BACKGROUND: Idiopathic panuveitis is a diagnosis of exclusion that lacks distinguishing features on fluorescein and indocyanine green angiography. Choroidal hypoperfusion or ischaemia has been implicated in panuveitis of different aetiologies. In this study, we use enhanced depth imaging optical coherence tomography (OCT) to examine the choroid and its vasculature in patients with this disease. METHODS: In this retrospective, cross-sectional study, OCT-derived measurements of retinal and choroidal thickness were obtained after manual segmentation using custom software. Choroidal measurements were further subdivided into Haller's large vessel layer (HLVL) and Sattler's medium vessel layer (SMVL), and correlated with clinical parameters. RESULTS: Twenty-one eyes from 21 patients were included. A reduction in hypo-reflective spaces, corresponding to vascular lumens, was observed in HLVL. The mean thickness of both the choroid (233.7 ± 73.3 µm), and HLVL (167.8 ± 53.7 µm), was less than that previously reported for normal eyes. Choroidal thickness expressed as a ratio to retina thickness showed significant correlation to visual acuity (r = 0.58, p = 0.006). This correlation was maintained in the ratio between HLVL and retinal thickness (r = 0.56, p = 0.009), but not in SMVL to retinal thickness (r = 0.352, p = 0.12). CONCLUSIONS: This study reports novel OCT-derived parameters in patients with idiopathic panuveitis. We noted loss of hyporeflectivity in HLVL, and thinning of both HLVL and the choroid as a whole. The observed correlation between visual acuity and the ratio of choroidal to retinal thickness is a strong enhanced depth imaging (EDI)-OCT derived candidate for prospective validation in future studies.

AIM: We report a case of tuberculous choroidal granulomas in a patient with systemic lupus erythematosis (SLE) who developed miliary tuberculosis (TB) and bilateral granulomas while on immunosuppressive medication. METHODS: The case notes of this patient were reviewed. RESULTS: The patient was first seen for base-line ophthalmic screening before anti-tuberculous medication was started. The patient was subsequently followed up at 2 weeks, 4 weeks and 2 months with fundal examination and photography. One choroidal granuloma was found in the right eye while another peripheral lesion involved the left eye. Both resolved in response to anti-tuberculous medication over a 2-month period with resultant chorioretinal atrophy and scarring. CONCLUSION: This case demonstrates the need to screen immuno-suppressed SLE patients who have miliary TB for possible eye involvement.

BACKGROUND: The purpose of this study was to measure the activity of the sympathetic nervous system in patients with central serous chorioretinopathy (CSC) by power spectral analysis (PSA) of heart-rate (RR-interval) variability, a noninvasive method that reflects the balance of the sympathetic-vagal interaction. METHODS: The following four different groups of patients were measured; group 1, acute CSC (n = 11); group 2, acute recurrent CSC (n = 7); group 3, chronic persistent CSC (n = 4); group 4, complete remission of CSC (n = 9). The data recorded for these 31 patients (29 men and 2 women), with an average age of 44 years were compared with those noted for a group of 15 age-matched healthy individuals. The sympathetic-vagal balance is expressed by the ratio of the low-frequency component (LF) to the high-frequency component (HF) of the power spectrum. RESULTS: Significant differences in mean LF/HF ratios were found as follows for all but one of the sub-groups as compared with the normal controls (LF/HF = 1.1): group 1, LF/HF = 5.5 (P < 0.01); group 2, LF/HF = 5.4 (P < 0.05); group 3, LF/HF = 4.2 (P = 0.1); and group 4, LF/HF = 3.0 (P < 0.01). There was also a significant difference between active CSC and inactive CSC (P < 0.05). CONCLUSIONS: These results support the view that the pathogenesis of CSC is related to an increase in the sympathetic activity of the autonomic nervous system. Furthermore, the LF/HF ratios seem to correlate with the activity of the disease.
