RESUMO

Advances in technologies including omics, apps, imaging, sensors, and big data are increasingly being integrated into research by nurse scientists, but the impact on improving health equity is still unclear. In this article, nursing research faculty from one institution discuss challenges and opportunities experienced when integrating various technologies into their research aimed at promoting health equity. Using exemplars from faculty experiences, a three-pronged approach to keeping patients and communities and the goal of health equity central in research while incorporating advancing technologies is described. This approach includes establishing long-term engagement with populations underrepresented in research, adopting strategies to increase diversity in study participant recruitment, and training and collaboration among a diverse workforce of educators, clinicians, and researchers. Training nurse scientists in integrating data and technology for advancing the science on health equity will shift the culture of how we understand, collaborate, and grow with the communities in which we train and practice as nurse scientists.

Introducción: Es necesario que las enfermeras tengan habilidades investigativas para resolver problemas y necesidades en la gestión de salud. Objetivo: Identificar la relación existente entre las habilidades investigativas y la gestión en salud en las enfermeras de la Red de Salud Arequipa-Caylloma. Métodos: Estudio descriptivo correlacional, de corte transversal, en la Red Arequipa - Caylloma en Arequipa, Perú durante el año 2017. El universo de estudio quedó constituido por 153 profesionales de enfermería, con la aplicación de la fórmula de población finita para variable cualitativa, mediante muestreo aleatorio simple se obtuvo una muestra de 90 enfermeras. Las variables de estudio fueron habilidades investigativas y Gestión de salud en enfermería. Para obtener la información la investigadora utilizó dos cuestionarios. Para el análisis distribuciones de frecuencias absolutas y porcentajes, para la asociación de variables el χ² con una significación de p < 0.05. Resultados: Predominaron las enfermeras con nivel regular de habilidades investigativas (47,77 por ciento) y de gestión en salud (45,55 por ciento), con el estadístico no paramétrico el χ² se encontró asociación significativa (p = 0,001) entre las variables habilidades investigativas y la gestión de salud. Conclusión: Aunque predominaron los niveles regulares de gestión y de habilidades investigativas en enfermeras de la Red de Salud Arequipa Caylloma, se encontró relación significativa entre las variables habilidades investigativas y la gestión en salud(AU)


Introduction: It is necessary for nurses to have research skills to solve problems and needs in health management. Objective: To identify the relationship between research skills and health management in nurses from Arequipa-Caylloma Health Network. Methods: Descriptive, correlational and cross-sectional study carried out, during the year 2017, with the Arequipa-Caylloma Network in Arequipa, Peru. The study universe was made up of 153 nursing professionals, with the application of the finite population formula for qualitative variable. By simple random sampling, a sample of ninety nurses was obtained. The study variables were research skills and nursing health management. To obtain the information, the researcher used two questionnaires. For the analysis of absolute frequency distributions and percentages, as well as for the association of variables, the chi-square test was used, with a significance level of P < 0.05. Results: Nurses with a regular level of investigative skills (47.77 percent) and health management skills (45.55 percent) predominated. The nonparametric statistic chi-square test permitted to find a significant association (P = 0.001) between the variables of investigative skills and health management. Conclusion: Although regular levels of management and research skills predominated in nurses from Arequipa-Caylloma Health Network, a significant relationship was found between the variables of research skills and health management(AU)

RESUMO

The development of care research and its teaching in the Faculty of Nursing at Universidad de Antioquia cannot be understood outside the context in which it emerges and from the trajectory of its leading players. This is how this article will present a synthesis of the future of research, with its principal milestones and events in institutional history, in dialogue with the trajectories of four teaching nurses, protagonists of key moments in said history and living testimony of 70 years of institutional life. This panorama seeks to constitute a referent for strategic reflection, which inspires new generations to meet the research challenges and teach care research.

RESUMO

Sensor technologies for health care, research, and consumers have expanded and evolved rapidly. Many technologies developed in commercial or engineering spaces, lack theoretical grounding and scientific evidence to support their need, safety, and efficacy. Theory is a mechanism for synthesizing and guiding knowledge generation for the discipline of nursing, including the design, implementation, and evaluation of sensors and related technologies such as artificial intelligence and machine learning. In this paper, three nurse scientists summarize their presentations at the Council for the Advancement of Nursing Science 2019 Advanced Methods Conference on Expanding Science of Sensor Technology in Research discussing the theoretical underpinnings of sensor technologies development and use in nursing research and practice. Multiple theories with diverse epistemological roots guide decision-making about whether or not to apply sensors to a given use; development of, components of, and mechanisms by which sensor technologies are expected to work; and possible outcomes.

In this paper, I argue for the need to shift the focus of nursing research regarding breastfeeding from that of individual responsibility to include a more system-focused, population health approach. In the 2011, "Call to Action to Support Breastfeeding," the U.S. Surgeon General called attention to the pervasive disparities in breastfeeding outcomes in the United States. A plethora of nursing research exists aimed at addressing these disparities; however, this research leans toward a neoliberal perspective, mainly focusing on individual factors and failing to address the systemic inequities contributing to these disparities. A shift in nursing science that focuses research at a population level would more effectively support addressing disparities in breastfeeding outcomes and embrace a commitment to social justice. In focusing at a population level, specific inequities that must be addressed include the negative history of breastfeeding in the Black population, systemic racism, and gender inequality. Critical research methodologies are proposed as useful approaches to address these inequities. By engaging in this level of research and using a critical lens, nurse scientists broaden their scope of care to include the entire population, motivate needed social and policy change, facilitate the choice to breastfeed, and ultimately eliminate breastfeeding disparities.

PURPOSE: The purpose of this article is to describe the differences between quality improvement and implementation science, the urgency for nurses and nurse scientists to engage in implementation science, and international educational opportunities and resources for implementation science. ORGANIZING CONSTRUCT: There is a push for providing safe, effective, patient-centered, timely, efficient, and equitable health care. Implementation science plays a key role in adoption and integration of evidence-based practices to improve quality of care. METHODS: We reviewed implementation science programs, organizations, and literature to analyze the roles of nurses and nurse scientists in translating evidence into routine practice. FINDINGS: Implementation-trained nurses and nurse scientists are needed as part of multidisciplinary teams to advance implementation science because of their unique understanding of contextual barriers within nursing practice. Likewise, nurses are uniquely qualified for recognizing what implementation strategies are needed to improve nursing care across practice settings. CONCLUSIONS: Many international clinical and training resources exist and are supplied to aid interested readers in learning more about implementation science. CLINICAL RELEVANCE: Half of research evidence never reaches the clinical setting, and the other half takes 20 years to translate into clinical practice. Implementation science-trained nurses are in a position to be excellent improvers for meaningful change in practice.

RESUMO

As part of National Association of Hispanic Nurses' Strategic Plan, our organization focuses on the development of scholarship in our members. Scholarship is what moves nursing science forward and includes not only research but also teaching/education and clinical practice. Part of scholarship is identifying what is in the literature or what is the "evidence" which is detrimental to identifying the best practices for our Latino communities. This article is part of a scholarship development series that will run in Hispanic Health Care International. The purpose of this first article is to provide a background on how to conduct a review of the literature. The objectives of the article are to (1) define what a review of the literature is, (2) discuss various types of review articles that can be used for research, practice, or education, (3) identify the process of synthesizing evidence, (4) present standardized formats that can be used in writing a review, and (5) provide steps to begin the process of writing a review.

BACKGROUND: Collaborative partnerships are increasingly recognised as valuable and essential tools for improving community health. AIM: To present the process used to establish a collaboration between a university and a community, including a description of the conceptual model that provided guidance for one such project. DISCUSSION: While numerous studies explore the potential benefits of such efforts, few have addressed how to initiate partnerships. The initiation phase and formative work required are arguably the most crucial step, as they become the foundation on which all future efforts are built and through which sustainability is achieved. CONCLUSION: These formative steps are essential when working with Native American communities, which experience severe health disparities, but for which limited initiatives are available for community health interventions. IMPLICATIONS FOR PRACTICE: The approach used in this project can guide the establishment of a university-community collaboration, from initiation to evaluation.

OBJECTIVE: To develop a journal club on management topics in Nursing. METHOD: Action research, adopting content analysis as methodological reference and the journal club strategy. 12 nursing managers of a public hospital in the state of São Paulo participated from August to November 2016. RESULTS: data showed the participation of nurses in the journal club in five meetings planned collectively with the following topics: "aspects regarding organization", with the subtopics "planning", "work process", "structure" and "people management"; and the topic "aspects regarding the team", with the subtopics "autonomy", "behavior/attitude", "awareness" and "social problems". CONCLUSION: The study showed the comprehensiveness of topics the journal club can address regarding management, and how scientific basis is important in the daily life of nursing managers to improve quality of care and the ability to manage.

AIM: The purpose of this article is to present the derivation of the Practice Primed Decision Model from a naturalistic decision-making framework for use in guiding future nursing decision-making research. BACKGROUND: Acute care nurses make decisions in demanding environments under the influence of many factors. The influence of these factors on nurse decision-making is not well understood leading to gaps in understanding how to best support acute care nurse decision-making. METHODS: The strategy of theory derivation was used in the development of a new model for use in nursing research. This model incorporates important elements identified in naturalistic decision making, a Recognition Primed Decision Model and an integrative review of nurse decision-making literature. CONCLUSION: The new model, Practice Primed Decision Model, provides a new perspective to guide nurse decision-making research. This model includes factors influential to the nurse decision-making process that is more realistic in time limited, high stakes decision-making situations.

Conclusión: El estudio mostró el alcance de los temas que el club de lectura puede abordar en relación a la gestión, y la importancia del fundamento científico en el cotidiano de los gerentes de enfermería para mejorar la calidad de la asistencia y la capacidad para administrar.

Se realizó un estudio descriptivo y transversal de 139 estudiantes de segundo año de la carrera Licenciatura en Enfermería, pertenecientes a la Facultad de Enfermería-Tecnología de la Salud de Santiago de Cuba, desde septiembre de 2017 hasta enero de 2018, a fin de evaluar la utilidad de un material didáctico sobre Investigación Cualitativa como herramienta de apoyo a la asignatura Elementos de Investigación Cualitativa en Salud. Se obtuvo un elevado nivel de aceptación en los estudiantes, quienes confirmaron la claridad con que se exponen los contenidos, el nivel de profundidad y la importancia de contar con un texto que ejemplifique y vincule el contenido de metodología de la investigación con situaciones de su práctica diaria. Este material favoreció la calidad del proceso de enseñanza - aprendizaje, dada en la promoción alcanzada, que resultó cualitativamente superior a la de cursos anteriores.


A descriptive and cross-sectional study of 139 students of 2nd year of Licentiate in Nursing, belonging to the Health Nursing-Technology Faculty in Santiago de Cuba was carried out from September, 2017 to January, 2018, in order to evaluate the usefulness of a didactic material on Qualitative Investigation as a support tool to the subject Elements of Qualitative Investigation in Health. A high level of acceptance was obtained in the students who confirmed the clarity with which the contents and their quality are exposed and the importance of having a text that exemplifies and links the content of the investigation methodology with situations of the daily practice. This material favored the quality of the teaching - learning process, given the high marks, which were qualitatively higher to those of previous courses.

BACKGROUND: The authors designed the evidence-based nursing practice assessment tool (EBNPAT) to measure the competence of nurses in using the five-step evidence-based practice model: ask, acquire, appraise, apply and assess. However, they needed to assess its content validity to show it is a valid tool. Assessing the content validity of a newly developed measurement scale is often challenging. Although the content validity index (CVI) is widely used in nursing research, it is limited to evaluating the chance of variation in agreement and consistency among reviewers of the content. AIM: To demonstrate the content validity of EBNPAT. DISCUSSION: An expert panel of 42 nursing and health academics and researchers evaluated the representativeness, comprehensiveness and clarity of each construct item on EBNPAT. They then rated their agreement on a four-point ordinal scale. The authors used the item-CVI (I-CVI) augmented with the intraclass correlation coefficient (ICC) and Cronbach's to assess the results and determine the content validity of EBNPAT. All I-CVIs for representativeness, comprehensiveness and clarity of the EBNPAT items were greater than 0.9 and the overall agreement and consistency among experts were greater than 0.65. The strengths of this study were the good sample size and the substantial consensus among experts in their view of assessing competence in evidence-based practice. CONCLUSION: High content validity of all construct items supports the progression to the next step in psychometric evaluation of EBNPAT. IMPLICATIONS FOR PRACTICE: This paper has demonstrated a sophisticated statistical approach for evaluating content validity that may be of use to developers of measurement scales in the future.

Neoliberal ideology and exclusionary policies based on racialized identities characterize the current contexts in North America and Western Europe. Nursing knowledge cannot be abstracted from social, political and historical contexts; the task of examining the influence of race and racial ideologies on disciplinary knowledge and inquiry therefore remains an important task. Contemporary analyses of the role and responsibility of the discipline in addressing race-based health and social inequities as a focus of nursing inquiry remain underdeveloped. In this article, we examine nursing's engagement with ideas about race and racism and explore the ways in which nursing knowledge and inquiry have been influenced by race-based ideological discourses. Drawing on Henry and Tator's framework of democratic racism, we consider how strategic discursive responses-the discourses of individualism, multiculturalism, colour-blindness, political correctness and denial-have been deployed within nursing knowledge and inquiry to reinforce the belief in an essentially fair and just society while avoiding the need to acknowledge the persistence of racist discourses and ideologies. Greater theoretical, conceptual and methodological clarity regarding race, racialization and related concepts in nursing inquiry is needed to address health and social inequities.

Background Ethnography, originally developed for the study of supposedly small-scale societies, is now faced with an increasingly mobile, changing and globalised world. Cultural identities can exist without reference to a specific location and extend beyond regional and national boundaries. It is therefore no longer imperative that the sole object of the ethnographer's practice should be a geographically bounded site. Aim To present a critical methodological review of multi-sited ethnography. Discussion Understanding that it can no longer be taken with any certainty that location alone determines culture, multi-sited ethnography provides a method of contextualising multi-sited social phenomena. The method enables researchers to examine social phenomena that are simultaneously produced in different locations. It has been used to undertake cultural analysis of diverse areas such as organ trafficking, global organisations, technologies and anorexia. Conclusion The authors contend that multi-sited ethnography is particularly suited to nursing research as it provides researchers with an ethnographic method that is more relevant to the interconnected world of health and healthcare services. Implications for practice Multi-sited ethnography provides nurse researchers with an approach to cultural analysis in areas such as the social determinants of health, healthcare services and the effects of health policies across multiple locations.

AIM: This paper explores the potential for incorporating big data in nursing regulators' decision-making and policy development. Big data, commonly described as the extensive volume of information that individuals and agencies generate daily, is a concept familiar to the business community but is only beginning to be explored by the public sector. BACKGROUND: Using insights gained from a recent research project, the College and Association of Registered Nurses of Alberta, in Canada is creating an organizational culture of data-driven decision-making throughout its regulatory and professional functions. The goal is to enable the organization to respond quickly and profoundly to nursing issues in a rapidly changing healthcare environment. SOURCES OF EVIDENCE: The evidence includes a review of the Learning from Experience: Improving the Process of Internationally Educated Nurses' Applications for Registration (LFE) research project (2011-2016), combined with a literature review on data-driven decision-making within nursing and healthcare settings, and the incorporation of big data in the private and public sectors, primarily in North America. DISCUSSION: This paper discusses experience and, more broadly, how data can enhance the rigour and integrity of nursing and health policy. CONCLUSION: Nursing regulatory bodies have access to extensive data, and the opportunity to use these data to inform decision-making and policy development by investing in how it is captured, analysed and incorporated into decision-making processes. IMPLICATIONS FOR NURSING AND HEALTH POLICY: Understanding and using big data is a critical part of developing relevant, sound and credible policy. Rigorous collection and analysis of big data supports the integrity of the evidence used by nurse regulators in developing nursing and health policy.

BACKGROUND: There is increasing reference to complex patient needs in health care. However, little is known about how to measure patient complexity or the relationship between it and district nursing activity. AIM: To illustrate the use of group concept mapping (GCM) to conceptualise and develop items for a patient complexity assessment instrument to be used by district nurses. DISCUSSION: The first phase of this research used GCM conceptualisation and consensus methodology to identify items for a community-based patient complexity instrument. GCM helped to provide a conceptual understanding of community-based patient complexity through focused exploration of the term. RESULTS: indicated that a number of environmental, sociological, psychological, behavioural, physical and organisational factors needed to be included. This, in turn, showed that an existing taxonomy did not contain the relevant items. Consequently, amendments were made and a new instrument developed. CONCLUSION: GCM is a suitable consensus method for use in nursing theory and developing instruments. It proved successful in achieving consensus with no loss of participants' views. IMPLICATIONS FOR PRACTICE: GCM is a suitable method for nurses to use in research or practice development activities as it is based on a facilitative and engagement-led approach.
