BACKGROUND: Melioidosis is a serious bacterial infection caused by Burkholderia pseudomallei, a gram-negative bacterium commonly found in soil and water. It can affect both humans and animals, and is endemic in regions such as Southeast Asia and Northern Australia. In recent years, there have been reports of an emergence of human melioidosis in other areas, including New Caledonia. RESULTS: During standard laboratory analysis in New Caledonia in 2021, a strain of B. pseudomallei was isolated from a goat. The strain was characterized using both MLST and WGS techniques and was found to cluster with previously described local human strains from the area. In parallel, several serological tests (CFT, ELISA, Luminex (Hcp1, GroEL, BPSS1840), arrays assay and a latex agglutination test) were performed on animals from the farm where the goat originated, and/or from three other neighboring farms. Using two commercial ELISA kits, seropositive animals were found only on the farm where the infected goat originated and tests based on recombinant proteins confirmed the usefulness of the Hcp1 protein for the diagnosis of melioidosis in animals. CONCLUSIONS: Despite the regular reports of human cases, this is the first confirmed case of melioidosis in an animal in New Caledonia. These results confirm the presence of the bacterium in the region and highlight the importance of vigilance for both animal and human health. It is critical that all health partners, including breeders, veterinarians, and biologists, work together to monitor and prevent the spread of the disease.

RESUMO

Twenty-four new species of Eutegaeoidea from Australia and New Caledonia are described, and two new genera are proposed. These are Eutegaeus woiwurrung sp. nov., E. nothofagi sp. nov., E. bidhawal sp. nov., E. ptilosus sp. nov., Humerotegaeus carinatus gen. et sp. nov., H. concentricus gen. et sp. nov., Atalotegaeus crobylus sp. nov., Neoeutegaeus torsteini sp. nov., N. melipsilon sp. nov. N. malcolmi sp. nov., N. corniculatus sp. nov. (Eutegaeidae), Compactozetes goongerah sp. nov., C. crenellatus sp. nov. (Compactozetidae) and Pterozetes lawrencei sp. nov. (Pterozetidae) from temperate rainforests in Victoria and Tasmania; Compactozetes bundjalung sp. nov., C. calderi sp. nov., C. duonodulus sp. nov., Sadocepheus remus sp. nov. (Compactozetidae) and Porrhotegaeus githabul sp. nov. (Porrhotegaeidae fam. nov.) from temperate and sub-tropical rainforests of the Great Dividing Range in central and northern New South Wales and southern Queensland, Porrhotegaeus catherinae sp. nov. from scalybark closed forest on Lord Howe Island, Eutegaeus odontatus sp. nov. and Compactozetes pirumorpha sp. nov. from moist upland forest on Norfolk Island and Neseutegaeus wardi sp. nov. and Atalotegaeus deficiens sp. nov. from tropical rainforest and moss forest in New Caledonia. Based on the predominantly Southern Hemisphere distribution of Eutegaeoidea, indicating strong Gondwanan affinities, and the morphology of adults and immatures, this taxon is treated as distinct from the Cepheoidea which has a distribution almost entirely within the Northern Hemisphere. Eutegaeoid species previously described from Australia (Eutegaeus soror P. Balogh, 1985, Atalotegaeus mensarosi J. & P. Balogh, 1983, Neseutegaeus monteithi J. & P. Balogh, 1983, Neoeutegaeus phyllophorus J. & P. Balogh, 1983 and Porrhotegaeus ornatus J. Balogh & Mahunka, 1966) are redescribed based on type material and new distribution records provided. Species have distribution patterns predominantly indicative of short-range endemics associated with remnant Gondwanan rainforest. Neseutegaeus monteithi is recombined with Atalotegaeus Luxton, 1988a. The definitions of genera and families of Eutegaeoidea are revised, and their relationships are reconsidered. Birotegaeus Luxton, 1988a and Pareutegaeus Woolley, 1965 are designated junior synonyms of Eutegaeus Berlese, 1916. Immatures are described for the genera Atalotegaeus, Eutegaeus, Neoeutegaeus Aoki, 1964 and Porrhotegaeus J. Balogh & Mahunka, 1966. Neoeutegaeidae fam. nov. is established for Neoeutegaeus Aoki, 1964 and Humerotegaeus gen. nov., Porrhotegaeidae fam. nov. for Porrhotegaeus and Bornebuschiidae fam. nov. for Bornebuschia Hammer, 1966 and Dicrotegaeus Luxton, 1988 which had previously been placed in Cerocepheidae or Compactozetidae. Eutegaeus aysenensis Ermilov, 2021 and E. queulatensis Ermilov, 2021 from Chile, are recombined with Atalotegaeus. A key is provided to the genera of the eight families of Eutegaeoidea, as recognised herein: Eutegaeidae, Neoeutegaeidae fam. nov., Cerocepheidae, Compactozetidae, Bornebuschiidae fam. nov., Pterozetidae and Porrhotegaeidae fam. nov.

CONTEXT: Although there is strong evidence in the literature on the interest of the implementation of health policy and programs in schools, data on the determinants of the active participation of education professionals in health promotion programs remain scare. OBJECTIVE: Our study was conducted in 2018-2019 in New Caledonia, a South Pacific archipelago rich in cultural diversity but marked by major inequalities in health and education. It aimed to identify the factors that explain the involvement of teachers in an intersectoral health promotion program (development of a health education pathway led by an intersectoral team). It involved 40 schools, which corresponds to 20% of primary schools of the archipelago. RESULTS: Data show that of the 139 education professionals who were enrolled in the program by the education administration, 77 actively contributed. The work of these 60 teachers, 12 school principals and 5 district managers were studied in detail and categorized on the basis of the nature of their contribution. CONCLUSION: Analysis of the factors impacting active participation showed that two main parameters play a key role in teacher engagement: support and leadership at the school level. This work confirms, on the one hand, the stakes for primary schools to have local support in health promotion and, on the other hand, the need to strengthen the role of school principals in the field of health.

Pure European bovine breeding is compromised in South Pacific (i.e. Australia, New Caledonia) due to the impact of acaricide-resistant Rhipicephalus (Boophilus) australis ticks. Farmers need new solutions to sustainably maintain their activity. An integrated tick control program has been applied for 2 years in herds of European breeds (i.e. Charolais and Limousine cattle breeds) in New Caledonia. A field trial including the use of a vaccine based on a local Bm86 antigen, agropastoral measures and a chemical control of the tick was conducted to assess the possibility to reduce the number of acaricidal treatments. Nine farms including 1400 animals were monitored from July 2017 to June 2020. The year mid2017-mid2018 was the year of initial monitoring in order to get control data related to tick infestation during the seasons and to the number of treatments per herd. Vaccinations began between September and December 2018. Results obtained from mid2019 to mid2020 were then compared to the initial period (mid2017-mid2018) to assess the benefits of the vaccine. The average annual number of acaricide treatments decreased significantly from 7.44 before vaccination to 4.45 during mid2019-mid2020 (p = 0.026). Infestation scores, assessed by tick counts when farmers grouped the cattle to apply acaricides, showed a constant and low infestation level. Nevertheless, farmers were willing to tolerate slightly higher tick loads on vaccinated cattle with less acaricidal treatments compared to unvaccinated and frequently treated cattle. Local cutaneous reactions were recorded at the injection site with 46.6% of all injections evolving into nodules and 1% of injections giving depilation or abscesses. At the end of the trial, all the breeders wish to continue the vaccination and consider that it is now possible to serenely maintain their breed.

BACKGROUND: Biological control programs involving Wolbachia-infected Aedes aegypti are currently deployed in different epidemiological settings. New Caledonia (NC) is an ideal location for the implementation and evaluation of such a strategy as the only proven vector for dengue virus (DENV) is Ae. aegypti and dengue outbreaks frequency and severity are increasing. We report the generation of a NC Wolbachia-infected Ae. aegypti strain and the results of experiments to assess the vector competence and fitness of this strain for future implementation as a disease control strategy in Noumea, NC. METHODS/PRINCIPAL FINDINGS: The NC Wolbachia strain (NC-wMel) was obtained by backcrossing Australian AUS-wMel females with New Caledonian Wild-Type (NC-WT) males. Blocking of DENV, chikungunya (CHIKV), and Zika (ZIKV) viruses were evaluated via mosquito oral feeding experiments and intrathoracic DENV challenge. Significant reduction in infection rates were observed for NC-wMel Ae. aegypti compared to WT Ae. aegypti. No transmission was observed for NC-wMel Ae. aegypti. Maternal transmission, cytoplasmic incompatibility, fertility, fecundity, wing length, and insecticide resistance were also assessed in laboratory experiments. Ae. aegypti NC-wMel showed complete cytoplasmic incompatibility and a strong maternal transmission. Ae. aegypti NC-wMel fitness seemed to be reduced compared to NC-WT Ae. aegypti and AUS-wMel Ae. aegypti regarding fertility and fecundity. However further experiments are required to assess it accurately. CONCLUSIONS/SIGNIFICANCE: Our results demonstrated that the NC-wMel Ae. aegypti strain is a strong inhibitor of DENV, CHIKV, and ZIKV infection and prevents transmission of infectious viral particles in mosquito saliva. Furthermore, our NC-wMel Ae. aegypti strain induces reproductive cytoplasmic incompatibility with minimal apparent fitness costs and high maternal transmission, supporting field-releases in Noumea, NC.

Lichen biomonitoring and air mass trajectories were used to study the influence of mining activities in the atmospheric dispersion of metallic elements to assess the exposure of the population to dust emitted by mining activities. A map of forward trajectory densities from open mine surfaces throughout New Caledonia was computed and allowed to identify three preferred wind directions (trade wind, bent trade winds and oceanic winds) that could arise in mining particles dispersion all over New Caledonia. Areas where an air quality monitoring would be advisable to evaluate the exposure of the population to the Nickel dusts have been identified. Lichens collected around the industrial mining site KNS and in North Provence of New Caledonia were analysed for their Ni, Co, Cr, Zn and Ti contents. Backward trajectories were simulated from the lichen sampling point using FLEXTRA fed with ECMWF meteorological data, and densities of trajectories having overflown a mine were calculated. Ratio metal/Ti was then plotted as a function of air mass trajectory densities having overflown open pits. A positive correlation between trajectory densities and titanium-normalized metal in lichen for Ni, Co, Cr was highlighted, indicating that mining is a source of dispersion of these metals. For Zn, which is a tracer of fossil fuel or biomass (wood) combustion activity, no correlation was found. Graphical abstract.

OBJECTIVE: This scoping review aims to identify and describe the methods used to collect and analyze dietary intake data in residents of Pacific Island countries. INTRODUCTION: Dietary assessment explores associations between dietary factors and health outcomes. In regions, including the Pacific Islands, where diet-related, non-communicable diseases are increasing, this is a growing area of research. As this information is used to inform food and nutrition policies and practice, accurate collection, analysis, and interpretation of dietary assessment data relies on robust methods. A greater understanding of how nutrition studies are designed can strengthen the evidence on nutrition and health in Pacific Island settings and inform future research approaches. INCLUSION CRITERIA: The scoping review will consider studies published in peer-reviewed journals, including quantitative and qualitative study designs and gray literature, including government reports, research and technical reports, theses and dissertations that measure and/or assess dietary intake in resident populations of the Pacific Island member countries of the Secretariat of the Pacific Community: American Samoa, Commonwealth of the Northern Mariana Islands, the Cook Islands, the Federated States of Micronesia, French Polynesia, Guam, the Independent State of Papua New Guinea, the Independent State of Samoa, the Kingdom of Tonga, New Caledonia, Niue, Pitcairn Islands, the Republic of Fiji, the Republic of Kiribati, the Republic of the Marshall Islands, the Republic of Nauru, the Republic of Palau, the Republic of Vanuatu, Solomon Islands, Tokelau, Tuvalu, and Wallis and Futuna. METHODS: There will be no time limit and searches will be conducted in PubMed, CINAHL, CABI, Scopus, Cochrane Library, and Web of Science. Results will be limited to English-language articles. Data will be extracted independently by two reviewers into a charting table. Results will be presented graphically and with tables accompanied by a narrative summary.

BACKGROUND: The objective of this retrospective survey was to evaluate after one year, the conditions and impacts of a dental sealant intervention conducted in New Caledonia, within a health promotion program. A greater or at least equivalent quality and impact of the intervention was expected for children living in socially deprived regions with the greatest health needs. METHODS: The study population was the schoolchildren, aged 6 years in 2016, who benefited from the dental sealant program (n = 2532). The study sample was randomly selected in 2017 from that population (n = 550). The children's dental status was evaluated at school in 2017 and compared with that recorded in 2016 during the sealant intervention allowing the calculation of the retention rates and one-year carious increment on first permanent molars. Socio-demographic variables (gender, public/private school) and conditions of sealant placement (school/dental office, presence of a dental assistant) were recorded. The carious increment was explained using a mixed multiple random-effects regression. A mediation analysis was conducted to assess the respective contributions of the retention rates and the region of origin on caries increment. RESULTS: The participation rate was very high (89%) and on average, children had 83% of their dental sealants present after one year, 31% fully and 52% partially present. Caries increment varied depending on the sealant retention rate as well as on the region (North, South, Islands). The mediation analysis showed that living in a deprived area (The Islands) was a strong determinant for high caries increment particularly when the retention rates were low. CONCLUSIONS: This study showed a high participation rate and acceptable effectiveness as measured with the one-year retention rates, for a fissure sealant intervention conducted in real-life conditions and integrated in a large health promotion program. Nevertheless, the intervention was not effective enough to totally balance the influence of health determinants, especially in socially deprived sectors characterized by greater dental needs.

This cross-sectional study assessed sugar-sweetened beverage (SSB) consumption and its associations with the sociodemographic and physical characteristics, behavior and knowledge of New Caledonian adolescents. The survey data of 447 adolescents from ages 11 to 16 years were collected in five secondary public schools of New Caledonia between July 2015 and April 2016. These data included measured height and weight, SSB consumption, sociodemographic characteristics, body weight perception, physical activity, and knowledge (sugar quantity/SSB unit; energy expenditure required to eliminate a unit) and opinions about the SSB‒weight gain relationship. Ninety percent of these adolescents declared regularly drinking SSBs. Quantities were associated with living environment (1.94 L·week-1 in urban environment vs. 4.49 L·week-1 in rural environment, p = 0.001), ethnic community (4.77 L·week-1 in Melanesians vs. 2.46 L·week-1 in Caucasians, p < 0.001) and knowledge about energy expenditure (6.22 L·week-1 in unknowledgeable adolescents vs. 4.26 L·week-1 in adolescents who underestimated, 3.73 L·week-1 in adolescents who overestimated, and 3.64 L·week-1 in adolescents who correctly responded on the energy expenditure required to eliminate an SSB unit, p = 0.033). To conclude, community-based health promotion strategies should (1) focus on the physical effort needed to negate SSB consumption rather than the nutritional energy from SSB units and (2) highlight how to achieve sustainable lifestyles and provide tools for greater understanding and positive action.

Polyandry has been studied in many species, especially birds [1]. Exclusively fraternal polyandry (several full or half-brothers with one unrelated female) is only known in human societies [2, 3], in which it is an important mechanism for limiting reproductive output [3] in association with scarce environmental resources [2]. However, the social organization of the Kagu Rhynochetos jubatus, a bird species endemic to New Caledonia, has the characteristics of this mating system. Kagu are cooperative breeders and evolved in the absence of predators [4, 5]. Breeding birds and their helpers contributes to the care and defense of the chick [6]. Kagu populations occur in both poor and rich habitats [7] and differ substantially in food supply and associated reproduction rates [8]. This enabled us to verify whether fraternal polyandry increased reproductive output in low-density situations but limited reproduction in high-density populations. Our 15-year study revealed that, regardless of resource availability, Kagu were organized in facultative fraternal polyandrous families grouped in clans. Within a clan, all breeding females were unrelated, whereas all males were related. There was no extra-clan paternity. An average family size of four to five adults was optimal for breeding success. Males that have long-established families in their own territory regularly visited their parents. We conclude that fraternal polyandry in Kagu increases reproductive output under poor environmental conditions but limits population growth when the population is near carrying capacity because the clannish spatial organization prevents new families from establishing territories.

BACKGROUND: Dengue is a mosquito-borne virus that causes extensive morbidity and economic loss in many tropical and subtropical regions of the world. Often present in cities, dengue virus is rapidly spreading due to urbanization, climate change and increased human movements. Dengue cases are often heterogeneously distributed throughout cities, suggesting that small-scale determinants influence dengue urban transmission. A better understanding of these determinants is crucial to efficiently target prevention measures such as vector control and education. The aim of this study was to determine which socioeconomic and environmental determinants were associated with dengue incidence in an urban setting in the Pacific. METHODOLOGY: An ecological study was performed using data summarized by neighborhood (i.e. the neighborhood is the unit of analysis) from two dengue epidemics (2008-2009 and 2012-2013) in the city of Nouméa, the capital of New Caledonia. Spatial patterns and hotspots of dengue transmission were assessed using global and local Moran's I statistics. Multivariable negative binomial regression models were used to investigate the association between dengue incidence and various socioeconomic and environmental factors throughout the city. PRINCIPAL FINDINGS: The 2008-2009 epidemic was spatially structured, with clusters of high and low incidence neighborhoods. In 2012-2013, dengue incidence rates were more homogeneous throughout the city. In all models tested, higher dengue incidence rates were consistently associated with lower socioeconomic status (higher unemployment, lower revenue or higher percentage of population born in the Pacific, which are interrelated). A higher percentage of apartments was associated with lower dengue incidence rates during both epidemics in all models but one. A link between vegetation coverage and dengue incidence rates was also detected, but the link varied depending on the model used. CONCLUSIONS: This study demonstrates a robust spatial association between dengue incidence rates and socioeconomic status across the different neighborhoods of the city of Nouméa. Our findings provide useful information to guide policy and help target dengue prevention efforts where they are needed most.

BACKGROUND/OBJECTIVES: Understanding the factors underlying the spatio-temporal distribution of infectious diseases provides useful information regarding their prevention and control. Dengue fever spatio-temporal patterns result from complex interactions between the virus, the host, and the vector. These interactions can be influenced by environmental conditions. Our objectives were to analyse dengue fever spatial distribution over New Caledonia during epidemic years, to identify some of the main underlying factors, and to predict the spatial evolution of dengue fever under changing climatic conditions, at the 2100 horizon. METHODS: We used principal component analysis and support vector machines to analyse and model the influence of climate and socio-economic variables on the mean spatial distribution of 24,272 dengue cases reported from 1995 to 2012 in thirty-three communes of New Caledonia. We then modelled and estimated the future evolution of dengue incidence rates using a regional downscaling of future climate projections. RESULTS: The spatial distribution of dengue fever cases is highly heterogeneous. The variables most associated with this observed heterogeneity are the mean temperature, the mean number of people per premise, and the mean percentage of unemployed people, a variable highly correlated with people's way of life. Rainfall does not seem to play an important role in the spatial distribution of dengue cases during epidemics. By the end of the 21st century, if temperature increases by approximately 3 °C, mean incidence rates during epidemics could double. CONCLUSION: In New Caledonia, a subtropical insular environment, both temperature and socio-economic conditions are influencing the spatial spread of dengue fever. Extension of this study to other countries worldwide should improve the knowledge about climate influence on dengue burden and about the complex interplay between different factors. This study presents a methodology that can be used as a step by step guide to model dengue spatial heterogeneity in other countries.

Jatropha curcas L. is an inedible plant belonging to the Euphorbiaceae family that is growing in subtropical zones of all continents. We report a series of 24 cases of poisoning with J. curcas seeds or fruits reported to poison centers in Paris and Marseille between December 2000 and June 2014. Fifteen adults and 9 children ingested J. curcas seeds or fruits. All patients experienced gastrointestinal disorders, within the first hours following ingestion: nausea, vomiting, diarrhea and abdominal pain. Laboratory investigations performed in 10 patients revealed minor abnormalities: CK elevation (8 cases), dehydration (5 cases) with moderate elevation of serum creatinine levels (3 cases), and mildly increased serum bilirubin (8 cases). Complete remission of all clinical signs was observed within 48 hours in the 20 cases for which the outcome was known. Previously published cases of J. curcas poisoning were very similar to ours: As in our series, gastrointestinal disorders were always present. They were sometimes associated with neurological or cardiovascular signs, and hepatic or renal disorders; these were generally interpreted as complications of severe gastroenteritis, although direct toxic effects could not be formally excluded. In most cases, simple supportive measures were sufficient to ensure complete recovery within 24-48 hours. J Curcas poisoning incidence is certainly increasing because the plant is cultivated to produce biodiesel and is now largely present in most subtropical countries. As a consequence, local health professionals should be informed of the toxic properties of this plant.

BACKGROUND: Before implementing a new oral health promotion program in the French overseas territory of Nouvelle Calédonie, the health authorities needed recent data about dental status of the New Caledonian child population. OBJECTIVES: This study aimed to describe the dental status of 6, 9 and 12-yr-old New Caledonian children and to investigate the environmental and behavioural risk factors related to oral health. METHODS: A randomly selected sample of 2734 children (744 6-yr-olds, 789 9-yr-olds, and 1201 12-yr-olds) was examined clinically by seven calibrated investigators and participants responded to a questionnaire. The main variables were objective criteria about dental status and subjective criteria about experience of dental care, dental fear, self-perception of oral health, cultural or ethnic identity and environmental and behavioural risk factors. RESULTS: Overall, most of the children had infectious oral diseases: more than 50% had gingivitis, and 60% of 6- and 9 yr-olds had at least one deciduous or permanent tooth with untreated caries. The mean 12-yr-old number of decayed missing and filled teeth (DMFT) was 2.09±2.82. The number of carious lesions was related to the unfavourable lifestyle, deprived social status and no preventive dental care. Kanak, Polynesians and Caledonians (respectively 27%, 18% and 45% of the study sample) were more affected by caries than metropolitan French and Asian children. Children with many untreated carious lesions had negative perceptions of their oral health; they complained of chewing difficulty and had higher scores for dental anxiety. CONCLUSION: This study highlights the need for new strategies aimed at improving oral health and at reducing inequalities in New Caledonia. An oral health promotion program would need to be developed in connection with other health programmes using the common risk factor approach within the context of the local environment.

Numerous threats impact coral reefs and conservation actions are urgently needed. Fast production of marine habitat maps promotes the use of habitat-only conservation plans, where a given percentage of the area of each habitat is set as conservation objectives. However, marine reserves can impact access to fishing grounds and generate opportunity costs for fishers that need to be minimized. In New Caledonia (Southwest Pacific), we used fine-scale fishery catch maps to define nineteen opportunity costs layers (expressed as biomass catch loss) considering i) total catches, ii) target fish families, iii) local marine tenure, and iv) gear type. The expected lower impacts on fishery catch when using the different cost constraints were ranked according to effectiveness in decreasing the costs generated by the habitat-only scenarios. The exercise was done for two habitat maps with different thematic richness. In most cases, habitat conservation objectives remained achievable, but effectiveness varied widely between scenarios and between habitat maps. The results provide practical guidelines for coral reef conservation and management. Habitat-only scenarios can be used to initiate conservation projects with stakeholders but the costs induced by such scenarios can be lowered by up to 50-60% when detailed exhaustive fishery data are used. When using partial data, the gain would be only in the 15-25% range. The best compromises are achieved when using local data.

Fourteen trace elements (Ag, As, Cd, Co, Cr, Cu, Fe, Hg, Mn, Ni, Pb, Se, V, and Zn) were analyzed in livers and muscles from 22 fish species from the New Caledonia lagoon, which is subjected to important chemical inputs due to intense land-based mining activities (New Caledonia is the third largest world producer of Ni). The results of this baseline research indicated that livers generally concentrated trace elements to a greater extent than muscles. Nevertheless, the overall trace element concentrations in both tissues were barely above the levels reported in fish and thus contamination at the local scale was poorly discriminated. Although these levels were low, preliminary risk assessment from a global health standpoint suggests that As would be an element potentially leading to exposure of concern for fish consumers. Based on the trace element concentrations in livers and the fish trophic preferences, some trends have been observed among trophic groups: Ag, Cu, Fe, Hg, and Zn concentrations were generally higher in liver of fish with the highest trophic position whereas Cd concentrations were lower in these groups. The use of the leopard coral grouper Plectropomus leopardus as a resident top predator allowed determining the geographical variations in contamination levels with significant differences for six out of the fourteen elements investigated. The sampling sites influenced by anthropogenic inputs were revealed by high Ag, Cd, Cu, Hg, and Pb concentrations. Such geographic differences also applied to Zn but surprisingly not for the typical elements associated with Ni mining, i.e., Co, Cr, Mn and Ni.

BACKGROUND: Incidence of acute rheumatic fever (ARF) and prevalence of rheumatic heart disease (RHD) in the Pacific region, including New Caledonia, are amongst the highest in the world. The main priority of long-term management of ARF or RHD is to ensure secondary prophylaxis is adhered to. The objectives of this study were to evaluate rates of adherence in people receiving antibiotic prophylaxis by intramuscular injections of penicillin in Lifou and to determine the factors associated with a poor adherence in this population. METHODS: We conducted a retrospective cohort study and we included 70 patients receiving injections of antibiotic prophylaxis to prevent ARF recurrence on the island of Lifou. Patients were classified as "good-adherent" when the rate of adherence was ≥80% of the expected injections and as "poor-adherent" when it was <80%. Statistical analysis to identify factors associated with adherence was performed using a multivariate logistic regression model. RESULTS: Our study showed that 46% of patients from Lifou receiving antibiotic prophylaxis for ARF or RHD had a rate of adherence <80% and were therefore at high risk of recurrence of ARF. Three independent factors were protective against poor adherence: a household with more than five people (odds ratio, 0.25; 95% confidence interval [CI], 0.08 to 0.75), a previous medical history of symptomatic ARF (odds ratio, 0.20; 95% CI, 0.04 to 0.98) and an adequate healthcare coverage (odds ratio, 0.21; 95% CI 0.06 to 0.72). CONCLUSIONS: To improve adherence to secondary prophylaxis in Lifou, we therefore propose the following recommendations arising from the results of this study: i) identifying patients receiving antibiotic prophylaxis without medical history of ARF to strengthen their therapeutic education and ii) improving the medical coverage in patients with ARF or RHD. We also recommend that the nurse designated for the ARF prevention program in Lifou coordinate an active recall system based on an updated local register. But the key point to improve adherence among Melanesian patients is probably to give appropriate information regarding the disease and the treatment, taking into account the Melanesian perceptions of the disease.

Herein, we explore the outlines of an innovative method based on the chemical recovery of metal-rich biomass produced in phytoextraction technologies. Taking advantage of the adaptive capacity of some New Caledonian plants to hyperaccumulate Ni(2+) cations in their aerial parts, this technique is based on the direct use of metals derived from plants as "Lewis acid" catalysts in organic chemistry. Metallic cations contained in New Caledonian nickel hyperaccumulators are recovered through a simple cost-effective process and serve the preparation of heterogeneous catalysts used in synthetic transformations allowing access to molecules with high added-value. The design of all processes is in line with the principles of green chemistry; it is adapted to the new economic constraints; it offers a new relevant outlet for metal-rich biomass; and it represents an alternative to non-renewable mineral materials.
