OBJECTIVE: Bipedalism was a significant milestone in the evolutionary development of Homo sapiens sapiens, influencing neocortical evolution and subsequent behavioral changes. Coordinated visual and sensory inputs are crucial for posture, environmental interaction, and surgical planning, with horizontal gaze being a pivotal parameter. This narrative review aims to explore various geometric measures used to assess horizontal gaze in patients, highlighting their applications in surgical planning. METHODS: A literature review was conducted in indexed databases using Mesh terms like "Cervical Vertebrae" and "Visual Fields" along with keywords such as "horizontal gaze" and "sagittal spine parameters." Among 477 initially identified articles, 41 were selected for inclusion after rigorous filtering. RESULTS: The most recognized method for assessing horizontal gaze is the Chin Brow Vertical Angle (CBVA), initially described in patients with ankylosing spondylitis. Clinical photography is employed as a tool for CBVA calculation, while other measures like McGregor slope and Slope of the Line of Sight have been considered as alternatives to CBVA. Each method presents its unique advantages and limitations. CONCLUSIONS: This review highlights the need for further research into horizontal gaze measurement methods. Developing novel approaches to determine horizontal gaze can significantly enhance surgical planning and, consequently, improve patient outcomes. The ongoing exploration of these geometric measures offers promising prospects for advancing the field and optimizing patient care.

BACKGROUND: Lower limb amputation does not affect only physical and psychological functioning but the use of a prosthetic device can also lead to increased cognitive demands. Measuring cognitive load objectively is challenging, and therefore, most studies use questionnaires that are easy to apply but can suffer from subjective bias. Motivated by this, the present study investigated whether a mobile eye tracker can be used to objectively measure cognitive load by monitoring gaze behavior during a set of motor tasks. METHODS: Five prosthetic users and eight able-bodied controls participated in this study. Eye tracking data and kinematics were recorded during a set of motor tasks (level ground walking, walking on uneven terrain, obstacle avoidance, stairs up and ramp down, as well as ramp up and stairs down) while the participants were asked to focus their gaze on a visual target for as long as possible. Target fixation times and increase in pupil diameters were determined and correlated to subjective ratings of cognitive load. FINDINGS: Overall, target fixation time and pupil diameter showed strong negative and positive correlations, respectively, to the subjective rating of cognitive load in the able-bodied controls (-0.75 and 0.80, respectively). However, the individual correlation strength, and in some cases, even the sign, was different across participants. A similar trend could be observed in prosthetic users. INTERPRETATION: The results of this study showed that a mobile eye tracker may be used to estimate cognitive load in prosthesis users during locomotor tasks. This paves the way to establish a new approach to assessing cognitive load, which is objective and yet practical and simple to administer. Nevertheless, future studies should corroborate these results by comparing them to other objective measures as well as focus on translating the proposed approach outside of a laboratory.

Skilled reading involves processing the upcoming word in parafoveal vision before it is fixated, leading to shorter fixations on that word. This phenomenon, parafoveal preview benefit, is a key component of theoretical models of reading; it is measured using the invisible boundary paradigm, in which reading times on a target word are compared for instances when preview is accurate and when the target word is masked while in the parafovea. However, parafoveal masks have been shown to induce unintentional processing costs, thereby inflating measures of preview benefit. The degraded mask has been explored as a potential solution to this problem, leading to mixed results. While previous work has analyzed the preview effect by comparing mean reading times on the target word, the present study provides a more comprehensive analysis by examining the distribution of the preview effect across target word fixation times for unrelated and degraded masks. Participants read sentences containing target words whose preview was either identical, unrelated, or degraded, and their eye movements were recorded. Analyses revealed that although there were no mean differences between reading times for the unrelated and degraded conditions, the pattern of the effects varied as a function of target word fixation times. Unrelated masks resulted in positively sloped generally linear delta plots, while degraded masks resulted in relatively flat delta plots for fixations longer than 200 ms. These differences suggest that different cognitive mechanisms are involved in the processing of the two mask types. Implications for understanding and measuring preview benefit are discussed.

SIGNIFICANCE: Fixation disparity is a small vergence error that does not disrupt fusion. Fixation disparity measures correlate with binocular symptoms. This article covers methodological differences between clinical fixation disparity measurement devices, findings when objective and subjective fixation disparities are compared, and the potential impact of binocular capture on fixation disparity measurements. Fixation disparity is a small vergence error that occurs in nonstrabismic individuals and does not disrupt fusion. This article reviews clinical fixation disparity variables and their clinical diagnostic value. Clinical devices that are used to measure these variables are described, as are studies in which the output from these devices has been compared. Methodological differences between the devices such as the location of the fusional stimulus, the rate at which judgments of dichoptic alignment are made, and the strength of the accommodative stimulus are all considered. In addition, the article covers theories of the neural origins of fixation disparity and control system models incorporating fixation disparity. Studies in which objective fixation disparities (oculomotor portion of fixation disparity assessed with an eye tracker) and subjective fixation disparities (sensory portion of fixation disparity assessed psychophysically with dichoptic Nonius lines) have been compared are also examined, and consideration is given to why some investigators find differences in these measures, whereas other investigators do not. The conclusion thus far is that there are likely complex interactions between vergence adaptation, accommodation, and the location of the fusional stimulus that lead to differences in objective and subjective fixation disparity measures. Finally, capture of the visual direction of monocular stimuli by adjacent fusional stimuli and the implications for fixation disparity measures are considered.

PURPOSE: Surgical skill assessment is essential for safe operations. In endoscopic kidney stone surgery, surgeons must perform a highly skill-dependent mental mapping from the pre-operative scan to the intraoperative endoscope image. Poor mental mapping can lead to incomplete exploration of the kidney and high reoperation rates. Yet there are few objective ways to evaluate competency. We propose to use unobtrusive eye-gaze measurements in the task space to evaluate skill and provide feedback. METHODS: We capture the surgeons' eye gaze on the surgical monitor with the Microsoft Hololens 2. To enable stable and accurate gaze detection, we develop a calibration algorithm to refine the eye tracking of the Hololens. In addition, we use a QR code to locate the eye gaze on the surgical monitor. We then run a user study with three expert and three novice surgeons. Each surgeon is tasked to locate three needles representing kidney stones in three different kidney phantoms. RESULTS: We find that experts have more focused gaze patterns. They complete the task faster, have smaller total gaze area, and the gaze fewer times outside the area of interest. While fixation to non-fixation ratio did not show significant difference in our findings, tracking the ratio over time shows different patterns between novices and experts. CONCLUSION: We show that a non-negligible difference holds between novice and expert surgeons' gaze metrics in kidney stone identification in phantoms. Expert surgeons demonstrate more targeted gaze throughout a trial, indicating their higher level of proficiency. To improve the skill acquisition process for novice surgeons, we suggest providing sub-task specific feedback. This approach presents an objective and non-invasive method to assess surgical competence.

Age differences in emotion perception are now well documented. However, a key limitation of many studies in this literature is the reliance on highly artificial tasks that lack context and consequently have poor ecological validity. This study reports two separate experiments that investigated age differences in emotion perception abilities using a highly contextualised film-based assessment along with a traditional emotion perception task. Experiment 2 additionally included a middle-aged sample and an assessment of eye-gaze patterns to the emotional films. The inclusion of eye-tracking in Experiment 2 was motivated by the fact that older adults consistently show visual biases to static emotion stimuli, yet it remains unclear whether biases also emerge in response to dynamic contextualised emotion stimuli. Experiment 1 identified age effects recognising displays of anger in the traditional emotion perception task but no age differences emerged on the film-based task. This finding was replicated in Experiment 2 with significant group differences on the traditional emotion perception task but no age differences on the film-based task. Experiment 2 also showed that there were no age differences in gaze patterns to these stimuli, showing for the first time that age-related visual biases to emotion stimuli may be task dependent. These findings highlight the fact that task-related features play a key role in the evaluation of age effects in emotion perception.

We present a dataset of eye-movement recordings collected from 60 participants, along with their empathy levels, towards people with movement impairments. During each round of gaze recording, participants were divided into two groups, each one completing one task. One group performed a task of free exploration of structureless images, and a second group performed a task consisting of gaze typing, i.e. writing sentences using eye-gaze movements on a card board. The eye-tracking data recorded from both tasks is stored in two datasets, which, besides gaze position, also include pupil diameter measurements. The empathy levels of participants towards non-verbal movement-impaired people were assessed twice through a questionnaire, before and after each task. The questionnaire is composed of forty questions, extending a established questionnaire of cognitive and affective empathy. Finally, our dataset presents an opportunity for analysing and evaluating, among other, the statistical features of eye-gaze trajectories in free-viewing as well as how empathy is reflected in eye features.

Fixational eye movements are a hallmark of human gaze behavior, yet little is known about how they interact between fellow eyes. Here, we designed, built and validated a split-field binocular scanning laser ophthalmoscope to record high-resolution eye motion traces from both eyes of six observers during fixation in different binocular vergence conditions. In addition to microsaccades and drift, torsional eye motion could be extracted, with a spatial measurement error of less than 1 arcmin. Microsaccades were strongly coupled between fellow eyes under all conditions. No monocular microsaccade occurred and no significant delay between microsaccade onsets across fellow eyes could be detected. Cyclotorsion was also firmly coupled between both eyes, occurring typically in conjugacy, with gradual changes during drift and abrupt changes during saccades.

For complex dynamic interactive tasks (such as aviating), operators need to continuously extract information from areas of interest (AOIs) through eye movement to maintain high level of situation awareness (SA), as failures of SA may cause task performance degradation, even system accident. Most of the current eye movement models focus on either static tasks (such as image viewing) or simple dynamic tasks (such as video watching), without considering SA. In this study, an eye movement model with the goal of maximizing SA is proposed based on Markov decision process (MDP), which is designed to describe the dynamic eye movement of experienced operators in dynamic interactive tasks. Two top-down factors, expectancy and value, are introduced into this model to represent the update probability and the importance of information in AOIs, respectively. In particular, the model regards sequence of eye fixations to different AOIs as sequential decisions to maximize the SA-related reward (value) in the context of uncertain information update (expectancy). Further, this model was validated with a flight simulation experiment. Results show that the predicted probabilities of fixation on and shift between AOIs are highly correlated ([Formula: see text] and [Formula: see text], respectively) with those of the experiment data.

The traffic environment of a tunnel group is more complex than that of a single tunnel, which increases the driving risk. The eye gaze behavior of drivers can be used to evaluate driving safety and comfort. To analyze the fixation characteristics of drivers in a single tunnel and tunnel group of mountainous expressways, an actual vehicle test is conducted. The test area has a total length of 160 km and 38 tunnels, including 8 tunnel groups and 16 single tunnels. In the test, the difference in the gaze time of five drivers between single tunnels and tunnel groups is compared. The k-means method is used to cluster driver's gaze points dynamically. Based on the Markov theory, the attributes related to gaze transfer are obtained. The results show that when tunnels are of short or medium length, there is no significant difference in the gaze time and gaze point transfer between the tunnel group and a single tunnel. In contrast, when tunnels have long or extra-long length, the repeated fixation probability and the two-step transition probability of looking back of a driver in a tunnel group are higher than those in a single tunnel. The design and management method of a single tunnel cannot be directly used, especially for extra-long tunnels located at the back of a tunnel group with a long upstream tunnel length and a short interval distance from the upstream tunnel. Therefore, it is necessary to focus on the design and management methods of tunnel groups.

The degree of spatial similarity between the gaze of participants viewing dynamic stimuli such as videos has been previously measured using metrics which are based on the NSS (Normalized Scanpath Saliency). Methods currently used to calculate this metric rely upon a numerical grid, which can be computationally prohibitive for a variety of otherwise useful applications such as Monte Carlo analyses. In the present work we derive a new analytical calculation method for the same metric that yields equal or more accurate results, but with speeds than can be orders of magnitude faster (depending on parameters). Our analytical method scales well with dimensionality, and could also be of use for other applications. The drawback is that it can become very slow if the number of participants in the study is very large or if the gaze sampling rate is high. We provide performance benchmarks for a Fortran implementation of our method, and make available the source code developed.

The averted gaze of others triggers reflexive attentional orienting in the corresponding direction. This phenomenon can be modulated by many social factors. Here, we used an eye-tracking technique to investigate the role of ethnic membership in a cross-cultural oculomotor interference study. Chinese and Italian participants were required to perform a saccade whose direction might be either congruent or incongruent with the averted-gaze of task-irrelevant faces belonging to Asian and White individuals. The results showed that, for Chinese participants, White faces elicited a larger oculomotor interference than Asian faces. By contrast, Italian participants exhibited a similar oculomotor interference effect for both Asian and White faces. Hence, Chinese participants found it more difficult to suppress eye-gaze processing of White rather than Asian faces. The findings provide converging evidence that social attention can be modulated by social factors characterizing both the face stimulus and the participants. The data are discussed with reference to possible cross-cultural differences in perceived social status.

In the current education environment, learning takes place outside the physical classroom, and tutors need to determine whether learners are absorbing the content delivered to them. Online assessment has become a viable option for tutors to establish the achievement of course learning outcomes by learners. It provides real-time progress and immediate results; however, it has challenges in quantifying learner aspects like wavering behavior, confidence level, knowledge acquired, quickness in completing the task, task engagement, inattentional blindness to critical information, etc. An intelligent eye gaze-based assessment system called IEyeGASE is developed to measure insights into these behavioral aspects of learners. The system can be integrated into the existing online assessment system and help tutors re-calibrate learning goals and provide necessary corrective actions.

Subject calibration has been demonstrated to improve the accuracy in high-performance eye trackers. However, the true weight of calibration in off-the-shelf eye tracking solutions is still not addressed. In this work, a theoretical framework to measure the effects of calibration in deep learning-based gaze estimation is proposed for low-resolution systems. To this end, features extracted from the synthetic U2Eyes dataset are used in a fully connected network in order to isolate the effect of specific user's features, such as kappa angles. Then, the impact of system calibration in a real setup employing I2Head dataset images is studied. The obtained results show accuracy improvements over 50%, probing that calibration is a key process also in low-resolution gaze estimation scenarios. Furthermore, we show that after calibration accuracy values close to those obtained by high-resolution systems, in the range of 0.7°, could be theoretically obtained if a careful selection of image features was performed, demonstrating significant room for improvement for off-the-shelf eye tracking systems.

SIGNIFICANCE: This study establishes normative data and a testing procedure for the oculomotor assessment tool. The oculomotor assessment tool standardizes visual targets for the Vestibular/OculoMotor Screening assessment and provides additional metrics that may aid in the differentiation between those with normal and those with abnormal oculomotor function potentially caused by a concussion. PURPOSE: This study aimed to assess the oculomotor endurance of healthy participants with no self-reported history of concussions using the oculomotor assessment tool. METHODS: Healthy participants (n = 376, average age of 20.4 years, range of 11 to 34 years, with no self-reported history of concussions) were recruited to perform the following three tasks for 60 seconds each: (1) horizontal saccades, (2) vertical saccades, and (3) vergence jumps. The participants were instructed to alternate visual fixation between two targets for each of the tasks as fast as they could without overshooting or undershooting the visual target. The differences in the number of eye movements between the initial and latter 30 seconds of the 1-minute test were analyzed. RESULTS: A statistical difference (P < .001) was observed in the number of eye movements for all three tasks (horizontal saccades [70 ± 15 for initial 30 seconds, 63 ± 13 for latter 30 seconds], vertical saccades [68 ± 14, 63 ± 13], and vergence jumps [43 ± 11, 39 ± 10]) between the initial and latter 30 seconds. No significant differences were identified in the number of eye movements or the change in eye movements between the initial and latter 30 seconds based on sex. CONCLUSIONS: These results establish a normative database for various eye movements. These data could potentially be used to compare different patient populations who have binocular endurance dysfunctions potentially due to traumatic brain injury, such as patients with concussion(s).

Microtropia describes a primary ocular deviation of less than 10 prism diopters associated with harmonious anomalous retinal correspondence and reduced stereopsis. It is routinely accepted that children with microtropia are less likely to achieve equal vision following occlusion therapy than those with bifoveal fixation. The most commonly used methods of diagnosing a microtropia are the 4 diopter prism test (4∆PT) and assessment of ocular fixation. This study examines the agreement between the two tests. One hundred and twelve typically developing children without a manifest strabismus who were able to undertake a linear visual acuity test and had two or more lines of anisoacuity following refractive adaption to their full cycloplegic correction underwent assessment of the 4∆PT and ocular fixation along with their routine orthoptic examination. One hundred and twelve children (46 boys and 66 girls) attending the Orthoptic department who fitted the above criteria were included in the analysis. The mean age at examination was 6 years. 80.3% had anisometropia of at least 1.25 diopters. The 4∆PT indicated a microtropia in 74 cases, whereas assessment of fixation indicated a microtropia in 68 cases. In 88 cases (78.6%), the results of the two tests agreed. Analysis found only moderate agreement between the two tests (k = 0.540 (CI 0.379-0.700)). Logistic regression analysis comparing those children where the two tests agreed with those where they disagreed found no difference in the level of anisoacuity (p = 0.7823), degree of anisometropia (p = 0.9385), the vision in the worst eye (p = 0.5260), the refractive error in the "worst" eye (p = 0.865), or the age at the time of testing (p = 0.4485) between the two groups. When assessing for a microtropia, it was found that not all children who elicit a central suppression response on the 4∆PT are found to be fixing eccentrically and vice versa. This could potentially have implications for the treatment of their amblyopia. It is important not to rely on just one test at one time to make the diagnosis of microtropia. Rather, if one or other test indicates a microtropia the first time they are attempted, this should be reassessed regularly as treatment progresses and certainly before treatment is stopped and suboptimal visual acuity is accepted.

PURPOSE: To determine if the stress of normal eye movements results in gaze-induced globe deformations, vitreous chamber axial length and vitreous chamber axial volume (VCAV) change in highly myopic eyes. METHODS: A prospective imaging study was performed on 82 eyes of 43 patients with high myopia (>27 mm of axial length) with a clinical diagnosis of staphyloma. Three-dimensional MRI scans were acquired while subjects gazed in five directions (primary, nasal, temporal, superior and inferior). Surface renderings were generated, and a processing pipeline was created to automate alignment of the eye and to measure VCAV within 5.5 mm of the visual axis for each eye in every gaze. The degree of gaze-induced globe deformation was determined by calculating the Dice coefficient to assess the degree of overlap of the sclera at each eccentric gaze with that found in primary gaze. Each eccentric gaze VCAV was compared to VCAV in primary gaze using a fixed-effects regression allowing for subject-specific and eye-specific effects. RESULTS: The Dice coefficient showed significant gaze-induced eye shape changes in all gazes (all p<0.0001). There were no statistically significant gaze-induced VCAV changes when comparing primary gaze to nasal, temporal or upgaze. However, when changing from primary to downgaze, VCAV was increased by +4.79 mm3 (p=0.002, 95% CI 1.71 to 7.86). CONCLUSION: Significant gaze-induced globe deformation was noted in all gazes, but a reversible, instantaneous VCAV increase occurred only in downgaze, which is consistent with studies supporting the association of environmental factors such as near work with myopia development and progression.

There is a growing understanding that the parafoveal preview effect during reading may represent a combination of preview benefits and preview costs due to interference from parafoveal masks. It has been suggested that visually degrading the parafoveal masks may reduce their costs, but adult readers were later shown to be highly sensitive to degraded display changes. Four experiments examined how preview benefits and preview costs are influenced by the perception of distinct parafoveal degradation at the target word location. Participants read sentences with four preview types (identity, orthographic, phonological, and letter-mask preview) and two levels of visual degradation (0% vs. 20%). The distinctiveness of the target word degradation was either eliminated by degrading all words in the sentence (Experiments 1a-2a) or remained present, as in previous research (Experiments 1b-2b). Degrading the letter masks resulted in a reduction in preview costs, but only when all words in the sentence were degraded. When degradation at the target word location was perceptually distinct, it induced costs of its own, even for orthographically and phonologically related previews. These results confirm previous reports that traditional parafoveal masks introduce preview costs that overestimate the size of the true benefit. However, they also show that parafoveal degradation has the unintended consequence of introducing additional costs when participants are aware of distinct degradation on the target word. Parafoveal degradation appears to be easily perceived and may temporarily orient attention away from the reading task, thus delaying word processing.

PURPOSE: To investigate saccadic movements in subjects with eccentric fixation due to a deep central scotoma in Stargardt disease (STGD). METHODS: We studied 10 patients with STGD and 10 healthy subjects (control group). Saccadic movements of all the 20 subjects were assessed by using the eye tracker technique Tobii Glasses Pro 2. Standard measurements of reading ability (MNREAD charts), visual acuity (ETDRS charts), contrast sensitivity (Pelli-Robson charts), reading contrast threshold and speed (REX charts), retinal sensitivity and stability and localization of the fixation (MP1 fundus perimetry) were obtained in all subjects. RESULTS: The saccadic movements time was significantly slower in STGD than in healthy subjects (699 ± 193 ms vs 299 ± 40 ms, p < 0.001). When STGD patients moved fixation to the target localized in retinal scotomatous areas, the movement was significantly slower compared to non scotomatous areas in the retina (1103 ± 798 ms vs 524 ± 187 ms, p = 0.039). There was a trend toward a correlation between slow saccadic movements in STGD subjects and the reading performance indices, although statistical significance was not achieved. CONCLUSION: Ocular saccades guided by eccentric fixation in STGD patients are significantly slower than in the control group, especially when the target corresponds to retinal areas with a deep scotoma. These results can explain the worse reading performance in STGD subjects, in particular when a non-viewing area on the right part of the text is present.

The gaze-cueing effect is a robust phenomenon which illustrates how attention can be shaped by social factors. In four experiments, the present study explored the interaction between the ethnic membership of the participant and that of the face providing the gaze cue. Firstly, we aimed to further investigate the differential impact of White, Black, and Asian faces on the gaze-cueing effect in White individuals. Secondly, we aimed to explore, for the first time, the impact of faces belonging to different ethnicities on gaze cueing in Chinese participants. The results allowed to rule out alternative accounts and showed that White participants exhibit a gaze-cueing effect for White and Asian faces, but not for Black faces, consistent with previous studies. As regards Chinese participants, the overall findings suggested a stronger gaze-cueing effect for White faces than for Asian faces. The results are discussed with reference to differences in the perceived social status of the various groups, pointing to the need of taking into account different cultural contexts.
