INTRODUCTION: Loss of natural teeth is a debilitating and irreversible process, which leads to functional, cosmetic and psychological morbidities. Dental implant serves as one of the options of tooth replacement, which are stronger, functionally effective and more durable. However, public awareness regarding it has been found to be low in Asian countries. This study was designed to know the status of knowledge and awareness of dental implant. METHODS: Seventy nine patients visiting dental department of Patan Academy of Health Sciences were included in this study through non-probability sampling. Data on knowledge and awareness regarding dental implants were collected through a survey after ethical approval from Institutional Review committee of Patan Academy of Health Sciences. Data entry was done in Epidata and analyzed with the help of Statistical Package of Social Sciences version 20. RESULTS: Total 33 (41.8%) of study populations choose dental implant as alternative for replacing teeth, 22 (30.4%) choose fixed dental prosthesis. 36 (45.6%) and 32 (40.5%) of population stated that long treatment time and high cost respectively as the disadvantages of dental implants. Only 12 (15.2%) were aware that dental implants are anchored in jaw bone. The source of information about dental implants was dentists for 48 (60.8%) of the patients followed by friends, media and medical doctors. CONCLUSIONS: Within the limitations of this study, we conclude that awareness of patients attending tertiary care center of Kathmandu valley is low. Hence, there is a need of organizing dental education programs to create awareness about dental implants.

CONTEXT: India suffers from a heavy burden of oral diseases. Dental implants (DIs) are prescribed widely by the dental practitioners to replace lost natural teeth. There is no estimate, however, to determine the number of DIs or the number of people with peri-implantitis or the failure of implants after placement. In this modeling study, we attempted to estimate the prevalence of adult Indians who would choose DI in the near future and to calculate the peri-implantitis and failure of DI. MATERIALS AND METHODS: Using the Global Burden of Disease database (2016), the number of dental caries in permanent dentition, periodontal diseases, and edentulism was obtained. Empirical assumptions of patients with anodontia in urban and rural areas who opted for DI, percentage of implants placed, the affordability factors, and mathematical models for DI were formed and executed. Peri-implantitis and survival data from literary evidence were collated. RESULTS: Based on assumptions, 909,643 Indians, (830,231-858,703) would choose DI. Estimated number of peri-implantitis would be 145,543-254,700 and estimated number of failures should be 50,940-79,412 in the near future. CONCLUSIONS: In spite of the high economic challenge and the risks or complications of peri-implantitis, DIs are gaining prominence. It is the dentists' burden to face the renewed challenges due to emerge and provide remedial measures.

BACKGROUND: The mandibular canal and its content represent the vital structure, which can complicate dentoalveolar surgical procedures in the posterior region of the mandible. The purpose of the present study was to determine the path the mandibular canal takes in relation to the horizontal and the vertical anatomical reference planes in edentate subjects, in order to minimize the risk of affecting its neurovascular content during various oral surgery procedures. MATERIALS AND METHODS: Morphometric evaluations were performed on 12 dried fully edentulous human mandibles and on cone-beam computed tomography (CBCT) cross-sectional images of the mandible, from 20 patients with either partial or complete edentulism. Both methods were utilized, in three target areas (corresponding to the second premolar, to the first molar and to the second molar regions), in order to measure the distance between the mandibular canal and the following reference points: (i) the lateral (buccal) surface of the mandible (MC-BS distance); (ii) the medial (lingual) surface of the mandible (MC-LS distance); (iii) the alveolar surface of the mandible (MC-AS distance). The results were statistically processed in Stata MP/13 software package using analysis of variance (ANOVA) test. RESULTS: The mandibular canal crossed the trabecular bone from the posterior towards the anterior, and from the lingual towards the buccal, reaching the premolar region, distal to the mental foramen, where it was located in the centre of the trabecular bone, main topographic pattern encountered in 27 (84.37%) of the cases. In five (15.63%) of the cases, in the premolar region, the mandibular canal was located near the buccal cortical plate. The mandibular canal descended from the second molar region towards the premolar region, main topographic pattern found in 28 (87.5%) of the cases. In four (12.5%) cases, the mandibular canal had a descending trajectory in the molar regions and it took a slightly ascending course in the premolar region. CONCLUSIONS: According to the results, the second molar region represents the highest risk area in the accidental injury to the content of the mandibular canal, during various oral surgery procedures.

Objetivo: avaliar o torque de inserção de um novo desenho de implante. Material e métodos: pacientes foram recrutados para instalação de implantes, no período de março a dezembro de 2015. Dados como o diâmetro, o comprimento do implante, a sequência de fresagem, a área de instalação e o torque atingido foram considerados. A avaliação do torque foi realizada com um torquímetro cirúrgico. Resultados: foram avaliados 82 implantes, com diâmetros entre 3,5 mm e 5 mm, e comprimentos entre 7 mm e 16 mm, sendo instalados 46 implantes na mandíbula e 36 implantes na maxila. Os torques (média ± desvio-padrão) alcançados foram de 53 ± 9,9 Ncm para maxila e 57 ± 11,3 Ncm para mandíbula. Conclusão: o sistema de implantes Novo Colosso promoveu torque de inserção adequado, com boa estabilidade primária nos implantes.


Objective: to evaluate the insertion torque of a new implant design. Material and methods: patients were recruited for implant placement from March to December 2015. Data such as diameter, implant length, drilling sequence, installation area, and torque reached were considered. Torque evaluation was performed with a surgical torque wrench. Results: 82 implants were evaluated, with diameters between 3.5 mm and 5.0 mm, and lengths between 7 mm and 16 mm, with 46 implants in the mandible and 36 implants in the maxilla. The torques (mean ± standard deviation) reached 53 ± 9.9 Ncm for maxilla and 57 ± 11.3 Ncm for mandible. Conclusion: the Novo Colosso implant system promotes adequate insertion torque with good primary implant stability.

OBJECTIVE: The aim of this study was to evaluate the influence of implant diameter, length and shape on a surrogate parameter of implant survival; i.e. the implant return rate in a big data analysis. MATERIALS AND METHODS: A retrospective study was conducted and the factors influencing the success rates of 69,377 sold implants over a seven-year period were evaluated. The osseointegration program of a reseller provides reliable data of a single country. Implant loss rates were investigated using logistic regression models and regressed by implant type, diameter, and length. RESULTS: The return rate of 69,377 sold implants was 2.78% and comparable to implant loss rates in previous published prospective studies as its surrogate parameter. A total of 80% of implant returns had occurred within 157 days, and an additional 15% within 750.25 days. Diameters of 3.8 to 5.0mm showed the lowest return rates with its bottom in the 4.3mm implant whilst 6.0mm implants had significantly higher return rates. In comparison to the most sold implant length (13mm) shorter implants showed significantly higher early return rates. CONCLUSIONS: The study provides evidence that in cases of standard indications and sufficient bone, the use of screw typed dental implants with 3.8 or 4.3 diameter and 11 or 13 mm length shows the lowest implant return rates. Other implants may be selected only in specific indications.

PURPOSE: Loss of teeth is associated with a significant reduction in quality of life. The aim of this prospective multicenter study was to assess the impact of dental implants on oral health-related quality of life (OHRQoL). MATERIAL AND METHODS: Patients with various kinds of indications for dental implants ranging from single-tooth loss to edentulous jaws were included. Quality of life related to dental implants was assessed through the Oral Health Impact Profile (OHIP-G 21), which has a score from 0 to 20 in healthy patients. RESULTS: In total, 8689 patients from 17 centers from 2009 to 2014 were enrolled in the study. The sex distribution was almost even (53.3% men, 46.7% women). The most frequent indications for the insertion of dental implants were free-end gaps (30.6%) and posterior single-tooth gaps (27%). In all, 12.4% of patients had an edentulous jaw. For all indications, patients reported significant changes in mean OHIP scores after prosthetic reconstruction. The most significant improvements in the OHIP score occurred in the groups of patients with edentulous jaws (pretreatment score: 42.3) after prosthodontic reconstruction (score: 24.8) and in the patient group with an anterior single-tooth gap (pretreatment score: 36.4) after prosthodontic reconstruction (score: 24.8). CONCLUSION: The insertion of dental implants and prosthodontic rehabilitation led to an improved OHRQoL for patients with all indications for dental implants, with the most significant improvements in patients with edentulous jaws and anterior single-tooth gaps.

RESUMO

In the next few years there will be a great increase in the percentage of the population aged over 65. Not only will they have more teeth than previous generations, but also a large number of implants. The increase in age is accompanied by an increase in the prevalence and incidence of periodontal diseases. In addition, there is a decrease in manual dexterity and an increase in co-morbidity and medications affecting the oral cavity. Dental care in aged care facilities can be poor and access to dental professionals difficult. This article discusses these issues.

BACKGROUND: Objectives were to: (1) determine whether and how often general dentists (GDs) provide specific dental procedures; and (2) test the hypothesis that provision is associated with key dentist, practice, and patient characteristics. METHODS: GDs (n = 2,367) in the United States National Dental Practice-Based Research Network completed an Enrollment Questionnaire that included: (1) dentist; (2) practice; and (3) patient characteristics, and how commonly they provide each of 10 dental procedures. We determined how commonly procedures were provided and tested the hypothesis that provision was substantively related to the three sets of characteristics. RESULTS: Two procedure categories were classified as "uncommon" (orthodontics, periodontal surgery), three were "common" (molar endodontics; implants; non-surgical periodontics), and five were "very common" (restorative; esthetic procedures; extractions; removable prosthetics; non-molar endodontics). Dentist, practice, and patient characteristics were substantively related to procedure provision; several characteristics seemed to have pervasive effects, such as dentist gender, training after dental school, full-time/part-time status, private practice vs. institutional practice, presence of a specialist in the same practice, and insurance status of patients. CONCLUSIONS: As a group, GDs provide a comprehensive range of procedures. However, provision by individual dentists is substantively related to certain dentist, practice, and patient characteristics. A large number and broad range of factors seem to influence which procedures GDs provide. This may have implications for how GDs respond to the ever-changing landscape of dental care utilization, patient population demography, scope of practice, delivery models and GDs' evolving role in primary care.

PURPOSE: The aim of the study was to analyze the characteristics of implant dentistry claims in Italy based on insurance company technical reports for malpractice claims. MATERIALS AND METHODS: One hundred twenty-one technical reports of cases of professional malpractice in implant dentistry between 2006 and 2010 were included in the study. Data included the sex and age of the patient and dentist, the kind of negligence claimed, and the damages awarded as a consequence of the alleged misconduct. RESULTS: Of the cases examined in this study, 9.9% went to court. The patients were female in 73.6% of the cases. Most of the technical errors were committed during implant insertion (82.6%). In 50.4% of cases, the technical error involved the surrounding structures, such as damage to the inferior alveolar nerve (32.2%) or the lingual nerve (2.5%), invasion of the maxillary sinus (9.1%), or pulpal dental necrosis in adjacent teeth (6.6%). Incomplete clinical documentation was apparent in 54.5% of cases. In 9.9% of cases, a civil suit had already been filed before a visit, and medicolegal advice from the insurance expert had been procured. CONCLUSION: The discrepancy between the total number of cases examined and those that went to court indicates that implant malpractice claims in Italy are most often settled out of court. The large number of intraoperative errors seen and the high proportion of injuries to surrounding structures suggest that implant dentists would benefit from further specific training. Also, clinical documentation vital to a defense against any claims relating to professional misconduct was incomplete or absent in more than half of the cases.

BACKGROUND: Patient-related variables such as cost of treatment, length of the treatment period, and comfort provided by the interim prosthesis when treatment planning for full-arch rehabilitation are often neglected in dental publications. METHODS: Two patient cohorts were followed up longitudinally in this study: the "All-on-4 treatment concept group" and the "historical group." The number of implants, total treatment time, number of surgical procedures, number of sinus grafts, necessity for immediate provisional implants, adjusted cost associated for treatment in each group, and the quality of interim prosthesis were compared. RESULTS: The total adjusted cost for patients receiving All-on-4 treatment concept averaged at $42,422 ± 3860 (&OV0556;31,392 ± 2856), whereas the mean total adjusted cost for the historical group was $57,944 ± 20,198 (&OV0556;42,879 ± 2113) (P = 0.01). The difference in cost had a mean value of $7307 (&OV0556;5407) per jaw. Factors associated with complexity of treatment and patient comfort, such as the quality of interim prosthesis, number of surgeries, and duration of treatment time, all significantly favored the All-on-4 treatment concept group in comparison with conventional treatment modalities. CONCLUSIONS: When implant rehabilitation of the total jaw is sought, the All-on-4 treatment concept should be considered the least costly and least time consuming treatment option.

The aim of this study was to investigate the production of mandibular implant-supported fixed and removable prostheses among prosthodontic specialists in Sweden and to compare the results with findings in a similar study made in 2001 (17). Questionnaires regarding treatment with mandibular implant prostheses during 20011 were mailed to all specialists in prosthodontics in Sweden (n = 156, according to available data). Of the 156 questionnaires, 129 (83%) were returned and of those 114 were completed. The reported number of treatments with mandibular implant-supported prostheses varied much among the specialists. Fixed implant prostheses were more common than overdentures (means 11 and 3, median values 8 and 2, respectively). However, the range was large for both alternatives. Ten (9%) of the specialists reported no treatment with fixed implant prostheses while 29 (25%) had not made any implant overdenture during 2011. The most common anchorage system for overdentures in 2011 (as well as in 2001) was two un-splinted implants with ball attachments or Locator abutments. The most common reasons for choosing overdenture treatment instead of a fixed implant prosthesis in 2011 were the reduced cost and the patient's main wish to improve denture retention. A majority of the prosthodontists (58%) reported that patients with implant overdentures were as satisfied as those with fixed implant-supported prostheses, whereas 40% claimed they were less satisfied. Two respondents (2%) considered that overdenture patients were more satisfied than those with a fixed prosthesis. It can be concluded that the general attitude among Swedish prosthodontists towards implant overdentures has not changed much during the 10-year period between the present and the previous investigation.An overdenture is still a seldom-used option in implant treatment of patients with edentulous mandibles in Sweden. Instead, a fixed implant-supported prosthesis continues to be the preferred option,

PURPOSE: The National Institute for Health and Welfare in Finland (THL) has maintained the Finnish Dental Implant Register since April 1994. The aim of this study was to use the Dental Implant Register to assess the influence of patient characteristics and background factors on dental implant removals in Finland from 1994 to 2012. MATERIALS AND METHODS: THL granted permission to access the Finnish Dental lmplant Register (1994 to 2012) and assess the following factors: total implant placements, total implant removals, time from implant placement to removal, implant types, implant lengths, placed and failed implants by jaw and tooth type, and patient sex and age. RESULTS: A total of 198,538 dental implants (51 different types) were placed between 1994 and 2012. A total of 3,318 (1.7%) of the placed implants were removed during the observation period. A total of 1,856 (1.8%) of the placed implants were removed from the maxilla and 1,462 (1.5%) from the mandible (P < .001). A slight difference in the sex distribution concerning implant failures (3.1% in men vs 2.3% in women) was observed. The median removal time was 247 days postoperatively (range 0 to 11,383 days), and one-third of the implants were removed within the first 142 days. Most of the placed implants were 10 mm or longer (93.3%) with 12 mm being the most commonly used length (23.9%). Shorter implants (8 mm or less) were removed more often (2.5% removal rate) than 9 mm or longer implants (1.5% removal rate). The IMZ Implant system demonstrated the highest overall removal rates (8.5%), while Brånemark Nobel Direct (0.6%) demonstrated the lowest removal rates. Of the most commonly placed implant types (together comprising 60.9% of all implants placed), the Straumann (1.2%) and the Astra Implant (1.2%) systems demonstrated equally low removal rates. CONCLUSIONS: The overall dental implant removal rates in Finland are low. Only slight differences in gender and implant position were observed.

BACKGROUND: The FDI World Dental Federation is engaged in a global consultation process to assess the potential challenges and impacts of the introduction of a preventive model to existing systems for caries management. OBJECTIVES: The aims of this study were to evaluate the quality of dental disease data collected with the International Caries Detection and Assessment System (ICDAS) index and dentists' perceptions with regard to the collection of data using the 'European Global Oral Health Indicators Development' (EGOHID) survey methods, and to estimate the mean time required for completion of the dental records according to the practitioners' perceptions. MATERIALS AND METHOD: The data - 2877 clinical examinations and 2877 individual assessments - were collected in 2008 using a network of 146 sentinel dentists in eight European countries. A clinical survey was completed for each participant and the dentist gave a detailed assessment of each patient investigated. RESULTS: This study shows that practitioners' perceptions have an impact on the mean time required to complete the dental record. Mistakes originate from dentists' attempts to simplify the completion of many boxes. This results in a larger number of missing data than of error codes. These missing data have an effect on the time required for information collection. CONCLUSION: The quality of the data collected will allow the establishment of recommendations based on this method.

With the progressive realisation of the single European market, public interest has been directed towards cross-border healthcare services to an increasing extent. More and more dentures are being imported into Germany from foreign countries. Furthermore, patients are becoming ever more mobile, travelling to other countries to receive prosthetic treatment from dentists. The objective of this evaluation was to determine by means of a dedicated questionnaire the patients' individual preferences for foreign dentures and the potential savings. 1 368 individuals between the ages of 30 and 75 years were interviewed within a representative omnibus survey. The evaluation of the individual willingness-to-pay included 4 treatment scenarios, which were assessed by the participants in a "bidding game". Participants could choose between a "crown scenario" and an "implant scenario", both with the subcategories "foreign dentures" and "dental tourism". The direct comparison revealed a preference for the "foreign dentures" option over "dental tourism". Average willingness-to-pay for the dental tourism option in the crown scenario was calculated as 80 Euro, and in the implant scenario as 280 Euro less in comparison with the willingness-to-pay for the foreign dentures option. The willingness to switch to a less expensive dentist was one of the main determinants in the causal explanation for the variance in willingness-to-pay. Quality proved to be the decisive criterion and was indicated by 92.4% participants. A lower price for dentures played a subordinate role and was only stated as the decisive factor by 31.1% participants. In conclusion, the results clearly indicate that the decision for or against foreign dentures and the extent of willingness-to-pay depends on a range of criteria, of which "price" is only one and not the decisive factor.

STATEMENT OF PROBLEM: CAD/CAM template-guided surgery has gained attention as a method of improving the predictability of dental implant placement. However, due to possible variations during the manufacturing process and in the robustness of the template design, a virtual prediction of the potential positioning of the implants is needed. PURPOSE: The purpose of this study was to perform virtual variation simulations on virtually planned implant placements and to compare them with corresponding results from actual surgeries performed on human cadavers in a previous study. MATERIAL AND METHODS: Seventeen computer-aided plans were used for virtual variation simulation of surgeries conducted on 17 human cadavers and 145 implants placed in the cadavers. For each surgery, 10,000 virtual surgeries were performed, resulting in 1,450,000 implant placements. The results from the virtual variation simulations were statistically compared with the results from the actual surgeries. The Mann-Whitney U test was used to compare the implant distributions (alpha=.05). RESULTS: In the maxillae, the difference between the simulated average mean of the mean and the compared surgical average of the median was 0.22 mm (apex) and -0.35 mm (hex), and for the mandible, the corresponding values were -0.19 mm (apex) and -0.69 mm (hex). The simulated average mean of the range compared to the mean range of the maximum deviation results from the surgeries of the maxillae was 2.96 mm (apex) and 0.44 mm (hex), and 2.3 mm (apex) and 0.26 mm (hex) for the mandible. The implant distributions between the simulations and the surgeries were significantly different at both the hex (P<.001) and apex (P<.001). CONCLUSIONS: The implant distributions were neither static nor normally distributed. Thus, within the limitations of this study, the definitive geometrical variations of the implants were not static, as they depend on the individual anatomy of the jaws and the ability to place the CAD/CAM-guided surgical template in the proper position.

OBJECTIVE: The attitude towards oral health is influenced by gender, the level of education and the social background of the patients The aim of the present study was to determine a possible relationship of gender, the educational level and the oral health as well as the choice of dental restorations. METHODOLOGY: A total of 2374 outpatients (age: 18-80 years) from a University dental school were assessed. Inclusion criteria were 15 remaining teeth and good general health. In addition to anamnesis data, information concerning educational level and present occupation was collected. The dental assessment included number of teeth, endodontic treatment, type of restorations (root canal fillings, restoration materials) as well as dental panoramic radiographs (OPG). RESULTS: The female patients demonstrated a statistically significant higher percentage of restoration, more teeth with crowns and more tooth coloured fillings compared to men (P < 0.01). Patients with a higher level of education preferred more expensive restorations, whereas patients with a low level of education exhibited more inexpensive restorations. However, patients with private health insurance showed lower levels of carious lesions. CONCLUSION: The examined patients showed no significant correlation between educational level and type of dental restorations but female patients had a lower number of teeth and more restoration.

RESUMO

Dental professionals are often faced with challenges when formulating a treatment plan for patients presenting with a compromised tooth. A common dilemma involves the decision between tooth retention using endodontic treatment with crown restoration, and extraction and an implant-borne restoration. In this article the authors evaluate the 2 treatment modes, and observe that because outcomes are similar with both treatments, decisions should be based on the patient's informed decision concerning restorability, costs associated with the procedures, esthetics, potential adverse outcomes, and ethical factors.

AIMS: To investigate the attitudes of maxillofacial surgeons in the treatment and dental rehabilitation of oral cancer patients in the UK. MATERIAL AND METHODS: The survey was conducted by postal questionnaires with 17 close-ended questions. A total of 229 questionnaires were sent to members of the British Association of Oral and Maxillofacial Surgeons over a one week period. A follow-up was sent if a reply was not received within 12 weeks. These results were compared to a similar study that was carried out approximately 15 years ago. RESULTS: The response rate was 65.5% (150/229). Overall 62% of respondents (92/150) carried out maxillary resections, which represents a decline of 23% on the previous study. There has been an increase in surgeons reconstructing the maxillary defect from 38% in the 1995 study to 91% in the present study. Ninety-eight percent of respondents had their patients seen in a multidisciplinary team (MDT) clinic, but in only 30% of the cases was a restorative dentist present on these clinics. There has been an improvement in the accessibility of a restorative dentist for this patient cohort, from 65% to 90%. The use of implants for dental rehabilitation post-cancer surgery has increased from 43% to 93%. CONCLUSION: This study highlights the changes in the dental and oral rehabilitation of patients undergoing resective surgery for oral cancer and especially those undergoing a maxillectomy procedure. It illustrates the increased use of implants for post-surgery rehabilitation and shows the different trends in which these implants are placed. An important aspect of this study is the input of the dental team. Current national guidelines state that a consultant restorative dentist needs to be a member of the MDT; this survey shows that this was the case in only 30% of responses.
