still consistently higher than WHO air quality guidelines. Our findings illustrate that ICS tested in real-world settings can reduce exposures to HAP, but implementation of cleaner fuels and related stove technologies may also be necessary to optimize public health benefits.

BACKGROUND: Access to, and sustained adoption of, clean household fuels at scale remains an aspirational goal to achieve sufficient reductions in household air pollution (HAP) in order to impact on the substantial global health burden caused by reliance on solid fuels. AIM AND OBJECTIVES: To systematically appraise the current evidence base to identify: (i) which factors enable or limit adoption and sustained use of clean fuels (namely liquefied petroleum gas (LPG), biogas, solar cooking and alcohol fuels) in low- and middle-income countries; (ii) lessons learnt concerning equitable scaling-up of programmes of cleaner cooking fuels in relation to poverty, urban-rural settings and gender. METHODS: A mixed-methods systematic review was conducted using established review methodology and extensive searches of published and grey literature sources. Data extraction and quality appraisal of quantitative, qualitative and case studies meeting inclusion criteria were conducted using standardised methods with reliability checking. FINDINGS: Forty-four studies from Africa, Asia and Latin America met the inclusion criteria (17 on biogas, 12 on LPG, 9 on solar, 6 on alcohol fuels). A broad range of inter-related enabling and limiting factors were identified for all four types of intervention, operating across seven pre-specified domains (i.e. fuel and technology characteristics, household and setting characteristics, knowledge and perceptions, financial, tax and subsidy aspects, market development, regulation, legislation and standards, and programme and policy mechanisms) and multiple levels (i.e. household, community, national). All domains matter and the majority of factors are common to all clean fuels interventions reviewed although some are fuel and technology-specific. All factors should therefore be taken into account and carefully assessed during planning and implementation of any small- and large-scale initiative aiming at promoting clean fuels for household cooking. CONCLUSIONS: Despite limitations in quantity and quality of the evidence this systematic review provides a useful starting point for the design, delivery and evaluation of programmes to ensure more effective adoption and use of LPG, biogas, alcohol fuels and solar cooking. The USAID/WASHplus project conducted a comprehensive assessment to understand consumer needs and preferences as they relate to increasing the uptake and consistent, exclusive, and correct use of improved cookstoves (ICSs) in Bangladesh. The assessment included household ICS trials, fuel and stove use monitoring, and consumers' perceived value of and willingness to pay for ICSs. Results showed that cooks appreciated and liked the ICS, but that no models met consumer needs sufficiently to replace traditional stoves. Initially, many preferred ICSs over traditional stoves, but this preference decreased over the 3-week trial period. Complaints and suggestions for improvement fell into two general categories: those that can be addressed through fairly simple modifications to the stove design, and those more appropriately addressed through point-of-purchase consumer education and follow-up from service agents or health outreach workers. Most households using the ICS realized fuel use reductions, although these were lower than expected, partly because of continued parallel traditional stove use. When given the option to purchase the stoves at market value, only one of 105 households did so; however, a separate assessment showed that 80% of participants (12 of 15 households) preferred to keep the stove rather than receive a cash buyout at market value. This indicates that users value the ICS when acquisition barriers are removed and highlights the need for better financing options.
