RESUMO

A. Wade Boykin's scholarship has provided key insights into the psychological realities of racially minoritized people and catalyzed revolutionary changes in psychology and education. Combining insights from personal and research experiences, Boykin authored the foundational triple quandary (TQ), a framework describing how Black Americans must navigate the often conflicting values and priorities of dominant mainstream society, the heritage culture of Black communities, and dynamics associated with being racially minoritized. TQ describes the unique developmental challenges faced by Black children, for whom misalignment between home cultural socialization and U.S. schooling often leads to pathologizing mischaracterizations of their attitudes and behaviors, resulting in chronic academic opportunity gaps. Boykin used his training as an experimental psychologist to empirically test the validity and explanatory utility of the TQ framework and to determine whether Black cultural values could be leveraged to improve student learning. Focusing on cultural values such as expressive movement, verve, and communalism, studies with his collaborators consistently supported Boykin's framework and predictions for improving Black student achievement-related outcomes. Beginning in the early 2000s, Boykin and his colleagues began to scale the lessons of decades of empirical work into the talent quest model for school reform. The TQ and talent quest continue to evolve in their application, as scholars and practitioners have found them relevant to a diverse range of minoritized populations in American society and beyond. Boykin's work continues to bear on the scholarship, career outcomes, and day-to-day lives of many scholars, administrators, practitioners and students across disciplines and institutions. One of the heroes in American history, Associate Supreme Court Justice Thurgood Marshall (1908-1993) sought legal remedies against racial discrimination in education and health care. As director of the Legal Defense Fund (LDF) of NAACP from 1940 to 1961, his success in integrating law schools in Texas led to the first black medical student admitted to a state medical school in the South. Representing doctors and dentists needing a facility to perform surgery, the LDF brought cases before the courts in North Carolina that moved the country toward justice in health care. His ultimate legal victory came in 1954, Brown v. Board of Education of Topeka, the decision that declared racial segregation in public schools unconstitutional. In 1964, the LDF under Jack Greenberg, Marshall's successor as director, won Simkins v. Moses H. Cone Memorial Hospital, a decision that held that hospitals accepting federal funds had to admit black patients. The two decisions laid the judicial foundation for the laws and administrative acts that changed America's racial history, the Civil Rights Act of 1964 and the Social Security Act Amendments of 1965 that established Medicare and Medicaid. His achievements came during the hottest period of the American civil rights movement of the 1950s and 1960s. Well past the middle of the twentieth century, black Americans were denied access to the full resources of American medicine, locked in a "separate-but-equal" system woefully inadequate in every respect. In abolishing segregation, Marshall initiated the long overdue remedy of the unjust legacies of slavery and Jim Crow.

Global health is a multifaceted concept that entails the standardization of procedures in healthcare domains in accordance with a doctrine agreed upon by experts. This essay focus on the creation of health demonstration areas by the World Health Organisation (WHO) to establish core nodes for integrated state-of-the-art health services. Particular attention is paid to the historical importance of technical cooperative activities carried out by the WHO in regard to the implementation of health services, a long-term strategic move that contributed to the thematic upsurge of primary health care in the late 1970s.

Resumen Salud global es un concepto complejo que implica la normalización de los procedimientos de actuación sanitaria siguiendo una doctrina acordada por expertos. Este trabajo se ocupa del establecimiento de zonas de demostración sanitaria por la Organización Mundial de la Salud (OMS) a modo de núcleos de modernos servicios sanitarios integrados. Revisa el origen, las bases téoricas y los objetivos de esta técnica y examina diversas experiencias europeas durante los primeros veinte años de la OMS. Pone de manifiesto la importancia histórica de las actividades de cooperación técnica de la OMS en la puesta en marcha de servicios sanitarios, una estrategia de largo plazo que ayuda a entender la aparición de la atención primaria de salud a finales de la década de 1970.

INTRODUCTION: In 2016, individual training programs in regional anesthesiology and acute pain medicine (RA/APM) became eligible for accreditation by the Accreditation Council for Graduate Medical Education (ACGME), thereby culminating a process that began 15 years earlier. Herein, we review the origins of regional anesthesia training in the USA, the events leading up to accreditation and the current state of the fellowship. METHODS: We reviewed pertinent literature on the historical aspects of RA/APM in the USA, related subspecialty training and the formation and current state of RA/APM fellowship training programs. Additionally, a survey was distributed to the directors of the 74 RA/APM fellowships that existed as of 1 January 2017 to gather up-to-date, program-specific information. RESULTS: The survey yielded a 76% response rate. Mayo Clinic Rochester and Virginia Mason Medical Center likely had the first structured RA/APM fellowships with formalized curriculums and stated objectives, both starting in 1982. Most programs (86%), including ACGME and non-ACGME fellowships, came into existence after the year 2000. Six responding programs have or previously had RA/APM comingled with another subspecialty. Eight current programs originally offered unofficial or part-time fellowships in RA/APM, with fellows also practicing as attending physicians. DISCUSSION: The history of RA/APM training in the USA is a tortuous one. It began with short 'apprenticeships' under the tutelage of the early proponents of regional anesthesia and continues today with 84 official RA/APM programs and a robust fellowship directors' group. RA/APM programs teach skills essential to the practice and improvement of anesthesiology as a specialty.

The aim of this article is to document and understand how trends in educational variability and the gender gap in education developed jointly over time. Main questions include: Is the education distribution among women becoming more dispersed as their average attainment surpasses that of men? Is education variability among women higher than that of men? Does the reduction-and eventual reversal-of the gender gap in education go hand-in-hand with less educational variability overall? To answer these questions we first show how overall education variability can be decomposed into four clearly interpretable components (variability among women and men, educational advantage favoring women and favoring men). We subsequently document how these components have evolved over time in the world and its regions from 1950 to 2010 (with projections until 2040). Our findings suggest that (i) with education expansion, education variability tends to follow an inverted U-shape trajectory. (ii) The composition of education variability has been shifting dramatically over time; in particular (iii) variability among men was usually higher than variability among women until the turn of the millennium, from then onwards their educational attainment distributions have the same degree of dispersion. And (iv) while in the 1950s the educational advantage of men was by far the main contributor to education variability, nowadays the educational advantage of women has become the most important source of variability in high- and middle-income countries.

Physician-scientists are needed to continue the great pace of recent biomedical research and translate scientific findings to clinical applications. MD-PhD programs represent one approach to train physician-scientists. MD-PhD training started in the 1950s and expanded greatly with the Medical Scientist Training Program (MSTP), launched in 1964 by the National Institute of General Medical Sciences (NIGMS) at the National Institutes of Health. MD-PhD training has been influenced by substantial changes in medical education, science, and clinical fields since its inception. In 2014, NIGMS held a 50th Anniversary MSTP Symposium highlighting the program and assessing its outcomes. In 2016, there were over 90 active MD-PhD programs in the United States, of which 45 were MSTP supported, with a total of 988 trainee slots. Over 10,000 students have received MSTP support since 1964. The authors present data for the demographic characteristics and outcomes for 9,683 MSTP trainees from 1975-2014. The integration of MD and PhD training has allowed trainees to develop a rigorous foundation in research in concert with clinical training. MSTP graduates have had relative success in obtaining research grants and have become prominent leaders in many biomedical research fields. Many challenges remain, however, including the need to maintain rigorous scientific components in evolving medical curricula, to enhance research-oriented residency and fellowship opportunities in a widening scope of fields targeted by MSTP graduates, to achieve greater racial diversity and gender balance in the physician-scientist workforce, and to sustain subsequent research activities of physician-scientists.

A rich, collaborative program funded by the US NIH Fogarty program in 2004 has provided for a decade of remarkable opportunities for scientific advancement through the training of Brazilian undergraduate, graduate and postdoctoral students from the Federal University and Oswaldo Cruz Foundation systems at Albert Einstein College of Medicine. The focus of the program has been on the development of trainees in the broad field of Infectious Diseases, with a particular focus on diseases of importance to the Brazilian population. Talented trainees from various regions in Brazil came to Einstein to learn techniques and study fungal, parasitic and bacterial pathogens. In total, 43 trainees enthusiastically participated in the program. In addition to laboratory work, these students took a variety of courses at Einstein, presented their results at local, national and international meetings, and productively published their findings. This program has led to a remarkable synergy of scientific discovery for the participants during a time of rapid acceleration of the scientific growth in Brazil. This collaboration between Brazilian and US scientists has benefitted both countries and serves as a model for future training programs between these countries.

To think today in our Latin America and particularly in Colombia, where the overcoming of an important component of the armed conflict is at stake, which is derived from its unequal social and political structures. It is important that research be more than policiesleading to techno-bureaucracy of knowledge, and that, on the contrary, researchers can open the range of possibilities, which is a challenge as a traveler and eccentric kind. The present text, derived from a piece of research, presents how, among thechanges that have affected Higher Education in recent decades in the Western world and thus in Latin America, excels the imposition of another logic, which is proper of the private company, and has been accompanied by the implementationof a new language largely imported from the managerial area. This generates a few dreadful impacts on the way of conceiving the policies and planning when it comes to defining the same guidelines of Higher Education.

The present article, derived from a piece of research, provides some of themacroeconomic policies implemented in Colombia, imposed by the internationalfinancial institutions, the specific case being the International Monetary Fund and its extended agreement with Colombia, and how these have affected the development of the well-being of the population, making special emphasis on how the policies that have been imposed on Higher Education, have been affected.

En este ensayo nos proponemos compartir una serie de reflexiones sobre las condiciones existentes para la publicación de artículos académicos en Colombia. En particular nos interesa ofrecer elementos para el debate sobre la política de indexación de revistas académicas que ha definido el Departamento Administrativode Ciencia, Tecnología e Innovación para Colombia (Colciencias) por medio de su Sistema Nacional de Indexación y Homologación de Revistas Especializadas de CT+I Colombia (Publindex). En el ensayo destacamos algunas consecuencias de tal modelo, sustentado en la confianza y delegación que Colciencias tiene en un modelo de indexación establecido por dos empresas privadas y con ánimo de lucro: Thomson Reuters (Isi)™ y Scopus Elsevier™. El objeto principal del ensayo es contribuir al mejoramiento de las políticas editoriales agenciadas desde Colciencias.


This paper attempts to share a series of reflections on the existing conditions forthe publication of scholarly articles in Colombia. In particular, we are interested in providing some elements for the debate on the policy of indexing academic journals that the Administrative Department of Science, Technology and Innovation forColombia (Colciencias) and its National System of Indexing and Official Approval of Specialized Journals of CT+I Colombia (Publnidex), has defined, making emphasis on the consequences of this new model of indexing, which is supported in the almost exclusive recognition of journals that are indexed by two private profitmakingcompanies: Thomson Reuters (Isi)™ and Scopus-Elsevier.

El texto es una reflexión sobre la universidad como institución social, realizada a partir de varios estudios desarrollados en cuyos hallazgos se evidencia, en primer lugar, el interés de algunas estancias por convertirla en un escenario de manipulación y control, reduciéndola a la racionalidad de los organismos funcionales, frente ala dinámica propia que la universidad instaura como institución social cuya base se moviliza desde las significaciones imaginarias sociales. En este proceso, se muestra desde la lógica de la teoría de los imaginarios sociales, la forma como la universidad se configura e institucionaliza en una tensión constante, entre el sabery el control, entre la función y la institución y se dan las bases que la definen y posicionan como institución imaginaria social.


The text is a reflection on the University as a social institution, based on severalstudies developed on whose findings are made evident, firstly, the interest of some stances by turning it into a scenario of manipulation and control, by reducing it to the rationality of the functional agencies, against the very dynamic that the University establishes as a social institution whose base is mobilized from the social imaginary significations. In this process, it is shown from the logic of thetheory of the social imaginary, the manner how the University is configured and institutionalized at a constant pressure, between knowledge and control, between function and institution, and the foundations, which define it and position it as a social imaginary institution are defined.

El presente texto, recoge las discusiones y reflexiones derivadas de variasinvestigaciones en las que se describe, analiza y evidencia la participación de los actores que conforman la actual sociedad de la información como constantes de la educación superior en México, los paliativos que de ella se emanan, la transmisión de información y los procesos de formación actuales, el desafío de construir signos y símbolos cuando las señales ya no son controladas por el dispositivo pedagógico tradicional lo que exige, en un contexto cambiante, que la educación superior no sea más un interior autosuficiente, posibilitando la producción de subjetividades por las clases disciplinares y de control, legitimadas por medio de viejas y nuevas tecnologías.


The present text, includes discussions and reflections derived from several studies in which the participation of the actors that make up the current information society as constants of higher education in Mexico are described, analyzed, and madeevident, as well as the palliatives emanating from it, transmission of information and the current training processes, the challenge of building signs and symbols when the signals are no longer controlled by the traditional pedagogical device, which requires, in a changing context, that higher education is no longer a selfsufficient interior, making the production of subjectivity possible by the disciplinary and control classes, legitimized by means of old and new technologies.
