RESUMO

Dysfunction of the masseter muscle may cause pathological kinking of the parotid duct leading to parotitis; MR sialography is a non-invasive radiological examination that allows to evaluate dynamically the ductal system of the parotid glands. In the present study we aimed to assess the relationships between Stensen's duct and masseter muscle and their implications in the aetiopathogenesis of recurrent parotitis secondary to masseter muscle dysfunction. Forty-one patients with recurrent unilateral parotitis and nine with bilateral recurrent parotitis, all with a clinical suspicious of masseter muscle hypertrophy due to bruxism were enrolled. They underwent ultrasonography as a first line examination and then MR sialography and sialendoscopy. Different anatomical features were studied. Involved parotid glands had a wider duct compared to contralateral unaffected parotid glands of patients with recurrent parotitis (p = 0.00134); male subjects with parotitis had a longer duct compared to the salivary glands of healthy patients (p = 0.00943 for affected glands and p = 0.00629 for the contralateral). A concordance between the evidence of an acute duct angle during sialendoscopy and a wider duct in patients with parotitis was observed although not statistically significant. These initial findings suggest that the masticatory muscle dysfunction related to bruxism seems to condition alteration of parotid duct course and anatomy thus favouring the occurrence of recurrent parotitis. A specific diagnostic iter based on clinical evaluation, dynamic ultrasonography and MR sialography, is therefore, mandatory to confirm the relationship between masseter muscle anatomy and parotid duct anomalies; this is the premise for an adequate therapeutic approach to underlying masticatory muscle disorder.

SUMMARY: Fungal parotitis is rare and the sequela parotid abscess exceedingly so. We report our experience with Candida glabrata and Candida albicans parotid gland abscesses in critically ill HIV-positive patients and highlight the value of microbiological assessment to tailor management.

Los países de las Américas, con apoyo de la Organización Panamericana de la Salud (OPS), han logrado avances notables al proporcionar a los niños y las niñas una protección general contra las enfermedades prevenibles mediante vacunación. Los niveles sostenidos de elevadas coberturas nacionales de vacunación, la erradicación de la poliomielitis, la interrupción de la transmisión endémica del virus del sarampión y las iniciativas más recientes en favor de la eliminación de la rubéola y el síndrome de rubéola congénita son hitos continentales de este progreso. En la actualidad, los países administran vacunas a grupos etarios distintos de los incluidos en los programas tradicionales de inmunización infantil. La introducción de la vacuna estacional contra la gripe en los adultos en riesgo; la vacunación de adolescentes y adultos, hombres y mujeres, para la eliminación de la rubéola, y la definición de la carga de enfermedad del cáncer cervicouterino son las actividades que apoyan la necesidad fundamental de efectuar una transición de los programas nacionales de inmunización infantil a programas de inmunización de la familia. Una de las funciones de la OPS en su apoyo a los países es difundir información que destaque el progreso de la Región y los desafíos que enfrenta. Con ese fin, publica varios documentos técnicos relacionados con la inmunización, tales como los boletines de inmunización, los módulos de capacitación en inmunización y la serie de guías prácticas sobre rubéola, sarampión, tétanos neonatal, poliomielitis, vacuna pentavalente y fiebre amarilla.

OBJECTIVE: To review the current literature on drug-induced parotitis. DATA SOURCES: Literature was accessed through MEDLINE/PubMed (1980-May 2012), using the search terms sialadenitis/chemically induced and parotitis/chemically induced. EMBASE (1980-May 2012) was searched using the terms parotitis/diagnosis, sialadenitis/side effect, and parotitis/side effect. International Pharmaceutical Abstracts (1970-May 2012) was searched using the search terms parotitis and sialadenitis. All searches were limited to articles on humans written in English. Inclusion criteria were published letters, case reports, reviews, and clinical trials involving drugs that may be associated with parotitis. Articles pertaining to parotitis induced by iodine-containing drugs were excluded. References of all relevant articles were reviewed for additional citations. STUDY SELECTION AND DATA EXTRACTION: Review articles, clinical trials, background data, and case reports of drug-induced parotitis were collected and case reports were assessed for causality. DATA SYNTHESIS: Parotitis is an uncommon adverse effect; however, signs and symptoms of parotitis have been noted in case reports as an adverse drug reaction related to various medications. Assessing causality of an adverse drug reaction such as parotitis is challenging. To help determine the probability of causality for these events, algorithms such as the Naranjo probability scale have been developed. Eighty-four case reports of drug-induced parotitis from 40 different drugs were reviewed using a modified Naranjo probability scale that included criteria specific for parotitis. Medications that met the criteria for establishing causality included l-asparaginase with 7 case reports, clozapine with 13 case reports, and phenylbutazone with 13 case reports. CONCLUSIONS: Drug-induced parotitis is a rare adverse drug reaction. Based on the quantitative and qualitative evidence collected from the case reports, medications that are associated with drug-induced parotitis include l-asparaginase, clozapine, and phenylbutazone. Many other drugs have been implicated in the development of parotitis; however, the evidence supporting this association is insufficient to determine causality at this time.

Objetivo Determinar las manifestaciones orales y caries dental en niños expuestos al virus de inmunodeficiencia humana (VIH). Métodos Se estudiaron 37 niños que acuden a la consulta pediátrica del programa nacional de control de SIDA (PRONASIDA-Lara). Se informó a padres y representantes sobre los objetivos del estudio y su carácter confidencial. El examen de la cavidad oral lo realizó un odontólogo, quien además indagó sobre historia de manifestaciones bucales y corroboró la información con la historia clínica. Resultado La edad promedio fue 5,15±3,79 años, 62,2 % era del género masculino. Se infectaron por trasmisión vertical 91,8 %. La mayoría de los niños pertenecían a la categoría clínica A (35,1 %). El mayor porcentaje (37,8 %) de niños se encontró en la categoría 15-24 % de linfocitos CD4. Entre las manifestaciones orales más prevalentes se encontró la candidiasis oral, queilitis angular, eritema gingival lineal y parotiditis. Se observó diferencia estadísticamente significativa entre el porcentaje de linfocitos CD4 y queilitis angular (p<0,05). Cuarenta y cinco coma nueve (45,9 %) presentó caries dental y 29,7 % tenían necesidad de tratamiento dental "urgente". Por otra parte, el índice de necesidad de tratamiento calculado (INT) muestra que tres cuartos de los niños tenían necesidad de tratamiento dental. Conclusiones Los resultados del estudio permiten concluir que existe la necesidad de una estrategia basada en el manejo de las manifestaciones orales, frecuentes en la infección por VIH en niños, que permita un tratamiento desde un punto de vista integral, con un enfoque clínico y preventivo.

Parents and guardians were informed about the purpose of the study and its confidentiality. An oral examination was conducted by a dentist; parents/guardians were asked about their children's history of oral manifestations and this was corroborated with their clinical charts. Results The mean age was 5.15±3.79; 62.2 % were males. Ninety-eight point eight percent had become vertically infected. Most of the children were classified in the A immune suppression category (35.1 %). The highest percentage (37.8 %) of children were in the 15 %-24 % total CD4+ lymphocyte category. The most prevalent oral manifestations were oral candidiasis, angular cheilitis, linear gingival erythema and parotid enlargement. There was a significant statistical difference between percentages of CD4 lymphocytes and angular qualities (p<0.05). Forty-five point nine percent had dental caries and 54.1 % were in "urgent" need of dental treatment. The Index of Orthodontic Treatment Need (IOTN) showed that ¾ of the children needed dental treatment. Conclusions The results of the study led to concluding that there was a need for an oral manifestation management-based strategy focusing on clinical and preventative treatment, as oral manifestations are frequent in children suffering from HIV infection.

La parotiditis recurrente se define como una inflamación parotídea, generalmente asociada a una sialectasia no obstructiva glandular. Se realizó un estudio en 74 niños menores de 15 años con diagnñstico de parotiditis recurrente en el período de 2000 a 2007. A cada paciente se le realizó interrogatorio, examen físico y estudio inmunológico mediante cuantificación de inmunoglobulinas séricas M y G, rosetas espontánea y activa e índice opsonofagocítico. La enfermedad afectó de forma similar a los 2 sexos. La edad de presentación de la primera crisis fue alrededor de los 3 años, con un promedio de 7 crisis por niño y una duración de 6 d. El 95,9 por ciento de los pacientes presentó alguna alteración de la respuesta inmune, 41,8 por ciento de células T, 12,2 por ciento de células fagocíticas, y 41,8 por ciento combinadas


Recurrent parotiditis is defined as parotic inflammation that is generally associated to non-obstructive glandular sialectasia. Seventy four children under 15 years of age, diagnosed with recurrent parotiditis from 2000 to 2007, were studied. Each patient was questioned and they also underwent physical exam and immunological study through quantification of serum M and G immunoglobulins, the spontaneous and active rosettes and the opsonocytophagic index. The disease affected males and females in a similar way. The age of onset of the first crisis was 3 years, with an average of 7 crises per child and 6 days of duration. Of these patients, 95.9 percent presented with some disorder in the immune response, that is, 41.8 percent in T-cells, 12.2 percent in phagocytic cells and 41.8 percent combined

RESUMO

A variety of neoplastic and non-neoplastic conditions may present with parotid swelling. It is rarely possible to predict the nature of a swelling on clinical grounds alone and accurate imaging is necessary for further assessment. The parotid glands are superficial structures and are readily amenable to high resolution ultrasound examination. Ultrasound is able to: differentiate possible benign from malignant neoplasms; demonstrate whether a palpable lesion arises within the parotid gland, or is periparotid in location; and identify those entities that may not need surgical intervention. In this article the sonographic features of a range of parotid pathologies are reviewed and examples illustrated and the role of ultrasound in parotid assessment is discussed.
